Us Guided Management Of Undifferentiated Dyspneic Patient In The Ed, Kaleigh Miller

Longitudinal Scholar's Project

Intro: Undifferentiated dyspnea can be a complicated presentation muddled by patient comorbidities and similar symptomology shared among etiologies. Some studies have shown increased mortality and length of stay in the hospital when incorrectly initially diagnosed in the ED. US has been shown more effective at differentiating these causes and improves diagnostic accuracy. This study will implement US exam upon initial exam of patient and chart time to diagnosis/treatment, length of stay in ED, length of stay in hospital admissions versus discharge rates, and 30 day mortality. ADHF and COPD/asthma patient differentiation will be the focus.

Methods: Prospective cohort study of …

Scheduling Like A Pro, Joseph Gibney

Emergency Medicine

Scheduling is difficult, but believe it or not, it can also be rewarding. This is one chief’s take on three years of scheduling and the highs, lows, do’s, don’ts, and novel ways to help your fellow residents live their best residency life.

  • Show that resident wellness is impacted by scheduling
  • Demonstrate our scheduling process, some errors we made over time, and how we learned from them
  • Discuss some novel scheduling tactics, how other residencies may apply what we’ve learned, and how to adapt it to fit their needs

Teaching "Shock Pathophysiology" By Flipped Classroom: Views And Perspectives, Rehana Rehman, Satwat Hashmi, Rozmeen Akbar, Syeda Sadia Fatima

Department of Biological & Biomedical Sciences

Flipped classroom (FCR) is an active learning pedagogical method in which the students prepare prior to class using different modalities, for example, reading materials and videos, and afterward spend the time in class discussing the content and reinforcing the concepts. We chose to replace one problem-based case on "Shock" with flipped-style teaching in the respiration circulation module of a private medical university. Our objective was to use the clinical presentation of "Shock" to open a window to interrelate basic science concepts of cardiovascular physiology and pathology. It aimed to merge the case-based discussion with small-group discussions in the form of …

Family Medicine In Tanzania: Seize The Moment, Riaz Ratansi, Bernhard M. Gaede

Faculty of Health Sciences, East Africa

In the context of addressing the pressing health needs for the global population, the World Health Organization has repeatedly called for universal health coverage (UHC) to be prioritised by its member countries. This is to be achieved through a high-quality primary health care (PHC) approach that provides comprehensive and integrated generalist care as close to where people live as well as links the clinical care to health promotion and disease prevention. In this paper, we argue for the introduction of family medicines as a critical player in the healthcare system of Tanzania to strengthen the strategies towards UHC. The paper …

Remediation of struggling learners is a challenge faced by all educators. In recognition of this reality, and in light of contemporary challenges facing infectious diseases (ID) fellowship program directors, the Infectious Diseases Society of America Training Program Directors' Committee focused the 2018 National Fellowship Program Directors' Meeting at IDWeek on "Remediation of the Struggling Fellow." Small group discussions addressed 7 core topics, including feedback and evaluations, performance management and remediation, knowledge deficits, fellow well-being, efficiency and time management, teaching skills, and career development. This manuscript synthesizes those discussions around a competency-based framework to provide program directors and other educators with …

Assessing For Gender Disparities In The Selection Of Chief Residents Of Emergency Medicine Residency Programs, Emilio G. Volz, Justin Steinberg, Tony Zitek

HCA Healthcare Journal of Medicine

Introduction: Although the number of women physicians has been increasing, there may be gender disparities in the assessment of female emergency medicine residents. This study sought to determine if female emergency medicine residents are less likely to become chief residents than males.

Methods: In July 2017, an anonymous survey was distributed to the program coordinators of all accredited emergency medicine residency programs in the United States. The survey requested the number of males and females in each graduating class from 2015 to 2017. The percentage of female residents who were chief residents was calculated and compared to that for males. …

The Great Imitator Strikes Again: A Case Of A Lupus Flare-Up Presenting Like An Acute Abdomen, Emmanouil Kiriakopoulos, Vanessa Perez, Robyn Hoelle

HCA Healthcare Journal of Medicine

Systemic lupus erythematosus (SLE) is an autoimmune disease affecting all age groups and can manifest in various forms, often making the initial or successive presentations difficult to diagnose. Peritonitis secondary to lupus is a rare manifestation of this disease and the prevalence is said to be much lower in children. We present a case report of an adolescent male with a known history of lupus who presented to the emergency department with a clinical picture consistent with an acute surgical abdomen and underwent an appendectomy. Subsequent workup identified the culprit as a lupus-related peritonitis requiring corticosteroids for resolution.

A Shift In Reality: Virtual And Augmented Systems In Higher And Medical Education, Brian Meyer Feb 2020

A Shift In Reality: Virtual And Augmented Systems In Higher And Medical Education, Brian Meyer

Current Issues in Emerging eLearning

Virtual and augmented technologies provide a seamless solution for merging traditional, theoretical learning with practical application in context. Unlike traditional teaching pedagogies, in which lessons are restricted in terms of the use of additional apparatus, pedagogies that involve the use of virtual and augmented reality technologies enable educators to build upon taught concepts to demonstrate the application of those concepts in practice, and allow educators to generate multiple atypical scenarios in order to build competence in practical fields of endeavour. In medical education, virtual and augmented reality tools provide an especially important opportunity for preparation before treating patients in actual …

The Role Of Mindset, Impostorism, And Irrational Beliefs In Resident Wellness: Results From The Hca Nationwide Longitudinal Resident Wellness Study, Gregory Guldner, Jason Siegel, Brendon Ellis, Anne Brafford

Medical Education

Background The significant proportion of residents reporting poor levels of well-being has appropriately fueled a call for interventions. Yet the specific constructs that impact well-being have not been well researched and validated in the resident population. Mindset (fixed versus growth), impostorism (the belief that one’s success is fraudulent) and irrational beliefs (dysfunctional beliefs that contribute to emotional pain and anguish) are among individual variables postulated to relate to well-being in a high-pressure educational environment. Each of these constructs is amenable to interventions should they have causal relationships to well-being.

Objectives Our prior research resulted in a statistically validated model of …

Department of Psychiatry and Human Behavior Faculty Papers

CONTEXT: Research on associations between medical student empathy and demographics, academic background, and career interest is limited in number, lacks representative samples, and suffers from single institutional feature. This study was designed to fill the gap by examining associations between empathy in patient care, and gender, age, race/ethnicity, academic background, and career interest in nationwide, multi-institutional samples of medical students in the United States and to provide more definitive answers to the aforementioned associations with more confidence in the internal and external validity of the findings.

METHODS: Four nationwide samples participated in this study (n=10,751). Samples 1, 2, 3, and …

Department of Pharmacology and Experimental Therapeutics Faculty Papers

BACKGROUND: Diagnostic uncertainty occurs frequently in emergency medical care, with more than one-third of patients leaving the emergency department (ED) without a clear diagnosis. Despite this frequency, ED providers are not adequately trained on how to discuss diagnostic uncertainty with these patients, who often leave the ED confused and concerned. To address this training need, we developed the Uncertainty Communication Education Module (UCEM) to teach physicians how to discuss diagnostic uncertainty. The purpose of the study is to evaluate the effectiveness of the UCEM in improving physician communications.

METHODS: The trial is a multicenter, two-arm randomized controlled trial designed to …

Medical Faculty Engagement In Curricular Revisions: An Inquiry Into Individual And Organizational Factors That Support Participation, Tomoko Tanikawa Feb 2020

Medical Faculty Engagement In Curricular Revisions: An Inquiry Into Individual And Organizational Factors That Support Participation, Tomoko Tanikawa

Dissertations and Theses

In efforts to facilitate the education of highly qualified and best-prepared medical professionals, many medical schools are either planning for or engaging in curriculum revisions including a complete overhaul of the curricular structure, academic and medical content, and pedagogical delivery modalities. To be effective, such changes require faculty buy-in, participation, engagement, and innovation. Unfortunately, despite medical schools' efforts to support faculty involvement in curricular revision, a range of reactions exist, including resistance. Thus, understanding the interactions between individual faculty attitudes and behaviors toward organizational initiatives and structural support is critical in advancing the educational mission of medical schools. Based on …
