Oral health disparities continue to present a significant public health challenge for older adults in the United States, particularly among racial minority and low-income populations. Despite national discussions around healthcare equity, there remains limited empirical insight into how race and income influence dental service utilization among Medicare beneficiaries. In this quantitative, cross-sectional study, the extent to which these demographic factors predicted oral healthcare utilization, using nationally representative data from the 2021 Medicare Current Beneficiary Survey, was assessed. Guided by the social determinants of health framework, chi-square tests, binary logistic regression, and negative binomial regression were performed to analyze the data. …

The health outcome of reducing 30-day Medicare readmissions continues to be a constant measurement of quality patient care. Post-acute services, like home health care, play a vital role in national efforts to improve patient care, contributing to cost reductions. This quantitative study explored the relationship between home health agency (HHA) ownership (for-profit vs. nonprofit) and Medicare 30-day readmissions by race, age, and gender. The three research questions addressed the independent variable, type of HHA ownership (for-profit vs. nonprofit), and the impact on the dependent variables, 30-day Medicare readmissions by race, age, and gender in Illinois. Organizations must find ways to sustain nurses and healthcare workers beyond traditional human resource programs and focus on unhealthy practice environments. A program evaluation was conducted to determine whether developing Caritas behaviors through a Caring Science program improves compassion satisfaction in nurses and interdisciplinary teams professional quality of life.

Exploring The Black Woman Experience Related To Sexual Practice, Sexual Identity, And Sexual Expression: A Qualitative Study, Ashley V. Rodriguez

This study revealed the complex interplay between personal identities and social constructs, focusing on Black-identifying women and their lived experiences with sex, sexuality, and sexual expression. Over centuries, societal influences depicted Black women as hypersexual, simultaneously exposing them to criticism, shame, and judgment for their sexual expression and meaning-making related to sex and sexuality. Consequently, Black women remain a high-risk demographic for unsafe sexual practices and sexually transmitted infections. Using intersectionality theory and Black feminist theory, this study explored how Black-identifying women give meaning to sex, sexuality, and sexual expression as they shape the Black feminine experience. Expressions Of Compassion Fatigue By Emergency Department Nurses Caring For Patients With Substance Use Disorders, Elizabeth B. Dowdell

Compassion fatigue occurs in unhealthy and violent environments. This qualitative study interviewed 53 emergency department nurses at a Level I trauma center. The ED nurses expressed frustration with addicted patients, negative responses, and decreased job satisfaction. Creating a healthy work environment includes fostering self-awareness and strategies to avoid compassion fatigue.

The AACN Healthy Work Environment Standards include essential evidence-based guidelines for improving nursing and patient outcomes. However, the centering of dominant groups limits the implementation of these standards. This presentation examines strategies for interprofessional healthcare teams to become authentic allies with nondominant groups and to create healthy work environments.

PURPOSE: Coats disease is a rare, retinal vascular disorder characterized by telangiectasias, aneurysmal dilations, and progressive exudative retinal detachment. Limited understanding of the disease warranted the need to identify controversial issues through an extensive literature search and a debate and discussion among international panels of experts.
METHODS: Extensive literature search was done on multiple aspects of the disease-classification patterns, disease, Coats-plus, and Coats-like response. Other key factors included the etiology, possible genetic patterns, relationship with other vascular disorders, the role of inflammatory factors and vascular endothelial growth factor (VEGF) in the pathogenesis, diagnostic features, and complications. The population of Puerto Rico has been heavily affected by chronic diseases; however, data remains limited on the influence of diabetes mellitus (DM), hypertension, and obesity on the probability of COVID-19 diagnosis. The purpose of this quantitative study was to evaluate the association between non-communicable diseases that included DM, obesity, hypertension, and COVID-19 diagnosis, controlling for age and sex and person exposure in Puerto Rico. The sample consisted of 1,463 participants who underwent a polymerase chain reaction (PCR) test for COVID-19 from April 1, 2020, to March 31, 2021. BACKGROUND: "Tranq dope"-fentanyl adulterated with xylazine-has complicated the management of opioid withdrawal in emergency departments (EDs). A novel withdrawal protocol was implemented to address these challenges, utilizing concurrent agents to intentionally synergize and potentiate effects. Questions remain about the safety of its components, some of which may prolong the QT interval.
METHODS: We conducted a retrospective cohort study of adult ED patients with suspected fentanyl/xylazine exposure treated utilizing novel protocol across two urban hospitals in Philadelphia, PA between September 2022 and October 2024. The Improving Nurse-Patient Communication by Fostering Ethnocultural Empathy quality improvement project engaged bedside nurses at an East Bay urban medical center in online cultural competency curriculum and focus group discussion. Nurses Scale of Ethnocultural Empathy (SEE) was assessed pre- and post – training using the SEE assessment tool.

Enhancing interprofessional communication has been shown to improve patient outcomes. Completing case studies can prepare pre-licensure students how to use the SBAR tool effectively. By assessing students using a rubric in sequential nursing courses during simulation laboratory, the impact of interprofessional communication teaching strategies can be evaluated.

The value of formal mentoring in nursing noted in literature too often emphasizes goal attainment without focusing on establishing meaningful relationships. A shared value held by mentor and mentee to intentionally transform within the partnership depends on each embracing a growth mindset and vulnerability to overcome barriers to dyad development.

Regorafenib Pulmonary Toxicity: A Case Of Cavitary Lung Disease And Ards Treated With Infliximab, Michael Dong, Mackenzie Kramer, Tanmay Gandhi, Gautam George

Regorafenib-induced pneumonitis and acute respiratory distress syndrome (ARDS) are rare and poorly characterized toxicities. We describe a 43-year-old male with metastatic colorectal cancer who developed hypoxic respiratory failure after two cycles of regorafenib. Imaging revealed cavitary lung lesions and diffuse ground glass opacities. Despite antibiotics and high-dose corticosteroids, his condition deteriorated, requiring ICU admission and mechanical ventilation. Bronchoalveolar lavage revealed bacterial pathogens, and he was treated for pneumonia. After multidisciplinary discussion, he was assessed to have regorafenib-induced pneumonitis as a major driver of his respiratory failure, and after limited steroid response, infliximab was administered with marked respiratory improvement. Rates of lactate production and consumption reflect the metabolic state of many cell types, including neurons. Here, we investigated the effects of nutrient deprivation on lactate dynamics in Drosophila neurons by leveraging the limiting effects of the unstirred layer, a diffusion barrier that forms around cells in culture. We found that neurons constitutively consume lactate when the availability of trehalose, a glucose disaccharide preferred by insects, is limited by the unstirred layer. Acute mechanical disruption of the unstirred layer reduced this reliance on lactate. Trauma remains a leading cause of morbidity and mortality in part due to complex pathophysiological responses. Yet our abilities to predict these changes are limited. Plasma miRNAs have been proposed as DAMPs that drive immune response and organ injury. Here, we test a panel of plasma miRNAs-selected based on next-generation sequencing and nucleotide motifs identified via a machine-learning algorithm-for their abilities to predict subclinical pathophysiological injuries. We find marked and severity-dependent increases in the miRNA biomarkers following trauma, which are closely associated with various injury markers. Lysosomes are essential organelles for cellular homeostasis and signaling, with dysfunction linked to neurological disorders, lysosomal storage diseases, and cancer. While proteomics has advanced our understanding of lysosomal composition, the structural characterization of lysosomal membrane proteins in their native environment remains a significant challenge. Here, we developed a cryo electron tomography workflow to visualize lysosomal membrane proteins within intact, native lysosomal membranes. We isolated endolysosomes by independently targeting two lysosomal membrane proteins, transient receptor potential mucolipin 1 and transmembrane protein 192, enriching organelles that exhibited the expected morphology and proteomic composition of the endolysosomal system.
