Permanent Pacemaker Implantation After Tricuspid Valve Repair Surgery, Alyssa Morrison Jan 2024

Permanent Pacemaker Implantation After Tricuspid Valve Repair Surgery, Alyssa Morrison

Yale Medicine Thesis Digital Library

Recent studies suggest that permanent pacemaker implantation after tricuspid valve repair may be more common than previously thought. Our primary aim was to assess the rates of early (within 30 days of surgery) and late (more than 30 days after surgery) permanent pacemaker implantation (PPI) after tricuspid valve repair (TVr) surgery. Our secondary aims included exploring risk factors for PPI after TVr and assessing outcomes.

We performed a retrospective analysis of 231 isolated and concomitant tricuspid valve repairs for primary and secondary tricuspid valve regurgitation at a single quaternary academic center performed from 2014–2022.

Early pacemaker implantation was performed in …

Maternal And Donor Human Milk Support Robust Intestinal Epithelial Growth And Differentiation In A Fetal Intestinal Organoid Model, Lauren Smith

Yale Medicine Thesis Digital Library

Necrotizing enterocolitis (NEC) is a severe gastrointestinal complication of prematurity. Using small intestinal organoids derived from fetal tissue of a gestational age similar to an extremely preterm infant, this study aims to assess the effect of diet on intestinal epithelial growth and differentiation to elucidate the role nutrition type plays in intestinal development and modifies the risk for NEC.

Organoids were cultured for 5 days in organoid growth media and 5 days in organoid differentiation media supplemented 1:40 with four different diet conditions: maternal milk (MM), donor human milk (DHM), standard formula (SF), or extensively hydrolyzed formula (HF). 4x images …

Jetblue Airways Corporation, Jeffrey S. Harrison, Gabby Petrizzo

Robins Case Network

JetBlue commenced operations on February 11, 2000. The company was founded by David Neeleman, an extremely dedicated entrepreneur who, consistent with his Christian roots, wanted to create an airline with more humanity. Neeleman first started working at his grandfather's convenience store when he was just nine years old. The store had a heavy emphasis on customer service. On NPR’s “How I Built This” podcast, Neeleman explained that when the store didn't have something in stock that a customer was looking for, his grandfather would go to another store down the street, buy it, and then deliver it. Consistent with its …

Microsoft And Activision Blizzard: The Rise Of The Video Gaming Giant, Ramid Kadirov, Jeffrey S. Harrison

Robins Case Network

Satya Nadella, appointed CEO of Microsoft since February 2014, envisioned Microsoft at the forefront of the video gaming competition. In September of that year, Nadella led the company in the acquisition of video-gaming publisher Mojang, popular for its world-building game Minecraft, for $2.5 billion.2 In 2015, Microsoft released Windows 10, which elevated its gaming features such as built-in support for Xbox Live and game streaming functionality. Further enhancing its Xbox offerings, Microsoft launched Xbox Game Pass in 2017, which was essentially “Netflix for video games,” allowing users to play games by paying a subscription fee. A year later, Microsoft introduced …

Weight Health In The Modern Lens: Investigating The Intersectionality Of Obesity, William Bohne, Anthony Onochie

Scholarship in Medicine - All Papers

Purpose The purpose of this study is to investigate the relationship between these social determinants of health and obesity to provide a better understanding of geographical differences in obesity rates. Methods All data was collected using countyhealthrankings.org. The variables that were studied included access to exercise, food insecurity, and life expectancy due to previous literature demonstrating probable association between these variables and the prevalence of obesity. Results Statistically significant increase in number of adults with obesity from 2016 to 2022 (32% to 37%). Statistically significant weak, negative correlation between access to exercise and obesity (r=-.306). Statistically significant weak, negative correlation …

Associations Between The Lived Environment And Health Markers In Ohio Counties, Jake Trzybinski, Darbi Griffith, Brian Johns

Scholarship in Medicine - All Papers

Environmental exposures have known to be associated with poor health outcomes in adults, however there is scarce literature on which exposures that are county-specific influence health outcomes in the state of Ohio. There are known longitudinal effects of the environment on a person’s physical and mental health and demonstrated correlations could have the potential to improve health outcomes of those living in communities disproportionately impacted by environmental stressors. Through a retrospective review using County Health Rankings and the defined parameters of rurality, we looked at various measures of environmental exposures and living conditions and their effect on diabetes rates, chlamydia …

Transpersonal Approaches To Clinical Supervision, Harris L. Friedman Jan 2024

Transpersonal Approaches To Clinical Supervision, Harris L. Friedman

International Journal of Transpersonal Studies

Transpersonal psychology can provide unique approaches to clinical supervision by offering an interconnected self-expansive view of the self across time and space, as well as can provide a secular avenue to consider the importance of spirituality without relying on supernatural assumptions. Transpersonal supervisors can challenge conventional notions about what and how supervisees know, and even the nature of being a knower, through providing a more inclusive, perhaps even holistic, vantage that is both critical of mainstream approaches and surpasses their ability to provide a coherent “super” (i.e., going beyond the usual more-myopic understandings) vision. Background on transpersonal psychology related to …

Dreams, Synchrony, And Synchronicity, Terry Marks-Tarlow Jan 2024

Dreams, Synchrony, And Synchronicity, Terry Marks-Tarlow

International Journal of Transpersonal Studies

This essay presents the dreams of a long-term patient over the course of treatment in order to explore the transpersonal potential of the relational unconscious—those open channels of unconscious communication between people. At a key moment, the patient brings in the therapist’s own childhood dream, serving to break a period of impasse and reset the therapy. Dreams and their interpretation reveal the essence of fractal consciousness, by which a sliver of experience can shed light on the whole of the psyche. A fractal model of understanding suggests open boundaries between self/other and self/world at multiple levels. The deep intimacy of …

Comparison Of 3 Treatments For Fractures Of The Humeral Shaft, Neil Werthmann, Mason Poffenbarger, John Riehl

North Texas GME Research Forum 2024

INTRODUCTION: Over the past 40 years, surgical treatment for humeral shaft fractures has become more common. The purpose of the study was to evaluate complication rates seen with different treatments for humeral shaft fractures. Nerve injury (NI), infection, deep vein thrombosis (DVT), and nonunion (NU) were the specific complications investigated. METHODS: A retrospective database review was performed looking at all patients with humeral shaft fractures. Data was analyzed for complication rates for non-surgical (NS) treatment, open reduction internal fixation (ORIF), and intramedullary nailing (IMN). Inclusion criteria were: 1) patients ages of 18 to 89 and 2) those with humeral shaft …

A Unique Case Of Spontaneous Pectoral Abscess, Martin Sentmanat, Zachary Shields, David Ross Jan 2024

A Unique Case Of Spontaneous Pectoral Abscess, Martin Sentmanat, Zachary Shields, David Ross

North Texas GME Research Forum 2024

Pyomyositis is a rare disease that is often misdiagnosed. Formerly more common in tropic areas, it has been more prevalent in temperate climates in the past couple of decades. Most cases of pyomyositis have a known cause or the patient is immunocompromised, but there are a few documented cases of spontaneous pyomyositis in an otherwise healthy patient. Our case describes a 35-year-old male with no past medical history or history of IV drug use who presented to the emergency department with right sided chest pain. The patient was initially seen and diagnosed with a pectoral muscle strain and fever. He …

A Common Complaint With An Unexpected Diagnosis: A Case Of Mycobacterium Avium Complex Pulmonary Abscesses, Maryam Fallahi, Mark Cheneler, Qazi Haider, Madhukanth Reddy

North Texas GME Research Forum 2024

Introduction: Mycobacterial infections are an atypical organism for pulmonary infections. These infections are classically separated in tuberculous (MTB) and nontuberculous (NTM) groups due to their differing pathogenicity and microbiologic therapy regimens. MTB is typically treated with a 4-drug therapy known as RIPE (rifampin, isoniazid, pyrazinamide, and ethambutol) however NTM is typically treated with 3- or 4-drug therapy with a backbone of rifampin, macrolide, and ethambutol. Due to the long courses of therapy required for NTM, stringent diagnostic criteria of clinical, radiologic, and microbiologic findings are required. Despite meeting all 3 criteria for diagnosis, not every patient is a candidate for …

Four Cesarean Scar Pregnancies Managed At Accreta Center Of Excellence At Medical City Dallas Hospital: A Case Series, Ana Adams, Nicole Tenzel, Monica Rodriguez, Robert Cohen

North Texas GME Research Forum 2024

Cesarean Scar Pregnancy results from pathologic implantation of a gestational sac into a hysterotomy scar. This rare disease continues to increase in incidence due to the rise of Cesarean delivery in the United States. Current management recommendations by the Society of Maternal Fetal Medicine (SMFM) include termination of pregnancy, or in rare instances, hysterectomy. This case series examines four patients diagnosed with Cesarean Scar Pregnancies that either declined termination or were unable to receive abortion services in the first trimester, and who were managed at the Accreta Center of Excellence at Medical City Dallas Hospital. This novel management strategy included …

A Case Report Of Florid Liver Failure Responding Favorably To Targeted Therapy With Cetuximab And Encorafenib Following Treatment With Irinotecan And Bevacizumab In Metastatic Colorectal Cancer, Maryam Fallahi, Omar Garcia, Joseph Kim, Ray Page

North Texas GME Research Forum 2024

Introduction: Colorectal cancer, commonly known as colon cancer, is a globally widespread and formidable malignancy originating from the epithelial cells lining the colon and rectum. This disease, demonstrating geographical and demographic variations, particularly prevails in Western countries. The progression of colon cancer typically spans years, initiated by the development of adenomatous polyps, presenting an opportune timeframe for early detection and intervention. The substantial morbidity and mortality rates associated with colon cancer underscore its significance as a public health concern. Various risk factors, such as age, family history, genetic predisposition, inflammatory bowel diseases, and lifestyle factors like diet and physical activity, …

The Cascading Effect Of Adverse Childhood Experiences And The Urgency For A Trauma-Informed Approach, Whitney Lovett Jan 2024

The Cascading Effect Of Adverse Childhood Experiences And The Urgency For A Trauma-Informed Approach, Whitney Lovett

Integrated Studies

Adverse childhood experiences (ACEs) refer to any traumatic event from birth to 18 years of age that can have negative effects to a child’s development and health. Any form of abuse, neglect, or household dysfunction constitutes an adverse childhood experience initiating immediate and long-term outcomes. Reoccurring and prolonged exposures to adversity can turn into toxic stress. These experiences take a toll on children’s brain development, growth, immune and other regulatory systems, learning, behavior, and genetic outcomes. Although ACEs are common, some populations may be more susceptible to adversities. Families who have limited economic advantages and histories of ACEs, racism, and …

Communism As An Americanism: The Curious Case Of The Red Jeffersonians, Matthew H. Hill Jan 2024

Communism As An Americanism: The Curious Case Of The Red Jeffersonians, Matthew H. Hill

[Archive] Belmont University Research Symposium (BURS)

The communist movement in the United States has struggled with many issues over its long history. One of these problems is the problem of American history itself. The United States, in many ways the quintessential capitalist state, would seemingly represent the ultimate enemy for a communist. It is more than a little bizarre, then, to see the, sometimes intense, admiration that many American communists had for men like Thomas Jefferson and Abraham Lincoln. While contradictory at first, this essay shows the logic behind this admiration, exploring the long history of American communism’s love affair with iconic figures of American history. …
