The United States Public Health Service recommends that age appropriate preconception care be provided to all women of reproductive age. Preconception health care provides a woman the opportunity to assess her health status before pregnancy, make appropriate health promoting behavior changes and increase the potential for a healthy pregnancy outcome. Neural tube defects, fetal alcohol syndrome, congenital malformations and low birth weight are examples of adverse infant conditions that can be reduced through preconception health care interventions. Current research suggests that preconception health care according to United States Public Health Service recommendations is not practiced in the primary care setting. …

The rate of teenage sexual involvement is at an all-time high. Traditional sex education is obviously not working. Abstinence education programs have been initiated within some school districts in an effort to decrease the number of adolescent pregnancies and sexually transmitted diseases. The purpose of this quasi-experimental study was to investigate the perceptions of risks and intentions of sexual activity in eighth-grade students before and after an abstinence education program. Breast cancer risk increases with age, and disease mortality has a direct correlation to the stage of detection. Mammography remains the gold standard for early detection of the disease, yet females aged 65 and over have demonstrated a lack of compliance with recommended screening frequency. The purpose of this study was to determine the effects of an educational program on the variables of perceived susceptibility to breast cancer, benefits of and barriers to mammography, and intention to obtain mammography. Current research suggests that 36% of American women are considered overweight. Women who are overweight by definition are at risk for major health problems as well as altered levels of self-esteem. A correlational design was utilized to examine the following null hypothesis: There is no relationship between body weight and levels of self-esteem in overweight women. Nola Pender's Health Promotion Model was the theoretical framework used to guide this study. The sample consisted of 42 women who were overweight by definition. Background: Sepsis is a serious condition resulting in end organ damage and ultimately, death. Communication techniques for nurses and physicians on septic patients admitted to the hospital from the emergency department was evaluated. Paper communication was used in 2020 to facilitate serum diagnostic acquisition and antibiotic administration (i.e., sepsis bundle), but an electronic communication form was utilized in 2021.
Method: An observational, descriptive design utilizing a medical record review was completed to compare the same time-period in 2020 and 2021. Communication methods for sepsis bundle completion were compared.

Despite the continuing high number o f repeat pregnancies to teens and the negative consequences o f a second birth, the American health care system continues to focus pregnancy prevention efforts on never-pregnant teens through a message o f abstinence. While abstinence is an important component o f pregnancy-prevention efforts, this approach does not address the needs o f teens who are already mothers. The purpose of this study was to explore the healthcare professional’s experience during the implementation of information technology. The focus is on the implementation of a computerized medication ordering, dispensing, and tracking system in acute care departments of a 600- bed public hospital. Spradley’s qualitative, ethnographic research methods were used to discover the personal meaning of the implementation. How do pharmacists and nurses feel about the implementation of an advanced technology system? Are there any barriers to implementation? This article describes how one health care executive charted her career course to achieve the advanced management competencies defined by Longest. To achieve success in the new and complex world of integrated health care delivery networks, aspiring executives are encouraged to develop a broad base of knowledge and experience that capitalizes on their unique skills, talents, and interests, rather than pursuing a narrowly defined linear career ladder. The importance of management team competency and selection of educational preparation are highlighted in the context of examples of advanced management competency development.

As a result of the ever changing and expanding role of the nursing home administrator in conjunction with the stricter legislation governing nursing facilities over the past decade and the graying of America, a study specific to this population was warranted. The purpose of this study is to determine the relative change in the self-reported occupational stressors of nursing home administrators over a five year period (December 1994 to June 1999) secondary to the increased regulatory climate of the nursing home industry.

Chemotherapy can be considered both a physiologic and a psychological stressor. Anxiety, nausea, emesis, and immunosuppression are commonly associated with chemotherapy, and may adversely affect the outcome or the process of treatment. Chemotherapy-related anxiety can exacerbate negative side-effects. Some psychological stressors, such as anxiety, can result in down-regulation of certain immune functions. Studies have demonstrated that behavioral interventions, such as specific relaxation strategies, can modify stress-related immune modulation. These behavioral interventions have also been shown to decrease the degree of nausea and vomiting associated with chemotherapy. การศึกษานี้วัตถุประสงค์ เพื่อประเมินความพึงพอใจของผู้ให้บริการและผู้รับบริการคลินิกเวชปฏิบัติ ครอบครัว โรงพยาบาลศูนย์หาดใหญ่ โดยมีกลุ่มตัวอย่าง ดังนี้ 1) ผู้ป่วยที่มารับบริการ จำนวน 200 คน 2) แพทย์ผู้ให้บริการประจำคลินิกเวชปฏิบัติครอบครัว 2 คน 3) พยาบาลและเจ้าหน้าที่ จำนวน 27 คน 4) แพทย์ทั้งในและนอกคลินิกเวชปฏิบัติครอบครัว 4 คน 5) หัวหน้ากลุ่มงานเวชกรรมสังคม และ 6) ผู้อำนวยการโรงพยาบาลศูนย์หาดใหญ่ การเก็บรวมรวมข้อมูล โดยใช้แบบสอบถามและสัมภาษณ์ เจาะลึก ผลการศึกษาพบว่า 1) ผู้รับบริการส่วนใหญ่เป็นกลุ่มผู้ประกันสุขภาพ มีอาชีพค้าขายและรับจ้าง รายได้เฉลี่ย 6,600 บาทต่อเดือนต่อครอบครัว มีการศึกษาระดับประถมศึกษาตอนต้น มาใช้บริการเฉลี่ย 6 ครั้งต่อปี ผู้รับบริการส่วนใหญ่พักอาศัยอยู่ในชุมชนใกล้เคียงที่ต้องใช้เวลาในการเดินทางมารับบริการเฉลี่ย ประมาณ 18 นาที รวมเวลาที่ใช้ในการมารับบริการที่คลินิกประมาณ 37 นาที และคิดว่าจะยังคงมาใช้บริการ \nที่คลินิกเวชปฏิบัติครอบครัวเพราะมีความพอใจในบริการที่ได้รับและไม่เสียเวลาในการรอรับบริการมากกว่า เหตุผลอื่น 2) โดยเฉลี่ยมีความพึงพอใจในระดับมาก ในเรื่องการประชาสัมพันธ์ห้องรอตรวจ แพทย์ ห้องยา ห้องทำหัตถการ และบรรยากาศทั่วไป แต่ที่พอใจน้อยที่สุดคือ คุณภาพของยาที่ได้รับ 3) พยาบาลและ เจ้าหน้าที่ มีความพึงพอใจต่อการให้บริการและเห็นความสำคัญของการจัดให้มีคลินิกเวชปฏิบัติคู่การดูแล ทั้งชุมชนได้ดี 6) ผู้บริหารระดับกลางและสูงพอใจในระดับหนึ่ง การขยายงานต่อไป คือ การเป็นแม่ขาย ในการดำเนินงาน และเป็นแหล่งความรู้ของนักศึกษาแพทย์ พยาบาล และบุคลากรสาธารณสุข คลินิกเวชปฏิบัติครอบครัวจะยังคงดำเนินนโยบายและให้การสนับสนุนต่อไป และเน้นที่การเป็นแม่ข่าย ในการดำเนินงาน และเป็นแหล่งความรู้

ศึกษาระดับแอลฟา-ฟีโตโปรตีน (AFP) และฮอร์โมนฮิวแมน คลอริโอนิค โกนาโดโทรปิน (HCG) ในสตรีมีครรภ์ที่เข้ารับการตรวจครรภ์ที่โรงพยาบาลสมุทรปราการ โดยทำการเจาะเลือดที่ช่วงอายุครรภ์ \nระหว่าง 14-18, 22-26 และ 30-34 สัปดาห์ ตามลําดับ พบว่าระดับ AFP มีค่า 61.42 ± 41.81 IU/ml (74.32 ± 50.59 ng/ml), 208.54 ± 72.62 IU/ml (252.33 ± 87.87 ng/ml) และ 260.41 ± 96.08 IU/ml (315.10 ± 116.26 ng/ml) ตามลำดับ และระดับฮอร์โมน HCG มีค่า 35,533.35 ± 200000.45 mIU/ml(3,826.59 ± 2153.85 ng/ml), 24,150.62 ±13191.36 mIU/ml (2,600.78 ± 1420.58 ng/ml) และ 351917.35 ± 22697.51 mIU/ml (3,867.94 ± 2444.29 ng/ml) ตามลำดับ ส่วนน้ำหนัก เฉลี่ยของทารกแรกเกิดมีค่า 3,121.74 + 363.64 กรัม

การวิจัยเชิงพรรณนา (Descriptive Research) ครั้งนี้มีวัตถุประสงค์เพื่อ 1) ศึกษาค่าใช้จ่ายของ ผู้บาดเจ็บจากอุบัติเหตุจราจรทางบกที่เข้ารับบริการโรงพยาบาลมหาราชนครศรีธรรมราช 2) เปรียบเทียบ ค่าใช้จ่ายในประเภทผู้บาดเจ็บที่เป็นคนเดินเท้า คนขับขี่ และคนโดยสาร 3) เปรียบเทียบค่าใช้จ่ายของ การบาดเจ็บที่เกิดจากการใช้ยานยนต์ที่ต่างกัน 4) เปรียบเทียบค่าใช้จ่ายในกลุ่มผู้บาดเจ็บที่คาดเข็มขัด นิรภัย และไม่คาดเข็มขัดนิรภัย 5) เปรียบเทียบค่าใช้จ่ายในกลุ่มผู้บาดเจ็บที่สวมหมวกนิรภัยและไม่สวม หมวกนิรภัย 6) เปรียบเทียบค่าใช้จ่ายในกลุ่มผู้บาดเจ็บที่ดื่มแอลกอฮอล์และไม่ดื่มแอลกอฮอล์ 7) เปรียบเทียบ ค่าใช้จ่ายในช่วงเวลาที่ได้รับอุบัติเหตุ กลุ่มตัวอย่าง คือ ผู้บาดเจ็บจากอุบัติเหตุจราจรทางบกที่เข้ารับบริการ รักษาที่โรงพยาบาลมหาราชนครศรีธรรมราช จํานวน 351 ราย โดยการสุ่มตัวอย่างแบบ Cluster Sampling โดยจับสลากผู้บาดเจ็บที่มารับบริการในระยะเวลาหนึ่งเดือน เครื่องมือที่ใช้ในการวิจัยคือ แบบบันทึกการเฝ้าระวังการบาดเจ็บระดับจังหวัด และแบบบันทึกค่ารักษาพยาบาลโรงพยาบาลมหาราช นครศรีธรรมราช วิเคราะห์ข้อมูลโดยหาค่าร้อยละ ค่าเฉลี่ยส่วนเบี่ยงเบนมาตรฐาน ทดสอบความแตกต่าง ของค่าเฉลี่ย โดยการทดสอบค่า ที และการวิเคราะห์ความแปรปรวนทางเดียว \nผลการวิจัยพบว่า ผู้บาดเจ็บที่ไม่นอนโรงพยาบาล จํานวน 251 คน มีค่าใช้จ่ายเฉลี่ย 200 บาทต่อคน ผู้บาดเจ็บที่เป็นคนเดินเท้าจะมีค่าใช้จ่ายมากกว่าผู้ขับขี่และผู้โดยสาร ผู้บาดเจ็บที่การบาดเจ็บเกิดจาก ยานพาหนะอื่น ๆ มีค่าใช้จ่ายสูงกว่าการบาดเจ็บที่เกิดจากรถจักรยานยนต์ ผู้บาดเจ็บที่ดื่มแอลกอฮอล์ มีค่าใช้จ่ายที่สูงกว่า ผู้บาดเจ็บที่ไม่ดื่มแอลกอฮอล์ อย่างมีนัยสำคัญที่ระดับ 0.05 ส่วนค่าใช้จ่ายเฉลี่ย ของผู้บาดเจ็บที่คาดและไม่คาดเข็มขัดนิรภัย, ผู้บาดเจ็บที่สวมและไม่สวมหมวกนิรภัยผู้บาดเจ็บที่เกิดเหตุ ในเวลาที่ต่างกัน ไม่มีความแตกต่างกัน ผู้บาดเจ็บที่นอนโรงพยาบาล จำนวน100 คน มีค่าใช้จ่ายเฉลี่ย 7,196 บาทต่อคน ไม่มีความแตกต่างกัน ในค่าใช้จ่ายเฉลี่ยของผู้บาดเจ็บในกลุ่มของประเภทผู้บาดเจ็บ ประเภทยานยนต์ ผู้บาดเจ็บที่คาดและไม่คาดเข็มขัดนิรภัย, ผู้บาดเจ็บที่สวมและไม่สวมหมวกนิรภัย, ผู้บาดเจ็บที่เกิดเหตุในเวลาที่ต่างกัน

การวิจัยเชิงกึ่งทดลองนี้ มีวัตถุประสงค์เพื่อศึกษาประสิทธิภาพการบันทึกการพยาบาลผ่าตัดโดยใช้ กระบวนการพยาบาล และเพื่อเปรียบเทียบการรับรู้คุณค่าของวิชาชีพการพยาบาลก่อนและหลังการใช้แบบ บันทึก กลุ่มตัวอย่างคือ พยาบาลวิชาชีพและพยาบาลเทคนิคทุกคน ที่ปฏิบัติงานในแผนกผ่าตัด โรงพยาบาลระนอง จำนวน 15 คน และแบบบันทึกการพยาบาลผ่าตัด จำนวน 30 ฉบับ \nเครื่องมือที่ใช้ในการศึกษาคือแบบบันทึกการพยาบาลผ่าตัดโดยใช้กระบวนการพยาบาลพร้อม คู่มือการใช้แบบบันทึก แบบตรวจสอบประสิทธิภาพการบันทึกด้านความครอบคลุมตามกระบวนการบันทึก \nด้านการบริหารความเสี่ยงและด้านความถูกต้องตามกฎหมาย แบบสอบถามประสิทธิภาพการบันทึก ด้านความคล่องตัวในการบันทึก และแบบสอบถามการรับรู้คุณค่าของวิชาชีพการพยาบาล ทุกฉบับผ่าน การตรวจสอบความตรงตามเนื้อหาและความเที่ยง วิเคราะห์ข้อมูลโดยหาค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบน มาตรฐาน และสถิติทดสอบ Wilcoxon signed rank test \nผลการวิจัยพบว่า ประสิทธิภาพการบันทึกของแบบบันทึกการพยาบาลผ่าตัดโดยใช้กระบวนการพยาบาลด้านความครอบคลุมตามกระบวนการบันทึกอยู่ในระดับปานกลาง ด้านการบริหารความเสี่ยงอยู่ใน ระดับดี ด้านความถูกต้องตามกฎหมายอยู่ในระดับดีมาก และด้านความคล่องตัวในการบันทึกอยู่ในระดับมาก และการรับรู้คุณค่าของวิชาชีพการพยาบาลก่อนและหลังการใช้แบบบันทึกทางการพยาบาลผ่าตัดโดยใช้ \nกระบวนการพยาบาลไม่แตกต่างกัน

National surveys to determine trends in contraceptive use in the United States have occurred every 6 to 10 years since 1955.1,2,3 Since that time, there has been a rapid decline in the use of natural methods of family planning; a sharp rise and slow decline in the use of oral contraceptives and, in the past ten years, a major reliance on surgical sterilization for contraception. The purpose of this study was to examine the effect of guilt, degree of spousal support, role conflict, and decreased psychological well-being experienced by educated stay at home mothers with young children. Social role theory is utilized as a basis for the development of a theoretical model. Relevant research literature does not identify a link between guilt, degree of spousal support, role conflict, and decreased psychological well-being in stay at home educated mothers. Risks associated with home visiting have been acknowledged in the nursing literature since the 19th century, yet there is not a well-defined body of literature on this subject. This void in the literature needs to be addressed in view of the current emphasis on practice in the community and the increase in the number of nurses and other health professionals that are new to the field who currently make visits. Swimmers, like all athletes, require a well-balanced diet with adequate energy intake for normal daily activities and the demands of training and competition. Investigators have typically studied nutritional status of athletes during heavy or peak training (1), (2). These studies generally show low levels of energy intake relative to expenditure, and carbohydrate intake also well below recommendations. However, several studies of the effects of exercise on energy intake have shown that as exercise energy expenditure increases or decreases, dietary intake tends to remain the same (3), (4), (5). The growing feminization of the migration of ethnic indigenous women, such as Mixtecs, Zapotecs and Triquis from the southern Mexican state of Oaxaca, are altering the dynamics and configuration of traditional Mexican migration, making it especially significant for the health professions. Informed by a postcolonial ecofeminist paradigm, this bi-national grounded theory study explored the migration and health experiences of indigenous Oaxacan immigrant/migrant women. Data collection using participant observation and interviewing occurred in Oaxaca, Mexico, along the US-Mexico border, and the interior of California. Dimensional analysis was used as the method of data analysis to generate theoretical understandings of these phenomena. …
