We aimed to compare drug utilization in hypertensive old patients and very old patients in primary care, along with the evaluation of potentially inappropriate drug prescribing. Materials and methods: In this cross-sectional study, we compared prescriptions of 65-79-year-old (old patient prescriptions [OPP], n = 433,988) vs. ≥80-year-old (very old patient prescriptions [VOPP], n = 134,079) with "essential hypertension" diagnosis, issued by 3:1 systematically-sampled primary care physicians (n = 1431) in İstanbul throughout 2016. Drug utilization patterns and distribution of antihypertensives based on drug …

The Relationship Between Morning Blood Pressure Surge And Asymptomatic Episodes Of Paroxysmal Atrial Fibrillation In Patients With Systemic Arterial Hypertension, Ümmü Taş, Sedat Taş, İdi̇l Yavuz

Turkish Journal of Medical Sciences

Background/aim: Hypertension is a known risk factor for developing atrial fibrillation. However, there is limited data to investigate the association between morning blood pressure surge (MBPS) and paroxysmal atrial fibrillation (PAF). We conducted the present study to determine whether there is a relationship between asymptomatic PAF and MBPS and whether MBPS may be a predictor of asymptomatic PAF episodes. Materials and methods: This prospective study comprised 264 adult patients who were newly diagnosed with essential hypertension or were previously diagnosed but not receiving regular antihypertensive therapy. We evaluated the patients in 2 groups according to their 24-h electrocardiography monitoring results: …

Background/aim: The relationship between the activation of the renin-angiotensin system and the increase in erythropoiesis has been shown in many studies. In addition, the use of angiotensin converting enzyme inhibitors (ACEIs) or angiotensin-receptor blockers (ARB) has been reported to reduce hemoglobin levels in various patient groups at risk for secondary erythrocytosis/polycythemia. The aim of our study is to investigate whether there is a change in hemoglobin levels after starting ACEIs or ARBs in patients who have not used them before. Materials and methods: Three hundred and fifty-one patients who were started on renin angiotensin aldosterone system (RAAS) blockers were evaluated …

Turkish Journal of Medical Sciences

Aim: Studies about relationships between diabetes mellitus, diabetic complications, and cognitive functions have been initiative factors for this study. We evaluated various relations between procedure of the disease, diabetic complications, and cognitive functions among diabetic subjects. Materials and methods: Type-2 diabetic patients were evaluated for their disease status, complications, and cognitive functions (by Mini Mental State Examination (MMSE) test). We compared MMSE results of the patients with the control group. Results: We evaluated 75 patients with type-2 diabetes mellitus and 49 control subjects. Orientation (P = 0.006), attention and calculation (P = 0.002), and total (P < 0.001) scores of the control group were significantly greater compared to the diabetic group. We observed negative correlation between the length of disease and cognitive functions within the diabetic group. Recall scores of the diabetic subjects with 6 and less HBAIC levels were significantly greater than those with 6.1 and greater levels (P = 0.049). We observed negative relations between registration scores and retinopathy, but not between orientation and total scores and hypertension among diabetic subjects. Conclusion: Type-2 diabetes mellitus may destroy cognitive function. Length of disease, high serum HBAIC levels, hypertension, and retinopathy are significant additional risk factors. We conclude that cognitive function assessment should be routine procedure in the management of type-2 diabetes mellitus.

Effects Of Ramadan Fasting On Stroke, Selçuk Çomoğlu, Ahmet Temi̇zhan, Emel Peşi̇nci̇, İzzet Tandoğan, Şenay Özbakir

Turkish Journal of Medical Sciences

The effects of fasting on humans have not been adequately investigated. Ramadan fasting is a great opportunity for scientific research due to its peculiar nature. With this in mind, we conducted research on the effects of Ramadan on stroke, which is one of the most common causes of death in adults. Patients hospitalized solely for ischemic stroke and intracerebral hemorrhage were evaluated retrospectively. The ratio of these cases to other patients hospitalized in the neurology department was determined. The in-hospital clinical course and mortality rates of the patients with stroke were evaluated. Within the entire cohort, the most frequently encountered …

A Cell Protective Mechanism In A Murine Model Of Parkinson's Disease, Emi̇n Öztaş, Turgut Topal Jan 2003

A Cell Protective Mechanism In A Murine Model Of Parkinson's Disease, Emi̇n Öztaş, Turgut Topal

Turkish Journal of Medical Sciences

Parkinson's disease is characterized by progressive degeneration of the dopaminergic (DA) neurons in the substantia nigra (SN). However, the mechanism underlying DA cell death remains unclear. While apoptotic cell death has been implicated in DA cell death in Parkinson's disease, autophagy - a regulated cellular process for degrading intracellular proteins and organelles under stress - in Parkinson's disease is not known. Here, we report evidence of autophagy in DA neurons of the SN in a 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) model of Parkinson's disease. Mice were treated with saline or MPTP (40 mg/kg) and sacrificed after 7 days, and brain tissue samples were …

A total of 57 patients with thalamic hemorrhage, whose diagnosis was confirmed by cranial computerized tomography, were evaluated. The most significant risk factor was hypertension, while the most common initial clinic symptom was incontinence. Those patients who had small hemorrhagic size of low volume and the presence of ventricular …

The cause of reduced renal mass (RRM)-saline hypertension is not clear. In our study, it was aimed to investigate the role of vasopressin (AVP) in this type of hypertension by using selective V 1 antagonist of AVP. RRM was performed in 32 normotensive male Wistar rats under anesthesia in which we used a mixture of ketamine (130 mg/kg, i.m.) and chlorpromazine (1.3 mg/kg, i.m.). After a one-week of recovery period, all of the rats were given a low sodium diet and either distilled water (group 1) or 1% NaCl (group 2) as drinking water for 4 weeks. Five weeks after …
