Immunoglobulin A vasculitis (IgAV), formerly Henoch–Schönlein purpura, is a small-vessel vasculitis characterized by palpable purpura, arthralgias, gastrointestinal involvement, and renal disease. Although IgAV is common in children, adult-onset disease is rare and may be associated with more severe manifestations, particularly renal involvement. In adults, IgAV may be difficult to recognize because of overlap with more common gastrointestinal, infectious, or medication-related conditions. We present a case of adult-onset IgAV initially attributed to presumed diverticulitis and later to a drug eruption following antibiotic exposure. This case highlights diagnostic pitfalls and the importance of early recognition and renal surveillance.

Introduction: Occurrence of 2 (or more) Hodgkin's and non-Hodgkin's lymphoma concordantly or sequentially in a single patient is very rare.
Case: An 88-year-old Italian male with a history of mucosa-associated lymphoid tissue (MALT) lymphoma in remission and indolent follicular lymphoma under surveillance, presented with acute autoimmune hemolytic anemia (AIHA). While initially speculated to be secondary to follicular lymphoma relapse, stable mesenteric lymphadenopathy compared to prior CT abdomen/pelvis and smudge cells on peripheral smear prompted further evaluation. Peripheral blood flow cytometry indicated a mature B-cell neoplasm, which was confirmed to be chronic lymphocytic leukemia (CLL) with a bone marrow biopsy. Minimal …

Progressive Facial Swelling And Airway Symptoms Revealing A Deep Neck Abscess With Atypical Microbiology, Drew Knight, Hakam Sulaiman, Arash Bakian, Ilya Fonarov
Journal of Community Hospital Internal Medicine Perspectives
Deep neck abscesses have become less common in the antibiotic era; yet, they still pose major risks of morbidity and mortality if not addressed appropriately. Their proximity to the upper respiratory tract and mediastinal structures further exacerbates this risk. We present the case of a 56-year-old man with a medical history of type 2 diabetes mellitus, who exhibited a two-month history of recurrent left-sided facial edema, pain, and dysphagia. Computed tomography imaging confirmed a substantial fluid collection extending from the submandibular region into the mid-cervical area, from which aspiration yielded 110 mL of purulent material. Cultures indicated Cutibacterium acnes, …

Journal of Community Hospital Internal Medicine Perspectives
Left ventricular non-compaction cardiomyopathy (LVNC) is a rare genetic cardiomyopathy characterized by prominent trabeculations and deep recesses that predispose to arrhythmias and heart failure. We describe a 42-year-old man with persistent tachycardia and exertional fatigue found to have CTI-dependent atrial flutter with 2:1 conduction. Transesophageal echocardiography showed severe biventricular dysfunction with an ejection fraction of 15%, and cardiac MRI confirmed LVNC with a non-compacted to compacted ratio of 3.3. After successful ablation and normal coronary angiography, he was diagnosed with LVNC. This case highlights an unusual presentation of LVNC with typical atrial flutter and underscores the value of cardiac MRI …

Rasmussen’S Aneurysm: A Rare But Life-Threatening Complication Of Pulmonary Tuberculosis – A Case Report., Karan Puri
Journal of Community Hospital Internal Medicine Perspectives
Rasmussen’s aneurysm is a rare but potentially fatal complication of pulmonary tuberculosis. It represents a pseudoaneurysm of a pulmonary arterial branch caused by erosion of the arterial wall secondary to chronic inflammation within a tuberculosis cavity. Although uncommon, especially in non-endemic regions, rupture of this aneurysm can result in massive hemoptysis and carries a high mortality if not promptly recognized and managed. We report a case of a healthcare worker in the United States, a non-endemic region, who presented with massive, life-threatening hemoptysis. She was found to have active pulmonary tuberculosis. Computed tomography angiography demonstrated a 10-mm pseudoaneurysm arising from …

Rectal Dieulafoy Lesion With Severe Hematochezia In A Warfarin-Treated Patient, Carlos Elizondo Alatorre, Maria Gabriela Rubianes Guerrero, Julio C Valencia Manrique, Jennifer Harley
Journal of Community Hospital Internal Medicine Perspectives
Dieulafoy lesions (DLs) are rare vascular malformations in the gastrointestinal tract that often present with gastrointestinal bleeding, most commonly in the stomach. Rectal DLs are exceptionally rare, representing less than 2% of cases. Warfarin use increases the risk for bleeding and management represents a challenge. We present a case of a 60-year-old male on chronic warfarin therapy who presented with severe hematochezia due to a rectal DL, requiring more than one colonoscopy and prothrombin complex concentrate (PCC) administration. While guidelines recommend PCC over fresh frozen plasma (FFP), they acknowledge that this is based on very low certainty of evidence.

Immune-mediated necrotizing myopathy (IMNM) is a rare subtype of idiopathic inflammatory myopathies that can present with rhabdomyolysis. We report a 43-year-old woman with rapidly progressive proximal muscle weakness following a flu-like illness. Laboratory evaluation revealed markedly elevated creatine kinase (22,207 U/L) and transaminases, with positive anti-signal recognition particle (SRP) and Coxsackie B4 antibodies. Muscle biopsy demonstrated extensive necrosis, regeneration, and endomysial lymphocytic infiltration, confirming IMNM. The patient was treated with intravenous methylprednisolone, IVIG, and rituximab, followed by oral steroid taper and mycophenolate. At six-month follow-up, she showed significant clinical and biochemical improvement. This case highlights a potential viral-triggered autoimmune mechanism, …

Piperacillin-Induced Immune Hemolytic Anemia Masked By Intestinal Failure-Associated Liver Disease: A Case Report With Three Serial Exposures, Stephanie Ammari, Nawras Ibrahim, Khalid Mahmood
Journal of Community Hospital Internal Medicine Perspectives
Background: Drug-induced immune hemolytic anemia (DIIHA) secondary to piperacillin has an estimated incidence of 0.01–0.10% [1,2]. In patients with intestinal failure-associated liver disease (IFALD), chronic conjugated hyperbilirubinemia obscures typical hemolytic markers [3,4].
Case Presentation: A 27-year-old woman with traumatic short bowel syndrome requiring total parenteral nutrition for 6 months, IFALD (baseline total bilirubin 2.5 mg/dL), and chronic kidney disease experienced three piperacillin-tazobactam exposures over two months. First exposure (12 days): hemolysis developed after drug discontinuation (primary sensitization). Second exposure: severe hemolysis occurred within 3 days (anamnestic response) with hemoglobin declining to 5.8 g/dL, undetectable haptoglobin, elevated LDH, reticulocytosis, and strongly …

A Tight Squeeze – Cardiac Tamponade In A Scleroderma Patient, Ifrah Nadeem, Charles E Geringer
Journal of Community Hospital Internal Medicine Perspectives
Systemic Sclerosis (SSc) is an autoimmune connective tissue disease marked by multiorgan fibrosis, including cutaneous involvement, and vasculopathy. Primary heart involvement in systemic sclerosis (SSc-pHI) includes heart failure, arrhythmia, pericarditis, pericardial effusion, valve disorders, and myocarditis. SSc-pHI remains a leading cause of mortality in this population. Pericardial effusion in SSc is usually associated with heart failure, pulmonary hypertension, and end-stage kidney disease. Cardiac tamponade is a rare initial presentation of SSc-pHI but carries a worse prognosis. Diagnosis is often challenging due to subclinical manifestations and requires advanced imaging modalities such as transthoracic echocardiography with tissue doppler and cardiac magnetic resonance …

A Case Of N-Methyl-D-Aspartate (Nmda) Autoimmune Encephalitis In A 34-Year-Old Man Thought To Have Schizophrenia, Riyad Amin, Michael Wilson, Morgan Koontz, Khawaja Omar
Journal of Community Hospital Internal Medicine Perspectives
Background: N-methyl-D-aspartate (NMDA) autoantibody encephalitis is a rare autoimmune condition affecting approximately 1.5 per million people per year [1]. It predominantly occurs in young women and is often associated with tumors, particularly teratomas [3]. The diagnosis is aided by testing cerebrospinal fluid (CSF) or serum for NMDA receptor antibodies along with careful evaluation of symptoms. Misdiagnosis is common, as its symptoms can mimic other more prevalent conditions, such as upper respiratory illnesses or psychiatric disorders like acute psychosis or schizophrenia.
Case Report: We present the case of a 34-year-old man who was initially incarcerated and subsequently developed decreased communication and …

Echocardiography In Stroke Management: Necessary Or Overused?, Nicole Ann Tesoro, Jasvin Kurundrayil Manha, Jerold Fleishman, Sumanth Bandaru
Journal of Community Hospital Internal Medicine Perspectives
Stroke remains a leading cause of morbidity and mortality worldwide, leading to the development of efficient and evidence-based management strategies. Echocardiography is a crucial diagnostic tool for identifying cardiac sources of embolism, particularly in cardioembolic and cryptogenic strokes, yet its routine use in all stroke patients remains controversial. While evidence-based guidelines recommend a patient-specific approach, rigid hospital quality metrics have incentivized widespread echocardiography, even in low-yield cases such as large artery atherosclerotic strokes. This disconnect raises concerns regarding cost-effectiveness, resource allocation, and potential over-testing, delaying discharges and prolonging hospital length of stay. A more tailored approach, focusing on guideline-driven indications, …

Background
Liver transplantation remains the definitive treatment for end-stage liver disease (ESLD) and hepatocellular carcinoma (HCC). While hepatitis C virus (HCV) was historically the leading indication for liver transplant, the introduction of direct-acting antivirals (DAAs) has drastically reduced HCV-related ESLD. In contrast, metabolic dysfunction-associated steatohepatitis (MASH) and alcohol-associated liver disease (ALD) have emerged as predominant causes of liver transplantation. This study evaluates racial and ethnic disparities in liver transplantation indications from 2004 to 2023 and projects future trends through to 2040.

Management Of Skeletal Class Iii Malocclusion With Deep Bite And Moderate Overjet Using Improved Super-Elastic Ti-Ni Alloy Wire (Isw), Wen-Hsuan Lin, Ching-Yu Yang, Yuan-Hou Chen, Jian-Hong Yu
Taiwanese Journal of Orthodontics
A 20-year-old female presented with concerns about poor dental alignment and a tendency to bite on her lip easily. Clinical examination revealed a concave and brachy facial profile, accompanied by a deep overbite and a moderate overjet. Radiographic examination showed skeletal Class III with a low mandibular plane angle. Correction of the deep bite was achieved by using the Improved Super-elastic Ti-Ni alloy Wire (ISW) and Intermaxillary elastics (IMEs). To correct the moderate overjet, interproximal reduction (IPR), the Multi-bend Edgewise Archwire (MEAW) technique, and arch lengthening of the lower arch were applied. The active treatment duration took approximately 54 months, …

Improved Super-Elastic Ti-Ni Alloy Wire For The Treatment Of Skeletal Class Iii Malocclusion With Anterior Crossbite And Missing Lower Molar, De-Lun Zhuang, Yuan-Hou Chen, Jian-Hong Yu
Taiwanese Journal of Orthodontics
This case report describes the use of Improved Super-elastic Ti-Ni alloy wire (ISW), developed by Tokyo Medical and Dental University, in managing a skeletal Class III malocclusion with anterior crossbite and a missing lower molar. A 26-year-old female presented with a chief complaint of poor dental alignment. Clinical and radiographic evaluation revealed Angle Class III malocclusion, anterior crossbite, a missing mandibular first molar (36), and a centric occlusion–centric relation (CO–CR) functional shift, consistent with a skeletal Class III pattern. Treatment involved the use of ISW for leveling and alignment of both arches and correction of the anterior crossbite, while maintaining …

Purpose: This study aimed to clarify the anatomical boundaries of mandibular molar distalization, emphasizing the risk of inferior alveolar nerve (IAN) contact. Patients and Methods: In total, 30 skeletal Class III patients undergoing orthognathic surgery (59 mandibular hemi-arches) were analyzed using pretreatment three-dimensional (3D) computed tomography scans. The shortest horizontal distance between the mandibular second molar and the inferior alveolar canal (M2-IAC) was measured at multiple depths below the cementoenamel junction, along with the distance to the lingual cortical plate (M2-MLC). Samples were categorized by vertical facial pattern (low-, normal-, and high-angle case). Simulated distalization cases predicting IAN contact were …

Comparative Evaluation Of Selected Properties Of Tetrahedral Amorphous Carbon (Ta-C) Coated And Uncoated Beta Titanium (Β-Ti) Archwires - An In Vitro Study, Abhinaya Jotheswaran, Kurunji Kumaran Navaneethakrishnan
Taiwanese Journal of Orthodontics
Objective: The development of newer archwires can be achieved either by discovering novel materials or by modifying existing ones. The objective of the present study was to coat beta-titanium (β-Ti) archwires with tetrahedral amorphous carbon (ta-C)/hard carbon and to evaluate selected physical and mechanical properties. Methods: Hard carbon–coated (n = 5) and uncoated (n = 5) β-Ti wires were assessed for surface roughness, wear resistance, load-deflection characteristics, and friction using appropriate testing apparatus under dry in vitro conditions. Results: Compared to uncoated β-Ti wires, ta-C–coated β-Ti wires demonstrated a statistically significant reduction in surface roughness, increased wear resistance, and lower …

The Significance Of Medical Terminology In The Arabic 111 Course At King Saud Bin Abdulaziz University For Health Sciences (Ksau-Hs): An Analytical Pragmatic Study, Nuwair Abdullah Khalaf Al-Anzi
UB Journal for Humanities
Medical terminology in the “Arab 111” curriculum varies in its use within the educational context, which may affect students’ comprehension and understanding. Therefore, this study seeks to examine the suitability of these terms from a pragmatic perspective and their relationship to the effectiveness of scientific communication. Medical terminology possesses multiple contextual dimensions and is not confined solely to the scientific context among physicians, patients, and society. Pragmatics thus focuses on the way these terms are employed in everyday communication, since their circulation requires simplification to suit non-specialists. Moreover, misunderstanding medical terminology may lead to errors in diagnosis or treatment, making …

Profiling Students’ Academic Language Readiness For English‑Medium Instruction In Saudi Higher Education, Hanan Sarhan Alsubaiai
UB Journal for Humanities
Saudi universities have been rolling out English‑Medium Instruction (EMI) at a fast pace. But no standardized and validated instrument currently exists to assess students’ readiness. The present study developed the Academic Language Readiness Test (ALRT), administered it to undergraduate students, and determined the score cut-offs distinguishing those who are Ready, Borderline, and those who are Not Ready. A crosssectional, multisite quantitative design was employed with 712 undergraduate students from seven colleges at Bisha University. The ALRT measured four subskills: academic listening, academic reading, academic vocabulary and grammar, and academic study skills (notetaking and visual data interpretation). Psychometric analysis combined Classical …

How Rude?: Prohibitions On Insults And Abuse In International Human Rights Complaint Proceedings, Lisa Reinsberg
Northwestern Journal of Human Rights
May international human rights mechanisms close their doors to individuals who insult their members or Member States? Several do so, but the basis, scope, and impact of such rejections remain mostly unexamined. This article maps the relevant practices of regional and United Nations human rights bodies and compares them to other courts’ responses to contempt of court or vexatious litigation, to understand whether this admissibility criterion is a proper application of the prohibition on abuse of the right of petition or a legitimate exercise of a human rights body’s inherent powers. Three approaches emerge: ignore, balance, and punish. While the …
