การติดตามผลการรักษาผู้ป่วยเท็มโพโรแมนดิบิวลาร์ดิสออเดอร์ที่ยุติการรักษา, วันทนี มุทิรางกูร, ประภาพร หอมจันทร์จีรัง, สัจจพร พรรคอนันต์

Chulalongkorn University Dental Journal

วัตถุประสงค์ เพื่อติดตามผลการรักษาและหาสาเหตุที่ผู้ป่วยเท็มโพโรแมนดิบิวลาร์ดิสออเดอร์ (ทีเอ็มดี) ยุติการรักษา วัสดุและวิธีการ ใช้แบบสอบถามทางไปรษณีย์ส่งไปยังผู้ป่วยที่เอ็มดีที่มารับการรักษาที่คณะทันตแพทยศาสตร์จุฬาลงกรณ์มหาวิทยาลัยที่ยุติการรักษาไป 316 คน ผลการศึกษา มีผู้ตอบแบบสอบถามกลับมาจํานวน 77 คน (อัตราการตอบกลับร้อยละ 24.3) เป็นชาย 17 คน (ร้อยละ 22.1) หญิง 60 คน (ร้อยละ 77.9) โดยกลุ่มนี้มี 58 คน (ร้อยละ 75.3) เป็นผู้มารับการรักษาด้วย อาการมากกว่าหนึ่งอย่าง และมีเสียงข้อต่อขากรรไกรขณะอ้า-หุบปากเป็นอาการสําคัญอันดับแรกที่ต้องการ รักษา (ร้อยละ 31.1) ผู้ป่วยทุกคนได้รับการรักษาเชิงอนุรักษ์โดยใช้เฝือกสบฟันเป็นหลัก พบว่า 1. หลังจากยุติการ รักษา ผู้ป่วยยังมีอาการอยู่ 55 คน (ร้อยละ 71.4) โดยอาการที่ยังคงมีไม่แตกต่างอย่างมีนัยสําคัญจากก่อน รักษา ยกเว้นอาการอ้าปากจํากัด (p = 0.013) มีผู้ที่ต้องการรักษาต่อ 43 คน (ร้อยละ 78.2) และผู้ที่ไม่ต้องการ รักษาต่อ 12 คน (ร้อยละ 21.8) 2. เหตุผลที่ทําให้ผู้ป่วยยุติการรักษาไปมีดังนี้ รู้สึกทนอาการที่มีอยู่ได้ (ร้อยละ 37.7) คิดว่าอาการนั้นรักษาไม่หาย (ร้อยละ 32.5) ไม่สะดวกมารับการรักษาในเวลาราชการ (ร้อยละ 31.2) ไม่สะดวกในการเดินทาง (ร้อยละ 26.0) ไม่ได้รับการติดต่อ (ร้อยละ 20.8) และอื่น ๆ สรุป การศึกษานี้ชี้ให้เห็นว่า ผู้ป่วยที่เอ็มดีอาจยังมีอาการอยู่ภายหลังยุติการรักษาไป และมีเหตุผลหลายประการ ที่เป็นสาเหตุของการยุติการรักษา (ว ทันต จุฬาฯ 2550;30:117-28)

ความสัมพันธ์ระหว่างภูมิลําเนาและพื้นที่ ปฏิบัติงานของบัณฑิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย, ชาญชัย ให้สงวน

วัตถุประสงค์ การวิจัยนี้มีวัตถุประสงค์เพื่อประเมินอิทธิพลของภูมิลําเนาที่มีต่อพื้นที่ปฏิบัติงานของบัณฑิตทันตแพทย์แห่งจุฬาลงกรณ์มหาวิทยาลัย วัสดุและวิธีการ ดําเนินการสํารวจกลุ่มตัวอย่างบัณฑิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัยที่สําเร็จการศึกษา ในรอบ 30 ปีที่ผ่านมา จํานวน 1,289 คน ซึ่งถูกคัดเลือกด้วยวิธีสุ่มตัวอย่างแบบเป็นระบบ เก็บข้อมูลเกี่ยวกับ ภูมิลําเนาและพื้นที่ปฏิบัติงานด้วยแบบสอบถามทางไปรษณีย์ มีทันตแพทย์ส่งแบบสอบถามกลับคืนซึ่งเป็น ทันตแพทย์ที่ยังปฏิบัติงานอยู่ 375 คน วิเคราะห์ความสัมพันธ์ระหว่างภูมิลําเนากับพื้นที่ปฏิบัติงานโดยควบคุม ตัวแปรอื่นที่เกี่ยวข้องด้วยสถิติแมนเทล-แฮนเซลไคสแควร์ ผลการศึกษา บัณฑิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัยส่วนใหญ่ร้อยละ 60.5 มีภูมิลําเนาในกรุงเทพมหานคร พบว่าภูมิลําเนามีความสัมพันธ์กับพื้นที่ปฏิบัติงานอย่างมีนัยสําคัญ ทันตแพทย์จากกรุงเทพมหานครและปริมณฑล ส่วนใหญ่ปฏิบัติงานตรงตามภูมิลําเนาของตน ส่วนทันตแพทย์จากภูมิภาคอื่นมีการกระจายตัวแบ่งเป็น 2 กลุ่มใหญ่ กลุ่มหนึ่งเลือกกลับไปปฏิบัติงานในภูมิลําเนาเดิม แต่อีกกลุ่มหนึ่งมุ่งเข้ากรุงเทพมหานคร หลังจากที่ได้ควบคุม อิทธิพลของตัวแปรอื่นที่เกี่ยวข้องแล้ว พบว่าทันตแพทย์จากต่างจังหวัดมีความน่าจะเป็นสูงกว่า 2.79 เท่าตัวที่จะ ปฏิบัติงานนอกพื้นที่จังหวัดภูมิลําเนา เมื่อเทียบกับผู้ที่มีภูมิลําเนาในกรุงเทพมหานครและปริมณฑล สรุป การศึกษานี้บ่งชี้ว่าภูมิลําเนาเป็นปัจจัยกําหนดที่สําคัญประการหนึ่งในการเลือกพื้นที่ปฏิบัติงานของบัณฑิต ทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย แต่การพัฒนานโยบายเพื่อแก้ไขปัญหาการกระจายโดยอิงเกณฑ์ภูมิลําเนา ควรคํานึงถึงปัจจัยอื่นที่เกี่ยวข้องร่วมด้วย (ว ทันต จุฬาฯ 2550;30:103-16)

The efficacy of a novel synthetic antimicrobial peptide (WLBU2) was evaluated against three oral microorganisms (grown planktonically): Streptococcus gordonii, Fusobacterium nucleatum, and Porphyromonas gingivalis. WLBU2 killed all three species, with F. nucleatum being the most susceptible. WLBU2 also reduced the bacterial burden of S. gordonii and F. nucleatum biofilms.

Objective: To compare as-received copper-nickel-titanium (CuNiTi) archwires to those used in patients by means of differential scanning calorimetry (DSC). Also, the thermal or phase properties of 27°C, 35°C, and 40°C CuNiTi archwires were studied to ascertain if their properties match those indicated by the manufacturer.

Materials and Methods: Six wires of 27°C, 35°C, and 40°C CuNiTi were tested as-received, and six each of the 27°C and 35°C wires were examined after use in patients for an average of approximately 9 and 7 weeks, respectively. Traumatic Neuroma Following Sagittal Split Osteotomy Of The Mandible, R. H. Kallal, F. G. Ritto, Luis Eduardo Almeida, D. J. Crofton, G. P. Thomas

A 16-year-old male underwent bilateral sagittal split osteotomy of the mandible to correct a mandibular deficiency. Twenty-one years later, a routine panoramic radiograph revealed a radiolucent lesion on the left side of the mandible. The lesion was biopsied. As the patient did not have symptoms and the lesion was connected to the inferior alveolar nerve, the lesion was not totally excised in order to preserve nerve function. The histological features were consistent with traumatic neuroma, and no further surgical procedure was planned.

“Early” or two-phase orthodontic treatment of Class II malocclusions is a highly debated topic in the orthodontic literature. We report here on a retrospective cephalometric study of patients with Class II, division 1 malocclusions. One group consisted of 32 consecutively treated patients who received “early” treatment with a Fränkel II appliance followed by treatment with full fixed appliance. These subjects were “matched” by sex and cephalometric value, to a subject treated with standard edgewise appliances in a single phase. Matching criteria focused on the bony facial characteristics, notably ANB, NAP, Y Axis, AOBO, FMA, overjet, and overbite. The question was …

Purpose: The objective of this study was to evaluate the osseointegration between plasma sprayed (PS) hydroxyapatite (HA) coated implants and electrophoresis deposited (EPD) nano hydroxyapatite coated implants in an animal model. The quantity of osseointegration was inferred by interfacial strength and percent tissue-contact length.Materials and Methods: Thirty-six cylindrical implants (18 PS and 18 EPD) were placed into dog mandibles. Nine implants from each group were evaluated with pull out testing and histological studies at 12 weeks after implantation. Malocclusions can involve arch-size tooth-size discrepancies that have to be resolved by premolar extractions. Treatment planning in such cases usually involves removing first (P1) or second premolars (P2). Choosing an extraction pattern is a learned skill that incorporates many diagnostic factors. This study compared the amounts of in-treatment tooth movement in two common extraction patterns (4/4 and 5/5) in order to gain insight into the criteria used for differential diagnosis and treatment planning. PURPOSE: The effectiveness of many commercially available composite finishing and polishing devices have been studied and compared. However, no studies have directly compared the effectiveness among the polishing brushes. This study set out to (1) investigate the surface roughness reduction and gloss value increase of two universal dental composites polished by three polishing brushes and two well-studied finishing and polishing systems, and (2) to evaluate resulting surface morphology using scanning electron microscopy (SEM).

Purpose: The aim of this study was to evaluate the reproducibility of mineral density measurement expressed in Hounsfield Units (HU) using a phantom device with known mineral densities and the NewTom 3G Cone Beam Computerized Tomography (CBCT).

Materials and Methods: A phantom device made of 21 HA cylindrical disks with known densities was fabricated. The phantom was scanned in 6, 9, and 12 inch fields of view using a NewTom 3G CBCT. Bacterial lipopolysaccharide (LPS) has been implicated in the pathogenesis of pulpal and periapical diseases. Several studies have evaluated the ability of various medicaments and irrigants used during root canal therapy to chemically detoxify LPS. The purpose of this study was to evaluate the ability of some endodontic sealers to detoxify LPS. This investigation compared the quantities of TNF-α released by human monocytes exposed to LPS or LPS pre-treated with sealers. The clinical success of dental implants is determined mostly by surface properties of implants and interactions with surrounding tissues. Furthermore, clinical success of dental implants is directed by implant surfaces and bone cell responses that promote osseointegration and long-term stability. Hydroxyapatite (HA)-coated Titanium (Ti) implants have been widely used due to its effect on bone response. However, microbial infection is common. This study explores the little understood process of evaluating the performance of department chairs/division heads in dental schools. Specifically, this research aimed to elucidate the methods, processes, and outcomes related to the job performance of department chairs/division heads. Forty-three deans and 306 chairs completed surveys with both close-ended and open-ended questions. In addition, ten deans and ten chairs were interviewed. Results indicate that 80 to 90 percent of department chairs are formally evaluated, although as many as 50 percent may lack job descriptions. Recommended best practices for performance appraisal--such as having at least yearly appraisals, holding face-to-face meetings, and setting …

PURPOSE: The purpose of this study was to measure the utility of various predictors used by the Old Dominion University Gene W. Hirschfeld School of Dental Hygiene baccalaureate degree dental hygiene program in selecting dental hygiene students who are most likely to graduate and be successful in passing the National Board Dental Hygiene Examination (NBDHE). The following factors were examined: grade point average (GPA); science GPA; final grade in various prerequisite courses; final grade in first-year dental hygiene courses; academic setting where prerequisite courses were completed; multiple attempts to achieve a passing course grade; and admissions criteria points (ACP).

PURPOSE: The purpose of this literature review is to document the contributions dental hygienists have made over the past 3 decades to improve the nation's oral health. This historical review encompasses selected literature that acknowledged dental Hygienists' direct involvement in U.S. school-based or school-linked oral health programs from 1970-1999.

This study was conducted to determine the effects of five different finger rest positions, (fulcrums: opposite arch, standard intra-oral, basic extra-oral, cross arch and finger on finger) on the muscle activity of four forearm muscles, (extensor carpi radialis longus, flexor carpi ulnar is, biceps brachii and pronator teres) during a simulated periodontal scaling experience. A convenience sample of 32 consenting right handed senior dental hygiene students who met inclusion criteria participated. The primary purpose of this study was to determine if tooth whitening product use influences oral health interests and values in an adult population. A two-group, randomized, pre-test, multiple post-test design was used for studying 60 adults between the ages of 18 and 60. Persons who use tobacco products, have composite/tooth-colored anterior restorations, less than eight anterior teeth, pregnant or lactating women, or those who have previously used a whitening product other than toothpaste were excluded from the study. This study determined if vital tooth whitening affects oral health-related quality of life (OHRQOL) in adults ≥50-years old, and if tooth whitening promotes individuals to seek regular professional dental care and participation in social activities. Using a two-group, single blind, randomized, pre-test-post-test design, 62 participants were enrolled. The experimental group used a whitening product twice daily for three weeks, while the control group used no whitening products. Tooth color satisfaction was assessed using the Tooth Color Satisfaction Scale and was correlated with overall oral health-related quality of life.
