This study examined whether students’ attitudes about community health practice, attitudes toward people who are indigent and homeless, and perceived leadership skills changed after participation in a planned interdisciplinary community health experience with an urban homeless or formerly homeless population. Data were collected from medicine, nursing, occupational therapy, physical therapy, and social work students who participated in the community health experiences and from students in these disciplines who did not participate in this curriculum. The Relationship Between Perceived Self-Efficacy, Regimen Adherence, And Glycemic Control In Adolescents With Type 1 Diabetes Mellitus: A Pilot Study, Antonieta Pelias Alteza

This pilot study examined the relationship between perceived self-efficacy and regimen adherence on glycemic control among adolescents, aged 11 to 18 years, with Type 1 Diabetes Mellitus. Thirty-six adolescent-parent dyads participated in this study. Bivariate analysis using Pearson product moment-correlations identified no significant relationships among the variables. Regarding treatment regimen, adolescents on insulin pumps had a mean glycosylated hemoglobin level of 8.09% whereas adolescents on daily insulin injections had a mean glycosylated hemoglobin level of 9.26%. Bandura's theory of self-efficacy provided the theoretical framework for this study. The Self-Efficacy for Diabetes scale and Questionnaire of Self-Care Behaviors scale were used …

When offering a Web-based course, the challenge is to deliver content in a format that enables the student to learn, think critically about the content, and grow as a result of the learning. A 15-year-old girl who came to the emergency department holding tissue to her right eye said that she had been shot. She had right eye pain and swelling below the eye, and she was bleeding. Changes in reimbursement and advances in technology have resulted in a decreased hospital stay for patients undergoing cardiac surgery. These patients are being discharged into the community setting earlier, resulting in an increased demand for transitional specialty home care. This article relates the experience of a Midwest home care agency charged with the development of specialty home care for these complex patients. Transportation arrangements are an integral but fragile element in the effectiveness of adult day care services in Vermont and nationwide. Almost by definition, adult day center participants generally cannot drive due to cognitive and/or physical limitations. Since adult day care services are congregate in nature and serve community-residing elders, this long-term care option is feasible only when there are arrangements to transport elders to and from service centers. Transportation is therefore a major issue for adult day care services.

The purpose of this study is to examine relationships among stress, coping, and years of service in rescue personnel. One hundred sixteen voluntary subjects employed in Fire/EMS service in a small southern city participated in the study. Subjects completed a demographic questionnaire, The Davidson Trauma Scale (DTS), and The Coping Inventory for Stressful Situations (CISS).

The incidence of symptomatic stress for the sample was 19.8 percent, higher than the general population but comparable to rates found in other studies of rescue workers. Calls involving children were rated as most disturbing by the subjects. There was no relationship found between years …

This paper projects the long term consequences of the rise in youth smoking in the 1990s by updating the state estimates for projected smoking-related deaths among youth in the U.S. using information from the Behavioral Risk Factor Surveillance System (BRFSS) 2000 and the U.S. Census 2000. This analysis is similar to that from an earlier study published by the Centers for Disease Control and Prevention (CDC), Office on Smoking and Health (MMWR, 45[44], November 8, 1996). The 1996 analysis used young adult smoking prevalence data from 1994 and 1995; whereas, the analysis presented here represents smoking prevalence data from 2000. …

Though often difficult, ethical decision making is necessary when caring for surgical patients. Perioperative nurses have to recognize ethical dilemmas and be prepared to take action based on the ethical code outlined in the American Nurses Association's (ANA's) Code of Ethics for Nurses with Interpretive Statements. Tennessee's registered nurse vacancy rate is the highest in five years and the nursing shortage has dramatically impacted all portions of the state. The purpose of this study was to describe organizational attributes of exemplar Tennessee hospitals, as perceived by Chief Nurse Executives, which may influence the recruitment and retention of registered nurses.

In this study, 52 Tennessee hospitals were identified as exemplars of quality patient care, organizational policy, and administration. Their Chief Nurse Executive's were surveyed utilizing a Hospital Characteristics Questionnaire and the Organizational Support Subscale from the Nursing Workforce Index Revised (NWI-R). This study examined home-care nurses’ knowledge and perceived preparedness to counsel patients on advance directives (ADs). Joyce Fitzpatrick’s “life perspective rhythm” model provided the framework for the thesis. Sixty-two registered nurses actively practicing in home-care in West Michigan were surveyed over a 4-week period. Each nurse completed a 16-question survey, which included an 11-question true-false test of knowledge of ADs. Survey results indicated that nurses who feel prepared to counsel patients are more knowledgeable than those who do not feel prepared to counsel (t=2.89, df=57, p=<0.05). Overall knowledge was high (87.4%) but only 40 nurses (64%) felt prepared to counsel patients on ADs. The study implies that factors other than knowledge may have an impact on preparedness to counsel. Possibilities for further study include more research on education of healthcare providers about ADs, patient education, and the need for mutual discussions on end-of-life care issues in relationship to completing ADs.

Prenatal maternal attachment and the practice of health promoting behaviors during pregnancy are considered universal phenomena to women. Yet, the understanding of these phenomena from the lived experiences of pregnant women has not been well researched. The purpose of this phenomenological study was to describe the experience of maternal attachment to the unborn child and how that attachment might relate to the practice of these behaviors during pregnancy. In 1999, the Supreme Court ruled that school districts were financially responsible to provide related services including nursing services, to medically fragile students. This decision applied to children attending public schools who have complex health problems and are tracheostomy dependent. The purpose of this grounded theory study was to explore role perceptions of eighteen school nurses caring for these children in classrooms. Six dimensions emerged from the data: Promoting Family/Nurse Connections, Adapting the Environment, Claiming Authority, “Standing Out There”, Striking a Balance and Experiencing Success. Aged widowers are at risk for increased incidence of health problems and higher mortality rates following the death of a spouse. Mens' abilities for dealing with loss have been questioned, but little research has been done with this group. Surviving widowers provided significant insights illuminating the experience of resilience and providing relevant information regarding this population. Utilizing interpretive phenomenology, a methodology advocated by van Manen, in-depth interviews were conducted researching the lived experience of resilience among nineteen elderly widowers between 71 years and 100 years of age.
