หลักการและเหตุผล: การเกิดไข้ในภาวะเม็ดเลือดขาวต่ำถือเป็นภาวะแทรกซ้อนที่เป็นสาเหตุสำคัญของการเสียชีวิตของผู้ป่วยมะเร็งต่อมน้ำเหลืองชนิดนอนฮอดจ์กินที่ได้รับการรักษาด้วยเคมีบำบัดสูตรริทูซิแมบ ไซโคฟอสฟามาย วินคริสติน ดอกโซรูบิซิน และ เพร็ดนิโซโลน (R-CHOP) โดยประโยชน์ของการให้ยาต้านจุลชีพเพื่อป้องกันภาวะแทรกซ้อนดังกล่าวยังไม่ชัดเจน โดยเฉพาะในผู้ป่วยที่ได้รับ G-CSF ร่วมด้วย จึงยังไม่มีคำแนะนำมาตรฐาน งานวิจัยนี้มีวัตถุประสงค์หลักเพื่อประเมินประสิทธิภาพของยาลีโวฟลอกซาซินในการป้องกันการเกิดไข้ตามหลังการให้เคมีบำบัดสูตร R-CHOP ในผู้ป่วยมะเร็งต่อมน้ำเหลืองชนิดนอนฮอดจ์กิน วิธีการศึกษา: เป็นการศึกษาแบบสุ่มที่มีกลุ่มควบคุม ทำการศึกษาผู้ป่วยโรคมะเร็งต่อมน้ำเหลืองชนิดนอนฮอดจ์กินที่ได้รับเคมีบำบัดสูตร R-CHOP ในโรงพยาบาลจุฬาลงกรณ์ ระหว่าง 1 มกราคม พ.ศ. 2566 - 31 ธันวาคม พ.ศ. 2566 โดยแบ่งกลุ่มการศึกษาเป็น 2 กลุ่ม ให้ยาลีโวฟลอกซาซินขนาด 500 mg หรือยาหลอก รับประทานวันละ 1 ครั้ง เป็นระยะเวลา 7 วัน เริ่มหลังให้เคมีบำบัด 1 วัน ผู้ป่วยทุกรายได้รับ G-CSF โดยประเมินผลการศึกษาหลัก คือการเกิดไข้ ในระยะเวลาทั้งสิ้น 120 วันนับตั้งแต่เริ่มเคมีบำบัด และ ผลการศึกษารอง ได้แก่การเกิดภาวะช็อกจากการติดเชื้อ การเสียชีวิตจากการติดเชื้อ การเสียชีวิตจากทุกสาเหตุ การลดขนาดยาเคมีบำบัด ระยะเวลาการเริ่มเกิดไข้ครั้งแรก และ ผลลัพธ์รวม ผลการศึกษา: ผู้ป่วยที่เข้าร่วมงานวิจัยทั้งหมด 58 คน แบ่งเป็นกลุ่มละ 29 คน อายุเฉลี่ย 61 ปี อัตราการเกิดไข้ร้อยละ 10.3 และ ร้อยละ 37.9 ในกลุ่มที่ได้ยาลีโวฟลอกซาซิน และ ในกลุ่มที่ได้ยาหลอกตามลำดับ (P = 0.014) ยาลีโวฟลอกซาซินลดอัตราการเกิดภาวะไข้ร่วมกับเม็ดเลือดขาวต่ำ (ร้อยละ 3.4 เทียบกับ ร้อยละ 24.1, P = 0.022), การลดขนาดยาเคมีบำบัด (ร้อยละ 0 เทียบกับ ร้อยละ 17, P …

A Pianist Looks At Gerald Finzi's A Young Man's Exhortation, Op. 14, Mary Ann Wilder

Gerald Finzi is a twentieth-century British composer. Though he also composed for orchestra and chamber ensembles, he is primarily known as a composer of vocal works, both solo and choral. In this solo vocal works, he favored the poetry of Thomas Hardy; six of his nine song cycles set Hardy’s poetry. One of these, A Young Man’s Exhortation, was Finzi’s second song cycle for voice and piano, but the first to earn notable success. Finzi writes for the piano lyrically and independently, as though the piano were a duetting voice—or in many cases, voices. This paper focuses on a …

X-ray crystallography is a robust and powerful structural biology technique that provides high-resolution atomic structures of biomacromolecules. Scientists use this technique to unravel mechanistic and structural details of biological macromolecules (e.g., proteins, nucleic acids, protein complexes, protein-nucleic acid complexes, or large biological compartments). Since its inception, single-crystal cryocrystallography has never been performed in Türkiye due to the lack of a single-crystal X-ray diffractometer. The X-ray diffraction facility recently established at the University of Health Sciences, İstanbul, Türkiye will enable Turkish and international researchers to easily perform high-resolution structural analysis of biomacromolecules from single crystals. Prison reentry programs attempt to equip justice-involved veterans with life skills necessary for their transition out of prison. This qualitative study addressed the scant understanding of the impact pre-released prison reentry programs have on justice-involved veterans’ transition and reintegration back to the community. AbstractVaccination hesitancy is high among African Americans and other people of color. The practice gap addressed in this project was the lack of a culturally sensitive clinical guideline for vaccination education relevant to the African American community. The practice-focused question was answered by developing an evidence-based Clinical Vaccination Education Guideline for the African American Community (CVEGAAC) that was validated by a group of four content experts using the Appraisal of Guidelines, Research, and Evaluation tool. Since the introduction of antiretroviral therapy, the survival rate of HIV patients has been on the rise with a predicted increase by 2030, but this has also contributed to early onset of comorbidities such as cardiovascular disease, osteoporosis, and kidney failure. Invasive mosquitoes threaten the environment and humans, as they transmit 17% of infectious diseases including malaria, West Nile virus and yellow fever. This essay analyzes the wartime framing of “fighting” a war on COVID-19—to “fight that invisible enemy,” coronavirus. How might lawmakers address social crises, such as COVID, with the urgency of an emergency without reinforcing our overly-militarized, securitized, and policed state? The problem with a securitized approach is that the notion of security combines conceptual ambiguity with heightened urgency and this indeterminacy can shape and even distort legality in exceptional moments.

The eastern oyster (Crassostrea virginica) is a keystone species that stabilizes sediments, cycles nutrients, filters water, and provides habitats and food for estuarine fauna. Oysters are prone to several macroparasites, including oyster pea crabs (Zaops ostreum), mud blister worms (Polydora websteri), and boring sponges (Cliona sp.). Prior research has shown that shellfish infected with blister worms and boring sponges exhibit weaker shells, are more vulnerable to damage and predation, and potentially experience reduced condition. Oxyclozanide is a salicylanilide derivative anthelmintic drug with a well-known effect on parasites that cause infections in humans and animals. In this study, the effect of oxyclozanide on DNA damage in hemocytes of Galleria mellonella (L.) (Lepidoptera: Pyralidae) larvae, which has been used as a model organism in many fields, was investigated. Hemolymph was collected from the last instar larvae (7th instar) reared on artificial diets containing oxyclozanide at different concentrations (0.003%, 0.03%, 0.3%, and 1.5%) under laboratory conditions and then hemocytes suspension was prepared. Environmental variation plays an important role in shaping patterns of parasite and pathogen infections within ecosystems and populations. Effective management of wildlife diseases hinges on the detection of parasites and pathogens, followed by the identification of trends in infections that can be used for predicting their spread and implementing control measures. Beavers are ecosystem engineers and keystone species that serve as important sources of environmental variation. The outbreak of the novel coronavirus (SARS-CoV-2) in Wuhan, China and subsequent global pandemic illustrated the need of accurate forecasting and a greater understanding of the underlying dynamics of infectious diseases.

In this dissertation, we examine the six most populous towns in the State of Montana through the lens of the classic SIR model. With this, we show that the six towns in question exhibit similar disease dynamics and population behavior within each wave. Furthermore, we conduct analysis on the age demographics of COVID-19 cases and deaths. This analysis corroborates the findings of the SIR model fits, in that the …

UNLABELLED: The roles of preexisting auto-reactive antibodies in immune-related adverse events (irAEs) associated with immune checkpoint inhibitor therapy are not well defined. Here, we analyzed plasma samples longitudinally collected at predefined time points and at the time of irAEs from 58 patients with immunotherapy naïve metastatic non-small cell lung cancer treated on clinical protocol with ipilimumab and nivolumab. We used a proteomic microarray system capable of assaying antibody reactivity for IgG and IgM fractions against 120 antigens for systemically evaluating the correlations between auto-reactive antibodies and certain organ-specific irAEs. OBJECTIVES: This study aims to assess the association between daily temperature and sickness absence episodes in the Mediterranean province of Barcelona between 2012 and 2015, according to sociodemographic and occupational characteristics.

METHODS: Ecological study of a sample of salaried workers affiliated to the Spanish social security, resident in Barcelona province between 2012 and 2015. The association between daily mean temperature and risk of new sickness absence episodes was estimated with distributed lag non-linear models. The lag effect up to 1 week was considered. We collected >40,000 mosquitoes from 5 provinces in South Africa during 2011-2018 and screened for zoonotic flaviviruses. We detected West Nile virus in mosquitoes from conservation and periurban sites and potential new mosquito vectors; Banzi virus was rare. Our results suggest flavivirus transmission risks are increasing in South Africa.

BACKGROUND: Delayed diagnosis and inadequate treatment caused by limited biomarkers are associated with the outcomes of COVID-19 patients. It is necessary to identify other promising biomarkers and candidate targets for defining dysregulated inflammatory states.

METHODS: The triggering receptors expressed on myeloid cell (TREM)-1 and TREM-2 expression from hospitalized COVID-19 patients were characterized using ELISA and flow cytometry, respectively. Their correlation with disease severity and contrast with the main clinical indicators were evaluated.

RNA-based therapies are currently used for immunisation against infections and to treat metabolic diseases. They can modulate gene expression in immune cells and hepatocytes, but their use in other cell types has been limited by an inability to selectively target specific tissues. Potential solutions to this targeting problem involve packaging therapeutic RNA molecules into delivery vehicles that are preferentially delivered to cells of interest. In this review, we consider why the kidney is a desirable target for RNA-based therapies in cardiovascular disease and discuss how such therapy could be delivered.
