An Unusual Case Of Refractory Hypoxia: The Many Ways Of Providing Oxygen, Saliha Erdem, Mir Babar Basir

Journal of Shock and Hemodynamics

This case report outlines the complex clinical course of a 26-year-old female patient who presented with altered mental status after an N-methyl-3,4-methylenedioxy-amphetamine overdose. The patient exhibited profound hemodynamic instability and respiratory failure. She subsequently developed shock, necessitating advanced life support measures, including veno-venous extracorporeal membrane oxygenation with a dual lumen right atrium to pulmonary artery cannula (ProtekDuo; LivaNova, London, UK) and Quadrox Oxygenator (Getinge, Wayne, NJ) system. This resulted in worsening hypoxemia, which was attributed to an increased left ventricular preload. In response, an intra-aortic balloon pump was used to decrease the left ventricle’s afterload. After balloon placement, the patient’s …

John Wain: The Inkling In The Middle, David Bratman

Mythcon

John Wain (1925-1994) - novelist, poet, and general man of letters – was caught between two literary generations. A tutorial pupil of C.S. Lewis’s at Oxford, he was invited by Lewis to join the Inklings, a private literary club consisting mostly of dons. While Wain enjoyed the company of his seniors, he felt at odds with them on basic matters of literary preference, in particular seeing no point to fantasy. Wain is more often classed with his contemporaries, post-WW2 writers known as The Movement, the University Wits, or the Angry Young Men, poets and novelists in a realist, often sarcastic …

Why There Is No Evidence That Pyridine Killed The English Crabs, Alex T. Ford, Mark F. Fitzsimons, Crispin Halsall

School of Geography, Earth and Environmental Sciences

The North East coast of England experienced a mass mortality event in late 2021 affecting millions of crabs and lobsters. The die-off coincided with the redevelopment of one of the UK's flagship ports, prompting local scientists to suggest the remobilization of dredged industrial contaminants as a cause. A multi-agency investigation found no definitive causal factor; however, re-evaluation of data by consultants drew a different conclusion, linking the industrial compound pyridine to the crustacean deaths. Authors of an unpublished study subsequently claimed that their data demonstrated pyridine to be exceptionally toxic and that their modeling explained the coastal distribution of washups. …

We investigate unsteady, two-dimensional laminar fluid flow around cylinders, focusing on understanding the impact of injecting methane gas through two diametrically opposite arcs on the cylinder in the crossflow of the second fluid. This study encompasses applications in mixing and dispersion, which are crucial in various technological and natural processes. Our analysis addresses velocity field’s contribution to the spatiotemporal distribution of transported quantities. We observed that mixing induces a transition from laminar to turbulent flow. Depending on the injected-to-crossflow velocity ratios, the wake vortices downstream of cylinder arrays separate from or connect to the injected gas. This phenomenon significantly impacts …

Nucleic acid-sensing Toll-like receptors (TLR) 3, 7/8, and 9 are key innate immune sensors whose activities must be tightly regulated to prevent systemic autoimmune or autoinflammatory disease or virus-associated immunopathology. Here, we report a systematic scanning-alanine mutagenesis screen of all cytosolic and luminal residues of the TLR chaperone protein UNC93B1, which identified both negative and positive regulatory regions affecting TLR3, TLR7, and TLR9 responses. We subsequently identified two families harboring heterozygous coding mutations in UNC93B1, UNC93B1+/T93I and UNC93B1+/R336C, both in key negative regulatory regions identified in our screen. These patients presented with cutaneous tumid lupus and juvenile idiopathic arthritis plus …
