Among many of the innovative changes in nursing education, high-fidelity simulation is expanding roles and providing students with new perspectives. The purpose of this quantitative, causal-comparative, posttest only, nonexperimental, between-groups research design study was to determine whether students in active supervisor (observer) roles had an effect on associate degree nursing (ADN) students’ self-perceived knowledge, skills, and attitudes in comparison to students in active participant roles. A convenience sample of second-semester nursing students and third-semester nursing students enrolled in an ADN program in the 2023 academic year was utilized for this study. Objectives: The purpose of this study was to investigate the impacts of using scaffolded clinical simulations on nursing students’ confidence in clinical reasoning, clinical judgment, and critical thinking skills. Next, we also attempted to gain insights into students’ perceptions of the benefits of scaffolded clinical simulations.

Method: We used a mixed-methods research design to investigate the impacts of using scaffolded clinical simulations on 133 second-semester baccalaureate nursing students’ confidence in clinical reasoning, clinical judgment, and critical thinking skills.

The literature review found suspension of disbelief (SOD) in clinical simulation heavily weighted on educators alone within high-fidelity environments. The project examined a co-created narrative background story applied to a simulated patient’s clinical profile to determine achieving an improved connectedness toward the simulated patient leading to enhanced SOD and enhanced levels of learning and reaction. The aim of this study was to compare nursing students experiencing simulations (SIM group) (n=25) versus clinical placement (CPG) (n=55) in perinatal care. Questionnaires on satisfaction, self-confidence and perceived support were completed by both groups. Knowledge acquisition was assessed using standardized course evaluation. The Student’s t-test showed that differences between groups were not statistically significant for knowledge acquisition and satisfaction, while they were statistically significant for self-confidence and perceived support with higher scores in the CPG group. The American Association of Colleges of Nursing (AACN) (2019) reported in 2018-2019, U.S. nursing schools turned away more than 75,000 qualified applicants from baccalaureate and graduate nursing programs due to insufficient numbers of faculty and clinical sites. Two-thirds of the nursing schools related a shortage of nursing faculty and/or clinical preceptors as a reason for not admitting qualified applicants (AACN, 2019).

Transfusions are the most common procedure that hospitalized patients undergo. One of the risks associated with a transfusion is a transfusion related adverse event (TAAE). Transfusion associated adverse events can occur in any patient receiving a transfusion. Some patients are at risk of certain TAAEs (e.g., heart failure patients at risk of transfusion associated circulatory overload) while other events (e.g., allergic reactions) cannot always be anticipated. Evidence-based remediation options are limited for nursing students who fail their clinical competency evaluations. Scholarly literature provides a paucity of studies related to the use of simulation-based technology to remediate nursing students. The research question focused on the difference in the initial competency demonstration evaluation scores of associate degree nursing students compared to the reevaluation scores after remediation with simulation-based technology. Benner's novice to expert and Kolb's experiential learning theories were used to explain how nurses acquire and develop skills.
