Problem: Health care systems, especially acute care settings, are becoming overburdened with patients who are older, have medical complexity, and have limited access to health care resources. Community paramedicine (CP) programs use paramedics to offer preventive care, health education, and connections with the community that is underserved and experiencing homelessness. CP programs have emerged to reduce acute care utilization and emergency department visits, and to improve health care outcomes.

Approach: A local CP team was paired with the MaineHealth cardiovascular disease fellows' clinic to expand cardiovascular care to patients at high risk. Individuals were identified by clinicians as requiring additional …

Bridging Fiscal Sustainability And Health Equity: A Policy Analysis Of California’S Healthcare Coverage Safeguards, Guadalupe Castaneda Martinez

Without structural policy intervention, approximately 4.6 million Californians remain at risk of being uninsured by 2030, worsening racial and ethnic health disparities and hospital uncompensated care burdens. This policy analysis advocates for a hybrid universal coverage model combining sliding-scale subsidies (California Senate Bill 78) with full-scope Medi-Cal expansion for all income-eligible residents regardless of legal status (California Assembly Bill 133). The framework relies on existing funding mechanisms, including the Managed Care Organization tax and the Health Care Affordability Reserve Fund. Comparative analysis indicates that market-only subsidies leave severe equity gaps, whereas immediate single-payer transitions pose substantial fiscal and administrative risks. …

Nepal’s constitutional commitment to a “socialism-oriented” state places social justice, equitable access to resources and housing, and protection of marginalized populations at the center of national development discourse. However, persistent landlessness, informal settlements, unemployment, youth outmigration, and unequal access to infrastructure reveal a significant gap between constitutional aspirations and socioeconomic realities.

This article examines lessons Nepal can draw from Tanzania’s experience with Ujamaa, the African socialist development model initiated by President Nyerere in 1967. Rural municipal governments across Texas and the United States face a convergence of workforce and leadership challenges that threaten long-term service delivery and organizational sustainability. An aging public-sector workforce, persistent recruitment and retention constraints, and fragile leadership continuity interact in ways that uniquely affect small and rural communities. This study includes insights regarding potential supervision challenges that are faced by probation officers overseeing domestic and intimate partner violence (IPV) probationers, emphasizing the importance of effective management to enhance offender accountability and victim safety. The relationship dynamic between probation officers and domestic and IPV probationers influences recidivism rates, highlighting the importance of effective and empathetic supervision. The purpose of this study was to identify gaps in training, supervision strategies, and resources, with a focus on integrating attachment-based practices to improve outcomes. Picture books serve as a foundational medium in early childhood education, utilizing interrelated visual and verbal elements to foster linguistic, cognitive, and social development. Despite their pedagogical utility, certain literary works inadvertently perpetuate traditional gender roles, constraining children's conceptualizations of identity. This study evaluates gender representation within eleven selected titles of The Berenstain Bears series. Employing a qualitative multimodal content analysis, the study integrates Butler's gender performativity framework (1990), Callow's multimodal narrative approach (2020), and Adam and Harper's evaluative criteria (2021) to examine how domestic, social, and recreational roles are constructed. The project team, in collaboration with partners and stakeholders, developed a four-hour training course designed to build a foundational understanding of how wildfire hazards affect rail systems infrastructure operations and how this knowledge can help equip urban rail operators with tools and best practices for proactive risk management.
