Background/Objective: Penicillin allergy labels (PALs) contribute to broad-spectrum antibiotic use. Thorough documentation can help prescribers identify and remove unnecessary PALs. We aimed to improve documentation of PALs in three pediatric urgent care (PUC) clinics, using a nurse-initiated quality improvement (QI) approach. Methods: QI interventions included a survey to assess prescriber and nurse confidence, an online educational module, and an algorithm to aid in clarifying PALs. We measured the percentage of PALs with a clarified reaction severity as our primary outcome using annotated control charts. Descriptive and inferential statistics evaluated survey responses between nurses and prescribers. Results: Clarified PAL reaction severity …

Background: The association of overall cardiovascular health (CVH) with changes in DNA methylation (DNAm) has not been well characterized.
Methods: We calculated the American Heart Association's Life's Essential 8 score to reflect CVH in 5 cohorts with diverse backgrounds (mean age 54 years, 55% women, and enrollment year ranging from 1989 to 2012). Epigenome-wide association studies (EWAS) for Life's Essential 8 score were conducted, followed by bioinformatic analyses. DNAm loci significantly associated with Life's Essential 8 score were used to calculate a CVH DNAm score. We examined the association of the CVH DNAm score with incident cardiovascular disease (CVD), cardiovascular …

Excessive daytime sleepiness (EDS) is linked to adverse clinical outcomes. This study evaluated changes in a validated tool to assess EDS, the Epworth Sleepiness Scale (ESS) and mortality risk. This retrospective cohort study included Veterans receiving sleep-related services in the Department of Veterans Affairs (VA) from October 4, 1999 to August 18, 2018, with two qualifying ESS measures. ESS values were extracted from patient notes using a validated natural language processing (NLP) pipeline (96% accuracy). ESS scores were categorised as Normal (0-10) or Abnormal (11-24). Patients were grouped based on ESS changes: Normal-Normal, Normal-Abnormal, Abnormal-Abnormal and Abnormal-Normal. Cox proportional hazards …

Biomolecular phase separation is an emerging theme for protein assembly and cellular organisation. The collective forces driving such condensation, however, remain challenging to characterise. Here we show that tracking the dilute phase concentration of only one component suffices to quantify composition and energetics of multicomponent condensates. Applying this assay to several disease- and stress-related proteins, we find that monovalent ions can either deplete from or enrich within the dense phase in a context-dependent manner. By analysing the effect of the widely used modulator 1,6-hexanediol, we find that the compound inhibits phase separation by acting as a solvation agent that expands …
Strategies To Improve Psychiatrist Understaffing In Psychiatric Hospitals In Rural And Underserved Communities: Addressing Administrative Challenges, Carla Aramouni
Walden Dissertations and Doctoral Studies
Psychiatric hospitals across the United States face a worsening shortage of psychiatrists, particularly in rural and underserved communities, which limits timely mental health care, increases wait times, and places unsustainable strain on the psychiatric workforce. The purpose of this integrative review was to identify administrative strategies that can mitigate psychiatrist understaffing and support sustainable care delivery in psychiatric hospitals. An integrative review methodology was used to explore both empirical and nonempirical literature, resulting in 92 articles, including 32 peer-reviewed sources and several supporting studies selected through a systematic search, screening, and critical appraisal. Thematic analysis of the evidence revealed five …

To explore potential chemoresistance mechanisms and identify therapeutic opportunities in muscle-invasive bladder cancer (MIBC), we conduct comprehensive proteogenomic characterization of 46 pre- and 14 post-treatment MIBC tumors incorporating genomics, transcriptomics, proteomics, and phosphoproteomics. Multi-omics clustering not only recapitulated established molecular subtypes but also revealed subtypes associated with chemotherapy sensitivity. Protein isoform level analysis identifies protein abundance of a short isoform of ATAD1 and RAF family proteins as biomarkers of chemosensitivity. Integration of proteomic and phosphoproteomic data reveals Wnt signaling via GSK3B-S9 phosphorylation and the JAK/STAT pathway as potential targets to overcome chemoresistance. Correlations between PD-L1 and TROP-2/NECTIN-4 indicate an additive …

B complex vitamins, a group of eight water-soluble vitamins, play interconnected roles in maintaining nervous system health. Thiamine (B1), riboflavin (B2), and niacin (B3) are essential as co-enzymes in numerous metabolic reactions related to energy production. Thiamine is involved in the Krebs cycle, riboflavin in the electron transport chain, and niacin plays a key role in both glycolysis and the Krebs cycle. These metabolic processes are vital for sustaining the integrity of the nervous system, as the energy produced is critical for the functioning of nerve cells. Deficiencies in these vitamins can lead to significant neurological and psychiatric conditions, including …

In Vera Institute of Justice v. Department of Justice, as in a raft of other cases involving constitutional, statutory, and regulatory challenges to the rapid mass cancellation of federal grants or grant programs, the government has deployed jurisdictional arguments in an effort to restrict the judiciary’s ability to review the legality of executive actions. Specifically, the government has attempted to confine jurisdiction over the plaintiffs’ claims to the Federal Court of Claims, which is powerless to remedy the alleged violations. The arguments by which the government would deny judicial review of the plaintiffs’ claims conflate two distinct considerations: first, …

Development Of A New Ground-Based Instrumentation Technique For Low-Resolution Exoplanet Transmission Spectroscopy, Mary Anne Limbach, Luke M. Schmidt, Ryan J. Oelkers, Darren L. Depoy
Physics & Astronomy Faculty Publications
We present a new instrumentation technique for high-precision, ground-based spectrophotometric measurements ideal for low-resolution (R ∼ 20–60) exoplanet transmission spectroscopy. This technique employs novel thin-film coating technology for wide-band, simultaneous multi-band imaging, enabling high-precision differential photometry through self-referencing and comparison stars. Theoretical calculations and on-sky results show that this method effectively reduces 96% of amplitude scintillation noise and other systematics that typically limit ground-based spectrophotometric precision for bright stars. On-sky results are demonstrated using the custom-built Exoplanet Transmission Spectroscopy Imager (ETSI), deployed at the McDonald Observatory 2.1 m telescope in 2022. For a V = 9.7 mag star, ETSI …

An Educational Intervention Focused On Identifying Adverse Childhood Experiences, Laconia Bacon
An Educational Intervention Focused On Identifying Adverse Childhood Experiences, Laconia Bacon
Walden Dissertations and Doctoral Studies
Adverse childhood experiences (ACEs) refer to traumatic childhood experiences that individuals may have been exposed to during their youth, such as child maltreatment, domestic violence, or interparental violence. Research has demonstrated that experiencing ACEs in childhood may result in physical and mental illnesses that led to dangerous lifestyle patterns in adulthood. In the local organization, several patients with ACEs presented in the emergency department (ED). The nurses, unaware of ACEs, were unable to detect and respond to the consequences of ACEs. Thus, the purpose of this Doctor of Nursing Practice (DNP) project was to determine whether an educational intervention focused …

Strategies For Volunteer Firefighter Recruitment And Retention, Teddy Gene Abbey
Walden Dissertations and Doctoral Studies
Volunteer fire departments (VFDs) provide essential services in rural areas that cannot afford full-time staff, and they are negatively impacted by declining recruitment and retention. VFD administrators who lack strategies to recruit and retain volunteer firefighters risk reduced emergency services and increased costs for hiring paid firefighters. Grounded in Herzberg’s two-factor theory, the purpose of this qualitative pragmatic inquiry project was to identify and explore strategies some VFD administrators use to recruit and retain members and maintain necessary emergency services. Participants were six administrators from VFDs in Oklahoma and Arkansas with community populations of 1,500 citizens or fewer residents who …

The Perceived Impact Of Officer-Involved Domestic Violence On Law Enforcement Administrators, Victoria Smith
Walden Dissertations and Doctoral Studies
The frequency of officer-involved domestic violence (OIDV) is a serious problem in the law enforcement profession. This qualitative phenomenological study aimed to determine what measures and specific domestic violence policies are working well and what is working poorly in Florida. This study is critical because it can aid upcoming law enforcement administrators (LEAs) and current LEAs in improving the policies and measures they use in their agency to combat OIDV among their officers and recruits. Jacques’s organizational culture theory (OCT) provided a framework for this study. I used purposeful snowball sampling methods to recruit participants for this study. Six law …

Educating Nurses On The Agency Of Healthcare Research And Quality Pressure Ulcer Prevention Program To Reduce The Rate Of Pressure Ulcers, Marie France Destaul
Walden Dissertations and Doctoral Studies
A staff development project was conducted to address the practice problem encountered in a healthcare setting. The identified gap in practice was related to a lack of knowledge of pressure ulcer injury prevention among inpatient staff. Leadership deemed it essential to address this practice problem in nursing care because the prevalence of pressure ulcer injuries in hospitalized patients was high. The following practice-focused question guided the project: For staff in an inpatient setting, will education on the Agency of Healthcare Research and Quality (AHRQ) Pressure Ulcer Prevention Program increase knowledge toward the goal of decreasing the incidence of pressure ulcers? …

A Qualitative Study On The Experiences Of Burnout Among Mental Health Practitioners Rendering Clinical Care During Covid-19, Mykiyaa Marvella Walker-Sanchez
Walden Dissertations and Doctoral Studies
Prior to the COVID-19 pandemic, between 21% and 67% of mental health practitioners in the United States experienced signs of burnout. The pandemic exacerbated these challenges, with an increased demand for mental health services. By 2022, 45% of practitioners reported burnout, and the APA’s 2023 Practitioner Pulse Survey noted that psychologists were working at the limits of their capacity, with high levels of burnout. While research on COVID-19 and burnout has expanded for physicians and nurses, less focus has been placed on mental health practitioners' experiences. The purpose of this generic qualitative study aimed to understand how mental health practitioners …

Towards Scalable Schema Mapping Using Large Language Models, Christopher Buss, Mahdis Safari, Arash Termehchy, David Maier, Stefan Lee
Computer Science Faculty Publications and Presentations
The growing need to integrate information from many diverse sources poses significant scalability challenges for data integration systems. These systems often rely on manually written schema mappings, which are complex and costly to maintain. While recent advances suggest that large language models (LLMs) can assist in automating schema mapping, key challenges remain. We motivate future research in schema mapping generation by highlighting key challenges, presenting a competitive bidirectional schema matching pipeline, and exploring the limitations of current methods for generating more complex mappings.

The development of motor control over sensory organs is a critical milestone, enabling active exploration and shaping of the sensory environment. Whether the onset of sensory organ motor control directly influences the development of corresponding sensory cortices remains unknown. Here, we confirm and exploit the late onset of whisking behavior in mice to address this question in the somatosensory system. Using ex vivo electrophysiology, we describe a transient increase in the intrinsic excitability of excitatory neurons in layer IV of the barrel cortex, which processes whisker input, immediately following the onset of active whisking on postnatal days 13 and 14. …

Corticotropin-releasing hormone (CRH) is a peptide associated with stress and anxiety that acts as a potent modulator throughout the nervous system. The thalamic reticular nucleus (TRN) displays high expression of the CRH receptor 1 (CRHR1), but whether CRH modulates key TRN functions, such as sleep spindle rhythmogenesis, remained unexplored. Combining polysomnographic and photometric recordings in mice, we show that CRH release in TRN during non-rapid-eye movement sleep (NREMS) oscillates with a ~50-s periodicity, anti-correlating with sleep spindle dynamics. Optogenetic manipulations of CRH release in TRN modulated NREMS fragmentation through microarousals with corresponding changes in sigma and delta power. In ex-vivo …

Viral infection induces robust reprogramming of metabolic pathways in host cells. However, whether host metabolic enzymes detect viral components remains unknown. Our group and others previously identified O-GlcNAc transferase (OGT), an important glucose metabolic enzyme, as a crucial mediator of the antiviral immune responses. Here, by studying a mouse model with a catalytically impaired OGT, we discover a catalytic activity-independent function of OGT in restraining influenza A virus (IAV) infection in addition to its catalytic activity-dependent effect on MAVS-mediated antiviral immunity. Biochemical studies reveal a critical antiviral effect based on OGT interacting with IAV genomic RNA that requires its N-terminal …
