Lymph node metastasis (LNM) is a critical prognostic factor for patients with oral squamous cell carcinoma (OSCC). Previous research has implicated the partial epithelial-to-mesenchymal transition of tumor cells and myofibroblastic cancer-associated fibroblasts (myCAFs) in the LNM process. However, the underlying molecular mechanisms remain poorly understood. Here, we conducted a comprehensive molecular analysis integrating original and publicly available OSCC data from bulk genome and transcriptome, single-cell transcriptome, and spatial transcriptome analyses. We found that myCAFs were quantitatively and functionally activated in LNM-positive samples and spatially colocalized with OSCC cells within the invasive tumor front (ITF), providing a niche that may facilitate …

Aberrant DNA methylation has been described in nearly all human cancers, yet its interplay with genomic alterations during tumor evolution is poorly understood. To explore this, we performed reduced representation bisulfite sequencing on 217 tumor and matched normal regions from 59 patients with non-small cell lung cancer from the TRACERx study to deconvolve tumor methylation. We developed two metrics for integrative evolutionary analysis with DNA and RNA sequencing data. Intratumoral methylation distance quantifies intratumor DNA methylation heterogeneity. MR/MN classifies genes based on the rate of hypermethylation at regulatory (MR) versus nonregulatory (MN) CpGs to identify driver genes exhibiting recurrent functional …

Many peptide drugs rely on nonproteinogenic amino acids and chemical modifications for improved activity and proteolytic stability. However, these features also make drug production expensive and challenging to scale. Here, we engineered small, linear, proteinogenic peptides that bind human programmed death-ligand 1 (hPD-L1) with high affinity and stability using mRNA display affinity maturation. The resulting peptides, SPAM2 and SPAM3, have antibody-like affinities for hPD-L1 (dissociation constants between ~250 and 300 pM) and are selective for hPD-L1. Both SPAM2 and SPAM3 compete with hPD-L1 ligands known to interact with the programmed cell death protein 1 site and are stable in human …

Treatment-resistant depression (TRD) represents a severe and complex subtype of major depressive disorder (MDD), affecting approximately 30% of patients who fail to respond adequately to multiple standard antidepressant therapies. While the pathophysiology of TRD remains incompletely understood, emerging evidence suggests that sex-based biological differences might influence its onset, progression, and treatment response. Women are disproportionately affected by depression and are more likely to experience residual symptoms and treatment resistance, potentially due to hormonal fluctuations, immune system differences, and variations in brain circuitry and neuroplasticity. This narrative explores the current literature on the mechanisms underlying TRD, with a particular emphasis on …

The most contentious action taken by reform prosecutors has been the issuance of categorical declination policies. Opponents decry this as bureaucratic nullification of democratically enacted offenses, while the prosecutors themselves counter that they are responding to the will of their local electorate. Democracy claims, it appears, have taken center stage in this debate, and they are deployed by both sides. How should we think about democracy and categorical declinations? The most comprehensive scholarly work on this subject is a 2021 article by Professor Kerrel Murray. In this essay, I hope to continue the work begun by Murray, offering additional insights …

Introduction: Military-dependent young adults (17-25 years old) with autism spectrum disorder (ASD) face significant barriers to accessing services during their transition to adulthood. Frequent relocations, disrupted care, and limited ASD-tailored interventions exacerbate the challenges for these families, with many young adults experiencing a "service cliff" as they age out of pediatric care and school-based services. Addressing these gaps is critical, particularly given the rising mental health challenges and executive dysfunction in this population. This study evaluates the preliminary efficacy of an adapted telehealth intervention, the Military-Launching! program, designed to support young adults with ASD and their military families.

Old Frames On New Media: Health Misinformation In Black Digital Communities, Merriah Croston
Brown School Graduate Student Theses and Dissertations
Growing evidence suggests that health misinformation has the potential to proliferate on social media. However, our understanding of the problem’s magnitude continues to evolve. Despite several studies on health misinformation and Black communities in the United States (U.S.), few of these studies characterize misinformation within digital settings. This dissertation seeks to address this gap by: 1) synthesizing the literature on health misinformation and Black communities to identify prevalent misinformation topics and claims, characteristics of exposure to and belief in these topics and claims, and successful intervention strategies; 2) quantifying and characterizing the volume of COVID-19 misinformation on Black Twitter during …

Composite materials used in aircraft during in-flight service are vulnerable to impacts, which can lead to undetected damage and deterioration. Impact incidents can cause considerable structural harm that might not be immediately apparent, resulting in a decline in the mechanical properties of material and overall performance. There are currently limited advanced models capable of assessing the state of compressive strain (or stress) in a compression after impact (CAI) test based on acoustic emission (AE) data. Developing such a model may enable real-time monitoring and evaluation during in-flight service, providing users with alerts when composite material failure is approaching. This paper …

Triboelectric nanogenerators (TENGs) have been considered as an effective approach for self-powered systems. Currently, coronary heart disease remains the leading cause of death in the United States. This can be easily resolved by balloon angioplasty or a specialized mesh tube called a stent. This study demonstrates a stent sensor made of nitinol, a nickel—titanium alloy used in the medical field for its pseudo-elasticity and strong corrosion resistance, poly(vinylidene fluoride) (PVDF) and polydimethylsiloxane (PDMS), which can measure several physiological parameters while placing it in the arteries. This nitinol health monitor sensor (NHMS) device thus integrates the TENG with a specific medical …

Background The ESCAPE-MeVO (Endovascular Treatment to Improve Outcomes for Medium Vessel Occlusions) and DISTAL (Endovascular Therapy plus Best Medical Treatment vs Best Medical Treatment Alone for Medium Vessel Occlusion Stroke) trials failed to demonstrate the superiority of endovascular thrombectomy over best medical management for medium and small vessel occlusions. Potential limitations of these trials include older patient populations, lower presenting National Institutes of Health Stroke Scale (NIHSS) scores, higher rates of premorbid disability, delayed revascularization times, inclusion of both medium and small vessel occlusions, and widespread use of stent retrievers. Here we present M2 occlusion data from the Imperative Trial, …

Total hip and knee arthroplasty are common surgical procedures aimed at improving mobility and quality of life. Despite surgical advances, postoperative complications remain a concern, particularly for patients with comorbidities and poor nutritional status. Immunonutrition, the targeted administration of nutrients that modulate inflammation, such as arginine, omega-3 fatty acids, and antioxidants, has been shown to reduce infections, complications in wound healing, and shorten hospital length of stay. This review examines current evidence on the role of immunonutrition in hip and knee arthroplasty, focusing on the mechanisms of key nutrients, clinical application of supplementation, and areas for future investigation. Despite promising …

Challenges And Opportunities In Reducing Risk Of Diabetes-Related Cardiovascular Disease: Making Every Contact Count, Michelle Clapham
School of Nursing and Midwifery
Despite recent Government initiatives to reduce rates, the prevalence of type 2 diabetes isexpected to continue rising to almost 10% of the population by 2035. Complications of type 2diabetes, such as cardiovascular disease (CVD), contribute to approximately 22 000 deathsannually in England, indicating a need for population health management through preventativehealthcare. The Making Every Contact Count (MECC) initiative recommends the building ofpreventative strategies into routine primary care appointments to reduce modifiable risk factorsassociated with both type 2 diabetes and CVD. This article explores the unique opportunitiesgeneral practice nurses have to promote primary and secondary prevention of CVD.

The chapter shares critical insights into and reflective analysis of the screenwriting process for the author's essay film Father-land (2018), made collaboratively with the sound artist and film-maker Stuart Moore through an artists' research residency in Nicosia, the only divided capital city in Europe. During the residency, the film-makers lived close to the demilitarized Buffer Zone - a physical border controlled by the United Nations, which has separated the Greek Cypriot south from the Turkish-occupied north since the violent conflict that tore the island in two in 1974. Their day-to-day experience of this fractured urban landscape, inflected by their childhood …

Aim: The purpose of this study was to describe and evaluate the effectiveness of a six-month structured bowel management programme (SBMP) for children with therapy-resistant functional constipation (FC). Method: A retrospective review of medical records with a pre-post design was conducted at an outpatient paediatric clinic in Sweden. Bowel frequency, stool form, faecal incontinence, and abdominal pain were compared before and after the intervention. Predictive factors for successful discharge and duration of care through long-term follow-up were calculated. Results: Of the 142 children enrolled in the SBMP, 132 completed the programme. Despite previous FC therapy resistance, the SBMP achieved a …

Rationale & Objective: Social gradient in health (a “social gradient in health” refers to the observed pattern in which individuals with lower socioeconomic status typically experience poorer health outcomes than those with higher socioeconomic status. This indicates that health disparities exist across different social levels, with the most disadvantaged groups experiencing the worst health outcomes) is significant and established in patients with kidney failure, but the pathways of this relationship are unknown. We aimed to assess the mediating effects of multimorbidity and geographical remoteness in the socioeconomic status (SES)-death associations. Study Design: A cohort study. Setting & Participants: All patients …

Introduction: For a blood-based biomarker to be considered a confirmatory test for the detection of abnormal amyloid beta (Aβ) levels, the sensitivity and specificity must be equivalent to that of current cerebrospinal fluid tests. Methods: In the current study we assessed the ability of phosphorylated tau (p-tau)217 and Aβ42/40 from the Lumipulse G p-tau217 and β-amyloid ratio (1-42/1-40) tests, individually and combined, to predict Aβ positron emission tomography status in two sub-cohorts from the Australian Imaging, Biomarkers, and Lifestyle Study of Ageing. Results: Testing an Alzheimer's disease continuum cohort, the area under the curve (AUC), sensitivity, specificity, and accuracy for …

Objectives: The leading cause of epilepsy mortality is Sudden unexpected death in epilepsy (SUDEP). National and international guidelines recommend that people with epilepsy are counseled on SUDEP risk at the point of diagnosis or at the earliest opportunity. This study compares neurologist views from five European countries on attitudes and relevance of SUDEP communication. Methods: A cross-sectional online survey of 17 Likert questions using validated themes was circulated via different ILAE and/or equivalent organizations using a non-discriminatory exponential snowballing technique, leading to a non-probability sampling. Data were analyzed using descriptive statistics, Kruskal–Wallis, Chi-square, and Fisher's exact tests. Significance was accepted …
