Nutritional epidemiology aims to link dietary exposures to chronic disease, but the instruments for evaluating dietary intake are inaccurate. One way to identify unreliable data and the sources of errors is to compare estimated intakes with the total energy expenditure (TEE). In this study, we used the International Atomic Energy Agency Doubly Labeled Water Database to derive a predictive equation for TEE using 6,497 measures of TEE in individuals aged 4 to 96 years. The resultant regression equation predicts expected TEE from easily acquired variables, such as body weight, age and sex, with 95% predictive limits that can be used …

Heart failure (HF) affects 6 million people in the US, and type 2 diabetes (T2D) is a significant risk factor for HF. Since 2008, regulatory agencies have required cardiovascular safety trials for new T2D therapies, and these have consistently demonstrated the cardiovascular benefits of glucose-lowering drugs. Sodium-glucose cotransporter inhibitors (SGLTi) and glucagon-like peptide-1 (GLP-1) receptor agonists (RAs)/glucose-dependent insulinotropic polypeptide (GIP) reduce major adverse cardiovascular events and deaths, regardless of diabetes status, and improve cardiorespiratory fitness. SGLTis, such as dapagliflozin and empagliflozin, benefit patients with reduced and preserved ejection fraction, significantly reducing HF hospitalizations and cardiovascular deaths. The DAPA-HF and EMPEROR-Reduced …

The incidence of obesity related glomerulopathy (ORG) is rising worldwide with very limited treatment methods. Paralleled with the gut-kidney axis theory, beneficial effects of butyrate, one of short-chain fatty acids produced by gut microbiota, on metabolism and certain kidney diseases have gained growing attention. However, the effects of butyrate on ORG and its underlying mechanism are largely unexplored. In this study, a mice model of ORG was established with high-fat diet (HFD) feeding for 16 weeks, and sodium butyrate treatment was initiated at the 8th week. Podocytes injury, oxidative stress, and mitochondria function were evaluated in mice kidney and validated …

Diaprepes abbreviatus is an agricultural pest known to affect around 270 plant species across the Caribbean and the United States, posing significant challenges to pest management. Chemical control dominates management, but environmental and health concerns motivate microbiome-informed alternatives. However, limited information exists on the gut anatomy, physicochemical environment, and microbial composition of D. abbreviatus. In this study, we provide the first comprehensive characterization of the gut morphology, pH, and microbiota of adult D. abbreviatus in both females and males collected in Puerto Rico. Using dye-based gut tracing, we identified foregut, midgut, and hindgut or posterior gut compartments, and confirmed the …

Introduction: The intestine has molecular and functional diversity across the proximal-distal and the crypt-villus axes, so it is imperative to determine the common and compartment-specific molecular actions of vitamin D. However, very little work on vitamin D mediated gene regulation has been done in normal human intestine. Here, we examined the impact of 1,25-dihydroxyvitamin D (1,25(OH)2D3) on cultures of human intestinal epithelium derived from duodenum (Dd) and distal colon (Co) biopsies of 6 subjects per tissue.
Methods: Human enteroids and colonoids were cultured for 3 days to promote a stem cell phenotype (undifferentiated, Un) or to induce differentiation (Diff) and …

Background: Cardiac arrest in children is associated with high morbidity and mortality, primarily due to neurological injury. Biomarkers linked to brain injury, released into circulation from compromised elements of the neurovascular unit, act as significant prognostic indicators in patients suffering from hypoxic-ischemic brain injury (HIBI) subsequent to the restoration of spontaneous circulation (ROSC) after pediatric cardiac arrest. The aim of this systematic review and meta-analysis is to evaluate the prognostic utility of brain injury biomarkers in predicting neurological outcomes and survival in patients following cardiac arrest in the pediatric population.

Preterm birth (PTB) is a syndrome arising from multiple etiologies that manifest as a final common phenotype, delivery before full term. Current knowledge gaps in epidemiologic, basic science, and clinical fields have limited our understanding of this complex pregnancy syndrome. Lack of insight into the cellular and molecular pathways underlying spontaneous PTB (PTB) has thus limited effective clinical management and restricted the investigation of novel treatments or druggable targets. Here, we examine several areas of domain-driven research that may lead to a better understanding of PTB, including infection and inflammation that drive early labor, social factors and their biological consequences …

BACKGROUND: People living with Sickle Cell Disease (SCD) experience higher rates of common mental disorders (CMD). There is an alarming treatment gap in the provision of adequate mental health services for CMDs in low- and middle-income countries (LMIC). One solution is the implementation of task-sharing interventions such as the Friendship Bench which utilizes concepts of problem-solving therapy (PST). This investigation uses a qualitative study design to evaluate the acceptability and feasibility of implementing a PST-based task-sharing mental health intervention for SCD populations in LMICs using the Consolidated Framework for Implementation Research (CFIR).
METHODS: Purposive, convenience, and snowball sampling strategies were …

Type 1 diabetes (T1D) is a chronic autoimmune disease characterized by the T cell-mediated destruction of insulin-secreting pancreatic β cells, leaving patients reliant on exogenous insulin to establish normoglycemia. Despite advancements in glucose management, the precise pathological mechanisms linking genetic predisposition and environmental triggers to the loss of immune tolerance remain incompletely understood, hindering the development of preventative therapies. This comprehensive review synthesizes clinical, experimental, and epidemiological data to detail the intricate pathogenesis of T1D, focusing on the convergence of autoimmunity, high-risk genetics, and enteroviral infection. We discuss how T1D is fundamentally a disease caused by failures in central and …

Colorectal Cancer (CRC) is the second leading cause of cancer-related deaths for adults in the United States. There have been significant advances in preventative medicine for CRC, but screening completion lags behind other cancers, with only 61% reported adherence based on USPSTF guidelines. About two-thirds of the patients who are diagnosed with CRC already have distally spread disease. Detecting CRC early is vital, as early detection is linked to smaller tumor sizes and better outcomes. The current research study asked three questions with the objectives of assessing the frequency of CRC screening in primary care clinics, evaluating documented barriers to …

Identifying the role of marine and estuarine habitats in supporting fish and invertebrate populations during vulnerable juvenile life stages is essential to achieve effective conservation and fisheries management. There remains general agreement that: (i) the quality of juvenile habitat is best measured as the contribution of juveniles to adult populations (here “juvenile–adult contribution”) and (ii) this contribution may be measured directly or inferred from habitat-specific abundance, growth and survival. Obtaining effective estimates of juvenile habitat quality using these four metrics, however, is challenging. Through a systematic review of approaches to measure juvenile habitat quality, we critically evaluate current abilities to …

Improving Food Safety Practices Among FSMA-Exempt Small-Scale Produce Growers: A Manuscript-Based Dissertation on Risk, Adoption, and Practical Barriers
Jesica Temple
Foodborne illness outbreaks associated with fresh produce continue to pose a public health risk in the United States. Although the Food Safety Modernization Act (FSMA) Produce Safety Rule (PSR) established science-based standards to prevent contamination, many small-scale farms are exempt from these requirements. Consequently, voluntary adoption of Good Agricultural Practices (GAP) varies widely across regions. Strategies to improve engagement among FSMA-exempt growers are key to advancing postharvest food safety and reducing contamination. This manuscript-based dissertation examined how knowledge, attitudes, …
Improving Social Determinants Of Health Screening Rates Among Healthcare Providers, Sarabeth Lee Clark
Improving Social Determinants Of Health Screening Rates Among Healthcare Providers, Sarabeth Lee Clark
DNP Projects
Social Determinants of Health (SDOH) are non-medical factors that influence a patient's health outcomes. The WHO Commission on Social Determinants of Health has stated that improving SDOH is one of the most effective means of enhancing the well-being of all individuals and reducing disparities (Chelak et al., 2023). Implementing increased screening measures to identify deficits can positively impact patient outcomes by providing crucial resources that address patients' needs. This approach can significantly enhance the quality of healthcare services (Wark et al., 2021). The purpose of this Doctor of Nursing Practice quality improvement project was to evaluate self-reported screening rates for …

Using Algal Turf Scrubber Biomass As A Feed Source For Juvenile Eastern Oysters, Taofeek Saka Jimoh
College of Graduate Studies: Theses & Dissertations
Algal turf scrubber (ATS) is an engineered system that promotes natural wastewater treatment processes and improves water quality by passing a shallow stream of nutrient rich water over the surface of a gently sloped flow-way. Algae turf scrubber biomass is a combination of micro and macroalgae, as well as micro-invertebrates, ciliates, bacteria, and more. The overall aim of the study was to reduce the feeding cost of juvenile eastern oysters by replacing the commercial microalgae diet with ATS biomass. The experiment was designed to assess the viability of replacing commercial microalgal diet with ATS biomass at different substitution levels (0, …

Syphilis disease has recently re-emerged and can result in serious, life-altering consequences, making it a significant public health concern. Untreated syphilis can have devastating effects, including brain damage, blindness, and paralysis. Congenital cases can cause miscarriage and infant death (Winny, 2024). The U.S. had its lowest rates of syphilis in 1999, potentially due to the efforts against the HIV epidemic. However, in the last two decades, syphilis transmission has been on the rise. According to the Centers for Disease Control and Prevention (CDC), there was an 80% increase in reported syphilis between 2018 and 2022 (Centers for Disease Control and …

In the past, we did not worry much about elderly poverty because retirement was short for most Americans – a brief jaunt of post-work life was soon met with death. But with the 100-year life becoming a reality for more Americans, an elderly poverty crisis looms. The American dream of abundant retirement savings remains elusive for many, particularly low-wage workers. While government initiatives emphasize individual responsibility and financial education as a way toward retirement success, the reality is that governmental policies are barriers, including asset limitations that prevent those who receive public benefits from saving. This chapter urges reform in …

Trichloroethylene (TCE) is a volatile synthetic chemical used in various industrial processes like metal degreasing. Large amounts of TCE have been released into the environment. Exposure to TCE can occur through routes, such as inhalation for workers using TCE or ingestion of drinking water in contaminated areas. Macrophages are key immune cells in virtually all tissues in the human body, including the fetal membranes, making them a plausible target for DCVC-induced immunotoxicity. Macrophages are critical for maintaining anti-microbial defenses during pregnancy, but little data exists on TCE immunotoxicity during pregnancy. We previously showed that the TCE metabolite, S-(1,2-dichlorovinyl)-L-cysteine (DCVC), …

Science, technology, engineering, and mathematics (STEM) education has become widely promoted. Family and Consumer Sciences (FCS) uses science to improve home life while reinforcing technology, engineering, and mathematics principles. However, FCS is not usually recognized as a STEM subject. The purpose of this study was to determine what factors may affect FCS teachers’ level of self-efficacy in teaching STEM to provide supports that improve teaching practices. This quantitative study surveyed secondary FCS teachers in Pennsylvania using the T‐STEM Survey for Elementary Teachers. Results were analyzed using descriptive statistics and simple linear regression analysis was performed to determine the predictive …

Demographic Profiles Of San Diego And Imperial Counties, Tessa Tinkler, Eo Hanabusa, Laura Deitrick, Connelly Meschen
Nonprofit Sector Issues and Trends
The Demographic Profiles of San Diego and Imperial Counties provides a snapshot of the two counties' vulnerable populations, including low-income communities, youth, immigrants, refugees, individuals experiencing homelessness, and retirement-age adults. While these counties differ in population size by more than an order of magnitude, the backdrop of California's rising cost of living, shifting immigration policies, income and health disparities, and the projections for an increasing aging population present compounding barriers to financial stability and access to services for all ages. Comprehensive data to understand these community needs was more readily available for San Diego County than Imperial County. This report …
