There are inadequate numbers of dentists able and willing to treat America's children, specifically children from low income and minority populations. This has led to the well-publicized disparities in oral health among children. In the early part of the 20th century New Zealand faced a significant problem with oral disease among its children and introduced a School Dental Service, staffed by allied dental professionals with two years' training in caring for the teeth of children, "school dental nurses." Adhesive Fragment Reattachment After Orthodontic Extrusion: A Case Report, S. Burcak Cengiz, Ilken Kocadereli, H. Cem Güngör, Nil Altay
School of Dentistry Faculty Research and Publications
In the treatment of crown fractures, adhesive fragment reattachment provides a good alternative to other restorative techniques, offering several advantages. The present paper reports a case in which the treatment of a cervical crown fracture was accomplished by reattaching the tooth fragment with a flowable resin composite. Orthodontic root extrusion was performed with a modified Hawley appliance prior to fragment reattachment. The clinical and radiographic results after 2.5 years were successful.

Consistency in clinical decision making may be necessary for reliable assessment of student performance and teaching effectiveness, yet little has been done to examine variation in periodontal diagnosis and treatment planning among dental school faculty. The purpose of this investigation was to examine variation among faculty in diagnosis and management of common periodontal diseases. Twenty-seven clinical instructors (periodontists, general dentists, dental hygienists, and first- and second-year periodontal graduate students) reviewed three web-based cases and answered a brief questionnaire focusing on radiographic interpretation, periodontal diagnosis, and treatment planning. Response rates for the three cases ranged from 62 percent to 70 percent. …

วัตถุประสงค์ เพื่อศึกษาความแตกต่างของค่าระยะทางและค่ามุมที่วัดได้จากภาพรังสีกะโหลกศีรษะด้านข้าง ด้วยโปรแกรมคอมพิวเตอร์สําเร็จรูปสําหรับวิเคราะห์ภาพรังสีกะโหลกศีรษะ 5 โปรแกรม เปรียบเทียบกับการวิเคราะห์ด้วยมือตามเกณฑ์ของสไตเนอร์ และเกณฑ์ของริกเกทส์ วัสดุและวิธีการ เลือกกลุ่มตัวอย่างจากภาพรังสีกะโหลกศีรษะด้านข้างของผู้ป่วยที่มารับการรักษาทางทันตกรรม จัดฟันที่คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย จํานวน 32 คน อายุระหว่าง 11-27 ปี (เฉลี่ย 16.06 ปี) ฟันอยู่ในระยะฟันแท้ ไม่มีฟันคุดหรือพยาธิสภาพมาบังคับรากฟันหน้าและฟันกรามแท้ซี่ที่หนึ่ง นําภาพรังสีมา ลอกลาย และกําหนดจุดอ้างอิงตามวิธีการวิเคราะห์ของโปรแกรมคอมพิวเตอร์สําเร็จรูป 5 โปรแกรม คือ 1. Dentofacial planner 2. Quick-ceph image 3. RMO's Jiffy Orthodontic Evaluation 4. Compu-ceph 5. OTP รวมทั้ง วิเคราะห์ด้วยมือจากภาพลอกลาย วัดค่าระยะทางและค่ามุมตามเกณฑ์ของสไตเนอร์และเกณฑ์ของริกเกทส์ รวม 18 ตัวแปร เปรียบเทียบค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐานด้วยสถิติวิเคราะห์ความแปรปรวนแบบทางเดียวที่ระดับนัยสําคัญ 05 เมื่อพบความแตกต่างอย่างมีนัยสําคัญ จึงทดสอบแบบจับคู่พหุคูณตามวิธีของทุกย์ ผลการศึกษา พบความแตกต่างของค่า Pog-NB (มม.) ของโปรแกรม JOE และค่า U6-PTV (มม.), L1-APO (องศา) และ Facial axis (องศา) ของโปรแกรม Compu-ceph ซึ่งมีเพียง 4 ค่า จากค่าตัวแปรที่ใช้ทั้งหมด 18 ค่า สรุป โปรแกรมคอมพิวเตอร์สําเร็จรูปทั้ง 5 โปรแกรม สามารถใช้ทดแทนการวิเคราะห์ด้วยมือได้ การเลือกใช้อุปกรณ์ สําหรับนําข้อมูลเข้าเครื่องควรพิจารณาความเหมาะสม ควรรู้วิธีการใช้โปรแกรมอย่างถูกต้องและควรมีความรู้ทางด้าน การวิเคราะห์ภาพรังสีของกะโหลกศีรษะ เช่น คํานิยามของจุดอ้างอิงที่ใช้และการกําหนดจุดให้ถูกต้อง ความแตกต่าง ที่พบจากการศึกษานี้เนื่องจากคํานิยามที่แตกต่างของจุดอ้างอิงของโปรแกรมสําเร็จรูปที่ใช้ และวิธีการวัดของโปรแกรม (ว ทันต จุฬาฯ 2548;28:29-38)

การให้ยาระงับความรู้สึกทั่วไปสําหรับการทําศัลยกรรมช่องปากและแม็กซิลโลเฟเชียล วิสัญญีแพทย์มีโอกาส พบปัญหาการช่วยหายใจ และการใส่ท่อช่วยหายใจลําบากได้เสมอ การประเมินสภาพทางเดินหายใจในระยะก่อนผ่าตัดและการเตรียมความพร้อมอย่างเหมาะสมทําให้การระงับความรู้สึกทําได้อย่างปลอดภัยปราศจากภาวะแทรกซ้อน รายงานผู้ป่วย 2 ราย ซึ่งได้รับการจัดการกับปัญหาใส่ท่อช่วยหายใจต่างกัน ผู้ป่วยรายที่ 1 เป็นชาย อายุ 16 ปี เข้ารับการผ่าตัดเสริมสร้างขากรรไกรล่างด้วยกระดูกเชิงกรานร่วมกับใช้แผ่นโลหะไททาเนียมตามกระดูกลักษณะกายวิภาคของใบหน้าที่บ่งบอกภาวะใส่ท่อช่วยหายใจลําบาก คือ ขากรรไกรล่างสั้น หลุบเข้าด้านใน และขยับลิ้นได้น้อย ซึ่งเกิดจากการผ่าตัดหลายครั้งก่อน เลือกใช้เทคนิคการใส่ท่อช่วยหายใจโดยใช้กล้องตรวจ กล่องเสียงชนิดท่อใยแก้วนําแสง ภายใต้การสูดดมยาสลบได้อย่างปลอดภัย ผู้ป่วยรายที่ 2 เป็นหญิง อายุ 75 ปี ได้รับ การวินิจฉัยเป็นลุควิกส์แองไจนา ต้องผ่าตัดเจาะระบายหนองออก ภายใต้การดมยาสลบ วิสัญญีแพทย์วางแผน ขั้นต้นจะทําการประเมินทางเดินหายใจ และใส่ท่อช่วยหายใจโดยวิธีไดเร็กต์ลาริงโกสโคปี หลังจากให้ยาสงบประสาท เข้าหลอดเลือดดํา ผู้ป่วยหมดสติร่วมกับทางเดินหายใจส่วนบนอุดกั้นอย่างสมบูรณ์ ไม่สามารถช่วยการหายใจด้วย หน้ากากช่วยหายใจ ความอิ่มตัวของออกซิเจนในเลือดแดงต่ําลงมากร่วมกับชีพจรเต้นช้า ผู้ป่วยได้รับการ ผ่าหลอดลมคอฉุกเฉิน เพื่อเปิดทางเดินหายใจและช่วยหายใจจนสัญญาณชีพกลับมาปกติ หลังผ่าตัดผู้ป่วยพื้นจาก ยาสลบ รู้สึกตัวดี แต่หายใจเองไม่เพียงพอจําเป็นต้องใช้เครื่องช่วยหายใจ จึงย้ายผู้ป่วยไปรับการรักษาต่อใน หออภิบาลผู้ป่วยหนักของโรงพยาบาลตํารวจ ผู้ป่วยกลับบ้านได้หลังจากเข้ารับการรักษานานเกือบ 2 เดือน เนื่องจากมีภาวะแทรกซ้อนของระบบหัวใจและหลอดเลือด (ว ทันต จุฬาฯ 2548;28:51-8)

วัตถุประสงค์ เพื่อประเมินประสิทธิภาพและความปลอดภัยของการให้ยาโพรพอฟอลหยดเข้าหลอดเลือดดํา เพื่อทําให้ผู้ป่วยสงบ ในผู้ป่วยนอกที่มารับการผ่าตัดเล็กของช่องปาก ในโรงพยาบาลคณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย วัสดุและวิธีการ ศึกษาผู้ป่วยจํานวน 50 ราย สภาพร่างกายอยู่ในระดับ 1 หรือ 2 ตามเกณฑ์ของสมาคม วิสัญญีแพทย์แห่งประเทศไทย อายุระหว่าง 12-77 ปี มารับการผ่าตัดเล็กของช่องปากภายใต้การฉีดยาชาเฉพาะที่ ทําให้ผู้ป่วยสงบผ่อนคลายโดยฉีดโพรพอฟอลขนาด 0.75-1.0 มก./กก. ทางหลอดเลือดดําเพื่อเริ่มนําให้สงบ แล้วตามด้วยการหยดเข้าหลอดเลือดดําแบบต่อเนื่องในอัตรา 3 มก./กก./ชม. ปรับอัตราการหยดยาในระหว่าง การผ่าตัดเพื่อรักษาระดับการสงบประสาทให้เพียงพอกับความต้องการของผู้ป่วย บันทึกความดันเลือด ชีพจร อัตราการหายใจ คลื่นไฟฟ้าหัวใจ และความอิ่มตัวของออกซิเจนในเลือด ตลอดเวลาการผ่าตัด ประเมินคุณภาพการ สงบประสาท ภาวะเสียความจํา ความพึงพอใจของผู้ป่วย และผลข้างเคียงจากยา ผลการศึกษา ปริมาณยาโพรพอฟอลที่ใช้ฉีดเข้าหลอดเลือดดําเพื่อนําสงบเฉลี่ย (ค่าเบี่ยงเบนมาตรฐาน) 51.52 (11.35) มก. (21-75 มก.) ขนาดยาโพรพอฟอลที่ใช้ในการหยดเข้าหลอดเลือดดําแบบต่อเนื่องเฉลี่ย (ค่าเบี่ยงเบน มาตรฐาน) 3.16 (0.58) มก./กก./ชม. (2.24-4.67 มก./กก./ชม.) ระยะเวลาในการผ่าตัดเฉลี่ย (ค่าเบี่ยงเบน มาตรฐาน) 72.34 (32.03) นาที (17-150 นาที) ศัลยแพทย์ประเมินคุณภาพของการสงบประสาทอยู่ในเกณฑ์ดี ร้อยละ 84 ปานกลางร้อยละ 16 ผู้ป่วยร้อยละ 54 เกิดภาวะเสียความจําอย่างสมบูรณ์ต่อเหตุการณ์ขณะฉีดยาชา เฉพาะที่ ไม่พบภาวะแทรกซ้อนรุนแรงต่อระบบหัวใจและหลอดเลือด พบกดการหายใจในผู้ป่วย 1 ราย ซึ่งเกิด หลังจากฉีดโพรพอฟอลเข้าหลอดเลือดดําเพื่อนําสงบ ผลข้างเคียงอื่น ๆ ได้แก่ อาการปวดขณะฉีดยาโพรพอฟอล พูดมากขึ้น คัน และปวดศีรษะ สรุป การให้โพรพอฟอลหยดทางหลอดเลือดดําแบบต่อเนื่อง อาจเป็นอีกทางเลือกหนึ่งในการทําให้ผู้ป่วยสงบนอกจากการให้มีดาโซแลมฉีดทางหลอดเลือดดํา หรือการสูดดมไนตรัสออกไซด์ร่วมกับออกซิเจน แต่จําเป็นต้องมีการเฝ้าระวังเตรียมพร้อมเพื่อป้องกันและแก้ไขภาวะฉุกเฉินเพื่อความปลอดภัยของผู้ป่วย (ว ทันต จุฬาฯ 2548;28:1-10)

This article presents proliferative verrucous leukoplakia (PVL), a distinct form of oral leukoplakia which exhibits aggressive behavior due to its high rates of recurrence and malignant transformation. It is a disease of unknown origin. The prevalence and distribution of this lesion are different from those of other forms of oral leukoplakia, and its clinical-pathologic appearance is varied, depending on the stage of development of the lesion. PVL is resistant to most kinds of therapy; therefore, the control of this disease is difficult, especially in the late course of disease in which multiple lesions usually occur. วัตถุประสงค์ (1) เพื่อศึกษาพฤติกรรมการออกกําลังกายของนิสิตทันตแพทย์ และความสัมพันธ์ระหว่างนิสิต ทันตแพทย์ชั้นปีต่างๆกับเวลาที่มีสําหรับการออกกําลังกาย (2) เพื่อศึกษาความชุกของการปวดคอและปวดหลังของนิสิตทันตแพทย์ วัสดุและวิธีการ เก็บข้อมูลโดยใช้แบบสอบถามชนิดตอบด้วยตนเองจากนิสิตทันตแพทย์ชั้นปีที่ 1-6 คณะทันต แพทยศาสตร์ จุฬาฯ ปีการศึกษา 2548 กลุ่มตัวอย่างได้มาจากการสุ่มแบบแบ่งชั้น ผลการทดลอง นิสิตจํานวน 194 คน ตอบแบบสอบถาม คิดเป็นอัตราการตอบกลับเท่ากับ ร้อยละ 100 เป็นนิสิตชาย 57 คน หญิง 137 คน พบว่ามีนิสิตที่ออกกําลังกายเป็นประจําร้อยละ 14.4 โดยนิสิตปี 6 ออกกําลังกายมากที่สุด (ร้อยละ 27.3) และนิสิต ปี 3 ออกกําลังกายน้อยที่สุด (ร้อยละ 3.2) สาเหตุส่วนใหญ่ที่ทําให้ไม่สามารถออกกําลังกายได้คือไม่มีเวลา (ร้อยละ 66) นอกจากนี้อาการปวดคอและปวดหลังยังพบได้ร้อยละ 43.8 และร้อยละ 47.4 ตามลําดับ ทั้งนี้ความถี่ในการ ออกกําลังกายของนิสิตทันตแพทย์ชั้นปีที่ 1-6 มีความแตกต่างกันอย่างมีนัยสําคัญ (p < 0.05) นอกจากนี้เพศยังมี ความสัมพันธ์กับพฤติกรรมการออกกําลังกายอย่างมีนัยสําคัญ (p < 0.05) สรุป ชั้นปีการศึกษาต่างๆ กัน มีพฤติกรรมการออกกําลังกายแตกต่างกัน อาการปวดคอและปวดหลังพบได้ร้อยละ 43.8 และร้อยละ 47.4 ตามลําดับ นอกจากนี้นิสิตทันตแพทย์ชายจะมีความถี่และใช้เวลาในการออกกําลังกายสูงกว่า นิสิตทันตแพทย์หญิง (ว ทันต จุฬาฯ 2549;29:195-204)

วัตถุประสงค์ เพื่อสํารวจความชุกของการเกิดฟันกรามแท้ซี่ที่หนึ่งที่ขึ้นผิดตําแหน่งในเด็กนักเรียนอายุ 7-10 ปีเขตกรุงเทพมหานครและเพื่อศึกษาปัจจัยที่เป็นสาเหตุ วัสดุและวิธีการ ออกตรวจสุขภาพช่องปากเด็กนักเรียนอายุ 7-10 ปี จํานวน 6,957 ราย ในโรงเรียนสังกัด กรุงเทพมหานคร รัฐบาล เอกชน เพื่อสํารวจความชุกของการเกิดฟันกรามแท้ซี่ที่หนึ่งที่ขึ้นผิดตําแหน่งส่งแบบสอบถามเกี่ยวกับปัจจัยที่เป็นสาเหตุให้ผู้ปกครองของนักเรียนในกลุ่มตัวอย่างและกลุ่มควบคุมตอบคําถาม จากนั้นนําข้อมูลน้ําหนัก ส่วนสูง วันเดือนปีเกิดของทั้งสองกลุ่มไปประเมินภาวะการเจริญเติบโต นําข้อมูลจาก แบบสอบถาม และภาวะการเจริญเติบโตไปวิเคราะห์ความสัมพันธ์ในการเกิดฟันกรามแท้ซี่ที่หนึ่งที่ขึ้นผิดตําแหน่ง โดยใช้สถิติไคสแควร์ และสหสัมพันธ์เครเมอร์สวี ที่ระดับนัยสําคัญ .05 ผลการศึกษา ผลการวิจัยพบฟันกรามแท้ซี่ที่หนึ่งขึ้นผิดตําแหน่งในเด็กจํานวน 41 ราย จากเด็กนักเรียน 6,957 ราย คิดเป็นความชุกร้อยละ 0.59 และพบว่าการเกิดฟันกรามแท้ซี่ที่หนึ่งขึ้นผิดตําแหน่งมีความสัมพันธ์กับการมีปัญหา ในการบดเคี้ยวของเด็กในช่วงอายุ 2-6 ปี (p = .027) และสัมพันธ์กับภาวะการเจริญเติบโตของเด็ก (p = .000) โดยพบมากในกลุ่มเด็กเตี้ยและในกลุ่มเด็กที่มีน้ําหนักค่อนข้างน้อย สรุป ความชุกในการเกิดฟันกรามแท้ซี่ที่หนึ่งขึ้นผิดตําแหน่ง พบร้อยละ 0.59 และพบความสัมพันธ์ของการเกิด ฟันกรามแท้ซี่ที่หนึ่งขึ้นผิดตําแหน่งกับปัจจัยปัญหาในการบดเคี้ยวอาหารของเด็กในช่วงอายุ 2-6 ปีและปัจจัยภาวะการเจริญเติบโตของเด็ก (ว ทันต จุฬาฯ 2548;28:39-50)

การสบฟันแบบคลาสทรีเป็นความผิดปกติที่พบได้บ่อยในคนไทย การเลือกใช้เครื่องมือและวิธีในการรักษาทางทันตกรรมจัดฟันที่เหมาะสมขึ้นกับปัจจัยหลายอย่าง ได้แก่ ลักษณะความผิดปกติ อายุ และลักษณะทางพันธุกรรม ของผู้ป่วย ดังนั้น ทันตแพทย์จึงควรมีความรู้ความสามารถที่จะจําแนกความผิดปกติที่สามารถให้การรักษาในระยะ เริ่มแรกได้ การเริ่มให้การรักษาในระยะเวลาที่เหมาะสมเป็นสิ่งสําคัญที่จะทําให้ประสบผลสําเร็จ และอาจจะช่วยลดความจําเป็นในการจัดฟันร่วมกับการผ่าตัดขากรรไกร บทความนี้ได้นําเสนอสาเหตุและการจําแนกลักษณะการสบฟันแบบคลาสทรี ข้อบ่งชี้และข้อห้ามในการให้การรักษาในระยะเริ่มแรก รวมถึงวิธีและระยะเวลาที่เหมาะสม ในการให้การรักษาด้วยเครื่องมือชนิดต่าง ๆ (ว ทันต จุฬาฯ 2548,28:69-80)

วัตถุประสงค์ ศึกษาการเปลี่ยนแปลงของอัตราการไหลและความเป็นกรดด่างของน้ําลายภายหลังการดื่มชาเขียว ชนิดปราศจากนํ้าตาล และชนิดเติมน้ําตาล วัสดุและวิธีการ อาสาสมัครจํานวน 50 คน อายุ 20-22 ปี มีสุขภาพสมบูรณ์และไม่ใช้ยาทุกชนิดอย่างน้อย 3 เดือน ก่อนเข้ารับการทดลอง ในวันแรก ทําการเก็บน้ําลายในระยะพักเป็นเวลา 10 นาที โดยแบ่งการเก็บเป็น 3 ครั้งติดต่อกัน คือ ครั้งแรกเริ่มต้นเก็บน้ําลายครบ 1 นาที ครั้งที่สองเก็บต่อจาก 1 นาทีจนครบ 5 นาที และครั้งที่สามเก็บต่อจาก 5 นาทีจนครบ 10 นาที คํานวณอัตราการไหลของน้ําลายเป็นมิลลิลิตร/นาทีและวัดความเป็นกรดด่างของน้ําลายด้วย เครื่องวัดความเป็นกรดด่าง (pH meter, IQ125, USA.) ในวันที่สอง สาม และสี่ ให้อาสาสมัครดื่มน้ําปราศจาก แร่ธาตุ เครื่องดื่มชาเขียวสําเร็จรูปชนิดปราศจากน้ําตาล และชนิดเติมน้ําตาล ชนิดละ 250 มิลลิลิตร โดยใช้หลอดดูดเก็บน้ําลายและวิเคราะห์ด้วยวิธีการเดิม เปรียบเทียบความแตกต่างของค่าเฉลี่ยของอัตราการไหล และความเป็น กรดด่างของน้ําลายในระยะพัก หลังดื่มน้ํา หลังดื่มชาเขียวชนิดปราศจากน้ําตาล และชาเขียวชนิดเติมน้ําตาล ด้วย การวิเคราะห์ความแปรปรวนทางเดียวที่ระดับความเชื่อมั่นร้อยละ 95 หากพบมีนัยสําคัญจึงทําการทดสอบหาคู่ที่มี ความแตกต่างกันทีละคู่ด้วยวิธีผลต่างอย่างมีนัยสําคัญน้อยที่สุด ผลการศึกษา ค่าเฉลี่ยอัตราการไหลของน้ําลายระยะพัก หลังการดื่มน้ํา หลังการดื่มชาเขียวปราศจากน้ําตาล และ ชาเขียวชนิดเติมนํ้าตาล เท่ากับ 0.40±0.15, 0.41±0.15, 0.48±0.26 และ 0.59±0.21 มล./นาที ตามลําดับ ค่าเฉลี่ย ความเป็นกรดด่างของน้ําลายในช่วงเวลาตั้งแต่ 0-1, 1-5 และ 5-10 นาที ในระยะพักเท่ากับ 7.20±0.40, 7.20±0.38 และ 7.2010.39 หลังดื่มน้ําเท่ากับ 7.21±0.40, 7.2010.38 และ 7.19±0.38 หลังดื่มชาเขียวปราศจากน้ําตาลเท่ากับ 7.71±0.39, 7.42±0.38 และ 7.41±0.49 …

The purpose of this study was to quantitatively compare the fill density of MTA produced by hand condensation and hand condensation with indirect ultrasonic activation. Thirty acrylic block with 30 degree curved canals (group C) and 30 with straight canals (group S) were instrumented to a final apical size of 45 of 0.06 taper crown-down technique. After irrigating with water and drying with paper points, each block was weighed to the nearest 0.0001g with a digital electronic balance. In half of the specimens, chosen at random, the canal first filled with MTA using the hand condensation method (H) then weighed. …
