Theses
Pulmonary hypertension (PH) is a common, complex, and potentially lethal haemodynamic condition with many underlying aetiologies. It is poorly understood, and diagnosis is usually made late in the disease process resulting in poor outcomes even in the developed world. An innovative universal framework is required to achieve early and accurate diagnosis for improved outcomes, especially in remote and rural areas. The thesis documents my substantial body of work that unlocks a collective framework to assist patients with PH, particularly in areas not classically serviced by tertiary level PH care.

Cutaneous cytomegalovirus infection is a rare but serious complication in solid organ transplant recipients. We present a 47-year-old male kidney transplant recipient with a chronic, nonhealing right lower extremity ulcer. Initial biopsies revealed septic vasculopathy, leading to treatment with sodium thiosulfate and antibiotics for suspected calciphylaxis. Despite regular wound care, the ulcer continued to worsen. After completing 6 months of cytomegalovirus prophylaxis, surveillance viral levels remained undetectable, but the ulcer progressed considerably. Worsening severity prompted hospitalization, during which cytomegalovirus viremia was detected, and an ulcer biopsy confirmed cytomegalovirus inclusion bodies. Antiviral therapy was reinitiated, resulting in rapid and sustained wound …

Objectives: To identify the association of mothers with a career in higher educati--on and their children’s growth and development. Methods: This was an analytical study with a cross-sectional design carried out at Wiraraja University, Sumenep, Indonesia, from March to August 2023. The samples were female lecturers at Wiraraja University. A total of 107 mothers were selected using total sampling. The mother’s career was identified using a questionnaire and their child’s growth and development were measured using child development pre-screening questionnaire. Aims: Nigeria’s rapidly aging population presents urgent challenges for the healthcare system, particularly in oral health, which remains an overlooked aspect of well-being. While older adults are especially vulnerable to dental issues such as tooth loss, periodontal disease, and oral pain, access to care remains limited due to systemic, economic, and cultural barriers. This qualitative study explored the lived experiences of older Nigerians in accessing and utilizing dental care, with the goal of identifying key barriers, facilitators, and policy-relevant insights.

Background/Objectives: Zika virus (ZIKV) infection is associated with life-threatening diseases in humans. To date, there are no available FDA-approved therapies or vaccines for the specific treatment or prevention of ZIKV infection. Variation in the ZIKV envelope protein (Env), along with its complex quaternary structure, presents challenges to synthetic approaches for developing an effective vaccine and broadly neutralizing antibodies (bnAbs). We hypothesized that beta-cyclodextrin (BCD) could be used to uniquely inactivate infectious ZIKV without disruption of Env.

Archaeoparasitology has been conducted in the Southwest for 57 years. In the first decade of the period, two pictures of parasitism emerged. Analysis of compressed organic sediments from a latrine at Elden Pueblo presented a spectrum of geohelminths not different from medieval Europe. These findings contrast pronouncedly with an analysis of coprolites from all other studies that show consistent pinworm parasitism at other sites in the Southwest. We summarize the paleoepidemiological bases for these contrasting phenomena.

In all health-related fields, the need for well-informed, holistic healthcare providers is paramount. For the heathcare field to be impactful in positive ways, providers should emerge from multifaceted academic backgrounds, equipped with an array of knowledge and skills to address the complex needs of different patients. Central to this is the idea that all aspects of an individual's identity and the intersectionality of these identities should be considered when providing care. Intersectionality recognizes that various social and personal identities, such as race, gender, sexuality, and socioeconomic status, intersect to shape individual experiences and health outcomes (Kelly et al., 2022).

The exponential increase in medical data has created a greater demand for precise and efficient information retrieval systems. Existing Large Language Models (LLMs) face domain-specific difficulties such as sophisticated medical jargon, situational comprehension, and the continual advancement of healthcare knowledge. To tackle these challenges, we present MediLightRAG, an innovative two-stage system which integrates parameter-efficient fine-tuning of Large Language models with LightRAG’s graph-based retrieval. The first stage focuses on enabling accurate resource-efficient model adaptation for the medical domain through QLoRA fine-tuning. Zika virus (ZIKV) is a mosquito-borne human pathogen that causes mild febrile illness in adults and severe neurological complications and microcephaly in newborns. Studies have reported that ZIKV modulates methylation of human and viral RNA critical for its replication in vertebrate cells. In this study, we show that ZIKV modulates mosquito S-adenosyl methionine (SAMe)-synthase, an enzyme involved in the production of SAMe (methyl donor), and histone methylation for its survival in mosquito cells. Reverse transcription quantitative PCR followed by immunoblotting analysis showed increased amounts of SAMe synthase at both RNA and protein levels, respectively, in C6/36 mosquito cells infected with …

Eggs of Schistosoma mansoni are produced by adult female worms in mesenteries of infected hosts. Eggs can cross the intestinal barrier and form granulomas in the tissue or breach and exit the host through fecal excretion. These interactions may affect the host microbiome assemblages. Given the potential for schistosomal alteration of host gut microbiome and subsequent effects on the fecal bacterial composition, it is important to conduct controlled microbiome studies on model animals. All active stages of Haemaphysalis vespertina sp. nov. (Acari: Ixodidae), a tick previously identified as H. leporispalustris Packard, 1869, are described from specimens collected on the vegetation and from leporids in California and Oregon. The adults of H. leporispalustris Packard, 1969 are redescribed based on type material. Adults of the two species can be distinguished by their overall size, the dorsal shape of palpal segment II, the number and shape of dorsal and ventral setae on palpal segment II, the number of spurs on coxae II, the length of setae on scutum, legs and coxae, and the pattern of scutal …

Amblyomma maculatum Koch (Acari: Ixodidae), Ixodes scapularis Say, and Dermacentor variabilis Say are three hard-bodied ticks responsible for vectoring pathogens that cause most human tick-borne diseases in the United States of America (USA). Tick surveillance is critical to elucidate high-risk areas for targeted vector control and public health interventions. Despite tick-borne diseases having a higher annual incidence compared to mosquito-borne diseases in the USA, tick traps used for surveillance are grossly under-developed compared to the suite of sophisticated mosquito traps available on the commercial market. In this first report of endoparasites from endemic land-mammals of the Galápagos Islands, we describe a new species of cestode of the genus Raillietina (Cyclophyllidea: Davaineidae) from a species of Nesoryzomys and summarize the extent of helminth parasitism in both oryzomyine endemics and introduced species of Rattus. Up to the current time, no helminth parasites have been reported from rodents of the Galápagos, and little work has yet been done describing and synthesizing Galápagos parasite diversity. Introduction: High-consequence infectious diseases (HCIDs) such as Ebola virus disease (EVD) and other viral hemorrhagic fevers (VHFs) require interdisciplinary teamwork and collaboration, communication, and adherence to protocols in order to safely transport patients between agencies. Despite national preparedness efforts, standardized transport protocols for HCID patients remain limited and inconsistently applied nationally. This quality improvement (QI) project aimed to develop and assess evidence-based interfacility transport protocols for a regional Biological Isolation Care Unit (BICU) using a multidisciplinary, simulation-based learning activity.

Background: Longitudinal measures of viral load (VL) are critical in monitoring the HIV status. While multiple lab indicators exist for monitoring measures of VL, research on clustering historical/longitudinal VL measures is limited. Analyzing longitudinal VL patterns rather than aggregated measures offers deeper insights into HIV status. This study uses functional data clustering to classify longitudinal VL patterns and characterize each cluster by demographics, comorbidities, social behaviors, and CD4 count.

Background: In South Africa, there is no centralized HIV surveillance system where key populations (KPs) data, including gay men and other men who have sex with men, female sex workers, transgender persons, people who use drugs, and incarcerated persons, are stored in South Africa despite being on higher risk of HIV acquisition and transmission than the general population. Routinely monitoring viral rebound (VR) is important in the life course of people with HIV (PWH). This study examined risk factors for time to the first VR, the number of VRs and their association with VR history in men who have sex with men (MSM). It includes 8176 adult PWH diagnosed from January 2005 to December 2018, followed until July 2021. We used the Cox model for time to the first VR, the Poisson model for a number of VRs, and logistic regression for VR history in MSM. Background: HIV continues to be an important public health concern in South Carolina (SC). However, an examination of providers’ willingness to use mHealth technologies to address ongoing barriers to HIV care and prevention strategies, particularly among men who have sex with men (MSM) is currently lacking in SC. Background: COVID-19 continues to pose a major global health challenge. Despite its geographic distance from Brazil’s major urban centers, Acre state has experienced notable outbreaks. This study assessed the detection and persistence of SARS-CoV-2 in the rural settlement of Santa Luzia, located in the remote municipality of Cruzeiro do Sul, Acre state, Brazil. Methods: In July 2022, a cross-sectional survey was conducted at 40 sites from an ongoing environmental study, selected by deforestation patterns and proximity to health posts. Increased access to fluoride varnish and expanded health and wellness campaign is expected to improve oral health of children and adults. The end of 2024 brought two new laws that specifically affect dentistry. The first such law, Chapter 457 of the Laws of 2024, took effect on Nov. 15. It amended Section 6608 of the New York State Education Law to allow registered dental assistants to apply topical fluoride varnish. Topical fluoride varnish had previously been limited to dentists and dental hygienists. This study proposes a model of allocating beds in a hospital considering infection control during a pandemic. The problem was formulated using the quadratic assignment problem (QAP) in a multi-period planning horizon. The main objective is to

Inflammatory cytokines such as interleukin-6 (IL-6) and interleukin-12 (IL-12) are central regulators of immune signaling and key biomarkers of disease, yet existing assays for their detection remain slow, invasive, and lack multiplexing ability. This dissertation advances the development of single-walled carbon nanotube (SWCNT) optical nanosensors capable of real-time, multiplexed, and molecularly specific cytokine detection through innovative use of single-stranded DNA (ssDNA) interfaces.
First, an IL-6-specific DNA aptamer was employed as both a dispersing agent and recognition probe for SWCNT fluorescence sensing. Sequence modifications, including anchor domains, truncations, and thermally induced refolding, were systematically tested to optimize sensitivity and selectivity. The …
