International Journal of Speleology

Studying caves with minimal human contact offers insights into antimicrobial resistance before human influence. In this study, we isolated 315 bacterial strains from Sorcerer’s Cave in Texas and evaluated their resistance to seven common antimicrobials. Under half (41.4%) of the Gram-positive isolates and 68.4% of the Gram-negative isolates were resistant to three or more antimicrobials. The two most common genera identified were Pseudomonas and Bacillus. Compared to bacteria isolated in previous cave studies, Sorcerer’s Cave bacteria exhibited more extensive antimicrobial resistance. Physical inactivity among older adults remains a major national and global public health concern linked to preventable morbidity, mortality, and rising healthcare costs. Physical therapists are uniquely positioned to promote physical activity (PA) through health education, yet their engagement in this role is not well defined. This quantitative cross-sectional study examined associations between clinical practice factors—years of experience, board certification status, and practice setting—and three domains of the theoretical domains framework (knowledge, skills, and behavioral regulation) related to PA promotion among older adults. Global biomedical and health research is increasingly relying on genomic and computational approaches, largely driven by the increasing volumes of nucleic acid sequencing. Concurrently, epidemiological studies and clinical records are generating enormous amounts of data amenable to disease modeling, machine learning, and artificial intelligence techniques. Bioinformatics and data science expertise is therefore essential for improved population health. Acinetobacter baumannii is a Gram-negative, aerobic and multi-drug resistant bacterial pathogen commonly associated with nosocomial infections. This species has several strains and is known to cause pneumonia, septi cemia, meningitis, urinary tract infection and wound infection which are associated with high mortality rates. This study focused on a genome-wide compari son among target C123 strain and 15 other reference strains to predict the overall properties, and resistance mechanisms of the C123 strain. Electronic cigarettes (e-cigarettes) emerged and were marketed as a more viable alternative to traditional tobacco smoking, especially among the youth. Taal Volcano Island’s hydrothermal environment supports extremophilic microbial communities. This study isolated, biochemically characterized, and genetically profiled Staphylococcus sp. from shoreline soils. Soil samples were collected near active vents and cultured on acidified gelatin agar (pH 5.0). Biochemical tests—Methyl Red, Spirit Blue Agar, Motility, and Acrolein—were performed. DNA extraction and 16S rRNA sequencing identified Staphylococcus candidatus, with 99 % sequence similarity to GenBank entries JN794602.1 and KX250303.1. Phylogenetic analysis clustered the isolate with S. ureilyticus, S. cohnii, and S. equorum. Healthcare disparities exist in the U.S. between different races. This study investigated the frequency of COVID-19 infections and hypertension cases among five different racial groups (White, Black, Asian, Native American, and Native Hawaiian). The study also examined the correlation between COVID-19 infections and hypertension with the hypothesis that because of disease predisposition and socioeconomic barriers, Black populations would have the highest rates of COVID-19 infections and hypertension. Purpose of Review: The potency of illicit fentanyl has rendered standard methadone inductions inadequate for many hospitalized adults with opioid use disorder. Standard protocols often fail to achieve therapeutic doses quickly enough to manage withdrawal and cravings in fentanyl-tolerant individuals, contributing to poor retention and higher self-discharge rates. This review examines emerging evidence on dosing strategies, outcomes, and safety of rapid methadone inductions in acute-care hospitals. Recent Findings: Nine studies (five retrospective cohorts, four case series/reports) encompassing 380 patients met inclusion criteria. Importance  Buprenorphine is the most commonly prescribed medication for opioid use disorder, but rates of retention in treatment remain low. Prior authorization (PA) has been identified as a barrier to the administration of buprenorphine, and patients may need multiple PAs to continue treatment; thus, many states have passed laws to prohibit the use of PA for buprenorphine in private insurance.

Objective  To examine the association between PA prohibitions and buprenorphine treatment retention overall and by branded vs generic drug status.

Background and objectives: One Paleopathology is a novel concept in Paleopathology that extends the One Health paradigm into the past. A workshop at the University of Durham, UK, was held prior to the 2024 International Society for Evolution, Medicine, and Public Health (ISEMPH) meeting, firstly to define and expand the concept of One Paleopathology and secondly to generate transdisciplinary research and outreach under this framework. This article presents a logic model to evaluate how effectively the workshop met its goals.

Although grief is a universal human experience, its perspective is unique, and research has often ignored the varied experiences of racial/ethnic and cultural groups, particularly men of color in the United States. Human milk banks provide pasteurized donor milk for premature infants whose mothers are unable to produce sufficient amounts of milk, offering critical nutritional and immunological benefits for this high-risk population.1–4 Although 28 HMBANA-accredited milk banks operate in the United States, none are located within 4.5 hours of Rochester, NY. BACKGROUND: Cyberchondria, a combination of the words "cyber" and "hypochondriasis," is a condition that is receiving increasing attention from clinicians and researchers globally. Researchers are currently using multiple instruments to quantify it. Furthermore, the instruments have been translated into multiple languages.

OBJECTIVE: This study aimed to examine the extent to which researchers are measuring cyberchondria using the 33-item Cyberchondria Severity Scale (CSS) and its 12-item abbreviated version, the CSS-12. It also examined the relative use of cyberchondria instruments in different languages.

We report a 22-year-old male, who presented in 2020 to a clinic in Kahramanmaraş, Turkey, with a 5-year history of a fixed preoccupation with the belief of emitting a ‘plastic smell’ from his anal region. This case illustrates a clinically significant link with a history of childhood sexual abuse. The patient was diagnosed with olfactory reference disorder (ORD) with poor insight and comorbid depressive disorder, based on ICD-11 and DSM-5-TR criteria. Extensive medical workups ruled out organic causes. He responded well to a combination therapy of sertraline and aripiprazole, showing significant symptom reduction. Noninfectious manifestations of common variable immunodeficiency (CVID) are not formally summarized. We performed a systematic literature review to generate a comprehensive reference for the field. Splenomegaly was the most reported manifestation across 53 publications, occurring in a median of 35.2% of patients. Frequently reported digestive system manifestations included diarrhea (median 27.8%; 21 publications), hepato(spleno)megaly (median, 21.0%; 19 publications), portal hypertension (median 21.0%; 3 publications), nodular lymphoid hyperplasia (median, 17.0%; 9 publications), and enteropathy (median, 16.0%; 34 publications). Hematopoietic cell transplantation (HCT) is a potentially curative treatment for severe combined immunodeficiency (SCID). Since the initiation of newborn screening (NBS), survival rates have improved significantly, but the impact of HCT upon neurodevelopment for patients with SCID requires more investigation. We performed a cross-sectional study of subjects with SCID in North America to assess the impact of NBS, transplant conditioning regimen, and genotype on neurodevelopmental outcomes after HCT. 69 subjects with SCID from 17 PIDTC centers (excluding those with ADA deficiency), ages 6-16 years, received comprehensive standardized neurodevelopmental testing of cognitive, behavioral, and emotional function. Natural killer (NK) cells are integral to the innate immune system, playing a crucial role in immune surveillance and the rapid response to virally infected and tumor cells. Epigenetic gene expression regulation significantly influences NK cell function and differentiation. Using a high-throughput small-molecule drug screening approach, we identified bromodomain and extra-terminal domain (BET) inhibitors (BETi) as potent modulators of NK cell function, reducing proinflammatory cytokine secretion while increasing markers of NK cell maturation and cytotoxicity. During NK lineage specification from hematopoietic stem cells, we demonstrated that BETi reduced NK cell fate and promoted increased myeloid cell differentiation. Moreover, differentiated NK …

Background Violence against women and against children are human rights violations with lasting harms to survivors and societies at large. Intimate partner violence (IPV) and sexual violence against children (SVAC) are two major forms of such abuse. Despite their wide-reaching effects on individual and community health, these risk factors have not been adequately prioritised as key drivers of global health burden. Antibody and cytokine kinetics describe the dynamic response to immune events such as infection and vaccination. These dynamics are not fully understood, and mathematical characterization may help explain variability across demographic groups and pulmonary symptoms post-acute infection. We fit time-dependent probability models to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) data to obtain distributions of longitudinal antibody response and cytokine values. To assess differences between groups, an overlap metric is applied to the modeled response curves. Our antibody models suggest significant differences between male and female populations and demonstrate deficient antibody responses of less-healthy groups such as smokers. Our cytokine …

During the early phase of the COVID-19 pandemic, before vaccines were available, countries implemented diverse combinations of social-distancing (SD) measures shaped by cultural norms and public acceptance. These differences produced varied outcomes in controlling viral spread, offering lessons for future health-crisis preparedness. We performed a reverse-engineering analysis of early COVID-19 case data from five socioculturally distinct countries—India, Vietnam, Italy, Finland, and the United States—selected to capture a range of initial prevalence levels. This study employed an age-structured Susceptible-Infected-Recovered-Vaccinated-Deceased (SIRVD) model to design optimal COVID‑19 intervention strategies by integrating epidemiological dynamics with economic costs. Using empirical data from South Korea, we estimated transmission parameters and analyzed quarantine-based non-pharmaceutical interventions (NPIs) alongside targeted vaccination scenarios. Our results indicated that a stronger form of mild NPI (upper range of Level I), when combined with targeted vaccination that prioritized seniors, achieved substantial reductions in infection peaks and overall economic burden. These findings underscore the effectiveness of age-specific strategies in epidemic management and offer critical insights for policymakers seeking balanced, resource-efficient approaches.

IMPORTANCE: Antibiotics prescribed at hospital discharge are frequently unnecessary or suboptimal. Strategies to improve prescribing are not well defined.

OBJECTIVE: To evaluate whether a discharge-focused prospective audit and feedback process decreases antibiotic overuse at hospital discharge.

DESIGN, SETTING, AND PARTICIPANTS: This stepped-wedge cluster-randomized clinical trial was conducted across participating units at 10 hospitals with antibiotic stewardship (AS) teams and supporting staff from December 5, 2022, to November 17, 2023. After a 24-week baseline period, 1 hospital crossed into the intervention arm every 2 weeks.

Inappropriate antibiotic prescribing for upper respiratory infections (URIs) remains a major contributor to antimicrobial resistance, adverse drug events, and unnecessary healthcare costs, particularly in urgent care settings characterized by high patient volume and rapid clinical decision-making.
