BACKGROUND: Adults in rural Australia are at elevated risk of cardiovascular disease (CVD). To date, no intervention trials have evaluated the impact of dietitian delivered nutrition therapy using telehealth exclusively in patients recruited in the primary care setting. The primary aim was to assess effectiveness of telehealth delivered Medical Nutrition Therapy (MNT) on change in dietary intake energy, reported as percent energy derived from nutrient-dense (core) foods. Secondary aims included assessment of the intervention effects on percentage weight loss, quality of life, health literacy and patient engagement in their health (patient activation).

Childhood obesity is a major health issue globally. The prevalence rate remains high in Ohio, with associated complications among affected children and adolescents. The current study examined the association among obesity risk knowledge, risk perception, feeding behavior, and child weight outcome while controlling for race, gender, income, and education. We collected quantitative data through an online survey of parents in Northwest Ohio (n = 71) using the obesity risk knowledge scale and factors from the Child Feeding Questionnaire. Nutrition is critical to the health, development, and well-being of children and adolescents, operating through nutrients and the social and physical influences of dietary behaviors. Child and adolescent growth and development depend on complex biological systems interacting with the nutrition ecology. In this review, we focus on bidirectional interactions with the multilevel nutrition ecology, including distal systems (climate, geopolitical, and food systems) and proximal systems (community food options; health, social, and food services; and school policies and programs), all mediated by the family. Introduction: People experiencing homelessness (PEH) face food insecurity, unstable housing and fragmented services that render conventional diabetes pathways unworkable and amplify complications. Methods: Between January and April 2024, we conducted a nationwide, cross-sectional mixed-methods survey of front-line professionals via NHS, inclusion-health and voluntary-sector networks, analysing quantitative data (n = 104) with ANOVA, Kruskal–Wallis tests and ordinal logistic regression, and subjecting free-text responses to reflexive thematic analysis, before converging findings to develop the Integrated Holistic Diabetes Care Model for Homelessness (IHD-CMPH). Urine Cmv Pcr Positivity And Wheezing In Children Under Two Years Old: Implications For Management And Public Health, Jing Wang, Zhaocheng Zeng, Dushuang Zuo, Xiaoyan Zhang, Zehong Su

Objective: Wheezing in infants and toddlers poses a challenge to clinical management due to its multifaceted etiology, including viral infections. Cytomegalovirus (CMV) infection has been implicated in respiratory conditions, mostly in immunocompromised individuals, but its potential role in wheezing in young children remains understudied. Methods: A total of 82 children under two years of age with wheezing were enrolled in this study, including 61 infants (<12 months) and 21 toddlers (12 to 24 months). Children were categorized into CMV-positive (n=49) and CMV-negative (n=33) groups based on urine PCR tests. Hematological profiles were examined on admission in all children. For CMV positive children, additional hematological evaluations were conducted at 2 weeks and 4 weeks following initiation of antiviral therapy. These children received a ganciclovir regimen (Induction: 5 mg/kg IV, q12h for 2 weeks; Maintenance: 5mg/kg IV q24h, 24 weeks). Results: On admission, CMV-positive children had significantly higher white blood cell (WBC) counts (12.16 vs. 8.39 ×109/L, P=0.004), leukocyte counts (6.92 vs. 4.49 ×109/L, P=0.010), and platelet counts (341.02 vs. 258.27 ×109/L, P=0.002) compared to CMV-negative children. During ganciclovir treatment, the elevated WBC, leukocyte, and platelet levels in CMV-positive children showed significant reductions by the second week, with further notable decreases by the fourth week (P<0.05). Notably, wheezing symptoms in CMV-positive children significantly improved after ganciclovir treatment, with subsequent negative CMV test results, indicating a favorable clinical response to antiviral therapy. Conclusion: Considering the multifaceted etiology of wheezing in children under two years of age, this study suggests that CMV infection may be one of the potential causes of wheezing symptoms. These findings highlight the importance of including CMV testing as part of the diagnostic evaluation for wheezing in this population to ensure more comprehensive management.

Objective: We aimed to estimate Entecavir's long-term on-treatment safety and efficacy in young children with HBeAg-positive chronic hepatitis, who acquired perinatally Methods: This is an observational study of 14 nucleoside/nucleotide-naïve children with HBeAg-positive chronic hepatitis who were candidates for Entecavir. Treatment eligibility included persistent elevation of alanine aminotransferase (ALT) for at least 6 months, high HBV viremia, and moderate to severe hepatic inflammation and/or fibrosis. Pakistan’s relationship with the IMF during the 2000 – 2004 period was quite unique and distinctive. Unlike the earlier eight agreements that were concluded with the IMF during 1988-99 and were never implemented beyond the first tranche release it was only in this period that two successive agreements – a nine month Stand-by Arrangement (SBA) in the year 2000 followed by a three year Poverty Reduction and Growth Facility (PRGF) were successfully completed. A Twisting Trail To Diagnosis: Unraveling Morvan’S Syndrome In Caspr2-Associated Autoimmune Encephalitis, Yashfeen M

This case snippet describes the diagnostic journey and clinical management of a 72-year-old gentleman who presented with progressive cognitive decline, involuntary myoclonus, and severe insomnia. Through a detailed clinical workup, including electromyography and antibody testing, he was diagnosed with Contactin-Associated Protein-Like 2 (CASPR2) antibody-associated autoimmune encephalitis manifesting as Morvan’s syndrome, a rare but reversible neuroimmunological disorder. Prompt initiation of immunotherapy led to gradual improvement, underscoring the value of clinical suspicion, multidisciplinary care, and early intervention. This case serves as a reminder to consider autoimmune etiologies in atypical neuropsychiatric presentations, especially in elderly patients.

BACKGROUND: Prior studies showed worse outcomes in obese inflammatory bowel disease (IBD) patients, especially those related to hospitalizations, surgery, and steroid-free remission. Glucagon-like peptide-1 receptor agonists (GLP1-RAs) have demonstrated significant metabolic benefits for patients with type 2 diabetes mellitus (T2DM) and obesity. Hence, GLP1-RAs may improve clinical outcomes in patients with IBD, especially those with obesity. The objective was to systematically evaluate the impact of GLP1-RAs on clinical outcomes in patients with IBD.

Hydrogels are ideal candidates for biomedical applications such as wound healing, tissue engineering, and bone regeneration due to their porosity and similarity to the extracellular matrix of humans. A gelatin/ sodium alginate (SA) hydrogel with the inclusion of polyethylene oxide (PEO) was utilized to achieve enhanced mechanical strength. Hydrogels were prepared at different ratios of polymer solutions to select the hydrogel that exhibited an optimal gelation time. Sophora japonica, a plant leaf extract (SJLE) rich in flavonoids and isoflavonoids, was added to the hydrogel to enhance tissue repair. PURPOSE: Clinical screening systems (CSSs) for Cushing syndrome (CS) validated in Europe have not been evaluated for CS or Cushing disease (CD) in the United States (US). We aimed to evaluate existing CSSs in US patients and develop two new symptom-based CSSs to identify patients with high pre-test probability of disease warranting referral for definitive biochemical workup-one for broad CS screening and one specifically for CD.
