Purpose: The population of the United States continues to become more diverse each day, and this changing demographic impacts our healthcare system, demanding that healthcare providers begin to provide culturally competent services. In order to have culturally competent practitioners, it would be prudent to incorporate the concepts of cultural competence effectively into respective health professions curricula. Induced lactation refers to the production of breast milk in a woman who has never been pregnant. While induced lactation has been practiced developing nations for centuries, it has only recently been introduced to the United States. Induced lactation allows a woman to provide both nutrition and nurture an emotional bond with an infant in cases of adoption, surrogacy, same-sex couples, maternal-infant separation, and emergencies/natural disasters. Many protocols for inducing lactation have been described, though all vary widely in methodology and effectiveness. Never Too Early: Introducing Illness Scripts And Narrative Medicine As Foundational Clinical Approaches, Julia B. Frank, Seema Kakar, Gisela Butera, Thomas Harrod

Educational Challenge: The foundations course of our reformed, organ-based curriculum introduces clinical interviewing when students know little clinical medicine. An exercise that introduces the complementary concepts of “illness scripts” and “narrative medicine” helps them structure early encounters with patients. It provides a framework for organizing future clinical knowledge while reinforcing the humanistic values that may deteriorate over the course of students’ education.

Method: Students conduct and write up a semi-structured interview of someone they know with chronic illness. They reflect on their experience in pairs, and then analyze sample encounters in small groups with faculty facilitation.

Despite the fact that osteopathic research is essential for the continued advancement of our profession, such research is lacking. One barrier is the attainment of funding resources to launch and continue quality research studies. In the present article, the authors outline resources for the early stages of research and provide guidance for grant proposal preparation, if it is determined that external funding is needed. The Effect Of Positive And Negative Reinforcement On Sixth Graders’ Mental Math Performance, Tess Greene, Amber Todd

We examined the relationship between different forms of reinforcement and subjects’ mathematical performance. Thirty sixth-grade students were asked to mentally calculate answers to fraction problems, while holding a heart rate monitor, after receiving a positive, negative, or neutral form of reinforcement. While the results suggest that any motivation, positive or negative, may result in higher grades, the positive reinforcement group displayed the largest gain. However, the results were not statistically significant, though the trend was consistent with prior research. The heart rates of the positively-reinforced group were significantly lower than those of the negatively-reinforced and control groups. These results suggest …

Identification of incoming residents’ unique strengths and weaknesses in a clinical setting is important for developing an individualized educational curriculum and ultimately addressing specific needs. This resource presents and describes materials for a clinical development program for first year residents. The program is structured around three educational elements: an entrance Objective Structured Clinical Examination (OSCE), Individualized Learning Plan (ILP), and peer clinical coaching. Adult literature has demonstrated that patient preferences and understanding of discharge instructions can impact success of discharge and even predict return ER visits. There is very little data in pediatric literature describing what information should be discussed with families when providing discharge instructions, and most pediatric residents do not receive formal education on the topic. Introduction: The extraordinarily high incidence of HIV in sub-Saharan Africa led to the search for cofactor infections that could explain the high rates of transmission in the region. Genital inflammation and lesions caused by sexually transmitted infections (STIs) were a probable mechanism, and numerous observational studies indicated several STI cofactors. Nine out of the ten randomized controlled trials (RCTs), however, failed to demonstrate that treating STIs could lower HIV incidence. We evaluate all 10 trials to determine if their design permits the conclusion, widely believed, that STI treatment is ineffective in reducing HIV incidence.

There is a paucity of knowledge regarding the impact of the Affordable Care Act (ACA, or Obamacare) on low-income, primarily Latino populations. The purpose of the study was to evaluate the role of a free clinic in the post-Obamacare environment. The sample population (n=62) for surveying was composed of patients at Clínica Esperanza/Hope Clinic (CEHC), an inner city clinic in the Olneyville neighborhood of Providence, RI. Basic demographics, along with information regarding motivation for visiting the clinic and satisfaction with the clinic’s patient care, were collected. The patients of CEHC are primarily of central american (57.6%) and hispanic/latino descent (87.9%). …

With the launching of the Liaison Committee on Medical Education’s new accreditation Standard ED-19-A in July 2013, a new era in quality improvement/patient safety (QI/PS) has begun. Core curriculum of medical schools must now include multidisciplinary teamwork; that is, inclusion of practitioners and/or students from other health professions. This Standard ED-19-A is harmonious with the earlier Accreditation Council for Graduate Medical Education’s (ACGME) Competency IV.A.5.c., which calls for QI to be integrated into residents’ training curricula.
