Biostatistics Collaborative Core: Faculty Publications and Other Works
Background/Objectives: Total thymectomy is currently the gold standard operation for treating thymoma. However, recent studies have suggested the potential health consequences of thymus removal in adults, including possible increased autoimmune disease and all-cause mortality. In this context, we assess oncologic outcomes following total vs. partial thymectomy for early-stage thymoma. Methods: We identified patients diagnosed with WHO types A–B3 thymoma between 2010–2021 from a national hospital-based dataset. We excluded patients with stage II or higher disease, open resections and perioperative chemo-/radiation therapy. We stratified patients into total and partial thymectomy cohorts. (1) Background: The integration of digital technologies such as electronic health records (EHRs), telepsychiatry, and communication platforms has transformed the mental health sector a lot compared to in previous years. While these tools enhance service delivery, they also introduce unique stressors. Despite growing concerns, there is no validated instrument specifically designed to measure the digital stress experienced by mental health professionals. The role of midbrain dopamine (DA) activity in reward seeking and reinforcement learning is well-documented, contrasting the lesser-understood impact of inhibitory signaling on DAergic activity, especially in the context of aversive learning and avoidance. The rostromedial tegmental nucleus (RMTg) is a GABAergic midbrain structure activated by aversive stimuli and exerts inhibitory influence over ventral tegmental area (VTA) DAergic activity in response to aversive consequences. Stimulation of the RMTg decreases the firing rate of VTA DA neurons and direct inhibition of the RMTg – VTA circuit enhances persistent reward seeking despite aversive consequences, a major criterion of substance use disorder (SUD). …

Physician Assistant (PA) education is rigorous and comprehensive. In only 27 months, the students must learn medical knowledge and skills for effective patient care. Instruction must be efficient and effective to teach these students how to be safe and functional PAs. Of primary concern is teaching students how to perform like clinicians, which requires critical thinking, self-directed learning, and teamwork.
This convergent, mixed-methods action research explored the impacts of case-based learning (CBL) in the PA classroom. The findings support using CBL to develop clinical reasoning skills, improve comprehension of the material, and develop self-directed learning skills. CBL requires the students …

Can I eat this food or not? Is this food suitable for diabetes and why? Which AI pipeline is best suited for a given task and dataset? How should an end-to-end pipeline be constructed? These questions differ from factual question-answering tasks. Recipes and AI pipelines are processes consisting of several entities interacting with each other. A recipe consists of ingredients, cooking methods, and their interactions, while an AI pipeline includes datasets, preprocessing techniques, models, hyperparameters, tasks, and results. Conjure practices in the nineteenth-century American South, such as “Hoodoo” or “Voodoo,” are commonly portrayed as primarily “black” beliefs or entrenched in “Africanistic” cultures. For the enslaved, their black spiritual systems, including conjure, were a powerful tool of resistance against the white enslaver. Black conjure practitioners established themselves as an affordable and pervasive medical alternative to often ineffective or harmful professional medicines for black and white Southerners, regardless of class. In this way, conjure challenged the authority of racial capitalism and destabilized the perceived moral and intellectual superiority of whiteness. This dissertation consists of three studies integrating frequentist and machine learning techniques to examine physical fitness trajectories and future physical military readiness. Study 1 consists of a cross-sectional examination of Physical Fitness Test (PFT) scores from four cohorts of first-year Cadets at a Senior Military College from 2019 to 2022. Study 2 consists of a longitudinal examination of PFT performance from one cohort of Cadets across a baseline test and three years of training at a Senior Military College. The classical Median Voter Theorem (MVT) predicts that, in a two-party system with majority rule, candidates will converge toward the ideological center to capture the median voter. This logic, grounded in spatial models of political competition, assumes full voter participation and fails to account for the growing ideological polarization in modern democracies. This thesis reevaluates the MVT by incorporating probabilistic turnout into a utility-based model of voter behavior. It theorizes that turnout is shaped not only by ideological proximity but also by expressive motivations, including both strong affinity and strong aversion to candidates. Group testing involves pooling specimens from multiple individuals and offers an efficient means to surveil low-prevalence pathogens, but poses challenges for spatial cluster detection when only pooled results are observed. In this thesis, we develop a spatial scan statistic tailored to group-testing data with variable pool sizes. The statistic compares a null hypothesis of a homogeneous infection rate across all clusters to an alternative hypothesis that infection probabilities differ inside and outside a candidate cluster, with both models fitted by maximum likelihood estimation. We approximate the null distribution of the maximum likelihood ratio test via Monte Carlo simulation.

The study aims to test the existence of the N-shaped Environmental Kuznets Curve (EKC) and to determine the impact of fossil energy consumption and economic activity such as industrialization, urbanization, and trade openness on CO2 emissions in Indonesia from 1971 to 2021, using Autoregressive Distributed Lag (ARDL). The results of this study show that the relationship between per capita GDP and CO2 emissions in Indonesia has a reversed N-shape. Industrialization and per-capita consumption of fossil energy have a positive impact, while trade openness has a negative impact on CO2 per-capita emissions both in the short and long term. The COVID-19 vaccination is considered crucial for controlling the pandemic and accelerating economic recovery. However, empirical evidence on the impact of vaccination on health and the economy at the district/city level in Indonesia remains limited. This study aims to analyze the impact of vaccination on health outcomes and economic conditions. The methodology used is a fixed effect model with monthly panel data on vaccination and health outcomes (positivity rate and recovery), as well as annual data on per capita expenditure in 514 districts/cities from January 2021 to December 2022. Purpose: Acute leukemia is a group of hematologic malignancies categorized according to the immature cells that proliferate and replace the normal bone marrow. Flow cytometry has emerged as a cornerstone in the diagnosis of hematologic malignancies. Staged analysis with a screening tube containing specific lineage markers determines the need for subsequent testing if there is an abnormal population (blasts). The specific lineage panels to be analyzed are determined depending on the positive markers in the screening tube. Given the known relationship between CDKN2A homozygous deletion (HD) and worsened outcomes in both meningiomas and IDH-mutant astrocytomas, it is paramount to identify CDKN2A HD for accurate risk stratification of patients. Multiple array platforms can detect CDKN2A HD. However, these methods are expensive and are not readily available at every institution. To address this, we conducted a meta-analysis and literature review to evaluate 5'-methylthioadenosine phosphorylase (MTAP) expression determined by immunohistochemistry (IHC) as a surrogate of CDKN2A HD. Target trial emulation (TTE) aims to estimate treatment effects by simulating randomized controlled trials using real-world observational data. Applying TTE across distributed datasets shows great promise in improving generalizability and power but is always infeasible due to privacy and data-sharing constraints. Here we propose a Federated Learning-based TTE framework, FL-TTE, that enables TTE across multiple sites without sharing patient-level data. FL-TTE incorporates federated protocol design, federated inverse probability of treatment weighting, and a federated Cox proportional hazards model to estimate time-to-event outcomes across heterogeneous data. Objective: Unlocking clinical information embedded in clinical notes has been hindered to a significant degree by domain-specific and context-sensitive language. Identification of note sections and structural document elements has been shown to improve information extraction and dependent downstream clinical natural language processing (NLP) tasks and applications. Fever of unknown origin (FUO) in people living with human immunodeficiency virus (PLHIV) is clinically defined as recurrent fever lasting more than four weeks in the outpatient setting or more than three days during hospitalization, despite a thorough diagnostic evaluation. This evaluation typically includes a comprehensive medical history, physical examination, imaging studies (such as chest radiography), and an extensive range of laboratory tests, including complete blood counts, blood and urine cultures, and metabolic panels. Background: Venoarterial extracorporeal membrane oxygenation (VA-ECMO) is used in patients with cardiopulmonary collapse, but data regarding its use in pregnancy is limited. We aimed to identify the clinical characteristics and predictors of in-hospital mortality in female patients requiring VA-ECMO, including evaluating the role of pregnancy.
Methods: In this cross-sectional study, a cohort of female patients aged 19-45 years who required VA-ECMO were identified from the National Inpatient Sample from 2018 to 2021 and further stratified according to presence of pregnancy-associated diagnoses. Gastric outlet obstruction (GOO) is a clinical diagnosis characterized by early satiety, abdominal pain, post-prandial vomiting, and weight loss. Here, we present a rare case of an ectopic pancreatic cyst inducing GOO, highlighting the importance of a thorough diagnostic workup. A 40-year-old man with no prior medical or surgical history presented with a six-month history of intermittent epigastric pain, nausea, and vomiting with an inability to tolerate oral intake for a week. Background: Encephalitis is characterized by brain parenchymal inflammation caused by infection or autoimmunity. There are limited recent data on how immunocompromised patients with encephalitis differ from the general encephalitis population.
Methods: This retrospective study of 2 large medical institutions compares clinical characteristics and outcomes of immunocompromised and immunocompetent patients with all-cause encephalitis.
Results: Of the 657 patients, 151 (23%) were immunocompromised. Immunocompromised patients were more likely to have an infectious etiology, comorbidities, inflammatory cerebrospinal fluid (CSF) profile, abnormal neuroimaging, and worse clinical outcomes as assessed by discharge Glasgow Outcome Scale (GOS) and in-hospital mortality (all P < .05). The most commonly identified etiologies in immunocompromised patients were herpes simplex virus (HSV) and varicella zoster virus. HSV accounted for similar proportions in the immunocompromised (18%) and immunocompetent (14%) groups, though it was more commonly associated with a CSF neutrophilia in the immunocompromised group (P = .001). Patients with interrupted inferior vena cava (I-IVC) and azygos continuation who undergo Fontan completion via hepatoazygos shunting exhibit unique hemodynamic challenges. This study evaluates age-related shifts in systemic venous return dominance, hepatic flow distribution (HFD), power loss (PL), and flow disturbances using patient-specific computational fluid dynamics (CFD). Data analysis from 95 patients with I-IVC showed a nonlinear shift in upper-to-lower body systemic flow dominance with ratios of 2, 1, and 0.5 (correlating to ages ∼3, ∼10, and ∼20, respectively). Aims: To evaluate treatment failure of sodium-glucose cotransporter 2 inhibitors (SGLT2i) among adults with type 2 diabetes (T2D) and assess associated risk factors and clinical and economic outcomes in the real-world setting.
Methods: Adults with T2D who initiated SGLT2i were selected from the Komodo's Healthcare Map, an administrative claims database (2016-2024). Treatment failure was defined as discontinuation, add-on/switching, or uncontrolled glycated hemoglobin (HbA1c) without action. Risk factors, treatment patterns, and clinical and economic burden were assessed.

Background: Acute ischemic stroke (AIS) from distal medium vessel occlusion (DMVO) presents unique treatment challenges. Mechanical thrombectomy (MT) is emerging as a viable option for these patients, yet the role of pre-stroke aspirin treatment is unclear. Background: There is uncertainty about whether patients with M2 occlusion benefit from endovascular therapy (EVT) in the late (6-24-hour) time window. We evaluated the clinical outcomes of patients with M2 occlusion selected for EVT compared with those who received medical management (MM) in the late window.
Methods: This multinational cohort study was conducted at 66 sites across 10 countries (January 2014 to May 2022). Membrane protein topology studies offer guidance to membrane protein structure, folding, and function, serving as a credible scaffold for designing site-directed mutagenesis and biochemical experiments, helping to identify functionally significant extracellular and intracellular regions, modeling three-dimensional structures, and building reliable mechanistic models. Membrane protein structure as a function of given lipid composition and physiological state of the cell is best probed in whole intact cells. A described simple and advanced immunofluorescence protocol applied to the transmembrane orientation of extramembrane domains permits a topology analysis of plasma membrane proteins in their native state in living unperturbed eucaryotic cells.
