Context: Arginine (Arg) is an important amino acid in T2D as a potent insulin secretagogue and precursor for nitric oxide (NO). Citrulline (Cit), the substrate for de novo Arg synthesis, is mostly produced from glutamine (Gln).
Objective: We aimed to investigate their metabolism in T2D using novel stable isotope tracer approach.
Methods: We studied 42 individuals (21 with T2D, 21 controls). After overnight fasting, blood samples were collected following pulse administration of stable amino acid tracers. Plasma concentrations and isotopic enrichments were measured by LC-MS/MS, and compartmental analyses were performed to calculate their whole-body production (WBP) rates and kinetics.

Background: Type II diabetes mellitus is commonly related to diabetic peripheral neuropathy (DPNP), which manifests as a gradual progression of damage to the peripheral nerves after excluding other causes. As initial alterations may lie hidden, focus has moved to easy crude markers. Neuron-specific Enolase (NSE) was chosen since it increases early after axonal injury, Whereas, Nerve Growth Factor (NGF) reflects neurotrophic activity. The recent COVID-19 pandemic has heightened public interest in noninvasive methods for early diagnosis of infectious diseases. In addition, various government agencies have implemented "infectious disease preparedness" to mitigate future outbreaks. This review highlights conventional and advanced methods for infectious disease diagnosis with an emphasis on emerging mass spectrometry methods. Conventional methods for pathogen identification, such as culture-based techniques and molecular methods, have limitations with respect to sensitivity, specificity, and turnaround time. Recent advances in high-resolution mass spectrometry have revolutionized the field of infectious disease biomarker discovery. Background: Food insecurity affects >2.4 billion people globally and persists across income levels. Characteristics such as low income, low education, and unstable employment do not fully explain this persistence. Social resources embedded in networks, specifically cognitive social capital involving trust, reciprocity, and support, may offset constraints and buffer characteristics associated with a higher probability of food insecurity.
Objectives: This study examined whether social capital was associated with a lower probability of food insecurity and whether this as sociation varied by individual characteristics and country contexts, consistent with buffering and compensation.

This staff education project addressed a practice gap in a psychiatric facility where nonclinical staff frequently submitted incomplete referral documentation for patients with co-occurring medical and psychiatric conditions. Due to the facility’s inability to provide acute medical stabilization, medical clearance is required before nurse practitioners can determine admission appropriateness. The project question was, Does educating intake staff on a standardized referral checklist improve knowledge of referral criteria, as measured by pre- and post-education surveys? Introduction. In recent years, the utilization of central venous catheters (CVC) in medical practice has become increasingly common. One of the complications associated with central venous catheterization is embolization of catheter fragments, which arefound in 1% of cases. Currently, endovascular techniques, such as snare loop catheter, have been developed for the extraction of intravascular foreign bodies. However, there is no strong evidence regarding this method. Introduction. Congenital duodenal obstruction is an anatomical abnormality in newborns. Various intrinsic and extrinsic etiologies can cause duodenal obstruction. The surgical treatment that is commonly performed on duodenal obstruction in neonates is duodenoduodenostomy. However, several etiological conditions, such as preduodenal portal vein (PDPV) and pancreatic annulare cause duodenoduodenostomy nearly impossible to perform, so an alternative surgery procedure with a good outcome is necessary.

Introduction: Pediatric pancreatic injury isuncommon but carries significant morbidity. While non-operative management is standard for low-grade injuries, the optimal approach for high-grade trauma (AAST grade III–V) remains debated. Pancreaticoduodenectomy (Whipple procedure) is rarely performed due to high procedural risks and is typically reserved for severe pancreatic head injury with concomitant duodenal damage.
Case presentations: A 13-year-old boy presented three days after blunt abdominal trauma with epigastric pain and bilious vomiting. CT imaging revealed free fluid, a large retroperitoneal hematoma, and suspected pancreatic head injury. Background:
Children and adolescents with intellectual and developmental disabilities (IDD) are at greater risk for obesity and poor obesogenic behaviors (e.g., physical activity, screen time, diet, sleep) than their typically developing counterparts. The Structured Days Hypothesis (SDH) suggests that in typically developing children and adolescents, obesogenic behaviors worsen during periods of reduced structure (e.g., weekend or summer vacation). However, children and adolescents with IDD have unique factors that may alter how structure (i.e., pre-planned, segmented, adult supervised, out-of-home programs) influences obesogenic behaviors. Dexmedetomidine To Minimize Cognitive Dysfunction After Cardiac Surgery, Lakesha Portis Ivey, Lisa Herbinger

Delirium is a common finding after cardiac surgery and can lead to increased morbidity and mortality rates and prolonged intensive care stays. Patients suffering postoperative delirium can experience neurological decline 12 months after surgery regardless of baseline neurological status. Postoperative delirium associated with cardiac surgery is hypothesized to be caused by surgical stress, decreased cardiac output, decreased intracranial perfusion, and prolonged exposure to cardiopulmonary bypass leading to disrupted neuroinflammatory pathways leading to neuronal injury. Dexmedetomidine, an alpha-2 adrenergic agonist, can be neuroprotective and may decrease the incidence of postoperative delirium in cardiac patients.

A south-suburban Cook County, IL city lacks grocery stores with high-quality food options, which contributes to food insecurity. Residents often rely on highly processed, low-nutrient foods, contributing to nutritional vulnerability. Friendship Church’s food bank sporadically serves this food desert, but sustaining its operations is challenging. Using community-based participatory action research design and the food security theory, data were collected from 25 semistructured interviews with food bank beneficiaries and church staff/volunteers and analyzed using reflective thematic analysis. The purpose of this qualitative phenomenological study is to explore solvability factors and psychological stressors experienced by investigators and detectives who work on cold case sexual homicides. The study includes eight participants’ perspectives of conditions that influence case solvability and psychological challenges inherent in investigating unresolved sexual homicide cases. Guided by the resource-based theory, the research involved examining how investigative techniques, organizational support, and available resources contribute to the successful closure and clearance of cold cases. This Doctor of Nursing Practice (DNP) project was a practice-focused staff education initiative designed to improve clinical staff knowledge, confidence, and readiness to educate adult patients enrolled in a Tirzepatide weight-management program. Tirzepatide has demonstrated superior weight-loss outcomes compared to other glucagon-like peptide-1 (GLP-1) receptor agonists; however, inconsistent staff education practices related to medication administration, adverse-event management, and lifestyle counseling present a gap in practice. Variability in staff knowledge and teaching approaches may reduce confidence, contribute to inconsistent patient education, and limit the effectiveness of evidence-based care delivery. Objectives: This study aimed to perform a comparative evaluation of glycaemic control using fructosamine and glycated haemoglobin (HbA1c) and to investigate the correlations between the means of these biomarkers and mean glycaemia in pregnant women. Methods: This prospective longitudinal study was conducted from January to December 2020 and included pregnant women with pregestational diabetes, recruited at the National Institute of Nutrition of Tunis, Tunis, Tunisia, before 12 weeks of amenorrhoea and followed-up until delivery. Objective: To examine obesity research trends in Arab countries to inform policies and interventions.
Methods: A bibliometric analysis was conducted using the Web of Science database accessed via King Abdulaziz University’s electronic library. Publications on obesity research in Arab countries from 2004 to 2023 were analyzed using R-Bibliometrix. Key parameters included publication volume, leading authors and institutions, funding sources, keyword co-occurrence networks, and collaboration patterns.
Results: Obesity-related research output in Arab countries demonstrated consistent growth from 2004 to 2023, with a threefold increase in publications since 2015. Regional and international collaborations have intensified, although disparities in research productivity persist across …

Objectives: Celiac disease (CD) is a prevalent autoimmune disorder triggered by gluten consumption, leading to an immune response that damages the small intestine. Research shows a link between CD and autoimmune thyroid disorders (AITD). This study seeks to determine the prevalence of AITD among adult and pediatric patients with CD in the primary care setting in Riyadh, Saudi Arabia.

Colorectal cancer has become a major public health concern in Saudi Arabia. It is the most prevalent cancer among men and the third most prevalent among women. Although screening for people aged 45-75 is recommended by national recommendations, screening uptake is still below 10%, and less than 1% of older persons have a colonoscopy. With regional differences that vary from age-standardized rates of 3.2 per 100,000 in Jouf to 21.8 per 100,000 in Riyadh among men, approximately 29% of cases are detected at late stages.
