The healthcare industry is one of the most praised industries in the US due to its lifesaving capabilities and immersive care. Consumers regularly report that are satisfied with the care they receive from their physicians. Conversely, they also report frustrations and confusions associated with the costs of such care. In extreme cases, this frustration can turn to financial ruin as medical debt is the leading cause of bankruptcy in the US. Objective:
The objective of the current study was to determine if patients of a large health care system in Detroit who self-identify as food insecure live further away from healthy grocery stores compared with food secure patients. Second, we explored whether food insecurity and distance to healthy grocery stores are related to ecological measures of vehicle availability in the area of residence.

Design:
A secondary data analysis that uses baseline data from a pilot intervention/feasibility study.

Setting:
Detroit, Michigan, USA.

We discuss the problem of combining the conflicting objectives of equity and utilitarianism, for social policy making, in a single mathematical programming model. The definition of equity we use is the Rawlsian one of maximizing the minimum utility over individuals or classes of individuals. However, when the disparity of utility becomes too great, the objective becomes progressively utilitarian. Such a model is particularly applicable not only to health provision but to other areas as well. Building a mixed-integer/linear programming (MILP) formulation of the problem raises technical issues, because the objective function is nonconvex and the hypograph is not MILP representable …
