Identifying Best Practices To Decrease The High Rate Of 30-Day Rehospitalization Of Patients With Dementia From Skilled Nursing Facilities, Mavis Johnson Jan 2026

Identifying Best Practices To Decrease The High Rate Of 30-Day Rehospitalization Of Patients With Dementia From Skilled Nursing Facilities, Mavis Johnson

Walden Dissertations and Doctoral Studies

Abstract As CMS continues to prioritize quality care over quantity and positive patient outcomes, hospitals and nursing homes must ensure that their mission and goals align. The high rehospitalization rate is a current and ongoing concern among policymakers and healthcare administrators. Adverse patient outcomes and increased financial costs to both patients and Health Service Organization (HSO) administrators highlight the need to identify best practices to help reduce rehospitalization. The purpose of this integrative review is to identify best practices for reducing the high rate of 30-day rehospitalization among patients with dementia in skilled nursing facilities (SNF). The review question that …

Evaluating Theories On End-Of-Life Dreams And Visions With Similar Phenomena In Trauma-Related Populations, Christopher Channy Vinson Jan 2026

Evaluating Theories On End-Of-Life Dreams And Visions With Similar Phenomena In Trauma-Related Populations, Christopher Channy Vinson

Doctoral Dissertations and Projects

A grounded theory approach was used to conduct a study on participants (N=24) who were veterans with trauma-related dreams or who had a near-death experience. Research questions were designed to qualitatively examine the lived experiences of dreams and visions in a non-end-of-life setting. This allowed the data to give greater insight into the end-of-life phenomenon and deepen the body of knowledge on dreams and visions in general. A survey consisting of the PTGI-SF, LuCiD, NExS, and the Greyson NDE, along with open-ended survey questions designed to record dream narratives for a thematic analysis, was used. The dream narratives were coded …

Faculty, Staff and Student Publications

Objectives: To determine whether changes in caregiver self-efficacy (beliefs about one's ability to manage dementia-related problems and access help) differed by caregiver race and ethnicity across all participants enrolled in a large pragmatic trial of comprehensive dementia care.

Methods: In the Dementia Care Study (D-CARE), community-dwelling older adults with dementia and their unpaid family caregivers were randomized to receive usual care, community-based, or health system-based comprehensive dementia care. Caregiver self-efficacy was assessed at baseline, 6 months, and 18 months using a 4-item scale (range 4-20, higher scores indicate greater self-efficacy). Among all caregivers, we compared least squares means for overall …

Kindergarten Teachers’ Experiences And Training With Critical Reflection To Support Adaptive Teaching, Tessa Cushman Jan 2026

Kindergarten Teachers’ Experiences And Training With Critical Reflection To Support Adaptive Teaching, Tessa Cushman

Walden Dissertations and Doctoral Studies

The problem that was addressed in this study is that kindergarten teachers often lack training in critical reflection as part of their adaptive teaching practice. Despite much research on critical reflection, there was a lack of knowledge regarding training kindergarten teachers receive on critical reflection and how they use it to adapt their teaching. The purpose of this study was to explore the experiences of kindergarten teachers regarding the training they received on critical reflection and the support they felt they needed to implement critical reflection for an adaptive teaching practice. Schön’s reflective practice theory was the conceptual framework for …

Brief Overview Of Assessment And Prognostication Tools For Palliative Care Oncology Patients, Nicole Schwensow Jan 2026

Brief Overview Of Assessment And Prognostication Tools For Palliative Care Oncology Patients, Nicole Schwensow

Lynchburg Journal of Medical Science

Patients with life-threatening illnesses can be at risk for the progression of their disease over time. Consequences of such an illness can include recurrent hospitalizations, poor symptom control, and a lack of understanding by the patient and family of the disease, its usual course, and prognosis. Palliative care is a philosophy of care intended to address these issues. While receiving palliative care, patients can continue to undergo treatments directed at a cure, whereas hospice patients no longer receive treatment for underlying diseases. Both approaches focus on symptom control. To best care for palliative patients over time, several outcome domains should …

Multidisciplinary Approach To Reducing Healthcare-Associated Infections: A Dnp Project, Jovinsky Jonaldo Eliscard Jan 2026

Multidisciplinary Approach To Reducing Healthcare-Associated Infections: A Dnp Project, Jovinsky Jonaldo Eliscard

Walden Dissertations and Doctoral Studies

Healthcare-associated infections are increasingly common in healthcare facilities due to multiple concerns, including negligence, human error, and poor conditions. According to Chuwa (2024), secondary infections are a significant threat to patient safety and wellness, reducing an organization’s ability to realize clinical objectives. The facility is a large healthcare provider operating numerous hospitals and care homes across the United States. The organization continues to report increased infection rates across its branches owing to a lack of standardized infection prevention and control protocols, reducing its efficiency and appeal. Internal audits point to inconsistencies in general sanitation, cleanliness, and hygiene. The organization also …

Intravenous Thrombolysis In Patients On Direct Oral Anticoagulants: Analysis Of The Get With The Guidelines Stroke Registry, Shadi Yaghi, Opeolu Adeoye, Et Al.

2020-Current year OA Pubs

BACKGROUND: Intravenous thrombolysis for acute ischemic stroke (AIS) is a proven effective treatment. Whether thrombolysis in patients with AIS with recent direct oral anticoagulant (DOAC) use is safe and efficacious is not well established. We aimed to compare outcomes of patients with AIS and recent DOAC use who received thrombolysis to those otherwise eligible but excluded due to recent DOAC use.

METHODS: This study included patients for the GWTG (Get With The Guidelines) registry with a diagnosis of AIS within 4.5 hours from last known normal, on a DOAC, and either (1) received intravenous thrombolysis, or (2) were excluded from …

A Structured Discharge Bundle To Reduce Heart Failure Readmissions, Carolyn Hudson Jan 2026

A Structured Discharge Bundle To Reduce Heart Failure Readmissions, Carolyn Hudson

Doctor of Nursing Practice (DNP) Scholarly Projects - Archive

Background: Heart failure (HF) remains a major contributor to 30-day hospital readmissions, placing strain on patients, healthcare systems, and reimbursement structures tied to quality metrics. Inconsistent discharge practices and gaps in transitional care coordination increase the likelihood of preventable rehospitalizations.

Purpose: This project examined whether implementing a standardized, nurse-driven discharge bundle would improve transitional care outcomes and decrease 30-day readmissions among adults hospitalized with HF.

Methods: A practice-based quality improvement initiative was conducted in a 465-bed regional hospital over a 8-week period. Using iterative Plan-Do-Study-Act cycles, a structured discharge bundle was introduced for eligible HF patients discharged from medical-surgical and …

Implementing An Anxiety Management Bundle In Long-Term Nursing Facilities, Sundan G. Bryson Jan 2026

Implementing An Anxiety Management Bundle In Long-Term Nursing Facilities, Sundan G. Bryson

Doctor of Nursing Practice (DNP) Scholarly Projects - Archive

There is currently a gap in accurate screening and treatment of anxiety in Long-Term Care (LTC) nursing facilities. The question for this quality improvement project was: Will implementing interventions for long-term care residents aged 65 and older improve anxiety screening, non-pharmacological interventions, and mental health referrals by 10% compared to current practices? Interventions to enhance anxiety screening included the Geriatric Anxiety Scale-Long Term Care (GAS-LTC) tool to assess and score anxiety. In addition, a Non-Pharmacological Intervention (Non-PI) tool was created to evaluate interventions performed by facility staff. Referrals to the mental health care provider were submitted if a resident scored …
