BACKGROUND: Hepatitis B virus (HBV) infection disproportionately affects Korean Americans, who often experience persistent social and structural challenges that may influence quality of life. Recent community studies showed most Korean Americans with chronic hepatitis B (CHB) were disengaged from clinical care, emphasizing the need to better understand factors affecting their well-being. This study examined associations between social determinants of health (SDOH) and physical and mental health related quality of life (HRQoL) among Korean Americans with CHB.
METHODS: A total of 365 CHB patients completed the enrollment survey. SF-12 was used to measure quality of life, calculating physical and mental components …

Recent advancements in single-cell DNA methylation (scDNAm) sequencing technologies have enabled the profiling of epigenetic landscapes at unprecedented resolution, offering insights into cellular heterogeneity, differentiation and evolution. Trajectory inference, which orders cells along pseudotime, allows researchers to track genomics changes across continuous cell states and identify key loci exhibiting differential methylation. However, no methods currently exist to model methylation changes along pseudotime in scDNAm data. Here, we present a hierarchical Bayesian framework for scDNAm data analysis. We aim to develop an in vivo hematopoietic stem cell (HSC) gene therapy approach for the prevention and control of HIV-1 infection. Toward this goal, we engineered helper-dependent adenovirus (HDAd) 6/3+ vectors to directly transduce HSCs in vivo, enabling progeny cells to secrete eCD4-Ig, a decoy protein that broadly neutralizes HIV/simian immunodeficiency virus (SIV) isolates by mimicking the primary viral receptor CD4 and coreceptors such as CCR5. In rhesus macaques, the HDAd 6/3+ platform achieved long-term expression of an enhanced eCD4-Ig variant (“eCD4-Ig-Emm06”) that retained potent neutralization efficacy in vivo. Streptococcus gallolyticus subsp. gallolyticus (Sgg) is a gram-positive, non-beta-hemolytic bacterium that belongs to the Lancefield group D of streptococcal pathogens. For the past several decades, associations between Sgg bacteremia and colorectal cancer (CRC) have been apparent, with gastroenterologists routinely recommending colonoscopies for positive cases of Sgg in either blood or stool samples. However, current research has not wholly established whether Sgg acts as a causative agent of CRC or if pre-existing cancers make the lumen of the colon more hospitable to its colonization. Regardless, Sgg has been implicated in the modulation of host inflammatory pathways and subsequent remodeling …

Personal health monitoring devices may detect physiological changes but can lack environmental context to determine a cause, while environmental alert systems may rely on generic, non-personalized data. Systems and methods are described that can algorithmically fuse multiple data streams. These streams can include real-time physiological data from a wearable computing device (e.g., a smart watch, fitness tracker), such as heart rate variability; hyperlocal environmental data, such as air quality or noise levels; and contextual data, such as a user's location and activity level. This fusion of data may be used to compute a personalized risk index. The index may then …

Perioperative fluid therapy is a cornerstone of anesthetic management in cardiac surgery, where both hypovolemia and hypervolemia are associated with increased morbidity, forming a ``U-shaped'' relationship with outcomes. This narrative review synthesizes evidence from peer-reviewed literature (2000–2024) identified via PubMed, Scopus, and the Cochrane Database, focusing on randomized trials, meta-analyses, and guidelines for adult cardiac surgery. A central debate persists regarding fluid choice. Current evidence supports the preferential use of balanced crystalloids over normal saline, as large-volume saline is linked to hyperchloremic acidosis and reduced renal perfusion. Pediatric acquired subglottic stenosis is a high-risk airway condition that requires close coordination between anesthesiology and otorhinolaryngology teams, particularly during endoscopic interventions performed under shared-airway conditions. This report describes the interdisciplinary management and anesthetic–surgical coordination in a child with severe acquired subglottic stenosis undergoing endoscopic balloon dilation, highlighting key decision points and airway management strategies. A 2-year-old boy with a history of prolonged and repeated endotracheal intubations presented with persistent dyspnea and noisy breathing, consistent with fixed extrathoracic airway obstruction. This study addresses the critical need for improved therapeutic strategies to overcome the limitations of conventional treatments, particularly systemic toxicity and poor bioavailability of potent agents such as camptothecin (CPT). This research was designed to fabricate and characterize a superparamagnetic iron oxide nanocomposite (SPION-DEX-CPT-FA) used for the selective delivery of CPT and to evaluate its effectiveness in antibacterial applications against bacteria Escherichia coli and Klebsiella pneumoniae, Staphylococcus aureus, and Streptococcus pneumoniae from the urinary tract. The objective of this study is the development of bio-based nanoemulsions that were derived from natural sources, including olive, coconut, red raspberry, and tomato seed oils, to produce safe and effective UV- protective agents. The nanoemulsions were produced at the nanoscale with uniform droplets by employing ultrasonic emulsification utilizing deionized water and Tween 80. According to Dynamic light scattering (DLS), the oil nanoemulsification from red raspberry exhibited the smallest droplet size of (148.5 nm), low polydispersity index of (0.218) and high zeta potential of (–35.6 mV). Red blood cells (RBCs) transport oxygen but accumulate oxidative damage over time, reducing function in vivo and during storage, critical for transfusions. To explore the genetics of RBC resilience, we profiled proteins, metabolites, and lipids from fresh and stored RBCs from 350 genetically diverse mice. Our analysis identified over 6,000 quantitative trait loci (QTLs). Compared to other tissues, the prevalence of trans genetic effects over cis ones reflects the absence of de novo protein synthesis in anucleated RBCs. QTL hotspots at Hbb, Hba, Mon1a, and (storage-specific) Steap3 linked ferroptosis to hemolysis. Proteasome QTLs clustered at multiple loci, underscoring the importance …

Background: The Coronavirus disease 2019 (COVID-19) has led to over 1.2 million deaths in the United States. Given the staggering scale of the pandemic, rural hospitals faced heightened challenges due to limited resource availability and underlying patient characteristics.
Methods: In this context, we examined patient and hospital-related factors associated with inpatient mortality due to COVID-19. In vitro models have tremendous utility for improving our understanding of complex physiological and pathophysiological processes in a more controlled environment than animal models, in addition to incorporating human cells to overcome species-specific disparities. This dissertation describes innovations in biomaterial-based fabrication processes that advance in vitro models of the blood-brain barrier, providing the opportunity to evaluate the effects of cell-matrix interactions on barrier integrity, additives to reduce viscous drag of blood flow, and molecular mechanisms underlying the cellular response to amyloid beta in the context of Alzheimer’s disease. Many heterotroph species perish when faced with severe food limitation; others can persist, adapt, and thrive. Sea urchins are emblematic of this paradox: they can overgraze kelp forests to form barren habitats, but can then survive for decades in these nutritionally depauperate seascapes. Understanding the mechanisms enabling persistence under starvation and rapid recovery when food returns provides insights into how consumer resilience shapes ecosystem dynamics. We quantified how food abundance, quality, deprivation, and reintroduction influence bioenergetic performance in the red sea urchin (Mesocentrotus franciscanus), integrating field observations of kelp forest and barren populations with a controlled feeding experiment. Colorectal cancer (CRC) develops through both conventional adenoma-carcinoma and serrated neoplasia pathways, yet noninvasive screening still under-detects the advanced precursor lesions that enable true cancer prevention. Stool-based screening reduces CRC mortality, but its preventive impact remains constrained by limited detection of advanced precancerous lesions (APLs), including advanced adenomas and sessile serrated lesions. Next-generation multitarget stool DNA assays (mt-sDNA; e.g., Cologuard Plus) have established high sensitivity for CRC and specificity approaching 94%, leaving improved APL detection as the principal opportunity for innovation. Copyback viral genomes (cbVGs) are truncated non-protein coding viral genomes produced when an RNA virus polymerase detaches from the replication template and resumes elongation downstream, copying back a sequence complementary to the nascent strand. cbVGs can stimulate antiviral immunity and compete for viral proteins to suppress virus replication, but technical limitations of PCR-based approaches have painted an incomplete picture of the diverse cbVG populations generated during infection. Here, we sought to improve our understanding of cbVG molecular structure by harnessing long-read RNA sequencing tools. Purpose: NRG/RTOG 1016 was a phase III randomized noninferiority de-escalation trial comparing cetuximab versus cisplatin, concurrent with accelerated radiation 70 Gy/6 weeks, in p16+ oropharyngeal cancer. Quality of life (QOL) was a secondary endpoint.
Methods and materials: Eligible/consenting patients among the first 400 entered completed the EORTC QLQ-C30/H&N35 at baseline, end of treatment, 3, 6, and 12 months posttreatment, to provide 90% power to detect an effect size of 0.5 in the between-arm change in QOL scores from baseline to 6 months. Monoclonal gammopathy of undetermined significance (MGUS) is a necessary precursor condition to multiple myeloma (MM). Given the role of autophagy in modulating MM risk, we investigated whether genetic variation in autophagy-related genes influences susceptibility to MGUS. We analyzed the association of 34,042 common autophagy-related single nucleotide polymorphisms (SNPs) with MGUS across six independent cohorts, five from Europe and one from North America, comprising 2317 MGUS cases and 282,358 controls. We also assessed their impact on immune parameters, including absolute counts of 91 blood-derived immune cell subsets and 103 circulating immunological proteins. Microglia arise from yolk sac progenitors and are thought to persist throughout life with minimal input from adult hematopoiesis. However, whether brain-engrafted monocyte-derived macrophages (MDMs) exist at homeostasis and during turnover and how they function relative to yolk-sac-derived microglia (YSMs) remain unsettled. Here, we combine lineage tracing, pharmacological microglia depletion, and multi-omics profiling to define the ontogeny, identity, and function of brain parenchymal macrophages. Despite sharing the parenchymal milieu, MDMs display transcriptional and epigenetic landscapes distinct from YSMs. Fate-mapping reveals that brain-engrafted MDMs transiently express CD206, echoing a developmental stage of microglial precursors. Xerostomia, the subjective sensation of dry mouth, is a debilitating consequence of head and neck radiotherapy and autoimmune disorders such as Sjögren’s syndrome. It severely impairs oral health and quality of life by promoting mucosal ulceration, infection, malnutrition, and speech difficulties, yet effective regenerative treatments remain limited. Macrophages have recently emerged as critical regulators of salivary gland repair through their roles in coordinating inflammation, fibrosis, and epithelial regeneration; however, the molecular mechanisms governing macrophage activation and function in the injured salivary gland remain poorly defined.
