Proteomics is a growing field that offers insights into various aspects of disease processes and therapy responses. Within the field of orthopedics, there are a variety of diseases that have a poor prognosis due to a lack of targeted curative therapy or disease modifying therapy. Other diseases have been difficult to manage in part due to lack of clinical biomarkers that offer meaningful insight into disease progression or severity. As an emerging technology, proteomics has been increasingly applied in studying bone biology and an assortment of orthopedics related diseases, such as osteoarthritis, osteosarcoma and bone tumors, osteoporosis, traumatic bone injury, …

Medical-legal partnership (MLP) integrates the unique expertise of lawyers into collaborative clinical environments. MLP teams meet the needs of individual patients while also detecting structural problems at the root of health inequities and advancing solutions at the institutional, community, and system levels. Yet MLPs today operate in limited settings and survive on scant budgets. Expanding their impact requires secure funding. Financing MLPs as health care can do the following: (1) help address inequity at the point of care; (2) enable expert diagnosis and treatment of nonmedical drivers of health; (3) enhance team-based practice in health care organizations; (4) offer another …

Background: The epigenome, the set of modifications to DNA and associated molecules that control gene expression, cellular identity, and function, plays a major role in mediating cellular responses to outside factors. Thus, evaluation of the epigenetic state can provide insights into cellular adaptions occurring over the course of disease.
Methods: We performed epigenome-wide association studies of primary sclerosing cholangitis (PSC) and primary biliary cholangitis (PBC) using the Illumina MethylationEPIC Bead Chip.
Results: We found evidence of increased epigenetic age acceleration and differences in predicted immune cell composition in patients with PSC and PBC. Epigenetic profiles demonstrated differences in predicted protein …

Objective: Survival analysis is widely utilized in healthcare to predict the timing of disease onset. Traditional methods of survival analysis are usually based on Cox Proportional Hazards model and assume proportional risk for all subjects. However, this assumption is rarely true for most diseases, as the underlying factors have complex, non-linear, and time-varying relationships. This concern is especially relevant for pregnancy, where the risk for pregnancy-related complications, such as preeclampsia, varies across gestation. Recently, deep learning survival models have shown promise in addressing the limitations of classical models, as the novel models allow for non-proportional risk handling, capturing nonlinear relationships, …

Traumatic, life-threatening events are experienced commonly among the general U.S. population, yet Black individuals in the United States (i.e., Black Americans) exhibit higher prevalence rates of posttraumatic stress disorder (PTSD) and more severe symptoms than other populations. Although empirical research has noted a range of symptom patterns that follow traumatic injury, minimal work has examined the role of racial discrimination in relation to PTSD symptom trajectories. The current study assessed racial discrimination and PTSD symptom trajectories at 6 months postinjury across two separate samples of traumatically injured Black Americans (i.e. emergency department (ED)–discharged and hospitalized). Identified PTSD symptom trajectories largely …

Trichotillomania (TTM) is associated with impairments in response inhibition and cognitive flexibility, but it is unclear how such impairments relate to treatment outcome. The present study examined pre-treatment response inhibition and cognitive flexibility as predictors of treatment outcome, change in these domains from pre-to post-treatment, and associations with TTM severity. Participants were drawn from a randomized controlled trial comparing acceptance-enhanced behavior therapy (AEBT) to psychoeducation and supportive therapy (PST) for TTM. Adults completed assessments at pre-treatment (n = 88) and following 12 weeks of treatment (n = 68). Response inhibition and cognitive flexibility were assessed using the Stop …

Research evidence in the 21st century strongly indicates that prisons are inherently harmful environments in which to try to support rehabilitation, because of their destructive impact on identity, self-actualisation, mental health and social relationships. Yet HM Prison and Probation Service (HMPPS) continues to propose that prisons can be legitimate sites of rehabilitation as well as centres of punishment. This article begins by interrogating this proposition by looking at research conducted in an adult male prison. By specifically focusing on the interpersonal conflicts and tensions that the men experience in their daily lives, the research highlights the ‘relational’ and ‘self-actualising’ harms …

Successful treatment of glioblastoma multiforme (GBM), an aggressive form of primary brain neoplasm, mandates the need to develop new therapeutic strategies. In this study, we investigated the potential of PBI-05204 in targeting GBM stem cells (GSCs) and the underlying mechanisms. Treatment with PBI-05204 significantly reduced both the number and size of tumor spheres derived from patient-derived GSCs (GBM9, GSC28 and TS543), and suppressed the tumorigenesis of GBM9 xenografts. Moreover, PBI-05204 treatment led to a significant decrease in the expression of CD44 and NANOG, crucial markers of progenitor stem cells, in GBM9 and GSC28 GSCs. This treatment also down-regulated GRP78 expression …

The T2-Fluid-Attenuated Inversion Recovery (T2-FLAIR) mismatch sign is a radiogenomic marker that is easily discernible on preoperative conventional MR imaging. Application of strict criteria (adult population, cerebral hemisphere location, and classic imaging morphology) permits the noninvasive preoperative diagnosis of isocitrate dehydrogenase (IDH)-mutant 1p/19q-non-codeleted diffuse astrocytoma with near-perfect specificity, albeit with variably low sensitivity. This leads to improved preoperative planning and patient counseling. More recent research has shown that the application of less strict criteria compromises the near-perfect specificity of the sign but remains adequate for ruling out IDH-wildtype (glioblastoma) phenotype, which bears a far grimmer prognosis compared to IDH-mutant diffuse …

The RNA exosome is an evolutionarily conserved complex required for both precise RNA processing and decay. Pathogenic variants in EXOSC genes, which encode structural subunits of this complex, are linked to several autosomal recessive disorders. Here, we describe a missense allele of the EXOSC4 gene that causes a collection of clinical features in two affected siblings. This missense variant (NM_019037.3: exon3:c.560T>C) changes a leucine residue within a conserved region of EXOSC4 to proline (p.Leu187Pro). The two affected individuals show prenatal growth restriction, failure to thrive, global developmental delay, intracerebral and basal ganglia calcifications, and kidney failure. Homozygosity for the …

Mc-Keyboard: A Practical Phase I Trial Design For Targeted Therapies And Immunotherapies Integrating Multiple-Grade Toxicities, Liyun Jiang, Zhulin Yin, Fangrong Yan, Ying Yuan
Faculty, Staff and Student Publications
In targeted therapies and immunotherapies, the occurrence of low-grade (e.g., grade 1-2) toxicities (LGT) is common, while dose-limiting toxicities (DLT) are relatively rare. As a result, conventional phase I trial designs, solely based on DLTs and disregarding milder toxicities, are problematic when evaluating these novel therapies.
Methods: To address this issue, we propose a novel phase I design called a multiple-constraint keyboard (MC-Keyboard) that integrates multiple toxicity constraints, accounting for both DLT and LGT, for precise dose escalation and de-escalation, and identification of the maximum tolerated dose (MTD). As a model-assisted design, an important feature of MC-Keyboard is that its …

The commitment of stem cells to differentiate into osteoblasts is a highly regulated and complex process that involves the coordination of extrinsic signals and intrinsic transcriptional machinery. While rodent osteoblastic differentiation has been extensively studied, research on human osteogenesis has been limited by cell sources and existing models. Here, we systematically dissect human pluripotent stem cell-derived osteoblasts to identify functional membrane proteins and their downstream transcriptional networks involved in human osteogenesis. Our results reveal an enrichment of type II transmembrane serine protease CORIN in humans but not rodent osteoblasts. Functional analyses demonstrated that CORIN depletion significantly impairs osteogenesis. Genome-wide chromatin …

The cauda equina syndrome (CES) is a rare but critical disorder, which can result in devastating motor weakness and sensory deficit, alongside often irreversible bladder, bowel and sexual dysfunction. In addition to the clinical burden of disease, this syndrome results in a disproportionately high medicolegal strain due to missed or delayed diagnoses. Despite being an emergency diagnosis, often necessitating urgent surgical decompression to treat, we believe there is a lack of clarity for clinicians in the current literature, with no published Irish guideline concerning screening or detection. The current study aims to identify and analyse appropriate guidelines in relation to …

Total knee arthroplasty (TKA) risks persistent pain and long-term opioid use (LTO). The role of social determinants of health (SDoH) in LTO is not well established. We hypothesized that SDoH would be associated with postsurgical LTO after controlling for relevant demographic and clinical variables. This study utilized data from the Veterans Affairs Surgical Quality Improvement Program, VA Corporate Data Warehouse, and Centers for Medicare and Medicaid Services, including Veterans aged ≥ 65 who underwent elective TKA between 2013 and 2019 with no postsurgical complications or history of significant opioid use. LTO was defined as > 90 days of opioid use beginning …

Introduction: Veterans Affairs Surgical Quality Improvement Program (VASQIP) benchmarking algorithms helped the Veterans Health Administration (VHA) reduce postoperative mortality. Despite calls to consider social risk factors, these algorithms do not adjust for social determinants of health (SDoH) or account for services fragmented between the VHA and the private sector. This investigation examines how the addition of SDoH change model performance and quantifies associations between SDoH and 30-d postoperative mortality.
Methods: VASQIP (2013-2019) cohort study in patients ≥65 y old with 2-30-d inpatient stays. VASQIP was linked to other VHA and Medicare/Medicaid data. 30-d postoperative mortality was examined using multivariable logistic …

Cannabinoid receptor 1 (CB1) is a G protein-coupled receptor that regulates critical physiological processes including pain, appetite, and cognition. Understanding the conformational dynamics of CB1 associated with transitions between inactive and active signaling states is imperative for developing targeted modulators. Using microsecond-level all-atom molecular dynamics simulations, we identified marked differences in the conformational ensembles of inactive and active CB1 in apo. The inactive state exhibited substantially increased structural heterogeneity and plasticity compared to the more rigidified active state in the absence of stabilizing ligands. Transmembrane helices TM3 and TM7 were identified as distinguishing factors modulating the state-dependent dynamics. TM7 …

Chemical dyeing and finishing processes used in the clothing and textile industries are among the main contributors that can increase the impact of climate change. Photocatalysis using nanosized titanium dioxide (TiO2) has been continuously developed as a promising technology for purifying dye wastewater into simpler and environmentally friendly components. In addition, the decoration of iron cations (Fe2+) and (Fe3+) increases the reusability of the photocatalyst due to their magnetic properties, which are easy to collect for the recycling process. Magnetic Fex+/TiO2 microfibers have been successfully prepared using a hydrothermal method using …

Protective Factors For Suicidal Ideation In Transgender Adults Living In Rural Communities, Genel Gronkowski
Theses and Dissertations
Transgender adults living in rural communities within the USA may be at an increased risk for suicidal ideation compared to their cisgender peers. Meyer’s Minority Stress Model postulates that this may be due to unique stressors that result from a minoritized identity that may lead to poor mental health outcomes, such as suicidal ideation; individual and community level protective factors may buffer or protect from poor health outcomes. The purpose of this study is to identify protective factors that may be unique to transgender adults experiencing suicidal ideation in rural communities. Utilizing a grounded theory approach, a homogenous group of …

Machine Learning-Assisted Materials Development: Molecular Solubility Prediction, Polymeric Membrane Fabrication, And Scale Formation, Arash Tayyebi
Theses and Dissertations
Experiments are generally time-consuming, expensive, and rely on exhaustive planning, analysis, and testing efforts. The limitations of the traditional experimental methods in materials development can be overcome by the use of machine learning (ML) algorithms to guide and assess the discovery and fabrication processes. This dissertation involves developing different ML models, focusing on their utilization in three distinct, yet overlapping areas: Prediction of organic compound aqueous solubility, guiding separation membranes fabrication and predicting scale formation in commercially produced water systems.Prediction of organic compound aqueous solubility: A reliable and practical determination of a chemical species’ solubility in aqueous systems continues to …
Numerical Investigation And Seismic Performance Evaluation Of Liquefied Natural Gas (Lng) Cylindrical Steel Storage Tanks Under Earthquake Loading, Shafqat Ullah
Theses and Dissertations
Cylindrical steel tanks are an important structural components of various industries, power plants, oil refineries, and other lifeline facilities. These liquid-filled reservoirs are prone to local instability (buckling) when exposed to lateral seismic forces. This thesis aims to study the seismic buckling performance of an empty and partially filled cylindrical steel storage tanks using finite element modeling (FEM) approaches. To validate the accuracy of the FEA ABAQUS software, the cylindrical tanks are considered for both static linear buckling, non-linear, as well as dynamic (seismic non-linear) analysis. A total of 3 cylindrical shells having 1 perfect model without carbon fiber reinforced …

Using Libensemble To Enhance Numerical Ice-Sheet Flow Simulations, Kuang Hsu Wang
Theses and Dissertations
Here, we interface an ice sheet flow model, FastIceFlo v1.0, with the Python toolkitlibEnsemble to enable dynamic ensembles of computations. We test the interface’s capability to perform dynamic ensembles of computations by conducting a sensitivity analysis. The results indicate the interface’s ability to perform dynamic ensembles of computations. Future work could extend the interface to include mass balance to run prognostic simulations. The eventual goal is to run simulation ensembles of large-scale ice sheets forward in time to better assess its sea level contribution sensitivity to uncertainties in climate forcing parameterization.

Introduction
Nickel-titanium rotary endodontic files have been commercially available for decades, but more recent innovations have introduced heat-treated and surface-treated files. This study investigated the corrosion properties of various nickel-titanium files in normal saline and sodium hypochlorite (NaOCl).
Methods
Ten different file brands of size 40 with a 0.04 taper were subjected to electrochemical testing in 0.9% NaCl (saline) and 5.25% NaOCl at room temperature. The Open Circuit Potential (OCP) was observed for 1 hour followed by a cyclic polarization test from −300 to 700 mV and back to −300 mV (vs OCP). Nonparametric ANOVA and a pairwise comparison ( …
