Background:

Anticonvulsants are an essential treatment for bipolar disorder; however, there is relatively little known about their use in older age bipolar disorder (OABD). In this paper, which leverages a large international dataset, we aim to 1) describe the use of anticonvulsants in OABD compared to younger age bipolar disorder (YABD; ages < 50 years old) and 2) explore any demographic/clinical correlates.

Methods:

A secondary analysis was conducted on the international data from the Global Aging and Geriatric Experiments in Bipolar Disorder project. Allowing pathology residents to practice with high levels of autonomy helps prepare them for independent practice. The Accreditation Council for Graduate Medical Education (ACGME) recently added to the core requirements that residents must have opportunities to perform assigned clinical responsibilities under oversight supervision-defined as review and feedback after care is delivered. To understand the current state of resident autonomy, pathology residency programs were surveyed (32 responses) about the highest level of supervision achieved by senior residents for common pathology tasks, as well as the most significant barrier to achieving oversight supervision for each task. Humans with pathogenic variants of the manganese (Mn) transporter gene SLC39A14 exhibit highly elevated brain Mn concentrations and childhood-onset dystonia-parkinsonism. Here we show that Slc39a14-knockout (KO) mice, a preclinical model of the disease with elevated Mn concentrations in the CB, express deficits in physiological tremor implicating cerebellar (CB) dysfunction. Imaging of intracellular Mn in Purkinje cells (PCs) using synchrotron-based X-ray fluorescence microscopy confirmed highly elevated Mn concentrations in the PCs of Slc39a14-KO mice. Aims: We studied the efficacy and safety of icosapent ethyl (IPE) 4g daily in reducing the risk of myocardial infarction (MI) across different MI subtypes and sizes, among REDUCE-IT high-risk patients with hypertriglyceridemia.

Methods: REDUCE-IT was a phase 3b, double-blind multicenter trial. Patients with established CVD or diabetes who were treated with statins and had moderate hypertriglyceridemia were randomized to receive IPE 4g daily or placebo. The current analysis focused on MI subtypes (fatal MI, nonfatal MI, ST-segment elevation MI (STEMI), non-STEMI (NSTEMI)), as well as MI size (measured by multiples of troponin upper limit of normal) and MI-related complications. …

Background: Selective dorsal rhizotomy (SDR) is typically a one-time surgical procedure supported by intense rehabilitation to improve ambulatory capability in children with spastic cerebral palsy (CP).

Observations: The authors present the unique case of a teenager with spastic diplegic CP who had undergone L2-S1 SDR as a child and presented with residual focal spasticity in the right plantar flexors. He underwent revision SDR at the right L5 and S1 levels in addition to decompression of canal stenosis. Postoperatively, he had notable improvement in his gait with relief of preoperative right knee and foot pain, improved ease of ambulation, and improved …

Adolescence is a developmental phase full of challenges, particularly for students from disharmonious family backgrounds who are vulnerable to low self-confidence. This study aims to enhance students’ self-confidence through group guidance services using the sociodrama technique at a public junior high school in Yogyakarta. The research employed a quantitative approach with a one-group pretest–posttest design. The subjects consisted of six eighth-grade students identified as coming from disharmonious families and having low self-confidence. It has been more than two years since the war erupted in Sudan. Although the crisis initially started between two warring parties, it has expanded beyond the two with the involvement of multiple actors. However, the current conflict is complicated and has deep historical origins. It increased after the 2019 revolution, with the Sudanese Armed Forces (SAF) and Rapid Support Forces (RSF) battling for power and control. This report documents attacks on civilians and objects essential for their survival in Sudan. It concludes that both parties to the conflict violated international human rights and humanitarian law, with most violations amounting to war crimes. The Rapid Support Forces’ conduct may also constitute crimes against humanity, including persecution and extermination. The report calls for accountability and sets out a roadmap for justice.

The essay examines the ecological marginalization of Dalit and tribal women in India, especially by analysing the three points of intersection, namely, caste, patriarchy, and gender. Though it starts with a global outlook on the African water crisis, it substantiates the argument with Indian realities of water inequality and resource depletion. This case study investigates how a local community agency engages with the Somali diaspora to design and implement culturally responsive services, such as the Settlement and Integration Program. It also explores the potential of fostering meaningful dialogue with community members to inform the development of culturally sensitive programming. The research question guiding this study was developed by findings from previous research, which highlighted the significance of community engagement, sustained dialogue, and the necessity for agencies to design clearly defined programs. These programs should prioritize active participation from community members in both the creation and implementation of tailored initiatives.

Pulmonary veno-occlusive disease (PVOD) is a rare and severe subtype of pulmonary arterial hypertension, characterized by progressive remodeling of small pulmonary arteries and veins with no therapies. Using a mitomycin C–induced (MMC-induced) rat model, we previously demonstrated that protein kinase R–mediated (PKR-mediated) integrated stress response (ISR) drives endothelial dysfunction and vascular remodeling. To determine whether PKR is the primary mediator of ISR and the pathogenesis, we treated control (Ctrl) and PKR-knockout (KO) mice with the same dose of MMC. Consistent with rat data, Ctrl mice displayed ISR activation, vascular remodeling, and pulmonary hypertension after MMC treatment, while KO mice showed …

Ureaplasma species (Ureaplasma spp.) are commensal organisms of the lower urogenital tract, but overgrowth has been associated with vaginal infections such as bacterial vaginosis (BV) and sequelae, including pelvic inflammatory disease (PID), infertility, and pregnancy complications such as preterm labor. Intrauterine devices (IUDs) are an effective contraceptive method; however, their use has been linked to an increased risk of urogenital infections, including those involving Ureaplasma spp., which may disseminate and lead to adverse health outcomes. Diabetes is among the leading causes of morbidity and mortality in the United States; therefore, effective Diabetes Self-Management Education (DSME) is vital in improving patient outcomes. A needs assessment of the Practice site identified gaps in provider confidence and frequency and consistency of delivering DSME that correlated with poor patient adherence and readmissions. The purpose of this quality improvement (QI) project was to implement a structured, evidence-based DSME training program to improve healthcare provider self-efficacy to provide DSME. The present study examines the relationship between perceived parental styles, grandiose narcissism, vulnerable narcissism, and defense mechanisms with the aim of replicating the Imamgolu & Batigun (2020) research findings. A cross-sectional design was used. Covariates such as sex, age, parental dynamics in youth, and household dynamics in youth. To do this, 150 participants between the ages of 18-65 and residing in the United States were recruited via Prolific. Regarding sex, 66.7% of the participants were female, 32.7% were male, and 0.7% were intersex. Fish provides essential nutrients, including protein, omega-3 fatty acids (oily fish) and other micronutrients, and may be seen to have a prominent role in protecting against non-communicable diseases, especially cardiovascular disease. Recent UK National Diet and Nutrition Survey analysis suggests people are not meeting their weekly fish intake recommendation of at least two portions per week, of which one should be oily. Lower socioeconomic groups are more likely to eat poor-quality diets, with low fish intake, resulting in poor health outcomes. In the United States alone, the prevalence of NAFLD is currently 38% and there has been a 50% increase over the last three decades. The present study investigated the role of ultratrace mineral deficiencies, specifically magnesium (Mg) and zinc (Zn), and if so, what nutritional interventions could be made to ameliorate or protect against the development of NAFLD. Religiosity is generally negatively associated with attitudes toward suicide. Religious individuals typically have more negative attitudes toward suicide than individuals with lower levels of religiosity. Hence, using a cross-sectional, predictive design, we aimed to investigate the link between religiosity and suicide attitudes and how much religiosity predicts different suicide attitudes. Climate change and human activity have severely degraded watersheds, jeopardizing their sustainability. The willingness of residents to pay for watershed management is essential for developing sustainable environmental protection efforts. This study determined the respondents' willingness to pay (WTP) for the restoration and protection (RP) of the Ulot Watershed of Samar Island Natural Park using a double-bounded dichotomous choice contingent valuation approach. The researchers used the Kish grid method to draw respondents from 150 households. Acute pancreatitis (AP) is a common cause of gastrointestinal-related hospitalizations in the United States. It is characterized by pancreatic inflammation and necrosis, with diagnosis based on elevated pancreatic enzymes, characteristic imaging, and epigastric pain radiating to the back. While gallstones and alcohol are the predominant etiologies, drug-induced pancreatitis (DIP) accounts for up to 5% of cases and remains a diagnostic challenge. Over 500 medications, including mesalamine, have been implicated. Proposed mechanisms of DIP include pancreatic duct obstruction, toxic metabolite accumulation, hypersensitivity reactions, and localized angioedema. Prompt recognition and withdrawal of the offending agent are critical to prevent complications.
