Welcome to the fall 2020/winter 2021 issue of the JCIPE newsletter. 2020 was a year like no other. We witnessed and weathered the impact of the COVID pandemic in a variety of ways, and felt it on nearly every aspect of our professional and personal lives. We also collectively witnessed atrocities too numerous to count, resulting from the deep-seated systemic racism that is intrinsically tied to health disparities and inequities in this country.

Purpose: One way to address healthcare disparities experienced by the transgender community is by educating medical students on trans health. Currently, there is not a universal LGBTQ+ curriculum in medical schools, which has resulted in students feeling incompetent and uncomfortable when treating transgender patients. The purpose of this study was to evaluate the current trans health content in JeffMD and remedy any curricular gaps with trans specific content.

Purpose: There are currently gaps in breastfeeding education for OB/GYN and Family Medicine residents, consequently affecting their confidence in this area of patient care. To address this issue, we are conducting an analysis of the efficacy of the current curriculum to highlight areas for future improvement.

Methods: PGY1-4 residents in the departments of Family Medicine (FM) and Obstetrics and Gynecology (OB/GYN) completed a pre-test before their required annual breastfeeding education on August 6, 2020. Purpose: To characterize emergency department (ED) staff regard toward opioid use disorder (OUD) and analyze variation based on job category. The closure of Hahnemann Hospital, in combination with the increasing prevalence of OUD, is exacerbating the previously inadequate network of resources for these patients. Jefferson is strategically positioned to improve the care provided to this patient population, especially in an ED setting, and ultimately increase linkage to long-term recovery programs.

Purpose: Anatomy, general care and emergency management knowledge of tracheostomies and laryngectomies is crucial in preventing significant morbidity and mortality within the surgical airway population. An Articulate module was made to address and refresh learners on pertinent surgical airway knowledge. Our goal was to assess the efficacy of the new Articulate module, measuring airway knowledge and attitude treating the surgical airway population.

Purpose: Little research is available on the demographics and career goals of first-generation medical students. Undergraduate research suggests first-generation students are more likely to come from lower-income households, speak English as a second language, and experience higher drop-out rates. Purpose: Deep Brain Stimulation (DBS) is an underutilized surgical therapy for Parkinson’s Disease (PD). Both physician and patient hesitancies have been described as barriers to DBS, but specific perceptions of DBS have not been well characterized. To better understand DBS hesitancy, our objective is to pinpoint what specific concerns and fears our patient population have and use this information to enhance education for those contemplating DBS.

If they receive an interview offer at an institution, they are faced with another decision, when to schedule their interview. Studies have examined the possible effect of interview date on matching and on match rank. Purpose:

Partial nephrectomies to remove renal masses are kidney-sparing procedures that rely on anatomical comprehension. Prior research demonstrates complex and expensive 3D models (~$1000) increased surgeons’ confidence in selecting their operative plan. We aim to use simpler and cheaper 3D models to improve preoperative surgeon confidence and support operative management.

Methods:

3D printed models (~$35) of the affected kidney, mass, and renal vasculature were created using preoperative CT or MRI imaging of 17 patients presenting for partial nephrectomies at Thomas Jefferson University (TJU) between July and December 2020. The models were created at TJU using Ultimaker technology.

Purpose

This study describes the current state of formal lesbian, gay, bisexual, transgender and queer (LGBTQ)-related education at undergraduate medical schools in the Mid-Atlantic region of the United States. A review of literature shows the last time a similar study was conducted was in 2011 by Obedin-Maliver et al and was a survey that included all medical schools of the United States and Canada. Given the changes over time for both the LGBTQ and medical community , it is likely that the state of LGBTQ-related education in undergraduate medical education has changed greatly.

Obstructive Sleep Apnea (OSA) is a common sleep disorder characterized by long pauses in breathing over the course of a night’s sleep, leading to significant daytime fatigue and long-term pathologies if left untreated. While a sleep study is the diagnostic tool of choice for OSA, this test is costly. To maximize cost effectiveness, we developed a screening protocol to identify patients at high risk for OSA for referral for a sleep study.

Purpose:

3D printing is a growing tool in surgical education with the ability to visualize and teach complex procedures from multiple angles. Previous studies demonstrated 3D models as useful teaching tools for partial nephrectomy for patient and trainee education. We aim to create a cheaper and simpler 3D model to encourage these learning opportunities.

Methods:

The island of Bermuda is 21 sq miles and located approximately 650 miles east of North Carolina, US. [1] There is only one hospital to serve the population of approximately 65,000. Dental nerve blocks are a vital skill for physicians to have when treating non-traumatic dental pain; however, there is yet to be a simple, practical, and effective way to educate residents in performing this task. The purpose of this project is to explore the efficacy of a low-fidelity dental model task-trainer in teaching emergency medicine residents to perform dental blocks.

Introduction: Diversity in healthcare can improve access to healthcare for racial/ethnic minorities. However, barriers such as lack of educational opportunities have hindered URiM (underrepresented in medicine) students from matriculation to medical school. Enrichment programs such as STEP UP Medicine seek to break down these barriers. It provides its participants with MCAT preparation, advising, medical skills sessions, clinical shadowing experiences, and networking opportunities.
