The purpose of this investigation was to determine if there is a difference in the type of microorganisms found in oral sites classified as gingival health, mild-moderate periodontitis, or severe periodontitis as measured by the DMDx* Test. A purposive sample of one hundred oral sites was selected from adult patients. Subjects were excluded from the study based on: the presence of any systemic disorders, communicable diseases, diabetic conditions, blood dyscrasias, congenital heart disease, rheumatic heart disease, pregnancy, history of periodontal surgery, prosthetic joint replacement, hypertensive heart disease, or any medications that might alter the gingival tissues (e.g. antibiotics, hormonal therapy, …

Room Temperature Aging Of Pd-Cu-Ga Dental Alloys, William A. Brantley, Zhuo Cai, Qiang Wu, Alan B. Carr, John C. Mitchell

Specimens of three Pd-Cu-Ga dental alloys cast five years ago and subsequently stored at room temperature were reexamined and observed to have lower amounts of the eutectic constituents in the near-surface region than originally present, along with other microstructural changes. This previously unreported room temperature aging behavior of these alloys is attributed to the presence of high-diffusivity paths in the non-equilibrium ascast eutectic structures and to the essential role of the surface for the vacancy diffusion mechanism. These results may have important clinical significance for the ill vivo corrosion resistance and long-term biocompatibility of the Pd-Cu-Ga alloys.

ศึกษาถึงความเจ็บปวดจากวิธีการฉีดยาชาแบบอินฟิลเตรชันที่เพดานปากสามตําแหน่งจากประชากร 200 ราย พบว่าบริเวณส่วนหน้าของเพดานปาก การแทงเข็มฉีดยาให้แนวเข็มขนานกับแนวแกนของกระดูกเพดาน ความเจ็บปวดจะน้อยกว่าการแทงเข็มตั้งฉากกับแนวแกนของกระดูกเพดานอย่างมีนัยสําคัญทางสถิติ (P≤0.05) บริเวณส่วนหลังของเพดานปาก การแทงเข็มฉีดยาให้แนวเข็มตั้งฉากกับแนวแกนของกระดูกเพดาน ความเจ็บปวดจะน้อยกว่าการแทงเข็มขนานกับแนวแกนของกระดูกเพดานอย่างมีนัยสําคัญทางสถิติ (P≤0.05) ส่วนบริเวณส่วนกลางของเพดานปากไม่มีความแตกต่างอย่างมีนัยสําคัญทางสถิติ (P≥0.05) ระหว่างการแทงเข็มฉีดยาให้แนวเข็ม ขนานและตั้งฉากกับแนวแกนของกระดูกเพดาน การหันส่วนปาดลาดของปลายเข็มออกจากตัวฟัน ความเจ็บปวด จะมีความแตกต่างกันทางคลินิก โดยความเจ็บปวดจะน้อยกว่า คิดเป็นร้อยละ 36, 64 และ 56 ตามลําดับ เมื่อฉีดยาชาบริเวณเพดานปากตรงตําแหน่งปลายรากฟันของฟันตัดบนข้าง ฟันกรามน้อยบนที่ 1 และฟันกรามบน ซีที่ 1 แต่ไม่มีความแตกต่างอย่างมีนัยสําคัญทางสถิติ (P ≥ 0.05)

ยาที่กระตุ้นให้เกิดรอยโรคคล้ายไลเคน พลานัส, กอบกาญจน์ ทองประสม, กนิษฐ์ วงศ์รุ่งโรจน์กิจ, มนวิภา ศรีวงศ์จรรยา

ไลเคน พลานัส เป็นรอยโรคเรื้อรังที่ยังไม่ทราบสาเหตุแน่นอน การใช้ยารักษาโรคทางระบบหลายชนิด สามารถกระตุ้นให้เกิดรอยโรคในช่องปากและผิวหนัง ซึ่งมีลักษณะทางคลินิกและทางพยาธิวิทยาคล้ายไลเคน พลานัสได้ จากการศึกษาผู้ป่วยไลเคน พลานัส ในจํานวน 81 ราย โดยแบ่งผู้ป่วยออกเป็น 2 กลุ่ม คือ กลุ่มที่ได้รับยาและไม่ได้รับยารักษาโรคทางระบบ พบว่าผู้ป่วยส่วนใหญ่เป็นเพศหญิง ช่วงอายุที่พบบ่อยประมาณ 41-60 ปี บริเวณที่ตรวจพบรอยโรคมากที่สุดคือบริเวณกระพุ้งแก้ม รองลงมาคือ เหงือกกับบริเวณรอยต่อระหว่างกระพุ้งแก้มกับเหงือกริมฝีปาก เพดานปาก ลิ้น และพื้นช่องปากตามลําดับ จากการเปรียบเทียบกลุ่มผู้ป่วยที่ได้รับยาและไม่ได้รับยารักษาโรค พบว่าผู้ป่วยที่ใช้ยารักษาโรคมีความสัมพันธ์กับชนิดของการเกิดไลเคน พลานัสในช่องปากอย่างมีนัยสําคัญทางสถิติ (P<0.025) คือผู้ป่วยที่ได้รับยารักษาโรคจะมีโอกาสเป็น ไลเคน พลานัส ชนิดอีโรซีฟ มากกว่าชนิดไม่อีโรซีฟ โดยพบว่าผู้ป่วยมักใช้ยากลุ่ม antihypertensive มากที่สุด รองลงมาคือ NSAIDS, hypoglycemic และยาอื่น ๆ ตามลําดับ

จากการศึกษาต่าง ๆ ที่ผ่านมาพบว่า ปริมาณและระยะเวลาที่น้ำตาลตกค้างในช่องปากมีความสัมพันธ์กับอุบัติการณ์การเกิดโรคฟันผุ การศึกษาส่วนใหญ่กระทําโดยการหาปริมาณน้ำตาลในน้ำลายภายหลังการอมสาร ละลายกลูโคสแล้วบ้วนทิ้งด้วยวิธีเทียบสีซึ่งเป็นวิธีที่ยอมรับโดยทั่วไปว่ามีความถูกต้อง แม่นยําสูง อย่างไรก็ตามวิธี เทียบสีจําเป็นต้องใช้เครื่องมือยุ่งยาก คือ เครื่องเทียบสีและต้องมีวัสดุอุปกรณ์อื่นอีกหลายอย่าง ทําให้ไม่สะดวกในการนําไปศึกษาภาคสนาม การทดลองครั้งนี้จึงนําแผ่นพลาสติกตรวจน้ำตาลซึ่งสามารถบอกปริมาณวิเคราะห์ของน้ำตาลในน้ำลายได้ โดยสังเกตการเปลี่ยนสีด้วยตาเปล่ามาทดลองใช้ จากการหาปริมาณกลูโคสในน้ำลายที่เวลา 3,5,7,9 และ 11 นาที หลังการอมสารละลายกลูโคสความเข้มข้น 50% นาน 2 นาที พบว่า ปริมาณน้ำตาลในน้ำลายที่ตรวจวิเคราะห์ได้ด้วยแผ่นพลาสติกตรวจน้ำตาลไม่แตกต่างจากปริมาณที่วิเคราะห์ได้ด้วยวิธีเทียบสีอย่างมีนัยสําคัญทางสถิติ (P = 0.05) จากข้อมูลครั้งนี้กล่าวได้ว่า แผ่นพลาสติกตรวจน้ำตาลใช้แทนวิธีเทียบสีได้ สามารถนําไปใช้ศึกษาการ กําจัดน้ำตาลจากน้ำลายภาคสนามเป็นประโยชน์ในการพยากรณ์อุบัติการณ์การเกิดโรคฟันผุในชุมชน และการให้ทันตกรรมป้องกันต่อไป

การศึกษานี้มีวัตถุประสงค์ เพื่อสํารวจความพึงพอใจของประชาชนที่มีต่อการบริการทันตกรรมของโรงพยาบาลระยอง และต้องการทราบข้อมูลพื้นฐานสําหรับเปิดคลินิคผู้ป่วยทันตกรรมนอกเวลาราชการของโรงพยาบาลระยอง โดยเลือกกลุ่มตัวอย่างที่มารับบริการและประชาชนทั่วไป โดยใช้แบบสอบถาม ถามกลุ่มตัวอย่าง จํานวน 336 ราย ในช่วงระยะเวลา มกราคม-พฤษภาคม 2537 ผลการศึกษาพบว่ากลุ่มตัวอย่างมีทัศนคติในแง่บวก ต่อการบริการทันตกรรมของโรงพยาบาลระยองสูงถึง ร้อยละ 81.3 ซึ่งกลุ่มตัวอย่างประมาณครึ่งหนึ่งเคยไปใช้บริการทั้งภาครัฐและเอกชน เมื่อศึกษาหัวข้อของความพึงพอใจของกลุ่มตัวอย่างที่มีต่อการบริการทันตกรรมของโรงพยาบาลระยอง ส่วนใหญ่มีความพึงพอใจระดับดี แต่มีข้อที่ความพึงพอใจระดับต่ำ คือ ความยุ่งยากในขั้นตอนของการมารับบริการ และเวลาที่สูญเสียในการรอรับบริการ เมื่อเปรียบเทียบคะแนนความพึงพอใจของปัจจัย ที่มีระดับนัยสําคัญ (alpha = 0.05) คือ กลุ่มตัวอย่างมีระดับการศึกษาน้อย มีรายได้ต่ำ นอกจากนี้ยังพบว่า กลุ่มตัวอย่างที่มีรายได้ต่ำจะเลือก เข้ารับบริการของรัฐมากกว่า และกลุ่มตัวอย่างที่มีรายได้สูง จะเลือกรับบริการทั้งภาครัฐและเอกชน ผลจากการศึกษาครั้งนี้ สามารถนํามาเป็นแนวทางที่จะนํามาเปิดการบริการทันตกรรมนอกเวลาของโรงพยาบาลระยองได้ ผู้ที่คาดว่าจะมาใช้บริการส่วนใหญ่เป็นข้าราชการเพศหญิง

ถุงน้ำเดนเจอรัสเป็นถุงน้ำที่มีจุดกําเนิดที่เกี่ยวกับการเกิดของฟันโดยลักษณะของถุงน้ำจะคลุมอยู่รอบตัวฟันที่ยังไม่ขึ้น หรือในฟันที่กําลังจะโผล่ขึ้นมาในช่องปาก โดยที่ตัวฟันนั้นเจริญเติบโตเต็มที่เรียบร้อยแล้ว บทความนี้เป็นรายงานผู้ป่วย 2 ราย ที่มีถุงน้ำขนาดใหญ่ โดยผู้ป่วยรายแรกมีถุงน้ำเดนเจอรัสที่เกิดจากฟันเกินเบียดลึกเข้าไปในโพรงอากาศแม็กซิลล่า ข้างขวาบริเวณฟันกรามน้อยซี่ที่หนึ่งทําให้ใบหน้า 2 ข้างไม่เท่ากัน ส่วนผู้ป่วยรายที่ 2 มีถุงน้ำเดนติเจอรัสขนาดใหญ่ที่เกิดจากฟันคุดกรามล่างซี่สุดท้ายในกระดูกขากรรไกรล่างข้างขวาทําให้มีอาการชาบริเวณริมฝีปากล่างข้างขวา นอกจากนั้นยังได้รายงานถึงลักษณะอาการทางคลีนิก ลักษณะภาพถ่ายรังสี ลักษณะทางจุลพยาธิวิทยา การรักษาและการพยากรณ์ของถุงน้ำชนิดนี้ด้วย

การเปรียบเทียบผลของยาไอบูโปรเฟนกับยาพาราเซตตามอลเกี่ยวกับอาการปวดบวมและอ้าปากจํากัดภายหลังการผ่าตัดฟันคุดของฟันกรามล่างซี่ที่สาม, ชัยวัฒน์ ปิ่นน้อย, พรรณี เอื้อสุวรรณา, พรรณทิพ ธรรมภาณ

ศึกษาผลของยาไอบูโปรเฟน 800 มิลลิกรัม เปรียบเทียบกับยาพาราเซทตามอล 1000 มิลลิกรัม เกี่ยวกับ การระงับอาการปวด บวม และการอ้าปากจํากัดภายหลังการผ่าตัดฟันคุดของฟันกรามล่างซี่ที่สาม โดยทําการวิจัยในผู้ป่วย 26 ราย เป็นชาย 7 คน หญิง 19 คน อายุอยู่ระหว่าง 17 - 21 ปี แบ่งออกเป็น 2 กลุ่ม ในกลุ่มที่หนึ่ง ผู้ป่วย 12 คน ได้รับยาไอบูโปรเฟน มีระดับความยากในการถอนฟันเท่ากับ 1.94 ± 0.50 ใช้เวลาในการถอนฟัน เฉลี่ย 17.66 ± 4.80 นาที ในกลุ่มที่สอง ผู้ป่วย 14 คน ได้รับยาพาราเซทตามอล มีระดับความยากของการ ถอนฟันเท่ากับ 1.67 ± 0.41 ใช้เวลาในการถอนฟันเฉลี่ย 15.5 ± 5.06 นาที ระดับความยาก และระยะเวลา เฉลี่ยที่ใช้ในการถอนฟันทั้ง 2 กลุ่ม ไม่แตกต่างกันอย่างมีนัยสําคัญทางสถิติ (P > 0.01) จากผลการวิจัยพบว่า อาการบวมและการอ้าปากจํากัดของทั้ง 2 กลุ่ม ไม่มีความแตกต่างกันอย่างมีนัยสําคัญทางสถิติ (P > 0.01) แต่พบว่ายาไอบูโปรเฟน สามารถลดอาการปวดภายใน 24 ชั่วโมง หลังการผ่าตัด ได้ดีกว่ายาพาราเซทตามอลอย่างมีนัยสําคัญทางสถิติ (P < 0.01) ไม่พบอาการแทรกซ้อนของยาเกิดขึ้นตลอดการวิจัย

The aerospace, energy, and chemical industries have benefitted from favorable applications of titanium and titanium alloys since the 1950's. Only about 15 years ago, researchers began investigating titanium as a material with the potential for various uses in the dental field, mainly because of its proven biocompatibility and other attractive properties. The present review examines updated information on the use of titanium in a major area of dentistry. Titanium-based alloys used for casting, casting equipment and procedures, casting investment materials, and porcelain veneering are discussed. A murine monoclonal antibody (MAb F-91-55) raised against the complex of soluble CD4 and human immunodeficiency virus type 1 (HIV-1) gp120 had previously been found to inhibit syncytium formation without inhibiting the interaction of CD4 with gp120, and its binding site was localized within the first two domains (D1/D2) of CD4. We investigated whether this antibody inhibited the infectivity of HIV-1 in the CD4+ T cell lines A3.01, Sup-T1 and H9. Mycobacterium avium complex (MAC) causes serious opportunistic infections in AIDS patients. Previous studies with MAC-infected beige mice have indicated that weekly administration of liposome-encapsulated streptomycin can reduce significantly the CFU in the liver and spleen. We examined whether streptomycin encapsulated in recently developed sterically stabilized liposomes with prolonged circulation times would have a therapeutic effect in this animal model. Purpose:
To investigate the effect of a baking soda-hydrogen peroxide (0.75%) dentifrice on wound healing, plaque formation, gingival inflammation, wound healing, patient comfort, probing depth, and clinical attachment level following gingival flap surgery.
Materials and Methods:
A randomized, double-blind crossover study involving 25 patients requiring bilateral maxillary gingival flap surgery was completed. The effects of twice daily brushing with a baking soda-hydrogen peroxide dentifrice (Mentadent) or a placebo dentifrice (Crest) were observed over a 28-day post-surgical period.
