The ANTHEM-HF, INOVATE-HF, and NECTAR-HF clinical studies of autonomic regulation therapy (ART) using vagus nerve stimulation (VNS) systems have collectively provided dose-ranging information enabling the development of several working hypotheses on how stimulation frequency can be utilized during VNS for tolerability and improving cardiovascular outcomes in patients living with heart failure (HF) and reduced ejection fraction (HFrEF). Changes in heart rate dynamics, comprising reduced heart rate (HR) and increased HR variability, are a biomarker of autonomic nerve system engagement and cardiac control, and appear to be sensitive to VNS that is delivered using a stimulation frequency that is similar to …

Environmental data systems have largely escaped scrutiny in the past decades. But these systems are the foundations for evaluating environmental priorities, making management decisions, and deciding which perspectives to value. Information is the foundation of effective regulation. The decisions regulators make about gathering, assimilating, and sharing information are, in many cases, determinative of the outcomes they reach. This is certainly true in the case of the environment.
This paper looks at how current environmental regulation has created data systems that undermine scientific legitimacy and systematically prevent stakeholder participation in environmental decision-making. These data systems concentrate power within federal and state …

History, text, and precedent reveal an understudied and underutilized source of constitutional authority for environmental protection—the Property Clause of Article IV, Section 3. The Clause vests Congress with the “Power to dispose of and make all needful Rules and Regulations respecting the Territory or other Property belonging to the United States.” This work re-examines these words, the context in which they were written, and the limited judicial decisions interpreting them with an eye towards increased congressional reliance on the Property Clause in the face of daunting threats to our natural environment. Meningiomas are common primary central nervous system tumors derived from the meninges, with management most frequently entailing serial monitoring or a combination of surgery and/or radiation therapy. Although often considered benign lesions, meningiomas can not only be surgically inaccessible but also exhibit aggressive growth and recurrence. In such cases, adjuvant radiation and systemic therapy may be required for tumor control. In this review, we briefly describe the current WHO grading scale for meningioma and provide demonstrative cases of treatment-resistant meningiomas. We also summarize frequently observed molecular abnormalities and their correlation with intracranial location and recurrence rate. We then describe how …

Non-random community changes are becoming more frequent in many ecosystems. In coral reefs, changes towards communities dominated by other than hard corals are increasing in frequency, with severe impacts on ecosystem functioning and provision of ecosystem services. Although new research suggests that a variety of alternative communities (i.e. not dominated by hard corals) exist, knowledge on the global diversity and functioning of alternative coral reef benthic communities, especially those not dominated by algae, remains scattered. Whole-animal, in vivo protein metabolism rates have been reported in temperate and tropical, but not Antarctic fish. Growth in Antarctic species is generally slower than lower latitude species. Protein metabolism data for Antarctic invertebrates show low rates of protein synthesis and unusually high rates of protein degradation. Additionally, in Antarctic fish, increasing evidence suggests a lower frequency of successful folding of nascent proteins and reduced protein stability. This study reports the first whole-animal protein metabolism data for an Antarctic fish. Abstract Millions of people experience injury to the central nervous system (CNS) each year, many of whom are left permanently disabled, providing a challenging hurdle for the field of regenerative medicine. Repair of damage in the CNS occurs through a concerted effort of phagocytosis of debris, cell proliferation and differentiation to produce new neurons and glia, distal axon/dendrite degeneration, proximal axon/dendrite regeneration and axon re-enwrapment. In humans, regeneration is observed within the peripheral nervous system, while in the CNS injured axons exhibit limited ability to regenerate. Transports across the continental shelf edge enhance shelf-sea production, remove atmospheric carbon and imply an active boundary to ocean circulation. We estimate relatively large overall transport across three contrasted sectors of north-west European shelf edge: the Celtic Sea south-west of Britain, the Malin-Hebrides shelf west of Scotland, the West Shetland shelf north of Scotland. The estimates derive from measurements in the project FASTNEt (Fluxes across sloping topography of the North East Atlantic): drifters, moored current meters, effective “diffusivity” from drifter dispersion and salinity surveys, other estimates of velocity variance contributing to exchange. Introduction Recurrent pulmonary exacerbations lead to progressive lung damage in cystic fibrosis (CF). Inhaled medications (mucoactive agents and antibiotics) help prevent exacerbations, but objectively measured adherence is low. We investigated whether a multi-component (complex) self-management intervention to support adherence would reduce exacerbation rates over 12 months. Methods Between October 2017 and May 2018, adults with CF (aged ≥16 years; 19 UK centres) were randomised to the intervention (data-logging nebulisers, a digital platform and behavioural change sessions with trained clinical interventionists) or usual care (data-logging nebulisers). Introduction: Autism, like other neurodevelopmental disorders (NDDs), has a strong association with epilepsy. There are known common genetic pathways in both autism and epilepsy. There are also specific genetic syndromes associated with both complex epilepsy and the autism phenotype. Areas covered: This review explores the evidence for common genetic etiologies and pathophysiological pathways in relation to both epilepsy and autism. Autism with comorbid epilepsy are associated with a high prevalence of medical and psychiatric comorbidities. This paper discusses how this influences assessment, treatment, and outcomes. Single-use packaging items constitute a large proportion of the plastic litter found in the marine environment. Consumer decisions contribute to the accumulation of this pollution in the environment. Here we undertook two studies to assess consumer responses to different types of single-use packaging. Moreover, we introduce a new measure of ocean connectedness adapted from nature connectedness measures and investigate its association with consumer response. In Study 1, 60 UK undergraduate students completed a packaging rating task and a survey on ocean connectedness in a laboratory environment. Objective: An earlier follow-up study from the CogEx rehabilitation trial showed little change in symptoms of depression, anxiety and psychological distress during the first COVID-19 lockdown compared to pre-pandemic measurements. Here we provide a second follow-up set of behavioral data on the CogEx sample. Methods: This was an ancillary, longitudinal follow-up study in CogEx, a randomized controlled trial of exercise and cognitive rehabilitation in people with progressive MS involving 11 centres in North America and Europe. Hypertension is a major risk factor for kidney and cardiovascular disease. The treatment of hypertensive individuals by selected ACE inhibitors and certain di-and tripeptides halts the progression of renal deterioration and extends life-span. Renal reabsorption of these low molecular weight substrates are mediated by the PEPT1 and PEPT2 cotransporters. This study aims to investigate whether hypertension and ageing affects renal PEPT cotransporters at gene, protein expression and distribution as well as function in the superficial cortex and the outer medulla of the kidney. Neonicotinoids are designed to target insect pests, but their extensive use and long persistence in soils mean that non-target soil organisms such as earthworms are likely to be chronically exposed to them. The common use of neonicotinoids in agricultural systems makes them highly likely to come into contact with other agro-chemicals that may give rise to synergistic effects. Chronic exposure of common earthworm (Lumbricus terrestris) to neonicotinoids in single or combined use with inorganic NPK fertiliser may pose problems that are not accounted for in most biological risk assessments under laboratory conditions. The Mesoamerican biodiversity hotspot is threatened with agriculture related deforestation and global climate change, both of which drive widespread changes in forest composition. As the forests change, so too does the community of animals they can support; of which many reptiles have evolved micro-niches specific to these forests. Marine ecosystems support human life in multi-faceted ways and are depended on for food and income by many coastal communities. However, marine habitats are being altered by anthropogenic pressures, influencing the diversity and distribution of species, including species that are relied upon by commercial fisheries. Destructive fishing practices such as dredging and trawling can have adverse impacts on the seabed and associated species through the degradation and fragmentation of benthic habitats. Apis mellifera (the Western honey bee) is a predominantly human managed pollinator consisting of different evolutionary lineages each containing a number of subspecies. Apis mellifera is threatened by various factors for example, parasites, viruses and pesticide use. The native subspecies of the British Isles Apis mellifera mellifera is additionally threatened by introgression resulting from the importation of foreign subspecies perceived to have more desirable characteristics. This thesis investigates the integrity of Apis mellifera mellifera in the UK and how that can be assessed. Initially, introgression levels over time in a conservation program were assessed.
