Lefamulin is a newer pleuromutilin antibiotic approved by the FDA (2019) for the treatment of community-acquired bacterial pneumonia (CAP). In addition, recent studies have shown the off-label use of Lefamulin to treat multidrug-resistant tuberculosis (MDR-TB) and sexually transmitted infections (STIs) caused by Mycoplasma genitalium. Therefore, this study aimed to conduct a literature review evaluating the potential off-label applications of lefamulin. Treatment of MDR-TB is typically complex because of the increased resistance to the standard first-line therapy. In addition, the entry of antibiotics into macrophages is limited by hypoxia, granulomas, and necrosis caused by MDR-TB. Studies have shown that Lefamulin exhibits …

Introduction: Doxycycline is a tetracycline-class antibiotic that inhibits bacterial protein synthesis by binding to the 30s ribosomal subunit, thereby preventing bacterial growth. It has a broad spectrum of clinical indications including treatment of Chlamydia infections, Lyme disease, Anthrax, Rickettsia, Atypical pneumonia, Staph aureus, Cholera, Acne vulgaris, and Traveler’s diarrhea, as well as malarial prophylaxis. Although generally well tolerated, effective and affordable, doxycycline is associated with several mild adverse effects like gastrointestinal distress and photosensitivity. One clinically significant adverse reaction is phototoxicity which occurs following exposure to ultraviolet radiation. Clostridioides difficile is a bacterium that colonizes a patient's gastrointestinal tract after the use of systemic antibiotics. This infection, also called CDI, causes severe diarrhea and can be life-threatening. In some patients, CDI recurs, creating a dangerous cycle. Recurrent CDI (rCDI) persists because systemic antibiotics induce dysbiosis, disrupting the gut microbiota’s metabolic functions and compromising the intestinal epithelial barrier. This study was conducted as a literature review evaluating the safety, efficacy, mechanism of action, and clinical use of Rebyota in preventing rCDI. Rebyota is the first FDA-approved (2023) live biotherapeutic designed to restore eubiosis in adult patients after antibiotic treatment. …

Streptococcus equi subspecies zooepidemicus (SEZ) is a beta-hemolytic group C streptococcus commonly found in the upper respiratory tract of horses and other animals. Although SEZ is a frequent cause of equine “strangles,” human infection is exceptionally rare and associated with direct animal contact or consumption of unpasteurized animal products. Human SEZ infections have been reported to present with bacteremia, septic arthritis, endocarditis, or meningitis—particularly in immunocompromised individuals—but involvement of prosthetic joints is scarcely described in the literature. Women who experience recurrent urinary tract infections (rUTIs), defined as more than three infections per year, often face significant reduction in their quality of life, including travel and employment. Additionally, there is concern about the long-term effects of prolonged and repeated use of antibiotics, which are currently the standard for prophylaxis and treatment. Overuse often results in resistance. Multiple countries currently offer, or are in the process of developing, vaccines that can prevent rUTI. Uromune MV140 from Immunotek is one such vaccine. Cancer is the overgrowth of dysregulated or mutated cells that affects 1 in 3 Americans. Chalcones are natural products with many possible derivatives, such as 4’-trifluromethoxychalcone (4TF), developed in Dr. Krzysiak’s lab, which exhibit anticancer activity against cancer cell lines. Tumor Necrosis Factor-Related Apoptosis-Inducing Ligand (TRAIL) is a tumor surveillance cytokine that acts on two receptor types: death receptors and decoy receptors. One way cancer cells can become resistant to TRAIL is by upregulating decoy receptors and/or downregulating death receptors. Coccidioidomycosis is a serious fungal infection that can produce a wide range of clinical manifestations, from mild, self-limiting pulmonary disease to severe, disseminated infection that can involve nearly any organ system, including the skin. Coccidioidomycosis may also affect patients at the psychiatric level. Its clinical relevance has increased markedly in recent years, as incidence continues to climb, geographic distribution widens, and emerging presentations challenge traditional assumptions regarding fungal latency and host immunity. The social determinants of health, as defined by the World Health Organization (WHO), are “the conditions in which people are born, grow, live, work and age, and people’s access to power, money and resources.” Epidemiologists have long studied the nexus between social forces and disease and have widely agreed that such forces are directly causal of pathologies. This paper is an analytic review of the existing literature on several social determinants— namely socioeconomic status (SES), race and ethnicity, age, working and living conditions, and social integration— and how they pertain to the causation, progression, and treatment of cancer.

Oxidation of arachidonic acid generates biologically active derivatives that play a critical role in shaping the inflammatory response. One such derivative, carboxypropylpyrrole (CPP), is a stable end-product of arachidonic acid oxidation that has been previously described and detected in inflamed tissues; however, its pathophysiological contribution to inflammation has not been investigated. CPP forms covalent adducts with the extracellular matrix and plasma proteins, altering their biochemical properties and receptor-binding characteristics. Here, we examined how CPP formation impacts neutrophil migration during acute inflammation. Using cell adhesion assays and biolayer interferometry, we identified integrins αMβ2 and αDβ2 as receptors for CPP-modified proteins. Among …

It is predicted that by 2050, deaths due to antibiotic resistance will supersede cancer and cardiovascular disease as the number one cause of death in the world. The present research aims to study the polymicrobial interactions of microbial communities in the Appalachian region. Microbial samples were obtained from mountains surrounding East Tennessee State University, isolated and screened against common human opportunistic bacteria. One particular bacterium, S1S4-10, was found to be antagonistic against Bacillus subtilis and created zones-of-inhibition on agar media. Reverse testing found that the B. subtilis did not have a counter inhibition against S1S4-10. Co-cultures found that S2S4-10 could …

Research has indicated a relationship between faculty and students as it pertains to stress levels and subsequent well-being in occupational therapy (OT) programs (Grab et al., 2021), a potential indication of a stress contagion effect. This research aimed to determine if faculty stress levels affect student stress levels in OT graduate programs. The researcher used an ecological correlational study design with a nonprobability convenience sampling procedure. Faculty and students (n = 19 and n = 165, respectively) from 367 accredited OT graduate programs across the United States completed a survey consisting of demographic questions and the PSS-10. The emergence of illicitly manufactured fentanyl has increased clinical uncertainty during buprenorphine induction in medication-assisted treatment for opioid use disorder. The purpose of this quantitative, retrospective cohort study, guided by the Transtheoretical Model (TTM), was to examine whether prescriber-level buprenorphine induction practices and experiences with precipitated withdrawal were associated with reported buprenorphine induction challenges, and whether county-level fentanyl prevalence contributed to these challenges after accounting for geographic clustering. Secondary, de-identified archival data were obtained from the National Institute on Drug Abuse Clinical Trials Network study CTN-0134 and included 64 prescribers practicing in fentanyl-endemic settings. The purpose of this quantitative study was to assess the well-being of medical students and identify the primary factors influencing their well-being. In this manuscript-style dissertation, the empirical article assesses the well-being of medical students through the dimensions of wellness and investigates the significance of differences between each dimension and self-perceived levels of happiness, stress, and satisfaction with life. The practitioner article provides recommendations to practitioners to further support the well-being of osteopathic medical students. The purpose of this dissertation study was to understand the perceived effects of long-term COVID-19 on Black essential workers at the height of the pandemic and overall impact on mental health. The researcher interviewed 19 Black essential workers from Texas, across economic sectors (e.g., healthcare, sanitation), and who experienced long COVID symptoms. Findings centered across four themes: asking for help during the pandemic, COVID symptoms, being Black, and mental health. Using a theoretical approach incorporating critical race theory and social determinants of mental health, findings revealed the ways systemic racism affected these Black essential workers. Background/aim: Acute lymphoblastic leukemia (ALL) treatment is frequently complicated by infections, emergency visits, and therapy interruptions, yet early prediction of short-term clinical deterioration remains challenging. Traditional prognostic markers rely on static laboratory values, whereas dynamic hematologic fluctuations may provide earlier warning signals. This study presents and internally validates a clinically applicable prediction model based on dynamic hematologic parameters and clinician-documented symptoms for predicting short-term (7-day) clinical events in children with ALL.

Modeling Bitcoin Dynamics Using Differential Equations, Boone M. Fleenor

In this thesis, we develop and analyze two nonlinear systems of ordinary differential equations to model Bitcoin price dynamics. Analytical techniques are used to obtain exact or approximate solutions where possible. Then, numerical simulations using a fourth-order Runge–Kutta method are employed to explore system behavior beyond analytically tractable regimes. Finally, model outputs are compared to historical Bitcoin price data using normalized and resampled time series. These results suggest that deterministic models can provide meaningful insight into the structural behavior of Bitcoin markets, while highlighting the need for stochastic or time-dependent extensions for more realistic modeling.

Background/aim: Post-exposure prophylaxis (PEP) for human immunodeficiency virus (HIV) plays a vital role in preventing transmission among healthcare workers following occupational exposure. Despite its clinical importance, epidemiological data regarding PEP implementation in Türkiye remain scarce. This study aimed to assess PEP practices, prophylactic treatment regimens, and follow-up outcomes among healthcare workers at risk of occupational HIV exposure.

Background/aim: Osteoporosis is a multifactorial skeletal disease that predominantly affects postmenopausal women and is characterized by reduced bone mass and an increased risk of fractures. Growing evidence suggests that the gut microbiota plays a pivotal role in bone metabolism through immunological, endocrine, and metabolic pathways, forming the basis of the so-called gut– bone axis. This study aimed to characterize gut microbiota composition in postmenopausal women with osteoporosis compared with healthy controls.

Ciltacabtagene autoleucel (cilta-cel), an anti-B-cell maturation antigen (BCMA) chimeric antigen receptor T-cell (CAR-T) therapy, was approved in 2022 for heavily pretreated relapsed/refractory multiple myeloma (RRMM). This study evaluates the safety and efficacy of cilta-cel in RRMM patients reported to the Center for International Blood and Marrow Transplant Research registry between March 2022 and December 2023 who met commercial release specifications. Among 595 patients, median age was 64 years, 57% were male, and 70% had ≥1 comorbidity. Ready-to-use therapeutic and supplementary foods (RUTF/RUSF) are a primary treatment for childhood malnutrition in Community Management of Acute Malnutrition (CMAM) programs. However, measuring intervention compliance is labor intensive and unreliable. We applied FoodSeq, a fecal genomic dietary assessment biomarker, in malnourished infants (3-15 months) from Matiari, Pakistan. FoodSeq identified a significant increase in the abundance of DNA from chickpea, a primary RUSF ingredient, during RUSF administration and captured region-specific complementary feeding practices, including age-inappropriate feeding practices such as wide-spread tea consumption. Telehealth has improved patient access by reducing barriers to convenient, cost-effective, and quality health care. Telehealth mobile applications allow individuals to remotely access services through smartphone-based platforms. The purpose of this study was to evaluate leading telehealth applications available in the United States, analyzing key factors that influence platform selection, including cost, prescribing and diagnostic capabilities, user feedback, and data privacy practices. Applications were identified through keyword-based searches of major app stores using the term “telehealth,” and each app was systematically evaluated using predefined criteria.
