Aim: This paper reports a study to describe and interpret the meaning of being on long-term mechanical ventilation.

Background: Patients who require mechanical ventilation in a critical care unit for prolonged periods of time are typically sicker than those who are ventilated for shorter periods. Despite advances in treatment modalities for critically ill patients, many still require long-term mechanical ventilation for 7 days or more. Therefore, caring for a long-term ventilated patient is often an everyday occurrence for critical care nurses; however, there is insufficient evidence of the meaning of this experience from a patient perspective.

The Chronic Illness Quality of Life Ladder (CIQOLL) underwent psychometric testing in a sample of 278 women with HIV disease. The CIQOLL, a self-anchoring striving scale based on Cantril’s Ladder, measures seven domains (physical , emotional, financial, family and friends, spiritual well-being, peace of mind, and overall life satisfaction) across four time periods (present, past, future, life without a diagnosis of HIV). The domains were derived from focus groups with persons with HIV disease. Women with a diagnosis of HIV Infection, age 18 or older, residing in rural areas in the southeastern United States, completed questionnaires that measured physical functioning, …

There is the need for critical changes in the United States health care system to meet the rising costs of health care and the massive number of people without health insurance. The Berea Health Ministry Rural Health Clinic (BHMRHC) is a nurse-managed, rural primary health care clinic that was designed to respond to the health care needs of the underserved, uninsured, and underinsured populations in a designated nine county area of Appalachian Kentucky. The process used in the development and implementation of BHMRHC is described as a model to assist in the future development of similar clinics.

School Connectedness, Anger Behaviors, And Relationships Of Violent And Nonviolent Youth, Sandra Thomas, H. Smith

PROBLEM: Youth violence research often focuses on risk factors arising from early familial interactions rather than school-related factors. METHODS: Via an Internet questionnaire, 282 girls and boys (ages 7-19, mean 15.3) from 47 states and Washington, DC, reported on school connectedness, interpersonal relationships, and anger behaviors. FINDINGS: Substantial percentages of violent youth did not perceive themselves to be liked by classmates and reported loneliness. If not liked by classmates, 80% hated school. Likers and haters of school differed on seven variables (all p < or = .01). CONCLUSIONS: Insufficient attention is paid to the alienation experienced by disliked and lonely students. Mental health nurses could play a pivotal role in fostering change in the social climate of schools and helping youth to achieve better anger management and social skills.

A secondary meta-analysis of programs to reduce falls in the elderly is undertaken to demonstrate a Bayesian analysis. The Bayesian statistical tradition is carefully distinguished from the standard Neyman-Pearson-Wald (NPW) statistical tradition. In the 12 studies, the logit effect size is used to compare treatment groups using a prevention program to control groups without a program. To contrast the Bayesian analysis, independent-effects and fixed-effect meta-analyses are first conducted in the NPW tradition. This is followed by Bayesian independent-effects and fixed-effect meta-analyses that numerically replicate the NPW results but have conceptually different interpretations. Lemierre's syndrome (LS) typically occurs in previously healthy young adolescents and young adults who become acutely ill following an attack of pharyngotonsillitis. Also known as post anginal sepsis, those afflicted develop pyrexia, rigours and multiple metastatic abscesses that lead to septic thrombophlebitis of the internal jugular vein. In the pre-antibiotic era this particularly virulent syndrome had a mortality rate in excess of 90%, but since the introduction of antibiotics and the widespread treatment of throat infections, it has became almost unknown. This study examined how women’s relationship with their primary health care provider (PHP) and their perceptions about how effective their PHPs believe zidovudine (AZT) to be in decreasing perinatal transmission related to women’s AZT beliefs and intentions. It used a cross-sectional design to collect data from 59 HIV-infected African American women. Almost half the women (45%) had given birth since HIV diagnosis. Most of the babies born to HIV-infected mothers (87%) were seronegative. Data analysis with Pearson’s r indicated that the quality of the women’s relationship with their PHP was positively correlated to how important the PHP would be in …

The purposes of this study were threefold: (a) to describe self-care agency, (b) to describe self-care practice, and (c) to describe the relationship between self-care agency and self-care practice in adolescents. The usefulness of Orem's Self-Care Deficit Nursing Theory (SCDNT) with adolescents also was evaluated. Participants in this study included 173 adolescents, 14 to 19 years of age. Data were collected in classroom settings in two diverse high schools. Denyes Self-Care Agency Instrument-90 and Denyes Self-Care Practice Instrument-90 were administered. Interviewing was found to be an excellent teaching strategy in the promotion of cultural sensitivity in the course described in this article. Students were active learners in the acquisition of knowledge and critical thinkers in the analysis of the knowledge. Leininger (1991) emphasized the need for cultural sensitivity and cultural specific care in assisting supporting, facilitating, and/or enabling "individuals or groups to maintain or regain their well being (or health) in culturally meaningful and beneficial ways, or to help people face handicaps or death" (p.47).
