No Longer Beholden – Moving On From Elaborate Chart Notes, Jeffrey H. Millstein Oct 2022

No Longer Beholden – Moving On From Elaborate Chart Notes, Jeffrey H. Millstein

Journal of Wellness

Despite recent changes in Medicare billing documentation guidelines, physician chart notes remain far lengthier and more detailed than is required. This is worrisome, as documentation burden is a major contributor to physician burnout. Elements of medical culture which have contributed to the elaborate chart note standard may be used to transform notes, reduce charting burden and improve physician well-being.

A Moment Of Reflection As We Move Forward, Bruce Morgenstern Oct 2022

A Moment Of Reflection As We Move Forward, Bruce Morgenstern

Journal of Patient-Centered Research and Reviews

The newly appointed editor-in-chief of the Journal of Patient-Centered Research and Reviews reflects on the original editorial mission of the journal and its continuing commitment to publish patient-centered scholarly works that exhibit respect for each patient’s values and preferences; coordinated and integrated care; patient and family education; alleviation of pain, fear, and anxiety; continuity through care-site transitions; and access to care. The reasoning behind the pursuit of well-developed patient-centered care models, through the dissemination of peer-reviewed research findings and patient perspectives, has only been reinforced since the journal’s launch in 2014.

Purpose: Longitudinal patient-partnered experiences may promote medical student empathy, but evaluation of such programs is limited. The aim of this study was to compare areas of learning among first-year medical students randomized to a patient-centered track (PCT) or traditional track (TT) longitudinal clinical experience.

Methods: PCT students (n = 24) were paired with 2 patients and a physician to participate in their patients’ care across multiple settings. TT students (n = 56) were paired with a physician preceptor and participated in caring for a variety of patients in a single setting. This qualitative study used a phenomenological approach to template …

The Effects Of An Interprofessional Education Video On Dermatologists’ Understanding Of The Role Of The Dermatology Certified Nurse Practitioner, Danielle Elaine Spelich Oct 2022

The Effects Of An Interprofessional Education Video On Dermatologists’ Understanding Of The Role Of The Dermatology Certified Nurse Practitioner, Danielle Elaine Spelich

Doctoral Projects

Dermatology-certified nurse practitioners (DCNPs) have validated knowledge of a dermatology core curriculum and have at least 3,000 hours of formal or informal dermatology training (Dermatology Nurse Practitioner Certification Board [DNPCB], n.d.a). With their standardized education and training, DCNPs are meeting the benchmarks set out by dermatologists. Subsequently, DCNPs have become the new gold standard for dermatology advanced practice registered nurses (APRNs).

However, there is no mention of DCNPs by the American Academy of Dermatology (American Academy of Dermatology [AAD], 2022), and personal experience with dermatologists is that they lack knowledge about DCNPs. This lack of knowledge could create an impasse …

Implementing The Use Of The Emergency Severity Index Triage Tool In Urgent Care, Sakeena Spencer Oct 2022

Implementing The Use Of The Emergency Severity Index Triage Tool In Urgent Care, Sakeena Spencer

Doctor of Nursing Practice Projects

Background: The urgent care sector has quickly grown in the last decade. Patients who visit healthcare facilities seeking treatment for exacerbations of chronic conditions or episodic illnesses such as asthma without scheduled appointments receive healthcare services from Urgent Care Centers (UCC) or Emergency Departments (E.D.s). Upon visiting the UCC, these patients report life-threatening symptoms; hence require immediate medical attention. UCC healthcare workers should distinguish between non-acute and acute patients to prioritize providing healthcare to patients, ensuring they are not at risk for fatal outcomes. Using the Emergency Severity Index (ESI) triage tool while delivering healthcare services to these patients has …

On Conflict, Brenden Huynh Oct 2022

On Conflict, Brenden Huynh

be Still

Conflict

In the past, I’ve always been one to avoid conflict. Conflict always had a negative connotation in my mind. I did whatever I could to avoid it. Because of my aversion to conflict, I have had to compromise my time and my efforts in numerous situations. I’ve held my tongue to prevent problems; but sometimes, this would lead to even more. This has affected me all my life, whether its a friend who said something I wasn’t fond of or a waitress that messed up my order, I almost never said anything simply to avoid conflict. Last year, I …

Medicine Is Humbling, Victoria E. Coutin

be Still

As I near the last couple of months of third-year clinical rotations in medical school, this short letter represents my own reflection on the experiences this year that have shaped me.

During your third year of medical school, every month you may find yourself in a completely new environment. These were some of the thoughts that kept me grounded and helped me better integrate myself into each of these new environments.

Aapm Report 373: The Content, Structure, And Value Of The Professional Doctorate In Medical Physics (Dmp), Jay W Burmeister, Charles W Coffey, John D Hazle, Neil Kirby, Yu Kuang, Michael A Lamba, Brian Loughery, Niko Papanikolaou

Faculty, Staff and Student Publications

The Professional Doctorate in Medical Physics (DMP) was originally conceived as a solution to the shortage of medical physics residency training positions. While this shortage has now been largely satisfied through conventional residency training positions, the DMP has expanded to multiple institutions and grown into an educational pathway that provides specialized clinical training and extends well beyond the creation of additional training spots. As such, it is important to reevaluate the purpose and the value of the DMP. Additionally, it is important to outline the defining characteristics of the DMP to assure that all existing and future programs provide this …

Reducing Intubation Time In Adult Cardiothoracic Surgery Patients: A Review Of Data Under The Direction Of A Board Certified Critical Care Intensivist, Rebecca Gooch, Lauren Grip, Chance Nadritch, Kimberly Vent Sep 2022

Reducing Intubation Time In Adult Cardiothoracic Surgery Patients: A Review Of Data Under The Direction Of A Board Certified Critical Care Intensivist, Rebecca Gooch, Lauren Grip, Chance Nadritch, Kimberly Vent

Nursing Research and EBP Day 2022

Background: Prolonged intubation after cardiac surgery increases the risk of morbidity and mortality and lengthens hospital stays. Factors that influence the ability to extubate patients with speed and efficiency include the type of operation, the patient’s baseline physiological condition, and provider practice patterns. The purpose of this research was to prove that the change of practice patterns that followed the addition of an intensivist lessened extubation times in cardiac surgery patients. It was hypothesized that the interventions brought on by the introduction of an intensivist would greatly reduce the amount of time a patient is intubated while in the intensive …

Assessing Primary Care Rns Impact On Quality Patient Care, Elizabeth Carreira Sep 2022

Assessing Primary Care Rns Impact On Quality Patient Care, Elizabeth Carreira

Nursing Research and EBP Day 2022

The purpose of this work was to examine how a large multi-site primary care based organization established an organizational structure that supported the determination, definition, measurement, and reporting of nursing sensitive indicators (NSIs) specific to ambulatory care nursing. NSIs capture the value of care provided by nurses and are a critical component of ANCC Magnet designation. The organization considered that successful pursuit of Magnet designation in an all ambulatory organization would involve establishing a framework to report NSIs that included defining nursing units and NSIs meaningful to the organization.

A multidisciplinary team led by nursing was established to examine and …

Ehealth@Home: Avoiding Readmissions For Heart Failure Patients, Krysta Harrington Sep 2022

Ehealth@Home: Avoiding Readmissions For Heart Failure Patients, Krysta Harrington

Nursing Research and EBP Day 2022

The purpose of this evidence-based project was to reduce readmission rates for congestive heart failure (CHF) patients. It was hypothesized that targeted, nursing led care would reduce readmission rates. To test the hypothesis, we engaged patients post-discharge to enroll in eHealth@Home. eHealth@Home is a program at Rochester Regional Health (RRH) that serves high need CHF patients with frequent acute care utilization. These patients receive intensive monitoring and follow-up from a dedicated eHealth nurse, nurse practitioner and community health worker who work in tandem with a patient’s RRH provider to ensure that seamless and quality care is delivered. Lost In Translation: Mitigating The Effects Of Language Barriers On Patient Outcomes, Megan R. Van Dorp Sep 2022

Lost In Translation: Mitigating The Effects Of Language Barriers On Patient Outcomes, Megan R. Van Dorp

Nursing Research and EBP Day 2022

The purpose of this research was to describe the development, implementation, timeline, and future evaluation of an evidence-based initiative to improve the outcomes of limited English proficiency patients. It was hypothesized that bridging the communication gap between patients of lesser English proficiency and their healthcare providers would increase patients' participation and understanding of their hospital stay. Improving discharge instructions and ensuring comprehension would promote well-being and health as patients are adequately equipped at discharge to assume their own healthcare needs. To test the hypothesis, a literature search was conducted of professional databases. Fifteen scholarly articles contributed to the research base …
