Objective: Before a surgical intervention in the mandibular interforaminal area, it is important to have a full understanding of the anatomy of the mandibular anterior segment. The evaluation of anatomical structures, variations and pathologies is performed with radiological imaging methods. Today, cone beam computed tomography (CBCT) is an imaging method which offers all the features necessary for this. In addition to its importance in surgical applications, it is also valuable in terms of anatomical studies thanks to the reliability of its measurements and the fact that it allows for a three-dimensional evaluation. The mental foramen and anterior loop are …

Enigmatic Cervicofacial Actinomycosis Post Radiotherapy - A Case Report, Amanda Phoon Nguyen
Journal of Dentistry Indonesia
Although thought to be uncommon, it is believed that cervicofacial actinomycosis is underreported due to frequent misdiagnosis. It has been called the “most misdiagnosed disease by experienced clinicians”. A wide range of dental procedures or trauma, including tooth eruption, may precipitate actinomycosis by enabling microorganisms to penetrate the deeper tissue layers via an entry point. This infective disease is significant due to its potentially aggressive and locally destructive nature. This case describes a patient diagnosed with cervicofacial actinomycosis. Her history prior to this was significant for proliferative verrucous leukoplakia, and T4aN0M0 (AJCC 7th Edition) squamous cell carcinoma of the right …

The diagnosis and prognosis of teeth with endo-perio lesions present a major challenge for dentists. Proper diagnosis is critical in determining treatment and the long-term prognosis. However, treating complex endodontic and periodontal lesions is one of the most common challenges in current clinical practice. The coexistence of pulpal and periodontal tissue damage may complicate diagnosis and further influence dental prognosis. This article consists of a case report evaluating the efficacy of xenografts and resorbable membrane pericardium in treating bifurcation lesions associated with periodontal lesions of the left mandibular first molar. A 22-year-old female patient was referred by conservative dentistry with …

Machine Learning For Automated Cervical Vertebral Maturation Assessment On Lateral Cephalograms, Nuo Dong
Theses & Dissertations
Introduction: Skeletal maturation age is an important guide in growing adolescent patients to determine the proper time to start orthodontic treatment. The most practical and commonly used method to determine skeletal age in today’s clinical practice is the Cervical Vertebral Maturation (CVM) method [2]. Based on the morphology of the cervical vertebrae, six maturational stages can be determined – designated CS 1 to CS 6. The main drawback of this technique is the time required to determine CS number. Objective: In this research a machine learning (ML) algorithm to automatically determine CVM staging from the lateral cephalometric radiograph was developed. …

Three-Dimensional Analysis Of Condylar Displacement After Orthognathic Surgery In Patients With Facial Asymmetry, Teresa Bae
Theses & Dissertations
Facial asymmetry is apparent when there is a clinically significant variation between the two halves of the face. There can be varying degrees of facial asymmetry and mild asymmetry can be treated by orthodontics alone. For severe facial asymmetry a combination of orthodontics and orthognathic surgery may be necessary. The condyle must be in a stable position following surgery to have functional and structural stability and prevent relapse. This research proposal is to perform a retrospective CBCT study on the displacement of the condyle following orthognathic surgery in patients with facial asymmetry. The CBCTs were obtained from Taipei Veterans General …

Pacific Journal of Health
Over three years have passed since the beginning of the COVID-19 pandemic. During these times we have experienced significant changes in all areas of our life. In healthcare, more attention was focused on COVID-19 than other concerns. An unexpected consequence of the pandemic was a silent progression of an increased spread of multi-resistant pathogens. While antibiotic use during the pandemic varied across healthcare settings, antibiotics were prescribed for COVID-19 patients, even though antibiotics are known not to be effective against viruses.1 In dentistry, antibiotics were prescribed to prevent potential dental infections, since many dental offices were not open. Antimicrobial …

Negative Pressure Wound Therapy For Complex Oral Wounds, Mykaihla Sternick, James Gates, Allen Champion, Andrew Yampolsky
Department of Oral and Maxillofacial Surgery Faculty Papers
Negative pressure wound therapy provides a nonsurgical treatment option for many types of complex wounds. This therapy utilizes the vacuum-assisted closure device, which decreases air pressure, removes fluid that accumulates within the wound, and aids to facilitate changes that promote healing. Despite the increased use of negative pressure wound therapy in the head and neck region, there is substantially less data available on the management of transoral vacuum-assisted wound closure. Herein, we present a case of a novel approach for the creation and use of a transoral wound vac for a patient with a refractory orocutaneous wound in the setting …

Genetically Engineering Gingipains For High Yield Large Scale Purification., Andrew E. Fuchs
Electronic Theses and Dissertations
Porphyromonas gingivalis is a keystone oral pathogen in the progression of periodontal disease and has been implicated in the etiology of many other systemic human diseases. P. gingivalis produces primary virulence factors termed the gingipains, a group of cysteine proteases. There is a high demand for high purity gingipains for downstream in vitro studies where P.g. is implicated, both oral and systemic. Gingipain production candidate mutant P.g. strains were generated to isolate each gingipain for large scale purification and the purified gingipains characterized. Two mutants were determined not to be viable for large scale purification of gingipains, each for different …

Objectives
Periodontal disease is multifactorial in its aetiology, which encompasses biopsychosocial contributors, including psychological stress. Gastrointestinal distress and dysbiosis have been associated with several chronic inflammatory diseases yet have rarely been investigated with respect to oral inflammation. Given the implications of gastrointestinal distress on extraintestinal inflammation, this study aimed to evaluate the potential role of such distress as a mediator between psychological stress and periodontal disease.
Methods
Utilizing a cross-sectional, nationwide sample of 828 adults in the USA generated via Amazon Mechanical Turk, we evaluated data collected from a series of validated self-report psychosocial questionnaires on stress, gut-specific anxiety around …

Root And Canal Morphology Of Mandibular Second Molars In An Egyptian Subpopulation: A Cone-Beam Computed Tomography Study, Shehabeldin Mohamed Saber Prof.Dr.
Dentistry
Background: This study was using Cone beam computed tomography (CBCT) to examine the anatomical variations in mandibular second molars in an Egyptian sub-population. Methods: A total of 350 CBCT images (215 females and 135 males, aged 15–65 years) of mandibular second molars were evaluated. Samples were evaluated in terms of: number of roots, number of root canals, roots’ cross section as well as prevalence and configurations of C-shaped canals. Statistical analysis was done to highlight differences between different categories and their prevalence among genders (significance level was set at p < 0.05). Data were presented as frequency and percentage values and were analyzed using chi square test followed by pairwise comparisons utilizing multiple Fisher’s exact tests with Bonferroni correction. The significance level was set at p < 0.05 within all tests. Results: Of the 350 mandibular second molars evaluated, 87.2% were non-C-shaped while 12.8% were C-shaped with no gender-based statistically significant differences (χ2 = 0.19, p = 0.656). Most samples had three root canals (80%) followed by two (16%), then one (3.2%), and finally four (0.8%) root canals. Among the non-C-shaped molars, presence of two roots was most common (83.4%) followed by presence of a single root (16.2%), and only one sample (0.2%) had three roots, and this had no correlation with gender (χ2 = 1.86, p = 0.431). In the mesial roots Type IV Vertucci was the most common configuration found (68.8%), while Type I was the most prevalent in the distal roots (91.8%). The long oval configuration was the most commonly found cross section in mesial roots while “oval” was the most prevalent in distal roots. Conclusion: Egyptian sub-population shows highly variable morphological features in mandibular second molars, hence, CBCT is highly recommended on case-to-case conditions.

Dentistry And The Law: Requiring Proof Of Adoption/Retaining Orthodontic Models, Dan Schulte Jd
The Journal of the Michigan Dental Association
In this Dentistry and the Law column, a dentist seeks guidance on handling a situation where racial differences between a child and an accompanying adult raised concerns during an examination, leading the dentist to request proof of adoption. The response clarifies that asking for proof of adoption is not illegal, citing relevant Michigan adoption statutes. The column advises adopting an office policy, emphasizing human trafficking training. Additionally, a dentist inquires about retaining orthodontic models and X-ray films. The answer recommends assuming orthodontic models are part of dental records, subject to a 10-year retention period. Patients can be given their models, …

The Journal of the Michigan Dental Association
Managing patients with a history of intravenous bisphosphonates and antiresorptive medications poses challenges in clinical decision-making. Extractions are generally discouraged in these patients due to the risk of medication-related osteonecrosis of the jaw (MRONJ). This case study presents an 89-year-old female with metastatic breast cancer on Xgeva, facing a non-restorable tooth (#30). A collaborative approach involving the general dentist, endodontist, and oral surgeon employed endodontic therapy, crown amputation, and soft tissue advancement, avoiding extraction. The technique, akin to coronectomy, mitigates MRONJ risk by preserving bone integrity. Follow-up assessments indicate successful treatment without complications, highlighting the efficacy of this approach in …

Tooth eruption is a pivotal milestone for children's growth and development. This process involves with the formation of the tooth root, the periodontal ligament (PDL) and the alveolar bone, as the tooth crown penetrates the bone and gingiva to enter the oral cavity. This review aims to outline current knowledge of the adverse dental effects of antiresorptive medications. Recently, paediatric indications for antiresorptive medications, such as bisphosphonates (BPs), have emerged, and these agents are increasingly used in children and adolescents to cure pathological bone resorption associated with bone diseases and cancers. Since tooth eruption is accompanied by osteoclastic bone resorption, …
