Problem/Purpose: Antibiotic associated diarrhea (AAD) and clostridium-difficile diarrhea (CDAD) are the most common forms of infectious diarrhea in long-term care facilities. The purpose of this study was to determine the effectiveness of probiotics in preventing AAD and CDAD in the long term care geriatric population, and to identify interventions that can be used to improve clinical practice. Background/Significance: Prophylactic use of probiotics have been purported to decrease the incidences of AAD and CDAD. Conventional approaches to cancer treatment typically do not address the personal experiences of the women with breast cancer. The aim of this study was to develop a structural description of the lived experience of women with breast cancer who create art, specifically through painting, sketching or drawing. A phenomenological approach was employed to explore the meaning of creating art for four women with a diagnosis of breast cancer. The philosophical underpinnings for this study were based on the phenomenological method of bracketing which allows the researcher and subjects to focus on lived experience. Caring for parents whose infant has died is extremely demanding, difficult, and stressful. In some situations, nurses may experience personal failure, feel helpless, and need to distance themselves from bereaved parents because they are unable to deal with the enormity of the parental feelings of loss. The aim of the study was to describe and compare attitudes toward perinatal bereavement care across a sample of nurses working in five obstetrics and gynecology settings from three Asian cities, as well as the factors associated with these attitudes. Background: Accurate prognosis is vital to the initiation of advance care planning particularly in a vulnerable, at risk population such as care home residents. The aim of this paper is to report on the revision and simplification of the MDS Mortality Rating Index (MMRI) for use in clinical practice to predict the probability of death in six months for care home residents.
Methods: The design was a secondary analysis of a US Minimum Data Set (MDS) for long term care residents using regression analysis to identify predictors of mortality within six months.

Nursing education is a complex combination of classroom and clinical instruction. The clinical portion is usually divided into the pre-clinical conference, direct care, and the post-clinical conference. Many goals are listed in the literature as learning outcomes for the post-clinical conference. The purpose of this study was to describe the most important goals, meaning, rationale, and preparation for the post-clinical conference by clinical nursing instructors in associate degree and baccalaureate degree programs.

Significance: Nurses facing impediments to what they perceive as moral practice may experience inner turmoil, frustration, and moral distress. These scenarios may culminate in resignation from employment at the individual level, increased attrition at the systems level, and poor patient outcomes, including increased morbidity and mortality.
Purpose: To explore the relationships between moral distress and avoidance thoughts and behavior between nurses currently practicing in critical care and non-critical care settings.

Background: Depressive symptomology is an independent risk factor for cardiovascular disease. Over 15% of persons with cardiovascular disease have depressive symptoms, and women are twice as likely to experience these symptoms as men. Depressive symptoms in women “ at risk ” for cardiovascular disease has not been well studied.
Purpose: This study investigated the relationship between depressive symptoms, health-promoting lifestyle behaviors, heart disease risk awareness, cardiac risk, and quality of life in women at risk for cardiovascular disease. This study sought to bridge the gap of nurses' knowledge of pain management and translation into clinical practice by describing pediatric nurses' negotiation of the bureaucracy of caring. The purpose of this study was to describe factors that influenced pediatric nurses' actions to relieve children's acute post-operative pain on a pediatric post-surgical care unit. The bureaucracy of caring provided the theoretical framework. This double blinded placebo controlled pilot study examined the use of combined inhaled medication for patients with moderate-to-severe COPD. The inhalation of formoterol versus placebo was studied when combined with the routine use of tiotropium. Maximizing bronchodilitation through the use of combination inhaler therapy is felt to be superior to the use of a single inhaled medication, yet few studies are found in the literature. Currently the Global Initiative for Obstructive Lung Disease (GOLD) recommends the use of combination inhaled therapy in patients with moderate-to-severe COPD. The aim of this study was to explore the experiences of Muslim Afghan refugee children living in Canada. War has an impact on a child's life, including education and protection by the family and community. The actual course of events of war can never be told completely due to the occurrence of simultaneous adversities. Hermeneutic photography was used to understand the experiences of Afghan children. Unintentional injuries are the leading cause of death for children under 14 years of age. Of the unintentional injuries, a significant portion occurs within the home setting. Creating a safe home environment for children has become a focus in current efforts for health promotion and injury prevention. Our objective is to enhance caregivers’ knowledge of home safety thus decreasing the incidence of childhood unintentional injuries. The National Association of School Nurses advocates for body mass index (BMI) screening. Little research describes school nurse practice of BMI screening. In this descriptive study, 25 Ohio school nurses participated in three focus groups. An adapted Healthy People 2010 Determinants of Health Model guided the research questions. School nurses engaged in multiphasic data collection which was contingent on physical environment, workload, and referrals. Lack of system policy was key barrier in terms of data collection, referral, and follow-up. A key facilitating factor was physical education teachers in terms of reinforcing health. The Clinical Nurse Leader (CNL) is a master-prepared generalist accountable for patient outcomes through application of evidenced-based practice at the microsystem level. Accelerated nursing programs are educating entry level nurses as CNLs in a novel Model C program. This doctoral project evaluates entry-level master’s CNL graduates with the CNL end-of-program competencies to determine whether these graduates are able to have positive effects on patient, systems, and leadership outcomes in clinical settings.

Today's nurses work and live in a multicultural society where they encounter patients whose backgrounds are different from theirs, and who need care from nurses who are both proficient in their work and knowledgeable about the role that culture plays in patient treatment. In this study, 45 student nurses enrolled in a baccalaureate program at a northeastern urban college completed a survey about their perceptions of care and culture including their relevance and application to the practice of nursing. The purpose of this study is to determine the dependability and accuracy of Clinical Performance in Nursing Examination (CPNE®) scores obtained from trained raters. Intraclass correlations were calculated to estimate interrater agreement. Accuracy of scores was determined by identifying percent accuracy for each scored element. Generalizability (G) coefficients were calculated to estimate sources of variance in CPNE scores. The population for the study is adjunct faculty at Excelsior College who agreed to participate in the study and are clinical examiners trained as raters for the CPNE

Background: Inadequate chronic pain control has far-reaching implications including costs for missed time at work; utilization of health care resources and reduced quality of life. People at highest risk for inadequate pain control include indigent adults and minorities. Newman's theory health as expanding consciousness views the chronic pain experience as a life pattern within health. A descriptive phenomenological study was proposed to obtain a subjective description of the experience of living with intravenous prostaglandin treatment for pulmonary arterial hypertension. It was intended to determine what kinds of elements are common to the experience with study participants, to develop an aggregate structure of the experience from the individual descriptions of it, and add to what is now known about this phenomenon. It was anticipated that the stories told by the participants could help to determine how nurses can assist future patients to live optimally within the confines of a palliative treatment for this incurable illness.

The CDC estimates that one in 20 patients admitted to the hospital is a carrier of methicillin-resistant Staphylococcus aureus (MRSA). Staphylococci are commonly found on the skin and mucous membranes within the anterior nares, which provides the principle reservoir for this organism. These organisms can go on to cause surgical site infections in hospitalized patients. Mupirocin is an effective topical medication used to eliminate nasal carriage of Staphylococcus aureus (S. aureus). This study examined the rates and risks of premature neonates >2500grams developing intraoperative non-euglycemia events (IONEE). A retrospective chart review of 26 premature neonates >2500 grams who underwent surgical procedures between January 1 and December 31, 2009 was conducted. Statistical analysis was done using Chi square and t-tests. Ten of the 26 subjects (38%) experienced an IONEE. Hyperglycemia was the primary IONEE that was noted in the neonates.
