A 68-year-old male with a history of metastatic bladder cancer and urinary obstruction presented with intractable hiccups and chest pain is presented in this case. He was found to have a unilateral pleural effusion in addition to ipsilateral renal metastasis abutting the diaphragm. Imaging revealed empyema. Thoracentesis revealed positive cultures and a lack of malignant cells on cytology. Comorbid urinary obstruction and urinary tract infection suggested the rare diagnosis of an infected urinothorax. Thoracentesis also revealed pleural fluid creatinine greater than serum creatinine, meeting the most specific diagnostic criteria for urinothorax and establishing the diagnosis. The purpose of this quantitative cross-sectional descriptive research study was to explore nurses’ comfort levels toward caring for dying patients and what factors influence the bedside nurses’ comfort levels in providing end-of-life care in an acute care hospital setting. A convenience sample (N=63) of RNs voluntarily participated in an online survey from January 2025 to March 2025 that provided direct patient care. Data was collected through Question Pro. Data collected included sociodemographic questions, questions about current comfort level, factors that influence nurses’ comfort level, and selected items from the Frommelt Attitude Toward Care of the Dying Patient (FATCOD) scale. Problem: A Residential Care facility utilizes paper charting causing limited access to vital information such as code status, allergies, and/or diet for those caring for the resident.

Context: Exceptional charting only when a resident’s condition changes may not provide enough vital information for the caregiver to administer medication or provide proper support during an emergency, such as the resident’s DNR status.

This article describes the journey of a small dog as she is trained to be a therapy dog. Requirements for training and regulations to be a therapy dog are discussed. After training, the dog describes her therapy skills in a variety of settings that enriches the lives of patients and families in hospice settings and rehabilitation. The purpose of this quality improvement (QI) Doctorate of Nursing (DNP) project was to create and evaluate the impact of high-fidelity simulation-based training (SBT) on graduate nursing students’ knowledge and confidence in diagnosing and treating septic shock. Outcomes were measured with pre- and post-simulation tests featuring competency questions and Likert-scale confidence ratings.

Accordingly, the two research questions guiding this project were: (1) How effective is high-fidelity simulation training in improving students’ knowledge and competence in managing septic shock? Secondary hemophagocytic lymphohistiocytosis (HLH) is a rare, immunologically driven disorder with a high mortality rate. It is typically diagnosed on the basis of clinical and laboratory criteria. We present the case of a 53-year-old woman with multiple previous hospitalizations for fever of unknown origin and fatigue. Her workup ultimately led to a diagnosis of HLH. We discuss the presentation, diagnostic criteria, and clinical treatment of secondary HLH to guide workup and management for future patients affected by this rare disease.

Problem. The physiological and psychological stress caused by adverse social conditions affecting minority groups impacts their ability to function in academia, the workplace, and other areas of life.

Purpose. This literature review examines the impact of minority stress on nurses' and nursing students' perceptions of their environment and overall satisfaction with the profession.

Search Strategy. Articles for this literature review were found using electronic databases CINAHL and PubMed.

Results. Twenty-two articles were selected for analysis. All articles were appraised using Joanna Briggs Institute tools to ensure validity.

Background: Acute Lymphoblastic Leukemia (ALL) is the most common pediatric cancer, with significant advancements in treatment leading to over 90% five-year event-free survival rates in developed countries. However, disruptions in health behaviors during treatment, such as diet, activity, and sleep, can negatively impact treatment tolerance and increase risks of late effects. Inadequate interdisciplinary collaboration in end-of-life care can result in moral distress among nurses, reflecting conflicts between core nursing values and physician expectations. This quality improvement project seeks to reduce moral distress by enhancing collaboration between physicians and nurses in providing end-of-life care in a defined microsystem on the Transitional Care Unit (TCU). A pre-survey of registered nurses (RNs) revealed that moral distress occurs when caring for actively dying patients, regardless of their code status. Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis usually linked to autoimmune or inflammatory bowel disease and typically affects the lower extremities. This case is notable for extensive upper-body, oropharyngeal, and nasal involvement in a patient without an underlying autoimmune disorder. A 56-year-old man with chronic cocaine use and stage IV renal cell carcinoma presented with rapidly progressive ulcerations of the face, neck, shoulders, and back, along with nasal septal collapse and oropharyngeal destruction. Prior biopsy demonstrated sterile neutrophilic inflammation without vasculitis, infection, or malignant infiltration, supporting PG as a diagnosis of exclusion. It is highly preventable through routine screening and early detection, yet national data indicate that one in three eligible adults remains unscreened. This quality improvement (QI) project aimed to enhance adherence to the Kidney Disease Outcomes Quality Initiative (KDOQI) guidelines for placement of tunneled dialysis catheters (TDCs) in intensive care unit (ICU) patients. The project addressed inconsistent application of best practices related to TDC placement, improved documentation, improved communication using TigerConnect, a reduction in inappropriate catheter placements, and maintenance of a zero hospital-acquired tunneled dialysis catheter-related bloodstream infection (CRBSI) rate. Background: Cancer-Related Cognitive Impairment (CRCI), often termed "chemo brain," impacts 50-70% of chemotherapy patients and 12-75% of cancer survivors, with symptoms such as memory difficulties, attention issues, and executive dysfunction potentially lasting up to 20 years. Most research has centered on breast cancer, leaving gaps for other cancer types. Non-treatment factors can also induce CRCI, including the cancer itself. The mechanisms of CRCI share similarities with Alzheimer's disease. Currently, there is no standardized assessment tool for CRCI, emphasizing the need for tailored evaluation methods.
