Although the article by J Càmara, I Grau, A González-Díaz, N Santos, et al. (Antimicrob Agents Chemother 69:e00237-25, 2025, https://doi.org/10.1128/aac.00237-25) suggests that ceftriaxone is greatly superior to amoxicillin in treating pneumonia due to pneumococci with an MIC > 2 mg/L, much of the difference in efficacy of the two drugs may have been due to dosage, route, and times of administration of the amoxicillin.

BACKGROUND: Spectrophotometric testing to detect sample hemolysis is available from central laboratory chemistry analyzers. While the cause and preventative measures are known, hemolysis continues to be a common preanalytical error, especially for specimens collected from the emergency department (ED) and intensive care units (ICUs) where point-of-care analyzers are commonly used for whole blood electrolyte testing. Recently, these analyzers have employed technology to detect hemolysis directly on whole blood samples.

Objectives: This study aimed to develop a population pharmacokinetic model for vancomycin in pediatric patients with sepsis or septic shock and to propose tailored initial dose recommendations based on the model’s findings. Methods: We conducted a retrospective analysis of pediatric patients with sepsis or septic shock who received vancomycin treatment from January 2017 to December 2019. A population pharmacokinetic model was developed using NONMEM. Measles-associated hemophagocytic lymphohistiocytosis is a rare and life-threatening condition, particularly in immunocompromised individuals. This case report discusses the management and successful treatment of a 21-month-old unvaccinated male with Down syndrome who developed secondary Hemophagocytic lymphohistiocytosis during follow-up for pneumonia and respiratory failure. The patient presented with rashes, fever, and cough. Despite initial treatment for pneumonia, he developed respiratory failure, requiring transfer to the pediatric intensive care unit. Physical examination revealed a toxic appearance, bilateral coarse crackles, hepatosplenomegaly, and several mobile cervical lymphadenopathies. Objective: Streptococcus pyogenes, or Group A Streptococcus (GAS), can cause mild infections like pharyngitis and impetigo, as well as severe diseases such as necrotizing fasciitis and sepsis. Transmission typically occurs through droplets or direct contact, particularly in children and adolescents, with peaks during winter and spring. In late 2022, the UK reported a sharp increase in GAS cases, with 24 children under 10 dying from the infection. Objective: This study aimed to evaluate the effects of immunoglobulin use on clinical treatment outcomes, prognosis, and laboratory parameters in neonates with septic shock. Methods: In the study, 155 newborns treated in the Neonatal Intensive Care Unit of Selcuk University Faculty of Medicine between 2019 and 2024 were retrospectively analyzed. Newborns were divided into three groups: those receiving IgM-enriched IVIG (pentaglobin®) treatment, those receiving standard IVIG treatment containing only IgG, and those receiving no immunoglobulin treatment. Objective: The aim of this study is to determine whether there is a change in serum levels of the anti-microbial peptides Hepcidin-25 and angiogenin, in pediatric patients with Brucellosis compared to healthy controls. Objective: Ezrin, also known as villin-2 or cytovillin, is a cytoplasmic peripheral membrane protein encoded in humans with the potential implications for various human cancers, cytokine modulation, cytoskeleton remodeling, mucous membrane integrity, and epithelial or pulmonary damage. This study hypothesizes that the amounts of exosome-associated ezrin in the serum detected by ELISA may correlate with the diagnosis or severity of pneumonia in children. Musculoskeletal infections represent a significant proportion of the clinical workload for infectious diseases (ID) providers, often presenting complex diagnostic and therapeutic challenges. Many providers may feel inadequately prepared to manage these conditions, which require a nuanced understanding of microbiology, musculoskeletal anatomy, surgical intervention, and orthopedic hardware. We implemented a pair of educational interventions including an online library of high-yield musculoskeletal ID literature and an educational podcast. © The Author(s) 2025. IMPORTANCE: Studies of COVID-19, influenza, and respiratory syncytial virus (RSV) disease clinical epidemiology, disease burden, and vaccine effectiveness often rely on clinical testing to identify cases, particularly with increasing use of electronic health record data. Oral candidiasis (OC) is caused by Candida albicans, targeting immunocompromised individuals and developing drug resistance, highlighting the need for advanced therapeutics. Polyphenols such as caffeic acid phenethyl ester (CAPE) and ellagic acid (EA) display antifungal and immune-modulating properties. Incorporating CAPE and EA in gellan gum (GG) formulation enhances their applicability and effectiveness against OC. We developed GG-based formulations loaded with CAPE (1,000 µg/mL), EA (1,000 µg/mL), or CAPE + EA (1,000 µg/mL each) against C. albicans. Importance: COVID-19 remains a disease with high burden in the US, prompting continued debate about optimal targets for annual vaccination.
Objective: To project COVID-19 burden in the US for April 2024 to April 2025 under 6 scenarios of immune escape (20% and 50% per year) and levels of vaccine recommendation (no recommendation, vaccination for individuals at high risk only, vaccination for all eligible groups) and to assess the potential benefit of vaccine recommendations in reducing disease burden.

BACKGROUND: Cardiovascular disease remains the leading cause of maternal mortality. Mental health disorders are risk factors for cardiovascular disease in nonpregnant populations, but their association with delivery outcomes is unknown.
METHODS: This cross-sectional study used delivery hospitalizations from the 2021 National Inpatient Sample. We used generalized estimating equations to fit quasi-Poisson regression models for cardiovascular severe maternal morbidity and mortality (SMM) events as defined by the Centers for Disease Control and Prevention, and non-SMM cardiovascular conditions or complications as defined by the Alliance for Innovation on Maternal Health. Background: Mitis group non-pneumococcal streptococci (MGNPS), specifically Streptococcus mitis, Streptococcus infantis and Streptococcus oralis, have recently been shown to cause pneumonia and/or bacteremia. These organisms often have capsular (cps) operons resembling those in pneumococci, and some express cps-generated polysaccharides that antigenically cross-react with pneumococcal capsular serotypes. But, to date, a series of MGNPS isolates has not been studied by electron microscopy (EM) for the presence of a capsule.
Methods: We studied 21 MGNPS; 11 were isolated from sputum and determined to have caused pneumonia, 3 were isolated from blood, and 7 were commensal isolates cultured …

Background: COVID-19 pandemic has raised global concern regarding maternal and neonatal health owing to physiological and immunological changes that may heighten disease severity. Despite the global evidence linking COVID-19 infection with unfavorable pregnancy outcomes, data from Sub-Saharan Africa remain limited. This leaves an essential vacuum in our understanding of how COVID-19 status affects maternal and neonatal outcomes in counties like Mombasa, where health-care system problems may alter risks. Public wildlife law is clear: wild animals exist and are managed for the public writ large. This is a mistake. Wild animals are beings of moral concern; their lives can be good or bad, better or worse, and they should be treated accordingly. This article explores a novel legal instrument which can account for wild animals' unique interests and circumstances: private wildlife trusts, in which wild animals are the trust beneficiaries. The opioid crisis is a significant public health emergency, intensified by the rise of synthetic opioids such as fentanyl. Lawmakers have described fentanyl as “terroristic in nature” due to its often-fatal properties. According to the Center for Disease Control (“CDC”), an estimated 107,543 people died from drug overdoses in the United States in 2023. As fentanyl use has surged, it has increasingly shaped public health policy through harm reduction initiatives, expanded addiction treatment resources, and legislative reforms aimed at curbing overdose deaths. The COVID-19 pandemic has underscored the necessity of effective face masks in both public and healthcare settings for mitigating the spread of disease. However, traditional face masks impede communication, posing significant challenges for approximately 13% of the general population and 10% of healthcare and social assistance workers who have hearing loss and who rely on lipreading and other non-verbal cues. Traditional surgical masks and N95 respirator masks serve important roles in healthcare, but they differ in levels of protection. The European Society for Immunodeficiencies patient registry (ESID-R), established in 1994, is one of the world's largest databases on inborn errors of immunity (IEI). IEI are genetic disorders predisposing patients to infections, autoimmunity, inflammation, allergies, and malignancies. Treatments include antimicrobial therapy, immunoglobulin replacement, immune modulation, stem cell transplantation, and gene therapy. Data from 194 centers in 33 countries capture clinical manifestations and treatments from birth onward, with annually expected updates. This report reviews the ESID-R's structure, data content, and impact. The registry includes 30,628 patient datasets (aged 0-97.9 years; median follow-up: 7.2 years; total 825,568.2 patient-years), with 13,550 cases in …

The 2025 Annual Florida EMS Provider Survey provides a comprehensive overview of Florida’s EMS system, highlighting critical insights into workforce dynamics, resource allocation, medical direction, and service integration across EMS agencies statewide. Conducted from April to May 2025, the survey received robust participation from 249 agencies, covering 74.5% of licensed EMS providers. Key findings underscore regional variability in workforce recruitment, salary scales, mental health support, mobile integrated healthcare (MIH), and preparedness for mass casualty incidents.
