Epistemic injustice, the theory of unfairness related to knowledge, is a useful framework for understanding the ways in which historic and ongoing marginalization and stereotypes have shaped the ways that people with intellectual and developmental disabilities (IDD) have been impacted by the COVID-19 pandemic. We conducted a scoping review of the literature and divided findings into physical health (cases, hospitalization, and death) and psychosocial outcomes (access to services, mental health symptoms, community participation, etc.). ‘Best interests’ is a key ethical norm for children’s medical decision-making. While intended to guide parents and healthcare practitioners (HCPs) in their decisions, it has been criticized for its ambiguity and inconsistent application. In the context of childhood vaccination, ‘best interests’ faces unique challenges due to parental vaccine hesitancy, their preventative nature, and their dual benefits to individuals and the public. I employed critical interpretive review (CIR) methodology to analyze 58 empirical and non-empirical publications on children’s best interests and healthcare decisions, including vaccination. One barrier to participating in clinical research is that patients with low literacy skills (1 in 5 US adults) may struggle to understand the informed consent document (ICD). Writing consents using health literacy and plain language guidelines including simplified syntax and semantics can increase understandability and facilitate inclusivity of research populations with literacy challenges. Our study aim was to evaluate a simplified ICD for understandability while considering factors known to relate to comprehension (reading skills and working memory). Researchers are rapidly developing and deploying highly portable MRI technology to conduct field-based research. The new technology will widen access to include new investigators in remote and unconventional settings and will facilitate greater inclusion of rural, economically disadvantaged, and historically underrepresented populations. Brühlmann and colleagues recently examined the effects of “warning statements” on insufficient effort responding (IER) in crowdsourced research, specifically Amazon’s Mechanical Turk (MTurk). Among other findings, it showed that passively reading a warning statement did not significantly reduce careless responding relative to a control condition. This brief essay discusses the context around IER on crowdsourced data collection platforms, finding Brühlmann’s contribution valuable. It then points out how crowdsourced studies by U.S. researchers using mechanisms like attention checks to control for IER plausibly should include warning statements regardless of their effect on IER due to federal research regulations around payment transparency.

Clinical ethics consultations (CECs) require documentation. There is a paucity of literature and resources to guide clinical ethicists in documenting consults. High quality documentation encourages transparency between patients and clinicians, while keeping interdisciplinary team members knowledgeable about evolving care plans. Opiate overdose is an unfortunately common risk factor for suicide; suicide attempts accounted for an estimated 30% of fatal opioid overdoses during the pandemic. As a result, the Rowan School of Osteopathic Medicine Department of Psychiatry developed an Integrated Mental Health Awareness and Suicide Prevention program targeted at members of the community and frontline workers to increase awareness and provide brief interventions, which demonstrated an improvement of 89% in our English-speaking attendees.
