This mixed-methods study centers the voices, knowledge, and leadership of Black and Indigenous women working in Early Care and Education (ECE) while tracing how racism, sexism, and classism have shaped the field from colonization to the present day. Twenty in-depth interviews with sixteen Black and Indigenous educators across the United States are paired with secondary analyses of national workforce data to create a multilayered portrait of their professional realities. Grounded in Black feminist thought and Indigenous desire-based inquiry, the analysis surfaces six inter-locking themes: (a) enduring structural barriers that depress wages and limit advancement, (b) the day-to-day resilience that sustains …

The focus will examine the impacts horizontal and vertical bullying can have on teacher victims and bystanders within K-12th within California. Problem: Middle managers in healthcare experience high levels of stress, heavy workloads, and burnout. At a large medical center in Northern California, middle managers reported low perceptions of organizational support for well-being despite an abundance of available resources.

Context: Baseline data indicated a perceived support score of 6.5 out of 10 on the Employee Interest Survey and a People Pulse Culture of Health score of 60 for assistant nurse managers (ANMs), both below regional benchmarks. These findings suggested inadequate engagement with organizational well-being resources.

Billions of people today rely on traffic predictions to optimize their travels. Digital mapping services deliver accurate predictions by learning from vast troves of historical data. Impressive as these systems are, their assumptions do not always apply. They depend on an endless flow of sensitive user data to a central authority, a stable Internet connection, and trustworthiness on both sides of the traditional client-server model. In this paper, I explored program participant and staff perspectives on correctional education programs. Prior research measured success of correctional education through recidivism, post-employment opportunities, and post-secondary education. However, few research examined the perspectives of staff, program participants, and alumni of such programs to understand factors that aid in important educational experiences within correctional education programs. An unprecedented number of BIPOC teachers exited the profession since June 2020 largely due to the challenges posted by the COVID-19 pandemic (Doan et al., 2024). This study aimed to investigate ways these educators coped with the heavy toll of the converging pandemic(s) by asking what, if anything, sustained them? Participants provided individual and group testimonios and shared snapshots of what it was like to teach and exist during this time via the photovoice methodology and reflected on the challenges and sources of support. The bacterial type IV secretion systems (T4SS) are medically problematic for their roles in the dissemination of mobile genetic elements or effector proteins, but they also have great potential for new antimicrobial therapies. Recent studies have deployed the T4SS subfamily of conjugation systems to deliver gene editing CRISPR/Cas systems to disrupt drug resistance genes or kill targeted bacterial recipients. However, the therapeutic potential of conjugative CRISPR/Cas delivery is compromised by mutations or host repair systems that diminish the efficiency with which CRISPR/Cas induces double-strand breaks in new transconjugants. Introduction: Free hemoglobin's release into CSF from blood breakdown is a primary instigator of delayed cerebral ischemia (DCI) after aneurysmal subarachnoid hemorrhage (aSAH). Early clearance of subarachnoid blood with intrathecal (IT) fibrinolytics has shown potential to decrease incidence of DCI. However, the dosage of fibrinolytic needed is not known. We investigated the ability of low dose tissue plasminogen activator (tPA) to rapidly remove subarachnoid clot.

Purpose: This study examines cancer screening rates and the usual source of care among Hispanic survivors of adolescent and young adult (AYA, aged 15-39) cancer, a group at increased risk for poor health outcomes, including secondary cancers.

Methods: National Health Interview Survey data were analyzed to identify Hispanic survivors of AYA cancer and Hispanic age- and sex-matched non-cancer controls. Breast, cervical, and colorectal screening (ever screened and up-to-date according to the U.S. Preventive Services Task Force recommendations) as well as reporting a usual source of care were compared among survivors and controls eligible for screening using multivariable logistic regressions.

Objective: To compare the predictive value of minimal ablative margin (MAM) quantification using tumor segmentation on intraprocedural contrast-enhanced hepatic arterial (HAP) versus portal venous phase (PVP) CT on local outcomes following percutaneous thermal ablation of colorectal liver metastases (CRLM).

Methods: This dual-center retrospective study included patients undergoing thermal ablation of CRLM with intraprocedural preablation and postablation contrast-enhanced CT imaging between 2009 and 2021. Tumors were segmented in both HAP and PVP CT phases using an artificial intelligence-based auto-segmentation model and reviewed by a trained radiologist. In phase III oncology trials, superiority is defined by statistical significance using P thresholds. However, this approach has been criticized because P is continuous. Here, we reconstruct patient-level data for 230 phase III oncology trials to model the robustness of statistical significance by estimating the survival-inferred fragility index (SIFI), defined as the smallest number of patients changing arms that alters the statistical significance interpretation. The median SIFI was 8 patients (IQR 4-19), representing 1.4% of enrollments (IQR 0.7%-3%). As a continuous statistic, P-but not the significance interpretation-was correlated with SIFI. Moreover, overall survival endpoints were more fragile than surrogate endpoints. …

Lifileucel, a tumor-infiltrating lymphocyte (TIL) cell therapy approved for advanced melanoma, demonstrates promise for treating other solid tumors, including endometrial cancer (EC). The current study evaluates the feasibility of manufacturing TILs from EC tumors using Iovance’s proprietary 22-day Gen2 manufacturing process. Key parameters, including TIL yield, viability, immune phenotype, T-cell receptor clonality, and cytotoxic activity, were assessed. Of the 11 EC tumor samples processed at research scale, 10 (91%) successfully generated >1 × 109 viable TIL cells, with a median yield of 1.1 × 1010 cells and a median viability of 82.8%. Immune checkpoint inhibitors (ICIs), either as monotherapy (ICI-Mono) or combined with chemotherapy (ICI-Chemo), improves survival in advanced non-small cell lung cancer (NSCLC). However, prospective guidance for choosing between these options remains limited, and single-feature biomarkers like PD-L1 prove inadequate. We develop a machine learning model using clinicogenomic data from four cohorts (MD Anderson n = 750; Mayo Clinic n = 80; Dana-Farber n = 1077; Stand Up To Cancer n = 393) to predict individual benefit from adding chemotherapy. The emergence of both task-specific single-modality models and general-purpose multimodal large models presents new opportunities, but also introduces challenges, particularly regarding adversarial attacks. In high-stakes domains like healthcare, these attacks can severely undermine model reliability and their applicability in real-world scenarios, highlighting the critical need for research focused on adversarial robustness. This study investigates the behavior of multimodal models under various adversarial attack scenarios. We conducted experiments involving two modalities: images and texts. Our findings indicate that multimodal models exhibit enhanced resilience against adversarial attacks compared to their single-modality counterparts. Hydrological surface loads, such as snowpack and soil moisture, are the main drivers of seasonal crustal movements as seen by space geodetic techniques. In addition to that, seawater sometimes exerts additional forces for coastal stations, and the atmosphere often plays an important role for stations within continents. Here we report a case in and around the Arabian Peninsula. Despite little seasonal hydrological changes there, stations show fairly uniform seasonal vertical crustal movements, characterized by ~ 1 cm winter subsidence, over the entire region. Background:
This quality improvement project was implemented in the Transitional Cardiac Unit (TCU), a 32-bed telemetry unit primarily serving older adults recovering from cardiac procedures. A microsystem assessment revealed a hospital-acquired pressure injury (HAPI) rate of 1.49 per 1,000 patient days, exceeding the regional target of 0.82. Key barriers included limited skin assessment frequency, inconsistent documentation, and gaps in staff training.

Problem:
From June to December 2024, six HAPIs occurred in the TCU. Despite existing protocols requiring two-RN skin assessments at admission, transfer, and discharge, early detection of skin breakdown remained insufficient. Well-being captures an individual’s capacity to lead a mean ingful and engaged life. Interventions to address well-being go beyond managing symptoms or supporting daily activity performance. Designing trials to evaluate the effectiveness of such well-being interventions can be challenging. One such challenge is determining what the comparator or control arm should be. Several trials of complex interventions use usual care as a comparator, but this can be particularly challenging when planning trials across multiple countries and health care systems. Lineage switch (LS), defined as the immunophenotypic transformation of acute leukemia, has emerged as a mechanism of relapse after antigen-targeted immunotherapy, which is associated with dismal outcomes. Through an international collaborative effort, we identified cases of LS after a host of antigen-targeted therapies (eg, CD19, CD22, CD38, and CD7), described how LS was diagnosed, reviewed treatment approaches, and analyzed overall outcomes for this form of postimmunotherapy relapse. Background/Objectives: Despite medical advances, stigma remains a major challenge for people living with HIV (PLWH). This study examined clinical, sociodemographic, and psychological predictors of HIV-related stigma, and explored whether affective temperament moderates the impact of depression on stigma.

Methods: This cross-sectional observational study included 97 PLWH attending a tertiary infectious disease unit in Rome, Italy. Participants completed a battery of validated psychometric instruments assessing depressive symptoms, anxiety, manic symptoms, mixed affective states, general psychopathology, impulsivity, and affective temperament. Endometriosis affects approximately 190 million females of reproductive age worldwide. Magnetic Resonance Imaging (MRI) has been recommended as the primary non-invasive diagnostic method for endometriosis. This study presents new female pelvic MRI multicenter datasets for endometriosis and shows the baseline segmentation performance of two auto-segmentation pipelines: the self-configuring nnU-Net and RAovSeg, a custom network. The multi-sequence endometriosis MRI scans from two clinical institutions were collected. A multicenter dataset of 51 subjects with manual labels for multiple pelvic structures from three raters was used to assess interrater agreement. Although Japanese seniors are very healthy, research shows that they are not very happy. Compared to seniors in other developed countries, Japanese elders have lower ratings of subjective well-being and are increasingly alone. This paper argues that changes to Japanese family life help explain why Japanese elders are struggling with low subjective well-being. Specifically, declining marriage rates and declining rates of elders living with their children (co-residency) have made it harder for elders to get the support they need. Background/Objectives: Hemiarthroplasty (HA) and Reverse Total Shoulder Arthroplasty (RTSA) are both reliable treatment options for complex proximal humerus fractures. Background: Spinal cord injury (SCI) clinical trials typically rely on a single primary endpoint to assess drug efficacy. This strategy fails to adequately capture the full impact of treatment in heterogenous neurological conditions like SCI. A more patient-centric analysis requires assessment of neurological function, functional capacity, and quality of life, incorporating meaningful patient-reported outcomes. Coastal industrial plants dispose of their wastewater effluents into shallow coastal waters through sea outfalls. When two or more outfalls along a coastline are discharging, the long-term impacts are interdependent. Analytical solutions of two-dimensional advection-diffusion equations with a two-point source in a spring-neap oscillating tidal current on a sloping seabed are presented to study the far-field merging and interaction of two discharged effluent plumes in coastal waters. To treat this disease, antibacterial food sources are essential as therapeutic agents for orangutans.
