Turkish Journal of Medical Sciences

Background/aim: This study aims to investigate the diagnostic utility of new erythrocytes, leukocytes, and reticulocytes parameters for the identification of subclinical iron deficiency (ID) in children under 6 years with nonsevere acute infection in pediatric outpatients. Materials and methods: The study included 102 children with acute infections and 31 true ID. Traditional and new hematology parameters were measured in a Sysmex-XN® , along with C-reactive protein level, and iron parameters. Participants? ID were categorized as: the ferritin < 100 ng/mL, transferrin saturation < 20% was defined as ?subclinical or functional ID (FID) in Group 1?; ferritin < 30 ng/mL, transferrin saturation < 20%, as ?absolute-ID (AID)? in Group 2; ferritin < 12 ng/mL without anemia and infection, as ?true ID? in Group 3. Results: The frequencies of FID and AID among the 102 children with acute infection were 24% and 76%, respectively. Compared with the Group 2 patients, Group 1 had a significantly higher mean percentage of hypochromic erythrocytes (Hypo-He), and significantly lower levels of hemoglobin (Hb) and Hb content of reticulocytes (RET-He) (p < 0.05 for all). Compared with Group 2 and Group 3 patients, Group 1 had a significantly higher mean percentage of immature reticulocyte fraction (IRF) and immature granulocyte (IG) values (p < 0.05 for all). The RET-He, IRF%, Hypo-He%, and IG% cut-off values for predicting FID during infection were 27.0 pg, 10.6%, 2.5%, and 0.35% respectively. Conclusion: The RET-He, Hypo-He, IRF, and IG may be useful parameters for identifying subclinical ID in small children with nonsevere acute infection in pediatric outpatients.

Peripheral Arterial Disease Increases The Risk Of Multidrug-Resistant Bacteria Andamputation In Diabetic Foot Infections, Pinar Aysert Yildiz, Tuğba Özdi̇l, Murat Di̇zbay, Özlem Güzel Tunçcan, Kenan Hizel

Turkish Journal of Medical Sciences

Background/aim: The aim of this study was to investigate the microbiological profile and resistance rates of diabetic foot infections (DFIs) and to determine the effect of peripheral arterial disease (PAD) on the microbiology, clinical condition, and treatment outcomes. Materials and methods: Characteristics, laboratory and imaging data, and the treatment modalities of patients admitted to our hospital with a diagnosis of DFI (PEDIS classification 3-4) during 2005-2016 were analyzed according to the presence of PAD. Results: Of 112 patients who were included in this study, 86 (76.8%) had PAD. Patients with PAD were older and had higher amputation rates (P < 0.05). A microbiological profile of patients revealed a predominance of gram-positive bacteria (57.1%). Staphylococcus aureus and Streptococcus spp. were the most frequently encountered bacteria. Incidence of Pseudomonas spp. infection was higher in the PAD group (P < 0.05). Of all patients, 24.1% had multidrug-resistant (MDR) microorganisms in their wound cultures. Presence of MDR bacteria in patients with PAD was 4.9-fold higher than that in patients without PAD (P < 0.05). Conclusion: This retrospective study indicates that PAD has a significant role, especially in elderly patients with DFIs. Patients should be promptly evaluated and treated for PAD to prevent infections with resistant microorganisms and limb loss.

The Type And Frequency Of Infections Occurring In Collagen Tissue Diseases, Refi̇k Ali̇ Sari, Mehmet Şahi̇n, Göksal Keski̇n, İlhami̇ Ki̇ki̇, Murat Turgay

Turkish Journal of Medical Sciences

Corticosteroids, widely used in the treatment of collagen tissue diseases, increase the number of neutrophils and decrease the total numbers of lymphocytes, monocytes, eosinophils and basophils. In addition, corticosteroids alter important functional activities of both lymphocytes and monocytes. They suppress the bactericidial activities of phagocytes. In this manner, the resistance to infections is reduced. Cytotoxic drugs can have profound effects on the production and function of both phagocyte cells and lymphocytes. We followed up 115 patients with collagen tissue disease for two years and 22 of them had infections. Eight of these 22 patients had urinary tract infection, six had …
