The health outcome of reducing 30-day Medicare readmissions continues to be a constant measurement of quality patient care. Post-acute services, like home health care, play a vital role in national efforts to improve patient care, contributing to cost reductions. This quantitative study explored the relationship between home health agency (HHA) ownership (for-profit vs. nonprofit) and Medicare 30-day readmissions by race, age, and gender. Therapy Dogs As A Catalyst To Physical Exercise And The Effect On Transgender Adolescents’ Mental Health, Tatiana Sokolik

This dissertation examines the effects of therapy dog interactions on physical activity levels and mental health outcomes among transgender adolescents. Using a quasi-experimental mixed- methods design, participants engaged in three weekly sessions with a certified therapy dog, with data collected through validated measures of anxiety, depression, affect, and physical activity, supplemented by qualitative feedback. Quantitative results indicated no significant increases in physical activity, though participants reported feeling motivated to spend more time outdoors. Notably, negative affect decreased significantly post-intervention, suggesting short-term emotional benefits. Qualitative findings further highlighted enhanced mood, self-awareness, and coping strategies. While improvements in anxiety and depression were …

Background/aim: Malnutrition and muscle wasting are common in terminal cancer patients and adversely affect prognosis. Bedside muscle ultrasonography (US) has emerged as a practical tool to assess nutritional status and monitor therapy. This study evaluated the relationship between achieving individualized nutritional targets and changes in muscle indices measured by US in terminal cancer patients receiving palliative care.

We evaluated a targeted Candida auris screening program, which revealed a 0.96% positivity rate. The proportion of community-onset cases increased from 8% to 54% following implementation. No significant differences in demographic or clinical characteristics were observed between patients with colonization and those with infection.

Background: Hydromorphone is a semi-synthetic opioid agonist and a hydrogenated ketone of morphine. This study examined hydromorphone use in the United States (US) using three databases. Methods: The distribution of hydromorphone in the US (in grams) was provided by the US Drug Enforcement Administration’s Automated Reports and Consolidated Orders System (ARCOS) by state, zip code, and business type (pharmacies, hospitals, providers, etc.). Hydromorphone prescription claims were also examined using the Medicaid and Medicare Part D programs from 2010 to 2023. Results: Hydromorphone increased by +30.6% by 2013, followed by a decrease of −55.9% by 2023 in ARCOS. Medicaid prescriptions increased …

End-of-life discussions can be difficult for patients, their families, and clinical nurses. Yet, studies have shown that early end-of-life discussions increase patient/family acceptance of the illness as terminal, promote satisfaction and involvement with end-of-life care, and have the potential to reduce emergency department visits, hospital admissions, and length of stay. This pre-/post- evidence-based practice implementation aimed to improve clinical nurses' confidence in initiating and participating in end-of-life discussions with patients and families. Dysphagia, difficulty with swallowing, affects approximately 16% of US adults and can result from conditions such as stroke, neurodegenerative conditions, and cancer. Despite growing emphasis on the intersection of religion/spirituality (R/S) and health, little research has examined these connections in dysphagia. This qualitative study explored the impact of dysphagia on engagement in R/S rituals and communities and access to cultural food traditions. It also examined ways that R/S beliefs shaped coping practices in persons with dysphagia and their caregivers. Traditional Extension programs for older adults often adopt a reactive approach, focusing on addressing age-related challenges after they arise. While these programs can be beneficial, a proactive aging model offers a more comprehensive and empowering approach. The concept of proactive aging, emphasizing preventative strategies for maintaining and improving wellbeing, has been well-established in the literature (Kahana et al., 2014). End-of-life planning can be an intimidating, confusing, and emotional process, which may explain why only 37% of Americans have completed advance care directives. To address this need, we developed the Planning AHEAD (Advance directives, Handling financial changes, Estate planning, and Arriving at Decisions for the end of this life) curriculum. Planning AHEAD is a seven-session course designed to help participants make important decisions that reflect their end-of-life wishes and document this information to relieve stress for their loved ones. In the first two years of the program, Extension educators facilitated 45 Planning AHEAD series across Wisconsin, reaching over 600 participants. …

Many Stories, One Shape: Narrative Convergence In Ai-Generated Fiction, Marisol Hernandez Brito

Frontier large language models (LLMs) generate fluent outlines but often converge on repetitive structural templates. This exploratory study examined inter-model diversity in thriller novel outlines produced by five LLMs (ChatGPT-5(5.1), Grok-4, Gemini-2.5 Flash, DeepSeek-v3, Claude Sonnet-4.5) under baseline and enhanced prompting conditions (N=50 outlines). Blind AI evaluations revealed moderate inter-model diversity (baseline mean 64.45%; enhanced 58.89%), declining relative to intra-model baselines and between phases, particularly in Protagonist and Plot dimensions. Quantitative and qualitative analysis identified phase-specific convergence: a "forensic procedural" template (analytical female protagonists, corporate conspiracies, Quest/Monster arcs) under the baseline prompt, shifting to a "domestic psychological thriller" template (uniform …

Problem: Timely pain reassessment is a vital component of hospice care, directly influencing the quality of life for patients. Despite the high prevalence of pain, which affects 55% of hospice patients due to disease progression or adverse drug reactions, only 51% of those reporting moderate to severe pain at admission receive a documented reassessment. Problem: Sepsis is a life-threatening condition with high readmission risk, threatening patient recovery and survival. In the managed care organization (MCO) where this DNP project was implemented, sepsis ranks among the top five readmission diagnoses. The organization's 30-day readmission rate of 15.5% is substantially higher than the benchmark of 9.29%, highlighting a clinical quality issue, a strain on patient outcomes, and a strain on healthcare resources.

Objectives: Emergency department (ED) visits offer opportunities for seriously ill patients to formulate future medical care goals, yet ED clinicians lack practical strategies for these conversations. ED GOAL, a behavioral intervention, engages seriously ill yet clinically stable older adults in the ED to address advance care planning (ACP) with their outpatient clinicians. In a randomized trial, goals-of-care documentation was significantly higher in the intervention group compared to controls after three (24.3% vs. 9.9%, p = 0.03) and 6 months (31.4% vs. 12.7%, p < 0.01). This study is a sub-analysis to learn about intervention arm participants' perceived benefits and obstacles of the intervention.

Introduction: Enfortumab vedotin-ejfv (EV), an antibody-drug conjugate approved for advanced urothelial carcinoma (aUC), has limited real-world safety data and no validated predictive biomarkers. Retrospective studies suggest higher objective response rates (ORR) among patients developing EV-related dermatologic toxicities, but survival implications remain unclear. This analysis assessed the safety of EV monotherapy and the impact of dermatologic and neurologic toxicities on efficacy outcomes in a multi-institutional cohort.

Background. Despite trial data supporting oral stepdown therapy for infective endocarditis (IE), its use remains limited, especially in North America. We evaluated outcomes of patients with IE managed by a multidisciplinary team and treated with either intravenous (IV) or partial oral antibiotics. Methods. This was a single-center retrospective study of patients with definite IE identified from an institutional registry between 7 September 2021 and 1 March 2025. Clinical and outcomes data were analyzed using multivariable logistic regression. Results. Of 236 patients, 143 received IV therapy alone and 93 were transitioned to partial oral therapy.
