BACKGROUND: The Medicare Modernization Act of 2003 (Medicare Part D) added prescription drug coverage for senior citizens aged 65 years and older and applied managed care approaches to contain costs. The Patient Protection and Affordable Care Act of 2010 (ACA) had the goals of expanding health care insurance coverage and slowing growth in health care expenditures.

OBJECTIVES: To (a) describe the proportion of senior citizens who had prescription drug insurance coverage and the proportion who experienced financial hardship from purchasing medications in 2015, and (b) compare the findings with those collected in 1998 and 2001.

Background: One school of thought argues that transportation infrastructure is not an ultimate end goal of development and therefore shouldn’t be addressed within development funding decisions while the other argues that transportation infrastructure is the crucial foundation from which all development efforts are based and therefore needs to be addressed within development funding decisions. Within this framework, there is a lack of academic and other research addressing how physical access to health care for pregnant women can better be addressed when making decisions regarding funding of transportation infrastructure projects.

Even though emergency departments (EDs) were created to treat trauma and emergent cases, there has been an increase in emergency department (ED) utilization for non-emergent reasons over the past half of a century. As non-emergent utilization grows as a result of the ED becoming a prevalent substitute for primary care, overcrowding of the ED and increased wait times will continue. Additionally, unnecessary cost to both the ED and the patient will be incurred. Journal of Nursing & Interprofessional Leadership in Quality & Safety

The analysis of harmful errors is typically led by a team within the hospital and includes clinicians and staff who were involved at the time of the event. However, the patient and family are often left out of this process and are not asked to participate in the investigation. Because little guidance is available for facilitating patient input, an interprofessional team convened to develop a semi-structured tool to be used in eliciting patient feedback. Some 72 persons who had experienced adverse events were interviewed. Using a thematic analysis approach, the team learned that 51% of the interviewees preferred to participate …

Implementation Of A Comprehensive Clinical Toolkit For Postpartum Depression Screening: Breaking Barriers In Women's Health Care, Liza C. Rivera Dnp, Msn, Aprn, Whnp-Bc

Journal of Nursing & Interprofessional Leadership in Quality & Safety

A quality improvement initiative to increase screening of postpartum women to detect depression and appropriately refer women screening positive for follow-up care was implemented in an obstetrics and gynecology practice. Physicians in the practice (n = 13) completed an educational module on use and benefits of a toolkit and showed a significant post test increase. Review of patient medical charts found a significant increase in screening rate in an 8-week intervention cycle. Through means of establishing a protocol for appropriate referral for follow-up, the goal of referring 50% of women who screened positive was exceeded.

Health Management and Policy Presentations

Data from the National Longitudinal Survey of Public Health Systems show that hospital and employer contributions to population health activities vary widely across U.S. communities. As peripheral participants in the organizational networks that support population health activities, these stakeholders bring novel information and resources to multi-sector work. This fact may explain the tendency for population health improvement activities to be more effective when hospitals and employers are involved.

The purpose of this project is to describe the development and implementation of cancer SCPs at three health systems’ cancer centers in a rural state. Collectively, these centers serve most cancer patients residing in SD, making the partnership across the cancer centers innovative. Clinical Research and Leadership Faculty Publications

Estimates of biological age based on DNA methylation patterns, often referred to as "epigenetic age", "DNAm age", have been shown to be robust biomarkers of age in humans. We previously demonstrated that independent of chronological age, epigenetic age assessed in blood predicted all-cause mortality in four human cohorts. Here, we expanded our original observation to 13 different cohorts for a total sample size of 13,089 individuals, including three racial/ethnic groups. In addition, we examined whether incorporating information on blood cell composition into the epigenetic age metrics improves their predictive power for mortality. Telehealth has long been viewed as an important pathway for increasing access to care for underserved populations, while providing high quality care at low cost. The spread of telehealth in the United States, however, has been hampered by a range of reimbursement, equipment costs, and licensure barriers. Purpose: To examine self-care for diabetes in the incarcerated population within the framework of the Rediscovery of Self-Care (RSC), a newly developed care model for persons with incarceration experience

Organizing Construct: Diabetes is a chronic illness that requires the development and use complex self-care management skills. The RSC is a strengths-based model promoting the belief that inmate- patients are capable of re-discovering their own strengths for self-care.

Findings: Persons with an incarceration experience have person and environment exposures that reduce their self-care capabilities for diabetes. Using a clinical case management approach, clinicians can assist incarcerated persons with re-entry …

This paper introduces literature that supports the need for Community Health Centers and management of low income patients with chronic illnesses.
