Assessment Of Decentralized, Inpatient Pharmacist Blood Culture Audit And Provider Feedback, Brittany Brown, Thomas J. Dilworth
Assessment Of Decentralized, Inpatient Pharmacist Blood Culture Audit And Provider Feedback, Brittany Brown, Thomas J. Dilworth
Pharmacy and Pharmacology
Background: Hospitalized patients with blood stream infections (BSI) are at increased morbidity and mortality risk despite advances in therapy and overall care. Audit and provider feedback for positive blood cultures by pharmacists has been shown to improve care and outcomes for patients with BSI. However, previous studies employed pharmacy specialists with infectious diseases (ID) training in a centralized audit and feedback model. It is unknown if decentralized inpatient pharmacists, with support from ID pharmacists as needed, could impact patient care similarly.
Purpose: To evaluate antibiotic treatment and outcomes for patients with BSI for whom blood culture audit and provider feedback …

The University of Louisville Journal of Respiratory Infections
This study reported the efficacy of short-course corticosteroids and convalescent plasma (CP) transfusion in treating five patients with severe COVID-19 disease. Five adults (mean±standard deviation age: 70±9 years) with laboratory-confirmed COVID-19 and severe hypoxemia (PaO2/FiO2 [P/F]<100) requiring invasive mechanical ventilation received five days methylprednisolone (40 mg intravenously every 12 hours) and subsequent CP transfusion (250–400 ml). Compared with the baseline, P/F ratios increased by 46% and by 28% after short course corticosteroids and CP transfusion, respectively. Four patients survived. Short-course corticosteroids and CP transfusion may improve hypoxemia in patients with severe COVID-19 disease.

Internal Medicine Articles
BACKGROUND: There is no proven antiviral or immunomodulatory therapy for COVID-19. The disease progression associated with the pro-inflammatory host response prompted us to examine the role of early corticosteroid therapy in patients with moderate to severe COVID-19.
METHODS: We conducted a single pre-test, single post-test quasi-experiment in a multi-center health system in Michigan from March 12 to March 27, 2020. Adult patients with confirmed moderate to severe COVID were included. A protocol was implemented on March 20, 2020 using early, short-course, methylprednisolone 0.5 to 1 mg/kg/day divided in 2 intravenous doses for 3 days. Outcomes of standard of care (SOC) …

Spectrum Of Imaging Findings In Aidsrelated Diffuse Large B Cell Lymphoma, Edward Nganga, Samuel Gitau
Spectrum Of Imaging Findings In Aidsrelated Diffuse Large B Cell Lymphoma, Edward Nganga, Samuel Gitau
Imaging & Diagnostic Radiology, East Africa
Lymphoma in HIV-infected patients is AIDS defining. This is the second most common AIDS defining malignancy after Kaposi’s sarcoma. Development of lymphoma in HIV patients is related to immunosuppression and high viral load. Co-infection with other lymphotrophic viruses especially EBV is also strongly associated with development of lymphoma in HIV patients. Despite advances in HAART therapy, incidence of diffuse large B cell lymphoma in HIV-infected patients remains significantly higher than in the general population.
Early diagnosis is challenging due to presence of opportunistic infections and atypical presentation of the lymphoma in this subset of patients. Atypical imaging findings are not …

Intranasal Administration Of Pcrv Conjugated To A Tobacco Mosaic Virus Delivery Platform Protects Against P. Aeruginosa In A Murine Model Of Acute Pneumonia, Eman Barahim
NYMC Student Theses and Dissertations
Pseudomonas aeruginosa is a facultative anaerobic bacterium that is the most common type of Gram-negative bacteria causing nosocomial pneumonia in the United States. P. aeruginosa is an opportunistic pathogen ubiquitously found in the atmosphere that can colonize both plants and animals. It is known to cause a wide range of severe, persistent, and fatal infections ranging from acute and chronic pneumonia, skin and soft tissue infections to urinary tract infections, especially in immune-compromised individuals. Patients with cystic fibrosis are at high risk because of deficiencies in mucosal immunity and phagocytosis. Ultimately, Pseudomonas infections can be lethal. There is a very …

When Does A Major Outbreak Become A Public Health Emergency Of International Concern?, David N. Durrheim, Lawrence O. Gostin, Keymanthri Moodley
Georgetown Law Faculty Publications and Other Works
The process of determining whether a Public Health Emergencies of International Concern (PHEIC) exists and should be declared has drawn increasing criticism over the past few years with allegations that it is more political than technical. Delaying declaration, where appropriate, means that the opportunity of garnering international solidarity and necessary resources in the early phases of epidemics, when public health measures may be more effective, is lost. A reform agenda aimed at enhancing WHO/Emergency Committee transparency and objectivity for the PHEIC declaration process is required.

Modulating Matrix Metalloproteases And Inflammation In Huntington’S Disease, Alejandro Lopez Ramirez
Modulating Matrix Metalloproteases And Inflammation In Huntington’S Disease, Alejandro Lopez Ramirez
Natural Sciences and Mathematics | Biological Sciences Master's Theses
Huntington’s disease (HD) is a rare and incurable autosomal neurodegenerative disease affecting 1-10 in every 100,000 people in the world. There is no cure for HD and treatments available alleviate certain symptoms for short periods of time. Evidence suggests that neuropathology of HD begins with the proteolysis of the mutated Huntingtin (mHTT) protein. A variety of proteases, like the matrix metalloproteases, cleave mHTT creating proteinaceous fragments that are thought to be neurotoxic. As these fragments increase in the brain, the damage to neurons also increases, leading to chronic inflammation due to hyper reactive microglia and astrocytes attempting to minimize and …

Based on the reported data from February 16, 2020 to March 9, 2020 in South Korea including confirmed cases, death cases and recovery cases, the control reproduction number was estimated respectively at different control measure phases using Markov chain Monte Carlo method and presented using the resulting posterior mean and 95% credible interval (CrI). At the early phase from February 16 to February 24, we estimate the basic reproduction number R0 of COVID-19 to be 4.79(95% CrI 4.38 - 5.2). The estimated control reproduction number dropped rapidly to Rc ≈ 0.32(95% CrI …

Engagement Of Primary Stakeholders To Tailor A Comprehensive Transitional Care Model For Persons Who Have Experienced A Stroke And Their Caregivers, Lorre Laws
Dissertations
Stroke is the leading cause of disability in the US, affecting approximately 795,000 persons annually. Stroke care is delivered across multiple settings from hyperacute care in a hospital through chronic stroke management in the community. Considerable advancements have been made in the delivery of hyperacute and acute stroke care. Science and practice gaps exist in providing stroke transitional care across multiple providers and settings once an individual is discharged from an in-patient care facility to home.
Using a qualitative descriptive design, this study engaged and elicited descriptions from stroke survivors and caregivers affected by stroke to inform the refinement and …
Prediction Of Drug-Drug Interaction Potential Using Machine Learning Approaches, Joseph Scavetta
Prediction Of Drug-Drug Interaction Potential Using Machine Learning Approaches, Joseph Scavetta
Theses and Dissertations
Drug discovery is a long, expensive, and complex, yet crucial process for the benefit of society. Selecting potential drug candidates requires an understanding of how well a compound will perform at its task, and more importantly, how safe the compound will act in patients. A key safety insight is understanding a molecule's potential for drug-drug interactions. The metabolism of many drugs is mediated by members of the cytochrome P450 superfamily, notably, the CYP3A4 enzyme. Inhibition of these enzymes can alter the bioavailability of other drugs, potentially increasing their levels to toxic amounts. Four models were developed to predict CYP3A4 inhibition: …
