Appalachia is one of the most medically underserved areas in the nation. The region has provider shortages and limited healthcare infrastructure. Children and adolescents in this area are in poor health and do not receive the needed quality care. Implementation of section 2302 of the 2010 Patient Protection and Affordable Care Act (ACA) enabled children enrolled in Medicaid/Children's Health Insurance Program with a terminal illness to use hospice care while continuing treatment for their terminal illness. In addition to being more comprehensive than standard hospice care, this relatively new type of care is more culturally congruent with the end-of-life values …

Although concurrent hospice care became available more than a decade ago, little is known about the socio-demographic and health characteristics of children who received concurrent care; health care services they received while enrolled in concurrent care, their continuity, management, intensity, fragmentation; and the costs of care. “It’S Killing Us!” Narratives Of Black Adults About Microaggression Experiences And Related Health Stress, Joanne M. Hall, Becky Fields

Perceived racism contributes to persistent health stress leading to health disparities. African American/Black persons (BPs) believe subtle, rather than overt, interpersonal racism is increasing. Sue and colleagues describe interpersonal racism as racial microaggressions: “routine” marginalizing indignities by White persons (WPs) toward BPs that contribute to health stress. In this narrative, exploratory study, Black adults (n= 10) were asked about specific racial microaggressions; they all experienced multiple types. Categorical and narrative analysis captured interpretations, strategies, and health stress attributions. Six iconic narratives contextualized health stress responses. Diverse mental and physical symptoms were attributed to racial microaggressions. Few strategies in …

Persons with schizophrenia spectrum disorders (SSDs) face multiple health barriers that are improved with exercise, but they seldom exercise due to a variety of barriers, of which amotivation is primary. In the current study, we investigated the subjective experiences of persons with SSDs following a formal exercise program. Twenty seven persons with SSDs who had taken part in a 16-week walking program provided feedback during exit interviews. Our purpose was to ascertain whether comments provided evidence of development of intrinsic motivation as defined by Ryan and Deci’s self determination of behavior theory. This Grounded Theory study describes the process by which older persons “become” volunteers. Forty interviews of older persons who volunteered for Habitat for Humanity were subjected to secondary content analysis to uncover the process of “becoming” a volunteer. “Helping out” (core category) for older volunteers occurs within the context of “continuity”, “commitment” and “connection” which providemotivation for volunteering.When a need arises, older volunteers “help out” physically and financially as health and resources permit. Benefits described as “blessings” of volunteering become motivators for future volunteering. Findings suggest that older volunteering is a developmental process and learned behavior which should be fostered …

Specifically, the study examined the extent to which manuscript reviews provided constructive guidance for authors to further develop their work for publication, and for editors to make informed and sound decisions on the disposition of manuscripts.

Leininger developed the ethnonursing research method to study transcultural human care phenomena using her theory of culture care diversity and universality. The ethnonursing research methodology which uses an open, largely inductive process of discovery to document, describe, understand, and interpret people’s meanings and experiences will be presented. The ethnonursing researcher functions as co-participant with informants working together to discover how people experience and practice care in their daily lives. Systematic and reflective processes are used while focusing on the cultural context to explicate lifeways and understand their meaning for informants. Horizontal violence is a form of workplace violence, a phenomenon that is prevalent in the nursing profession. Research has revealed a variety of negative peer-to-peer behaviors that lower morale and lead to turnover. However, little research has been conducted on “eating our young” (violence occurring between individuals with unequal power, such as staff nurse and student). We propose “vertical violence” as the appropriate term when abusive registered nurse (RN) behavior is directed towards students. Mental health patients describe “being understood” as an experience that evokes feelings of importance, worthiness, and empowerment. However, the experience of “being misunderstood” is more prevalent in patients’ relationships with health care providers. Negative consequences such as vulnerability, dehumanization, and frustration reveal that being misunderstood has the potential to damage or destroy therapeutic relationships.

Our aim in this study was to explore HIV/AIDS peer counseling from the perspective of women actively engaged in this work within the context of a community-based program in rural areas of the southeastern United States. Based on this research we suggest that the embodied work of HIV/AIDS peer counselors is constructed around their personal identities and experiences. This work involves gaining entry to other HIV-positive women’s lives, building relationships, drawing on personal experiences, facing issues of fear and stigma, tailoring peer counseling for diversity, balancing risks and benefits, and terminating relationships. Persons with schizophrenia spectrum disorders (SSDs) are not only at risk because of disabling disease symptoms but because necessary medications create health risks associated with high rates of obesity. Despite the well-known benefits of exercise, persons with SSDs rarely adhere to such regimens; few interventions to motivate exercise behavior have been tested in this group.

The concept of the therapeutic milieu was developed when patients’ hospitalizations were long, medications were few, and oneto- one nurse–patient interactions were the norm. However, it is not clear how the notion of ‘therapeutic milieu’ is experienced in American acute psychiatric environments today. This phenomenological study explored the experience of patients and nurses in an acute care psychiatric unit in the USA, by asking them, ‘What stands out to you about this psychiatric hospital environment?’ Three figural themes emerged, contextualized by time, which was a source of stress to both groups: for patients there was boredom, and for nurses, pressure …

A narrative study of thriving adult female survivors of childhood maltreatment was undertaken to discover how they had achieved success. Life trajectory patterns, turning points, and setbacks were identified. Data consisted of 81 interview transcripts derived from a series of three interviews, spaced over 6 to 12 months, with 27 survivors. The childhood abuse was intrafamilial, beginning as early as infancy and continuing, in most cases, until participants left home. The onset and pace of the healing trajectory were quite variable, including a roller-coaster pattern as well as patterns of slow, steady progress and continued struggle. Four types of redemption …

The purpose of this study was to describe mental health service recipients' experience of the therapeutic relationship. The research question was ‘what is therapeutic about the therapeutic relationship?’ This study was a secondary analysis of qualitative interviews conducted with persons with mental illness as part of a study of the experience of being understood. This secondary analysis used data from 20 interviews with community-dwelling adults with mental illness, who were asked to talk about the experience of being understood by a health-care provider. Recent rampage killings compel greater attention to anger and violence on the college campus. In each of these tragic mass murders, vengeful individuals sought to address perceived grievances against faculty and/or other employees of the university. In each of these situations, numerous clues of impending violence were evident. Sadly, however, in each of these cases the schools failed to take preventive actions. While prediction of violent behavior will never be an exact science, universities must begin to enact violence prevention strategies. Maintaining an attitude that 'this couldn't happen here' hampers the necessary education of faculty, staff, and security personnel.
