New guidelines from the Accreditation Council on Graduate Medical Education for critical care ultrasound (CCUS) training went into effect on July 1, 2024, for critical care medicine and combined pulmonary and critical care medicine fellows. Fellowship programs will need to ensure that their fellows demonstrate competence in skills that are essential to CCUS, which currently encompasses applications in basic cardiac, lung/pleural, abdominal, and vascular ultrasonography (US). This may be a challenge for fellowship programs that have nascent CCUS curricula and limited faculty expertise. Redesigning Post-Incarceration Employment Tax Credits: Making Work Opportunity Credits Work For Former Felons, Stephanie Hunter Mcmahon

Former felons have a significantly higher unemployment rate than other groups of American workers. This unemployment increases recidivism as well as broader societal problems. Consequently, Congress enacted the Work Opportunity Tax Credit to encourage employers to hire former felons (and other groups of targeted workers) by offsetting their wages. For the credit to increase employment, employers must know whether potential workers would likely qualify for the credit before hiring decisions are made. Exonerated individuals are released from prison, but they cannot recover the time they were incarcerated. The time is gone, as is its potential to qualify for the nation’s social safety net. Most people earn a right to the safety net and for retirement through their employment; however, even when prisoners work, they are denied employment-related benefits that would be enjoyed by those not incarcerated. Inmate labor does not earn toward Social Security, Medicare, and Social Security Disability Insurance, and the exclusion applies even if the inmate is later exonerated of the crime for which they were imprisoned. Gerald Templer's Leadership In The Malayan Emergency (1948-1960): Its Enduring Relevance, Kumar Ramakrishna

This article explores the leadership contributions of General Sir Gerald Templer, who was both High Commissioner and Director of Operations, during the counterinsurgency campaign waged by the British colonial authorities in Malaya against the Malayan Communist Party, between 1948 and 1960. This campaign has become known as the Malayan Emergency. The article explores how Templer’s leadership—marked by a combination of high intellect, sound practical nous, and the ability to galvanize governmental action and ultimately inspire the population to eventual victory—was instrumental in transforming the nature of the problematic initial British response to the Communist insurgency. Earlier this year, the Paterson City Council proposed an ordinance to criminalize homelessness. Many residents were unhappy with the proposed ordinance, feeling that it was unjust to treat the homeless in Paterson that way, and that it wouldn’t effectively address the root causes. The ordinance was withdrawn, but homelessness in Paterson still needs to be addressed. This paper proposes alternate solutions to homelessness; through increasing shelter capacity, affordable housing, modifying the ordinance, increasing in voluntary commitment, measures, and alternatives to involuntary commitment.

Because of its life-saving benefits, perioperative IV fluid therapy remains a cornerstone of medical treatment. However, it also induces sustained edemagenic stress. The resulting persistent interstitial edema-excessive fluid accumulation in the interstitium-significantly delays recovery and worsens patient outcomes. Therefore, to gain a detailed understanding of the lymphatic functional consequences of perioperative fluid therapy, this study aimed to test the hypothesis that perioperative IV fluid therapy compromises lymphatic pump function within 3 days after major surgery. This paper presents the development and validation of CHIIP (Continuous Health Information for Infrastructure Protection), a wireless structural health monitoring (SHM) device designed to provide accessible, real-time data on structural performance of building structures following a seismic event. The system integrates inclinometers, accelerometers, linear potentiometers, and strain gauges with ESP32 microcontrollers to create a compact and robust data collection device that can transmit synchronized sensor data via Wi-Fi to a remote server. From the moment an Indigenous girl is born, her identity is shaped by two intersecting categories: gender and ethnic belonging. These identity categories influence various aspects of her environment, upbringing, education, work and life.

As a member of the Wayuu people, I understand the challenges of being an Indigenous woman. The Wayuu Indigenous people are the largest Indigenous group in both Colombia and Venezuela. Black women face unique challenges related to intersecting racial and gender-based discrimination, often referred to as gendered racism. Previous research has indicated this form of oppression may significantly affect mental health, particularly depression. However, few studies have examined how gendered racism interacts with racial identity and demographic factors (e.g., age, marital status, geographical location) to predict depressive symptoms. The transition to a blended family is an emotional and psychological adjustment for bio-childfree individuals. The present qualitative study investigated how self-concept and emotional adjustment of bio-childfree stepmothers change when they become members of their new family. With a phenomenological approach, semi-structured interviews were conducted with 12 female participants who were bio-childfree, in a committed relationship with a partner with children, and had cohabited for one year or more. St. Angela’s School is an elementary school in the Austin area of Chicago. This phenomenological qualitative study examined Sense of Community (SOC) at St. Angela’s, operationalized as “The feeling that St. Angela School is welcoming, psychologically and physically safe, that I matter, I am respected, and that I belong.” As an inner-city, parochial school, St. Angela’s has seen extensive changes in the neighborhood and the student population, both in ethnicity and enrollment. Youth migration from rural to urban areas in Ghana, particularly to cities like Accra, Tema, Kumasi, and Takoradi continues to rise, despite ongoing efforts to improve youth employability and reduce urban influx. This study examined the extent to which youth attachment to community and participation in employment initiatives predict migration intentions. Guided by the network, sense of community, neoclassical, and empowerment theories, the research employed a mixed-method correlational design. Despite growing awareness of mental health disparities, BIPOC (Black, Indigenous, and People of Color) communities continue to face significant barriers in accessing culturally responsive mental health care. This dissertation explores the intersection between formal mental health services and informal social supports such as family, religious institutions, and community networks to understand which systems are more trusted, accessible, and effective for BIPOC individuals. Intersecting Inequities: Latinx Access To Mental Health Services, Melissa Thorson

This study examined barriers to mental health service utilization among Latinx populations using data from the 2022 National Survey on Drug Use and Health (NSDUH). Through the theoretical frameworks of Minority Stress Theory and Toxic Stress Theory, the study investigated the relationships between age, language proficiency, socioeconomic status, and mental health service access among different Latinx subgroups. Statistical analyses included chi-square tests, multiple regression, and mediation analyses on a sample of Latinx adults (N = 9,852). Results indicated associations between age and English proficiency (χ² = 145.23, p < .001, Cramer's V = 0.38), with 45% of adults aged 50 and older reporting limited English proficiency. Socioeconomic analysis revealed that 32% of older Latinx adults reported family incomes below $20,000, with insurance status partially mediating the relationship between income and service utilization (indirect effect = 0.15, 95% CI [0.08, 0.23]). differences in utilization rates emerged among Latinx subgroups (F(6, 1243) = 8.76, p < .001, η² = 0.12), with Mexican-origin individuals showing higher utilization (10%) compared to Puerto Rican individuals (2-3%). Age and socioeconomic status demonstrated interaction effects (β = 0.18, p < .01), while language proficiency moderated the relationship between age and service utilization (β = 0.24, p < .001). These findings suggest that barriers to mental health service utilization among Latinx populations are complex and interconnected, requiring multilevel interventions that address linguistic, cultural, and socioeconomic factors simultaneously. Implications for healthcare policy and service delivery are discussed, emphasizing the need for culturally responsive and linguistically appropriate mental health services.
