The vast majority of periodontal pathogens are classified as gram negative bacteria. These bacteria have an outermost membrane consisting of lipopolysaccharide (LPS), a molecule that can act as an endotoxin and elicit an immune response. The lipopolysaccharide structure consists of three major regions: oligosaccharide, core, and lipid A. Oligosaccharide is exposed on the cell surface, and lipid A anchors LPS in the outer membrane. LPS has been known to hinder certain defensive mechanisms of the immune system by secreting products that interfere with signals that promote chemotaxis of leukocytes enabling the pathogens to go unrestricted. Achieving long-term stability after orthodontic treatment is a major challenge for orthodontists. The previous literature shows that relapse occurs at a considerable magnitude. The purpose of the present study was to determine the amount and type of long-term, posttreatment relapse that occurs in a homogeneous group of patients treated by a single experienced orthodontist in a private practice setting. Dental casts from a sample of 30 Class II division 1 Caucasian females treated without extractions were evaluated at pretreatment (mean age = 13.1 years), posttreatment (mean age = 15.9 years) and recall (mean age = 28.3 years) to determine the …

Purpose: The specific aim of this study is to evaluate the trends in dental health care for individuals with ectodermal dysplasia. Methods: This was a cross sectional analysis of subjects recruited through the National Foundation of Ectodermal Dysplasia (NFED). From 1997 to 2000, individuals with ectodermal dysplasia or their caregiver (if the individuals were too young to selfreport) voluntarily completed questionnaires. The questionnaire consisted of 37 items consisting of demographics, ectodermal dysplasia diagnosis, access to dental care, level of dental utilization, and type of dental services received. Purpose: The purpose was to examine the maternal influences on the development of infant oral biofim and dominant bacterial strains of at risk populations. Methods: The study used a cross-sectional design to examine factors influencing biofilm colonization and the identification of bacterial strains transmitted from mother to child. Participants were enrolled in Children’s Health Involving Parents of Greater Richmond (CHIP). Plaque and saliva samples were collected from mothers and their children ages 6-36 months. The colonized oral bacteria strains of the mother infant dyads were then compared. Oral bacterial strain identification was completed using the HOMIM Forsythe microbe identification array. …

Purpose: To assess the effect of dental Medicaid reforms in the state of Virginia on dental plan performance. Methods: This project is a retrospective cohort study of 825,000 dental claims obtained from the Virginia Department of Medical Assistance Services. This cohort includes dental claims for children enrolled in Virginia’s Medicaid program from July 1, 2002 through June 30, 2008. The independent variable was enrollment pre or post policy reform with July 1, 2005 as the period dividing date. The dependent variable was dental benefit utilization measured as the average number of restorative, preventative, and total procedures. Statistical methods include Welch’s …

Teeth are brittle and highly susceptible to cracking. We propose that observations of such cracking can be used as a diagnostic tool for predicting bite force and inferring tooth function in living and fossil mammals. Laboratory tests on model tooth structures and extracted human teeth in simulated biting identify the principal fracture modes in enamel. Examination of museum specimens reveals the presence of similar fractures in a wide range of vertebrates, suggesting that cracks extended during ingestion or mastication. The use of ‘fracture mechanics’ from materials engineering provides elegant relations for quantifying critical bite forces in terms of characteristic tooth …

Purpose: The purpose of this pilot study was to determine if there is a difference in post-operative pain experience for children following dental restorations and/or extractions under general anesthesia (GA), with and without local anesthetic (LA). The alternative hypothesis is that children will experience less post-operative discomfort and soft tissue trauma when using intra-ligamental local anesthetic during the intra-operative time period. Those with physical and mental disabilities are at an increased risk of oral complications such as gingivitis and periodontal disease. In comparison with this risk, this population struggles to access appropriate dental care that accommodates those with disabilities. This thesis reviews the demographics of those with special needs, dental education barriers to working with this population and financial barriers that hinder access to care. An overview of the oral health problems of common disabilities is presented as well as behavioral management techniques that could be easily implemented in the dental office. Our objective was to compare frictional resistance evident in aesthetic archwires to traditional (non-aesthetic) archwires. Methods: Archwires ligated with elasatics to fixed brackets were pulled through these brackets while frictional resistance (in lbf) was measured. Results: There were no confirmed significant differences between the frictional resistance of the aesthetic arch wires compared to the traditional non-coated wires for all wire sizes tested Conclusions: Our data suggests that a sacrifice of clinical performance with these aesthetic archwires as compared to traditional archwires is not likely

On January 7th, 2011 the U.S. DHHS recommended setting the level of fluoride in drinking water to the lowest end of the current optimal range (0.7 to 1.2 ppm). EPA is initiating a review of the maximum amount of fluoride allowed in drinking water based on scientific assessments of health effects from too much fluoride. MCL (Maximum Contaminant Level) is the legal threshold limit on the amount of a substance allowed in public water systems. EPA sets the MCL to address adverse health effects. Current MCL for fluoride is 4 mg/L. The EPA is unsure if they will revise the …
A Comparative Study Of Rotary Instrumentation Of The Maxillary First Premolar Buccal Root Utilizing Cone Beam Computed Tomography, Stephan Zigo

The study objective was to determine cementum-dentin wall thickness along the furcation groove in maxillary bifurcated first premolars after preparation with three successively larger, 0.04 tapered, nickel titanium rotary files. Pre-instrumentation and post-instrumentation imaging was accomplished utilizing Cone Beam Computed Tomography. All data was analyzed using an ANOVA. Instrumentation resulted in a significant reduction in dentin-cementum wall thickness (p < .001). At mid-groove, predicted dentin-cementum wall thickness (95% CI) was equal to or less than the proposed standard (0.50 mm) for apical file sizes 30, 35, and 40 respectively. Instrumentation of the mid-groove in maxillary first premolars reduces dentin-cementum wall thickness to levels that may be insufficient to ensure tooth integrity.

The purpose of this study was to determine the number of rotations to fracture (cyclic fatigue) of the Profile® Vortex™ files (Dentsply Tulsa Dental Specialties, Tulsa, OK) compared to the EndoSequence™ files (Brasseler USA, Savannah, GA) using an in-vitro apparatus simulating a curved canal. Two hundred Profile® Vortex™ files of 25mm length were divided equally into ten groups, one for each of the Profile® Vortex™ files 20/0.04, 20/0.06, 25/0.04, 25/0.06, 30/0.04, 30/0.06, 35/0.04, 35/0.06, 40/0.04, and 40/0.06. The author reflects on the need of legal counsel for the dental students before signing any employment contract in a private dental practice firm. He states that the counsel can save both the students and owners from the problems which he called "mines". He further elaborate these "mines" which include lack of revenue or patients at the work place, compensation scheme causing too much profit for the owner and little or no discussion of the practice's value with the new associate.

A 13-year-old sought treatment for a severely compromised maxillary left central incisor and an impacted fully developed left canine. Extraction of both teeth became necessary. As the key component of the revised comprehensive treatment plan, the right maxillary central incisor was moved into the position of the left central incisor. All other maxillary teeth were moved mesially to close any gaps. Active orthodontic treatment was completed after 34 months. Frenectomy, minor periodontal surgeries, and bonded lingual retainers were used to improve aesthetics and stabilize the tooth positions. The patient was pleased with the treatment outcome. Objective: To analyse the demographics surrounding and the sustainability of a course in Emergency Dental Care and Health Promotion developed and taught by a team of dentists from the United States to refugee camp health-care workers in two long-term refugee camps in Western Tanzania.
Methods: Refugee camp dental patient log books from Mtabila and Nyarugusu camps Kigoma, Tanzania were analysed and demographic data collected on each patient visit from the programme inception in November 2007 until August 2009. Data collection included information relevant to 1961 patient visits. Data were entered into SPSS Statistics 17.0 using the Freq application.

Purpose: The purpose of this study was to determine policies and practices regarding magnification loupes among accredited dental hygiene programs as measured by a 31- item, self-designed questionnaire titled Magnifying Loupes in U.S. Entry Level Dental Hygiene Programs. In addition, the study compared policies among dental hygiene programs in two year versus four year programs in terms of requirements for the use of magnification loupes.

Introduction: The polymerization and acid<–>base reactions in resin<–>modified glass<–>ionomers (RMGI) are thought to compete with and inhibit one another. The objective of this study was to examine the effect of visible light<–>cure (VLC) delay on the polymerization efficiency and orthodontic bond strength of a dual<–>cured RMGI.
Methods: An RMGI light<–>cured immediately, 2.5, 5, or 10 minutes after mixing comprised the experimental groups. The maxillary lateral incisor is the second most frequently missing tooth in the dental arch with clinical management requiring a complex and multidisciplinary treatment approach. It has been suspected that teeth could be smaller in the maxillary or mandibular arches in patients experiencing agenesis of the maxillary lateral incisor, making ideal occlusal relationships and space creation for restoration of the lateral incisor difficult to obtain. Purpose: The purpose of this study was to evaluate the shear bond strength between machined titanium and composite resin using different surface treatments.
Materials and Methods: Titanium (Ti-6Al-4V) specimens were ground with 600 grit SiC paper and randomly divided into 6 groups (n=20/group). Group #1 (Control): samples were sandblasted with 110 µm Al2O3 for 10 sec. Objective: To determine if American Orthodontics' (AO) new, experimental ligature demonstrates less friction in vitro when compared to four other ligatures on the market.
Methods: Four brackets were mounted on a custom metal fixture allowing an 0.018-in stainless steel wire attached to an opposite fixture with one bracket to be passively centered in the bracket slot. The wire was ligated to the bracket using one of five types of ligatures including the low friction test ligatures (AO), conventional ligatures (AO), Sili–TiesTM, Silicone Infused Ties (GAC), Synergy® Low-Friction Ligatures (RMO), and SuperSlick ligatures (TP Orthodontics). Aim: To determine the marginal ridge thickness (MRT) of maxillary incisors in orthodontic patients and the possible correlation between MRT and Bolton Index.
Methods: 120 pre<–>orthodontic treatment dental casts were collected, following the inclusion criteria: Quality pretreatment upper and lower casts, age 10<–>16, full permanent dentition (except 2nd and 3rd molars), no obvious wear/attrition/abrasion, no proximal restorations or crowns.
