Post-irradiation sarcoma is a well-defined complication of radiation therapy, yet few reports document such lesions in the head and neck. A 30-year-old man presented for evaluation of an expansile lesion of the left posterior maxilla. His medical history was significant for a childhood ocular malignancy - unilateral retinoblastoma - which was treated with a combination of surgical enucleation of the eye and external beam radiation therapy. Biopsy of his maxillary lesion revealed a spindle cell malignancy that was morphologically and immunohistochemically consistent with a diagnosis of leiomyosarcoma. The aim of the present study, performed during periodontal recall visits, was to compare the levels of pain experienced by patients during periodontal probing using probes with tip diameters of either 0.40 mm or 0.63 mm.
Prior to the maintenance procedures, three groups of 20 adult patients were examined by 3 different therapists. Diagonal maxillary/mandibular quadrants were probed with a 0.40 mm and a 0.63 mm probe, respectively. At the completion of probing of each quadrant, the patients were asked to describe their pain using a Visual Analog Scale (VAS).

The aims of the present study were to determine the level of pain experienced by patients from probing during periodontal examination, and to find out to what extent the examining periodontist and the dental assistant could estimate the degree of pain experienced by the patients.
For each of three periodontists, 20 patients referred for periodontal diagnosis and treatment were selected. The periodontist carried out his examination, which included probing at six sites per tooth. Following probing, the patients rated the pain using a Visual Analog Scale (VAS). Recently Platelet-Rich Plasma has been proven to be an effective regeneration adjunct when combined with autogenous bone in the reconstruction of mandibular defects. However, little is known about the effect of PRP when combined with a bone allograft in the maxillary sinus.
The purpose of this study was to quantitatively evaluate the ability of PRP to enhance bone regeneration in the maxillary sinus of sheep when combined with DFDBA + CCFDBA. Wound healing is a complex biologic process that involves chemotaxis and division of cells, neovascularization, synthesis of extracellular matrix proteins, and remodeling of the repaired tissues. Of equal importance is the restoration of normal tissue structure and function without scar formation. The aim of this study was to find out the clinical effects of anti-TGFß1 and Pycnogenol® on scarless wound healing on rat skin.

Air-Rotor Stripping In Orthodontic Treatment - A Literature Review, Udom Thongudomporn

Air-rotor stripping was first introduced as an alternative to extraction or expansion therapy in orthodontic treatment in 1985. Since then, the technique has been variously modified for the improvement of its efficiency and the ease of use. Air-rotor stripping has been claimed to be advantageous for the relief of mild to moderate crowding without sacrificing premolars or violating the original arch dimension. However, the opinions on the drawback of the procedure, such as demineralization of the stripped enamel, periodontal complication, and so on, have been varied. It is suggested that clinicians should take precautions whenever the technique is clinically applied.

ไจแอนท์ เซลล์ ไฟโบรมาในช่องปาก : รายงานผู้ป่วย, ศิวพร สุขอร่าม, สมศรี โรจนวัฒน์ศิริเวช

ไจแอนท์ เซลล์ ไฟโบรมา จัดเป็นเนื้องอกชนิดไม่ร้ายแรงซึ่งพบได้น้อยในช่องปาก และโดยเฉพาะใน เด็กเล็ก บทความนี้เป็นรายงานผู้ป่วย 1 ราย ซึ่งเป็นเด็กหญิงไทยอายุ 5 ปี มารับการรักษาที่กลุ่มงานทันตกรรม โรงพยาบาลราชพิพัฒน์ เนื่องจากมีก้อนเนื้อขนาดประมาณ 0.6x0.8x0.3 เซนติเมตร บริเวณเหงือกสามเหลี่ยมระหว่าง ฟันตัด กลางแท้ซ้ายล่างและฟันตัดข้างแท้ซ้ายล่าง เป็นระยะเวลาประมาณ 1 ปี ไม่มีอาการใด ๆ ไม่มีเลือดออก โดยไม่พบความผิดปกติของกระดูกขากรรไกรบริเวณนั้น ผู้ป่วยได้รับการวินิจฉัยทางคลินิกเป็นพาพิลโลมา ให้การ รักษาโดยวิธีผ่าตัด ผลการตรวจทางจุลพยาธิวิทยาเป็น ไจแอนท์ เซลล์ ไฟโบรมา รายงานนี้แสดงถึงการตรวจทาง คลินิก การวินิจฉัยแยกโรคทางคลินิก และการวินิจฉัยทางจุลพยาธิวิทยา รวมทั้งเสนอแนะการดูแลอนามัยช่องปาก ที่ไม่ให้เกิดคราบจุลินทรีย์ซึ่งเป็นตัวกระตุ้นรอยโรคในผู้ป่วยและจากการติดตามผลหลังการรักษาเป็นเวลา 2 ปี ไม่พบการกลับเป็นขึ้นใหม่

วัตถุประสงค์ ในปัจจุบันประเทศไทยมีหลักประกันทันตสุขภาพหลายประเภท ซึ่งมีรูปแบบการดําเนินงานแตกต่าง กัน การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาถึงแบบแผนการใช้บริการทันตกรรมของผู้ป่วย และเพื่อประเมินผลกระทบ ของการมีหลักประกันทันตสุขภาพประเภทต่าง ๆ ต่อพฤติกรรมการใช้บริการทันตกรรม วัสดุและวิธีการ ท่าการเก็บข้อมูลโดยการแจกแบบสอบถามให้แก่ผู้ป่วย ณ คลินิกบริการทันตกรรมพิเศษ คณะ ทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย เพื่อรวบรวมข้อมูลพฤติกรรมการใช้บริการทันตกรรมในรอบหนึ่งปีที่ ผ่านมา (ปี พ.ศ. 2544) ได้รับการตอบแบบสอบถามกลับคืนที่นํามาวิเคราะห์ได้จํานวน 416 รายผลการศึกษา ประมาณร้อยละ 62.3 ของผู้ป่วยมีหลักประกันทันตสุขภาพประเภทใดประเภทหนึ่ง โดยที่สวัสดิการ รักษาพยาบาลข้าราชการและรัฐวิสาหกิจเป็นกลุ่มใหญ่ที่สุด (ร้อยละ 46.9) รองลงมาคือประกันสังคม (ร้อยละ 10.1) ผู้ป่วยส่วนใหญ่ (ร้อยละ 92.8) เคยใช้บริการทันตกรรมในรอบปีที่ผ่านมา ชนิดของบริการทันตกรรมที่ได้รับมาก ที่สุดได้แก่ การขูดหินน้ําลาย การอุดฟัน และการรักษารากฟัน คิดเป็นร้อยละ 66.0, 64.1 และ 36.1 ตามลําดับ พบว่า มีความสัมพันธ์กันอย่างมีนัยสําคัญระหว่างประเภทของหลักประกันทันตสุขภาพกับความถี่ของการใช้บริการ ทันตกรรม (p = 0.024) ผู้ที่มีสวัสดิการรักษาพยาบาลข้าราชการและรัฐวิสาหกิจ มีอัตราการใช้บริการทันตกรรมสูง กว่าผู้ป่วยประกันสังคมสรุป สรุปได้ว่า ประเภทของหลักประกันทันตสุขภาพมีผลกระทบต่อแบบแผนการใช้บริการทันตกรรมของผู้ป่วย หลักประกันทันตสุขภาพต่าง ๆ ที่มีชุดสิทธิประโยชน์เหลื่อมล้ํากัน อาจกระทบต่อความเท่าเทียมกันในการเข้าถึงบริการทันตกรรมของประชาชน

ประสิทธิผลของโปรแกรมสุขศึกษาในการแนะนํา สุขภาพช่องปากต่อการเปลี่ยนแปลงพฤติกรรม ในการเลิกสูบบุหรี่ของผู้ป่วยในคลินิกทันตกรรม 654 สํานักอนามัย กรุงเทพมหานคร, ลาวัณย์ บุณยมานนท์

วัตถุประสงค์ การศึกษานี้เป็นการวิจัยแบบทดลองที่มีวัตถุประสงค์ทดสอบประสิทธิผลของโปรแกรมสุขศึกษาเรื่องคําแนะนําของทันตแพทย์ต่อการลดเลิกสูบบุหรี่ของผู้ที่ตั้งใจจะเลิกสูบบุหรี่ และมารับบริการทางทันตกรรมที่คลินิก ทันตกรรม 654 กองทันตสาธารณสุข กรุงเทพมหานคร วัสดุและวิธีการ ผู้สูบบุหรี่ 90 คนถูกจัดเป็นกลุ่มทดลอง 46 คน กลุ่มควบคุม 44 คน กลุ่มทดลองจะได้รับความรู้ และคําแนะนําจากทันตแพทย์เกี่ยวกับผลเสียของการสูบบุหรี่ต่อร่างกาย และอวัยวะในช่องปาก ส่วนกลุ่มควบคุม ไม่ได้รับความรู้เกี่ยวกับอันตรายจากการสูบบุหรี่เมื่อเริ่มต้นการศึกษาแต่จะได้รับความรู้ดังกล่าวเมื่อสิ้นสุดการศึกษาทั้งสองกลุ่มจะได้รับการสอนวิธีควบคุมแผ่นคราบจุลินทรีย์ และการทําความสะอาดฟันเมื่อเริ่มต้นการศึกษา 1 สัปดาห์ หลังการกษา 1 เดือน และ 12 เดือน ผลการศึกษา ผลการศึกษาพบว่าจํานวนมวนที่สูบต่อวันมีความแตกต่างกันระหว่างกลุ่มทดลองและกลุ่มควบคุม อย่างมีนัยสําคัญทางสถิติที่ระดับความเชื่อมั่น 95 % (p<0.05) ตลอดระยะการติดตามผลที่ 1 สัปดาห์ (p=0,000) 1 เดือน (p=0.000) 3 เดือน (p=0.008) 6 เดือน (p=0.026) 9 เดือน (p=0.016) และ 12 เดือน (p=0.002) เมื่อสิ้นสุดการศึกษากลุ่มทดลองหยุดสูบบุหรี่ 32% ในขณะที่กลุ่มควบคุมหยุดสูบบุหรี่เพียง 7.4% กลุ่มทดลองและ กลุ่มควบคุมหยุดสูบบุหรี่มากกว่าหรือเท่ากับครึ่งหนึ่งของจํานวนที่เคยสูบ 28 % และ 18.5 % ตามลําดับ ส่วนผู้ที่ สูบบุหรี่น้อยกว่าครึ่งหนึ่งของจํานวนมวนที่เคยสูบในกลุ่มทดลองและกลุ่มควบคุมมีค่าเท่ากับ 28 % และ 18.5 % ตามลำดับ สรุป การศึกษาครั้งนี้แสดงให้เห็นว่า การแนะนําเรื่องผลเสียของการสูบบุหรี่ต่อร่างกายและอวัยวะในช่องปากจากทันตแพทย์มีผลต่อการลดเลิกสูบบุหรี่ของผู้สูบบุหรี่ที่มารับบริการทางทันตกรรม

วัตถุประสงค์ของการศึกษานี้เพื่อเปรียบเทียบการจัดคลินิกปริทันตวิทยาของนิสิตปริญญาบัณฑิต ระหว่างแบบ ช่วงเวลาและแบบตลอดปี ในเรื่องเกี่ยวกับการบริหารจัดการในคลินิก ความร่วมมือของผู้ป่วย สมรรถนะของวิชาชีพที่นิสิต ทําได้ และคุณภาพการเรียนการสอน วัสดุและวิธีการ ทําการศึกษาโดยใช้แบบสอบถามประกอบด้วยข้อมูลทั่วไปของผู้ตอบแบบสอบถาม ข้อมูลเกี่ยวกับ การบริหารจัดการในคลินิก ความร่วมมือของผู้ป่วย สมรรถนะของวิชาชีพที่นิสิตทําได้ และคุณภาพการเรียนการสอน กลุ่ม ตัวอย่างจํานวน 195 คน มีผู้ตอบแบบสอบถามกลับคืนจํานวน 175 คน ประกอบด้วยนิสิตทันตแพทย์ที่มีรหัสเลขประจําตัวนิสิต 41 จํานวน 87 คน ปฏิบัติงานคลินิกปริทันตวิทยาแบบช่วงเวลา และนิสิตทันตแพทย์ที่มีรหัสเลขประจําตัวนิสิต 42 จํานวน 88 คน ปฏิบัติงานคลินิกปริทันตวิทยาแบบตลอดปี ในปีการศึกษาที่ 5 รวบรวมข้อมูลและวิเคราะห์ข้อมูลด้วยการใช้ SPSS- PC software package ผลการศึกษา ผลการวิเคราะห์พบมีความแตกต่างอย่างนัยสําคัญทางสถิติของการจัดคลินิกปริทันตวิทยาแบบช่วงเวลาและการ จัดคลินิกปริทันตวิทยาแบบตลอดปี ในหัวข้อนิสิตรักษาผู้ป่วยไม่เสร็จในการรักษาขั้นต้น (p-041) นิสิตของยูนิต ล่วงหน้า เพราะเลือกอาจารย์เช็คงาน (1,003) การทําเครื่องมือให้ไร้เชื้อไม่ทันเวลาทํางาน (p-003) และการนําความรู้ทางพรี คลีนิกไปประยุกต์ใช้ในคลินิก (p-01) ส่วนหัวข้ออื่น ๆ มีความแตกต่างอย่างไม่มีนัยสําคัญทางสถิติระหว่างการจัดคลินิกปริ ทันตวิทยาทั้ง 2 แบบ สรุป ผลสรุปจากคําถามปลายเปิดพบว่า การจัดคลินิกปริทันตวิทยาแบบช่วงเวลามีข้อดี คือนิสิตสามารถให้การรักษา ผู้ป่วยอย่างต่อเนื่อง นิสิตสามารถบริหารจัดการในการรักษาผู้ป่วยเฉพาะ 2 ภาควิชาได้ง่าย และนิสิตทุ่มเทกับการปฏิบัติงานคลินิกเฉพาะวิชาอย่างเต็มที่ ข้อเสียของการจัดคลินิกปริทันตวิทยาแบบช่วงเวลา คือช่วงเวลา 12-13 สัปดาห์ไม่เหมาะสําหรับ การรักษาลักษณะองค์รวม ปัญหาที่เกิดขึ้นมีความแตกต่างกันในแต่ละช่วงเวลา และนิสิตมีความเครียดขณะทํางานที่มีเวลาจํากัด ส่วนข้อดีของการจัดคลินิกปริทันตวิทยาแบบตลอดปีเป็นการรักษาผู้ป่วยในลักษณะองค์รวมในทุกสาขาวิชาที่เกี่ยวข้อง และติดตามการรักษาในช่วงเวลาคงสภาพ นิสิตรู้จักตัดสินใจ วางแผนและการบริหารจัดการผู้ป่วย ส่วนข้อเสียของการจัดคลินิก ปริทันตวิทยาแบบตลอดปี คือจํานวนคาบเวลาทํางานต่อสัปดาห์ที่มีน้อยทําให้นิสิตนัดผู้ป่วยได้ยากขึ้น เป็นผลให้เกิด ความเครียดในการบริหารจัดการผู้ป่วย ข้อสุดท้ายคือ จํานวนคาบเวลาของนิสิตทั้ง 2 กลุ่มไม่เท่ากัน ถึงแม้ว่าจํานวนชั่วโมงที่ คลินิกจะเท่ากัน ผลจากการศึกษานี้อาจใช้เป็นแนวทางหนึ่ง ในการพิจารณากําหนดการจัดคลินิกโดยวิธีแบบช่วงเวลา หรือแบบตลอดปี และเป็นข้อมูลสําหรับบุคลากรทุกฝ่ายที่เกี่ยวข้อง ในการทําความเข้าใจกับระบบดังกล่าว

วัตถุประสงค์ การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อ ศึกษาถึงการเรียนการสอนในคลินิกปริทันตวิทยาของนิสิตปริญญาบัณฑิต ใน เรื่องเกี่ยวกับด้านการบริหารจัดการในคลินิก ความร่วมมือของผู้ป่วย สมรรถนะของวิชาชีพที่นิสิตทําได้ และคุณภาพการเรียนการสอน วัสดุและวิธีการ ส่งแบบสอบถามประกอบด้วย ข้อมูลทั่วไปเกี่ยวกับผู้ตอบแบบสอบถาม ข้อมูลเกี่ยวกับด้านการบริหารจัดการ ในคลินิก ความร่วมมือของผู้ป่วย สมรรถนะของวิชาชีพที่นิสิตทําได้ และคุณภาพการเรียนการสอน ให้กับทันตแพทย์ที่มีรหัส เลขประจําตัวนิสิต 40 จํานวน 100 คน และนิสิตทันตแพทย์ที่มีรหัสเลขประจําตัวนิสิต 41 จํานวน 102 คน ผู้ตอบแบบสอบถามทั้ง2 รุ่นได้ปฏิบัติงานของคลินิกรวม และสหคลินิก โดยเฉพาะคลินิกปริทันตวิทยาในลักษณะเดียวกันในการศึกษาปีที่ 4,5 และ 6- หลังจากได้แบบสอบถามกลับคืนมา ได้รวบรวมข้อมูลเพื่อการวิเคราะห์ข้อมูลเชิงพรรณนา ด้วยการใช้ SPSS - PC software package ผลการศึกษา ได้รับแบบสอบถามกลับคืนจํานวน 156 ฉบับ พบปัญหามากและปัญหามากที่สุดในหัวข้อด้านการบริหารจัดการ ในคลินิก เรื่องการรักษาผู้ป่วยไม่เสร็จในการรักษาขั้นต้น และหัวขัดผงพิมพืชเสียง่าย ร้อยละ 36.46-40.38 สําหรับหัวข้อ ความร่วมมือของผู้ป่วย นิสิตปี 4 ปี 5 และปี 6 พบปัญหามากและปัญหามากที่สุดในเรื่องความไม่ร่วมมือในการแปรงฟัน และใช้ไหมขัดฟัน ร้อยละ 22.43, 42.95 และ 31.41 ตามลําดับ หลังจากจบการศึกษาปีที่ 4 ผู้ตอบแบบสอบถามมีสมรรถนะ ของวิชาชีพ คือสามารถจุดหินน้ําลายและเกลารากฟัน สามารถตรวจหาหินน้ําลาย และสามารถสอนและชักจูงผู้ป่วยให้ดูแล อนามัยช่องปาก ในรายที่มีร่องลึกปริทันต์ไม่เกิน 5 มิลลิเมตร ได้มากและมากที่สุดรวมร้อยละ 30.13 ส่วนหัวข้อคุณภาพ การเรียนการสอน พบว่าร้อยละ 49.01 ของจํานวนอาจารย์ประจําที่ได้สอดแทรกคุณธรรม และจริยธรรมทางทันตแพทย์ ร้อยละ 71.17 ของจํานวนอาจารย์พิเศษที่ตอบข้อซักถามของนิสิตให้เข้าใจได้ง่าย ส่วนหัวข้ออาจารย์ที่ไม่อธิบายเหตุผลในการเช็ค งานผ่านไม่ผ่าน มีจํานวนอาจารย์ประจําและอาจารย์พิเศษถูกประเมินน้อยสุด คือ ร้อยละ 5.60 และ 3.92 ตามลําดับ สรุป …

วัตถุประสงค์ เพื่อศึกษาขนาดความกว้างของขากรรไกรในกลุ่มที่มีโครงสร้างใบหน้าสั้นและใบหน้ายาว พร้อมทั้งเปรียบเทียบขนาดความกว้างขากรรไกรที่ศึกษาได้จากโครงสร้างใบหน้าทั้งสองแบบ วัสดุและวิธีการ คัดเลือกกลุ่มตัวอย่างจากภาพถ่ายรังสีกะโหลกศรีษะด้านข้างโดยใช้ค่าของมุมที่เกิดจากระนาบ กะโหลกศีรษะ (SN) กับระนาบขากรรไกรล่าง (MP) เป็นเกณฑ์ได้กลุ่มที่มีใบหน้าสั้น 38 คน หญิง 26 คน ชาย 12 คน) กลุ่มที่มีใบหน้ายาว 38 คน หญิง 23 คน ชาย 15 คน) สร้างแบบจําลองฟันของกลุ่มตัวอย่างนํามาวัดความ กว้างของขากรรไกรส่วนหน้าและส่วนหลังทั้งขากรรไกรบนและขากรรไกรล่าง ตามจุดอ้างอิงของ Korkhaus ด้วยดี ไวน์เตอร์ปลายแหลม ศึกษาค่าความกว้างต่ําสุด สูงสุด ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐานของขากรรไกรใน ใบหน้าแต่ละรูปแบบ และนําค่าเฉลี่ยมาเปรียบเทียบกัน การทดสอบทางสถิติใช้ t-test ผลการศึกษา ในกลุ่มใบหน้าสั้น มีค่าเฉลี่ยของขนาดความกว้างขากรรไกรบนส่วนหน้าและส่วนหลังเป็น 38,092 มม. และ 48.553 มม. ขากรรไกรล่าง ส่วนหน้าและส่วนหลังเป็น 37.750 มม. และ 48.803 มม. ในขณะที่กลุ่ม ใบหน้ายาวขากรรไกรบนส่วนหน้า และส่วนหลังมีค่าความกว้างเป็น 36,447 มม. และ 46.842 มม. ขากรรไกรล่าง ส่วนหน้าและส่วนหลังเป็น 36.137 มม. และ 47.263 มม. ตามลําดับ โดยค่าเฉลี่ยของขนาดขากรรไกรในใบหน้าทั้ง สองกลุ่มแตกต่างกันอย่างมีนัยสําคัญทุกค่า (P < 0.05) ยกเว้นเมื่อแยกพิจารณาตามเพศ ในเทศหญิงถึงแม้ ค่าความกว้างทุกค่าจะแตกต่างในลักษณะเดียวกัน แต่จะมีนัยสําคัญ (P < 0.05) เฉพาะความกว้างส่วนหน้าของ ขากรรไกรบนและขากรรไกรล่างเท่านั้น สรุป ขนาดความกว้างของขากรรไกรบนและล่างในคนที่มีโครงสร้างใบหน้าสั้นจะมีค่ามากกว่าคนที่มีโครงสร้าง ใบหน้ายาวทุกค่า

The purpose of the study was to determine floss frequency, habits and techniques of patients entering a private periodontal office (office site) vs. a hospital dental clinic (hospital site). It was found that the flossing frequency in a hospital dental clinic is far less than in a private practice, and patients who do floss may not be using the proper flossing technique. Patients need more instruction and care with regard to their interproximal oral hygiene.

Strategies used to enhance liposome-mediated drug delivery in vivo include the enhancement of stability and circulation time in the bloodstream, targeting to specific tissues or cells, and facilitation of intracytoplasmic delivery. pH-sensitive liposomes have been developed to mediate the introduction of highly hydrophilic molecules or macromolecules into the cytoplasm. These liposomes destabilize under acidic conditions found in the endocytotic pathway, and usually contain phosphatidylethanolamine (PE) and titratable stabilizing amphiphiles. Formulations without PE have also been developed. Encapsulated compounds are thought to be transported into the cytoplasm through destabilization of or fusion with the endosome membrane.
