In 2023, the United States emitted an estimated total of 4,807 million metric tons (“MMTs”) of Carbon dioxide, with only a 0.2% decrease in 2024. These figures are simply not up for debate. What is often debated, however, is how (and if) the U.S. should be tackling these carbon emissions. Many would argue that the U.S. must increase its reliance on renewable energy and decrease its consumption of fossil fuels, which are major contributors of Carbon Dioxide and other Green House Gas (“GHG”) emissions. Background: Complex socio-cultural, psychological, geographical, and service-related challenges are faced when recruiting people with dementia for clinical trials. The aim of Phase 1 of the Dementia PersonAlised Care Team (D-PACT) project was to assess the feasibility of recruiting (identifying, approaching and consenting) people with dementia, including those without capacity to consent, to a cluster randomized controlled trial of a primary care-based personalized dementia support intervention in England. COVID-19 necessitated a shift to remote working, creating the opportunity to compare recruitment strategies before and under lockdown constraints. Our perceptual systems detect information about how our body is moving relative to the surrounding environment. Such information is essential to our ability to maintain upright standing balance. Real-time postural feedback systems are technologies that appear to enhance our ability to detect information about the movements of our body, and as a consequence improve our capacity to control the posture of our body. Here we examine a system in which real-time measurements of head position were "felt" as changes in the intensity of vibration of tactors in a headband. Synergetics is an emerging paradigm which quantitatively describes and predicts the dynamics of complex living and nonliving systems. It has illuminated how complex systems can spontaneously self-organize by coordinating the activity of their constituent components toward some coherent, collective behavior. This coordinated activity can produce complex macroscopic spatiotemporal patterns without the need for central planning. In this dissertation, we develop and explore generalizations of the Haken-Kelso-Bunz (HKB) model of coordination dynamics. These generalizations include modeling higher-dimensional systems of coupled multistable oscillators with arbitrary diversity and coupling parameters, where the system components have the capacity to “learn” or adapt their intrinsic …

OBJECTIVES: Describing the development of a visual dashboard leveraging available tools for efficient recruitment for patient centered clinical trials in resource constrained settings.

MATERIALS AND METHODS: A real-time, visual dashboard was developed, facilitating interactive visualizations, detailed analyses, and data quality control. Daily automated REDCap data retrieval occurred via an R program using REDCap API and output was integrated into Power BI. An interrupted time series analysis was conducted evaluating effects of dashboard on clinical trial recruitment metrics.

RESULTS: The visual dashboard displayed key recruitment metrics, including individual participant progression and recruitment trends over time. Interrupted time series analysis showed improvements …

INTRODUCTION: Fenestration of the lateral semicircular canal has been proposed to improve intracochlear gene therapy delivery, but the relationship between the membranous and bony labyrinths at the canal apex has not been systematically defined.

METHODS: We examined 62 human temporal bones, including 31 unaffected specimens and 31 with chronic otitis media, using histologic review to assess the membranous-to-bony relationship at the apex.

RESULTS: In all specimens, the membranous labyrinth directly contacted the outer circumferential bony wall.

CONCLUSION: This consistent finding, independent of disease state, indicates that fenestration at the lateral canal apex carries a high risk of membranous injury. Potential …

BACKGROUND/OBJECTIVES: Patient satisfaction is increasingly recognized as a key indicator of the effectiveness of substance use disorder (SUD) treatment. While some studies suggest disparities in satisfaction across treatment settings, there remains limited research examining these differences, particularly among vulnerable populations. This study aimed to assess patient perceptions of satisfaction with opioid use treatment services and explore how demographic and socioeconomic factors influence these experiences.

As transit-oriented interiors increasingly evolve into complex commercial and social micro-environments and serve diverse user groups, seating design must respond beyond basic functionality to address the need for adaptation and inclusivity. This study explores adaptive and inclusive design strategies through the development of seating design in such a context. With up to 10 years of follow-up, we report results from the final analysis of RESONATE- 2, a phase 3 study of first-line ibrutinib vs chlorambucil for the treatment of chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL). Patients aged ≥65 years with previously untreated CLL/SLL without del(17p) were randomly assigned to receive either single-agent ibrutinib (420 mg/d; n = 136) or chlorambucil (0.5-0.8 mg/kg; ≤12 cycles; n = 133). Background: The prevalence of psychiatric and neurological disorders in populations with human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) presents significant public health concerns including impaired quality of life, reduced antiretroviral therapy (ART) adherence, and elevated morbidity and mortality rates. Research suggests that these correlations translate to millions of people globally, given that over 25 million people are living with HIV/AIDS in the African region alone. We use foreign trade data on both imports and exports of goods and services among a group of 15 advanced economies to determine the incomes and price elasticities of demand for exports and imports of goods, services and goods and services combined in the long run. We find that changes in foreign and domestic income have, as expected, a positive long-run impact on the demand for exports and imports, respectively, with the impact of income changes being typically greater on the demand for services than on the demand for goods. Lipid metabolism plays a significant role in the regulation of various critical pathways within cells, where enhanced lipid metabolism is a hallmark of cancer cell metabolism. The Hippo signalling pathway poses as an important signalling pathway that governs tissue growth control and tumorigenesis. The effector of the Hippo signalling pathway, Yes-associated protein (YAP), serves as a central regulator for this growth control. Once a tissue develops to its correct size, YAP is phosphorylated and inactivated within the cytoplasm by the activation of the Hippo pathway, where its inactivation results in YAP nucleus translocation. PURPOSE: AXL, a receptor tyrosine kinase related to oncogenic processes, is aberrantly expressed in various cancers and associated with treatment resistance. Enapotamab vedotin (EnaV), a novel anti-AXL human IgG1 and monomethyl auristatin E antibody-drug conjugate, demonstrated antitumor activity in preclinical models, including non-small cell lung cancer (NSCLC). This phase 1/2 study assessed the safety and preliminary efficacy of EnaV in solid tumors. PATIENTS AND METHODS: This study comprised dose-escalation and dose-expansion phases; both phases investigated EnaV once every 3 weeks (Q3W) and EnaV on days 1, 8, and 15 of a 28-day cycle (3Q4W). Background: The purpose of the Vigilan observational study (ClinicalTrials.gov, NCT02931682) was to prospectively assess the natural history and developmental course of creatine transporter deficiency (CTD).

Methods: Males with CTD aged 6 months to 65 years were evaluated at 6-month intervals for up to 4 years. Evaluations included neurodevelopmental assessments of intellectual functioning, adaptive functioning, and challenging behaviors and the onset and progression of medical comorbidities.

Results: Fifty participants (median age, 7.6 years) were enrolled. The predominant CTD phenotype consisted of significant intellectual disabilities and limited skill development over time. Most participants had a history of febrile or nonfebrile seizures, …

Radiation exposure impairs rapidly renewing tissues like the intestinal epithelium, yet translational insights from murine models have been limited by species-specific responses. Here, we use human intestinal organoids (HIOs) derived from jejunal epithelium to evaluate human epithelial responses to low dose proton and photon (gamma) radiation. Gamma irradiation induces a unique developmental and metabolic shift in crypt-like organoids, including enrichment of amino acid metabolism pathways and activation of fetal-associated transcription factors and morphology. Integrated multi-omic profiling reveals serotonin biosynthesis as a central regenerative node. Cohesin gene mutations occur in many malignancies, including acute myeloid leukemia (AML). Loss-of-function mutations in the four major cohesin complex genes (RAD21, SMC3, SMC1a, and STAG2) occur across most major genetic subtypes of AML but are notably absent in AML harboring CBFB::MYH11, suggesting that cohesin mutations yield distinct biological outcomes dependent on the genetic AML driver. We hypothesized that CBFB::MYH11-expressing leukemias would be dependent on intact cohesin genes given their near-mutual exclusivity. To investigate this, we combined either germline or inducible heterozygous deletions in cohesin genes Smc3 or Rad21, respectively, with an inducible murine model of Cbfb::MYH11 AML. This approach …

The female Aedes aegypti mosquito's remarkable ability to hunt humans and transmit pathogens relies on her unique biology. Here, we present the Aedes aegypti Mosquito Cell Atlas, a comprehensive single-nucleus RNA sequencing dataset of more than 367,000 nuclei from 19 dissected tissues of adult female and male Aedes aegypti, providing cellular-level resolution of mosquito biology. We identify novel cell types and expand our understanding of sensory neuron organization of chemoreceptors across all sensory tissues. Our analysis uncovers male-specific cells and sexually dimorphic gene expression in the antenna and brain. In female mosquitoes, we find that glial cells, rather than neurons, …

Unexpected infant deaths often present complex diagnostic challenges, where clinical histories alone may not reveal the underlying etiology. Autopsy examination remains essential in identifying uncommon neuropathologic findings that contribute to mortality.

A two-month-old male infant was found unresponsive in his car seat (day 0) and succumbed four days later despite intensive medical intervention. His medical history included neonatal respiratory distress with two prior admissions to the neonatal intensive care unit. Autopsy was conducted one day after death (approximately five days postictus), revealing ectopic lymphoid tissue within the medulla of the brainstem, accompanied by subacute infarction and early necrosis. A study examined the job barriers, supports, satisfaction, and preparation programs of Texas school social workers (TSSW). The study used a convergent parallel mixed method design to compare and discuss quantitative and qualitative data obtained through a survey emailed to 476 Texas school social workers. Data was gathered from 250 social workers. The findings highlight the need to better understand the role of TSSWs and improve university social work preparation programs. BACKGROUND/AIM: While surgery is a major component of treatment for managing rectal cancer, some individuals opt against it, potentially affecting their chances of survival. This study investigated the clinical and demographic elements linked to the decision to refuse surgery and assessed the potential impact of this decision on overall survival (OS). PATIENTS AND METHODS: We conducted a retrospective cohort study using the U.S. National Cancer Database to analyze factors linked to surgery refusal in patients with rectal cancer. Arthritis of the first carpometacarpal joint is a prevalent and often disabling condition for many older adults. The diagnosis is typically established through a comprehensive patient history, physical examination, and confirmatory radiographs. Nonsurgical management with splinting and anti-inflammatory medication has demonstrated effectiveness. Several surgical options have proven effectiveness for those who fail nonsurgical management. Surgical options include silicone implantation, resection arthroplasty, tendon interposition, ligament reconstruction, denervation, and joint implant arthroplasty. In the following review, we will elaborate upon the array of therapeutic approaches, culminating in the delineation of our preferred method.

Insect proteins, especially from Tenebrio molitor (TM), are emerging as sustainable alternatives to conventional proteins. This study aimed to design and evaluate TM-enriched food formulations for individuals with increased protein needs, assessing their nutritional, physicochemical, and sensory properties, and exploring feasibility. Two sensory evaluations were conducted using the Check-All-That-Apply (CATA) method. The first (CATA1) assessed the direct incorporation of TM powder (32 g/100 g) into a pasteurized dairy product (D) with hazelnut (DTM1H), hazelnut–vanilla (DTM1HV), or vanilla (DTM1V) aroma.
