Changing long-standing cultural practices—even when such practices are harmful—is difficult, but the impressive success in reducing the acceptability and incidence of female genital mutilation/cutting (FGM/C) in several settings shows it is possible. Research by FRONTIERS in Burkina Faso, Egypt, Ethiopia, Kenya, Mali, and Senegal has provided insight into the sociocultural and religious underpinnings of FGM/C and has identified approaches that, over time, have contributed to individual and community decisions to abandon the practice. This information can help reduce the risk of undergoing FGM/C faced by three million girls and women every year. This is one of eight Legacy Papers synthesizing …

The Population Council’s USAID-funded Frontiers in Reproductive Health (FRONTIERS) program, in collaboration with the Egyptian Ministry of Health and Population and the NGO Social Planning, Analysis and Administration Consultants, conducted an operations research study to measure the acceptability and effectiveness of two birth spacing message models in Upper Egypt. Overall, the study found that providing birth spacing messages to low-parity women during antenatal and postpartum care and also to husbands through community awareness activities was feasible and acceptable. As a result, birth spacing messages have been integrated into service delivery guidelines for antenatal and postpartum care, and IEC materials on …

A two-phase operations research study was launched in Egypt in early 2000 with the goal of demonstrating how improving the quality of client–provider interaction (CPI) could be achieved in large healthcare systems, specifically in relation to family planning. The study was designed to explore how CPI improvements could enhance family planning knowledge, method continuation rates, client satisfaction, and achievement of fertility goals. Client outcome variables were expected to improve as a result of improving client–provider interaction. The Egyptian Ministry of Health and Population and FRONTIERS discussed the need for investigating service delivery aspects of the Egyptian NORPLANT® program. This study emerged from those consultations. The study provides a comprehensive assessment of the program: how services are administered and used through the ongoing Introduction Program as it approaches five years of operation. The report lists major findings with program implications and offers recommendations regarding quality of care; tracking and locating NORPLANT® users; program sustainability; and information, education, and communication strategies.

This report evaluates the effectiveness of a two-year training program at the Cairo Demographic Center to increase the number of researchers using operations research (OR), and to help institutionalize the ability of the Center to offer training in operations research in reproductive health. It also provides feedback for OR curriculum development. The project trained researchers with program and policymaking responsibilities and provided participants with the experience of designing an OR project, and built participants’ skills in communicating research results to managers. This 1999 study examined the feasibility and impact of introducing the discussion of sexuality during family planning (FP) consultations. Conducted by the Population Council in collaboration with the Egyptian Ministry of Health and Population (MOHP), the study took place in four MOHP clinics and two affiliated private clinics. Nurses and physicians at all six clinics attended a two-day training session on contraceptives with an emphasis on barrier methods. Providers in the three clinics that had been randomly chosen as intervention sites also received three days of training on sexuality, gender, and counseling skills. This paper offers a broad definition of quality of care and presents a comprehensive conceptual framework for the assessment of quality of reproductive health services and methodological approaches for its measurement. It presents three studies that were conducted between 1990 and 1991 by members of the regional Reproductive Health Working Group from Egypt, Jordan, and Tunisia. The studies provide examples of applications of the framework and its measurement using multiple data sources. Selected findings are presented to illustrate comparative results between countries. This pilot study conducted in Cairo and Minya examined the effects of improving the medical care and counseling of post-abortion patients in Egypt. A pre-test/post-test, no control group study design was used to measure the effects of an intervention that upgraded physicians' clinical and interpersonal communication skills for the care of post-abortion patients, including counseling and family planning (FP). The study's surveys utilized direct interviews with staff working in the OB/GYN wards, structured observations of treatment procedures and counseling of post-abortion patients, and interviews with patients prior to discharge. An operations research study was conducted to investigate the concerns of postabortion patients in Egypt to improve counseling that women receive. A corollary use of the study results is the development of sensitive survey research questions for measuring incidence of induced abortion. This study utilized qualitative research methods by conducting in-depth interviews with postabortion patients in El Galaa and El Menia University Hospitals and focus group discussions with family planning clients and noncontracepting women. Egypt’s family planning program officially began in 1965. Impressive gains in contraceptive use have been made since 1980, and the contraceptive prevalence rate increased from 24 percent in 1980 to 47 percent in 1992. Egypt’s National Population Policy addresses the nation’s population growth through multiple interventions designed to reduce fertility, including family planning (FP) services. The present study examined the impact of the Clinical Services Improvement (CSI) Project's provider training program on clients' long-term satisfaction and method use. A sample of 154 clients who received a family planning (FP) method in one of 30 CSI clinics were followed up over a period of 19 months through a series of home interviews. The interviews monitored clients' use of the FP method received at the clinic as well as their satisfaction with that method and the services at CSI. The Government of Egypt launched its national family planning (FP) program in 1966. The early phase was mainly supply oriented and aimed at ensuring wide distribution of contraceptives primarily through pharmacies to meet rising demand. Between 1988 and 1992 a number of additional sources of FP services appeared, with an increase in clinic-based services through both the private and public sectors. In 1992 about one-fourth of users relied on pharmacies to get their FP supplies compared to about one-half in 1988. This change evolved through the influence of two forces: the initiation of a number of FP projects, and expanded …
