Multiple Endocrine Neoplasia Type 2b: Eighteen-Year Follow-Up Of A Four-Generation Family, Glen W. Sizemore, J. Aiden Carney, Hossein Gharib, Charles C. Capen

Seven members with multiple endocrine neoplasia type 2B from a 15-member family have been followed for 18 years. All affected had the neuroma phenotype in a distribution compatible with autosomal dominant inheritance. The phenotype features have allowed 100% initial and continuing prediction of affected versus nonaffected status in as early as 1.5 years. Among the affected: immunoreactive plasma calcitonin (iCT) concentration was high in 100%; thyroid palpation was false-negative in 71%; and thyroid scintiscan was false-negative in 83%. All had total thyroidectomy, plus lymphadenectomy in three, for bilateral medullary thyroid carcinoma (MTC) or C-cell hyperplasia (in the youngest). None has …

The primary objective of the Health Care Linkage Project, funded by a grant from the Chicago Community Trust, is to develop, implement, and evaluate a primary health care linkage network within the city of Chicago that creates formalized linkages between community health centers, the Chicago Department of Health clinics, and hospitals. Six linkage networks are currently operational, with an additional two sites phased in during 1991. Increasing pressures on private and public hospitals have necessitated a reassessment of urban health care delivery. Patients left unserved by stressed private hospitals have placed a greater burden on public institutions, which themselves are often old, underfunded, and in danger of closure. As policy analysts consider remedies, primary care in community-based settings has reemerged as an important component of planning. We present results of a comparative analysis of five public health care delivery systems (Boston, Dallas, Denver, Milwaukee, and Seattle), reflecting their economic, political, and cultural dynamics. The American Medical Student Association (AMSA) Foundation is assisting the U.S. Public Health Service in increasing the number of primary care physicians trained and committed to practice in medically underserved areas. In collaboration with the American Academy of Family Physicians, the Ambulatory Pediatrics Association, and the Society of General Internal Medicine, AMSA conducted an assessment of federally-funded residency programs to identify and describe their affiliations with federally-funded community and migrant health centers (CIMHCs). Actinomycotic infections, once common in humans and cattle, are now rare causes of disease in man. This general group of organisms belongs taxonomically between the true bacteria and the fungi; however, the organisms behave clinically like true anaerobes. The organism, although phagocytized by the host cells, is not killed. Therefore, d is defined as a facultative intracellular parasite of the host. The protean manifestations of actinomycotic infections often mimic infectious processes, such as osteomyelitis or granulomatous disease, as well as neoplasia. Diagnostic And Therapeutic Potential Of New Radiopharmaceutical Agents In Medullary Thyroid Carcinoma, Luigi Troncone, Vittoria Rufini, Giovina De Rosa, Amerigo Testa

Recently developed radiopharmaceuticals have been proposed for imaging medullary thyroid carcinoma (MTC) with some having therapeutic potential. This study compares the imaging results obtained with radioiodinated meta-iodo-benzylguanidine (MIBG), 99mTc (V) DMSA, and 131I F(ab')2 anti-carcinoembryonic antigen (anti-CEA) in a group of MTC patients. In 23 patients 131I MIBG imaging showed a high specificity (no false-positive results) but a less satisfactory sensitivity (50%). In 12 patients 99mTc (V) DMSA revealed a better sensitivity (77%) but a lower specificity (three false-positive results). Positive results were obtained in two of three patients studied with 131I F(ab')2 anti-CEA. These data suggest that the highly …

Through nationwide surveys, we collected and analyzed 242 patients of medullary thyroid carcinoma (MTC). Included were 40 patients with multiple endocrine neoplasia type 2A (MEN-2A), six patients with MEN-2B, and 36 patients with only MTC having a positive family history (82 total patients in the hereditary group). Ten-year survival rates were 81.5% for all cases, and 97.5% and 76.1% for the hereditary and the sporadic group, respectively. Epinephrine/norepinephrine ratio in the urine was found to be a good indicator of the adrenomedullary hyperfunction in patients with hereditary MTC. Prospects For The Medical Use Of Interferon In 1984, Bosko Postic, Julio C. Arroyo

Interferon, a natural and wide spectrum antiviral polypeptide that was discovered in 1957, has been extensively described. In addition to its antiviral effect, interferon has been found to suppress tumor growth and modulate the immune response. Until 1980, interferon was primarily prepared for clinical trials in human leukocytes derived from the buffy coat of whole blood. A major breakthrough occurred in the late 1970s when human interferon was produced by DNA recombinant methods in such heterologous cells as bacteria (genetic engineering). Interferon is not commercially available in 1984, but it may, based on clinical trials in humans, become part of …

Our approach to the management of extracranial cerebral vasculature is discussed first, and increasingly more complex areas are then presented. Recently, an association was reported between a form of hemolytic-uremic syndrome and the use of adjuvant chemotherapy with mitomycin C and 5-fluorouracil. Our report presents the clinical course of a patient with this syndrome along with detailed renal pathological studies (including electron and immunofluorescence microscopy). This information is not available for most previous reports.

A Role For Dentistry In Managing The Oro-Nasopharyngeal Irradiated Cancer Patient, Michael R. Britt

Radiation therapy for oral and nasopharyngeal carcinoma produces direct and indirect changes in the oral cavity. Before such therapy is begun, the cancer patient should be examined by a trained dental specialist so that associated problems such as oral mucositis, xerostomia, radiation dental caries, osteoradionecrosis, and ageusia can be addressed. During and after irradiation therapy as well, the dental specialist should be involved in the treatment program. In this way, it Is possible to maintain the patient's dental system through restorative measures, mandatory use of topical fluorides, dental rinses, and preventive prophylaxis.

Proper nutrition is essential in the care of the infant with cleft lip or palate, as these infants are often plagued with feeding difficulties from birth. This paper reviews recent literature on the many feeding practices in use today and discusses the current trend toward a multidisciplinary approach to treatment for the cleft lip/palate child. It also presents a brief review of experimental animal studies which have implicated nutritional deficiences in the causation of cleft lip/palate.
